Transplantation of HLA-mismatched CD34+ selected cells in patients with advanced malignancies: severe immunodeficiency and related complications.

This trial was designed to test the use of CD34+ selected haemopoietic stem cells (HSC) in HLA-mismatched donor-recipient pairs, following intensive conditioning with thiotepa, antilymphocyte globulin (ALG), cyclophosphamide and single-dose total-body irradiation (sTBI). 10 patients aged 16-50 with advanced malignancies and a two- or three-antigen mismatched family donor entered this study. Donor marrow and G-CSF primed peripheral blood cells were processed separately on CD34 columns (Ceprate). The median number of infused CD34+ cells were 5.66 x 10(6)/ kg, with 0.55 x 10(6)/kg CD3+ cells. Nin

Tumor-targeted interferon-α gene therapy for glioblastoma: a phase 1 trial.

Glioblastoma (GBM) is an immunologically cold brain tumor with poor outcome, characterized by a myeloid-driven immunosuppressive microenvironment. Here we report an interim analysis of a first-in-human phase 1/2a dose-escalation study evaluating Temferon-a genetically engineered autologous stem cell transplant designed to deliver interferon-α2 by means of myeloid progeny recruited to the GBM TME and locally activate antitumor immunity. Twenty-four newly diagnosed patients with GBM and unmethylated MGMT promoter were treated across eight cohorts following surgical resection and radiotherapy, te

Diagnosis and treatment of familial hemophagocytic lymphohistiocytosis with isolated central nervous system involvement: A case report.

Familial hemophagocytic lymphohistiocytosis (FHL) is the prototypical genetic form of hemophagocytic lymphohistiocytosis (HLH), a potentially fatal hyperinflammatory condition. Limited awareness of HLH with isolated central nervous system (CNS) involvement often leads to underdiagnosis or diagnostic delay. Here, we report a 13-year-old girl with FHL type 3 (FHL3) presenting with isolated CNS involvement who subsequently underwent allogeneic hematopoietic stem cell transplantation (HSCT). This case expands the clinical spectrum of FHL3 and highlights that isolated neurological symptoms may be t

HLA-haploidentical hematopoietic stem cells transplantation with regulatory and conventional T-cell adoptive immunotherapy in pediatric patients with very high-risk acute leukemia.

Allogeneic hematopoietic stem cell transplantation (HSCT) is still needed for many children with very high-risk acute leukemia. An HLA-haploidentical family donor is a suitable option for those without an HLA-matched donor. Here we present outcomes of a novel HLA-haploidentical HSCT (haplo-HSCT) strategy with adoptive immunotherapy with thymic-derived CD4+CD25+ FoxP3+ regulatory T cells (Tregs) and conventional T cells (Tcons) performed between January 2017 and July 2021 in 20 children with high-risk leukemia. Median age was 14.5 years (range, 4-21), 15 had acute lymphoblastic leukemia, 5 acut

Extracranial primitive neuroectodermal tumors. The Memorial Sloan-Kettering Cancer Center experience.

The clinical data of 54 patients (57% males) with extracranial primitive neuroectodermal tumors (PNET) seen over a 20-year period at Memorial Sloan-Kettering Cancer Center were reviewed. The age at diagnosis ranged from 1 month to 81 years (median, 17 years). One PNET arose in a previously irradiated site. One patient had an unaffected identical twin. Primary sites were thoracopulmonary (n = 25), pelvis (n = 12), retroperitoneum or abdomen (n = 10), limbs (n = 5), neck (n = 1), and unknown (n = 1). At diagnosis, epidural extension was present in 13 patients with truncal primaries, and 11 patie

Sensitivity of p53-deficient cells to oxaliplatin and thio-TEPA (N, N', N" triethylenethiophosphoramide).

P53 is known as a determinant of cellular responses to DNA damage, including apoptosis, cell cycle arrest, and DNA repair. Its role is most easily understood in the context of Burkitt lymphoma and other apoptosis-prone cell types. A number of epithelial cancer cell types, by contrast, exhibit a higher threshold for apoptosis induction in response to DNA damage. In fact, p53 mediates DNA repair and protective responses in the latter cell types, in some cases p53-deficient cells being more sensitive to DNA damage, antithetical to the situation in Burkitt lymphoma and other apoptosis-prone cell t

Experience with newer intravesical chemotherapy for high-risk non-muscle-invasive bladder cancer.

The definitive treatment for patients with high-risk non-muscle-invasive bladder cancer (NMIBC) who fail to respond to intravesical bacillus Calmette-Guérin (BCG) is cystectomy. However, many patients who experience recurrence after BCG are either poor operative candidates or refuse surgery due to the long-term impact on their quality of life. In the last decade, there has been an increased interest in alternative intravesical therapies, and several novel chemotherapeutics have emerged as promising agents for high-risk NMIBC patients unable or unwilling to undergo cystectomy. Additionally, ext

Nitroxyl radicals decrease toxicity of cytostatic agents.

Nitroxyl radicals of a series of piperidineoxyles and pirrolinoxyles increase the tolerance of experimental animals to the injection of otherwise lethal doses of anti-tumor cytostatic agents. Simultaneous injection of nitroxyl radicals in different doses with 6-mercaptopurine, thiophosphamide, cyclophosphamide and the other cytostatic agents results in decreased toxicity and survival of animals. Nitroxyl radicals normalize the level of the oxidized form of P450 cytochrome that is decreased by the injection of lethal doses of cytostatic agents.

Cytotoxic, cell cycle, and chromosomal effects of methylxanthines in human tumor cells treated with alkylating agents.

Human tumor cells, like rodent cells, are sensitive to effects of methylxanthines (MEX) on lethality, cell cycle delays, and chromosome aberrations after DNA damage by anticancer drugs. Enhanced cytotoxicity of alkylating agents was observed when T24 human bladder tumor cells in culture were exposed to nontoxic concentrations of MEX such as caffeine or pentoxifylline. Tumor cell lethality was increased up to 10-fold by either caffeine or pentoxifylline (1 mM) present during the first cell cycle (16-24 h) after exposure to nitrogen mustard (HN2) or thiotepa. Reduced-Intensity Compared to Nonmyeloablative Conditioning in Patients with Non-Hodgkin Lymphoma Undergoing Allogeneic Hematopoietic Stem Cell Transplantation.

Reduced-intensity conditioning (RIC) and nonmyeloablative (NMA) conditioning are preferred for patients with non-Hodgkin lymphoma (NHL) undergoing allogeneic hematopoietic stem cell transplantation (allo-HCT). Although prior studies have suggested that higher-intensity regimens in RIC-NMA conditioning are associated with inferior outcomes in patients with NHL, the optimal conditioning regimen remains unknown. We performed a retrospective single-center analysis to determine outcomes of adult patients with B cell and T cell NHL who underwent allo-HCT and received either RIC or NMA conditioning b

Pharmacologic approaches to the treatment of meningeal malignancy.

The meninges are a unique site of recurrence for certain malignancies because of the limited penetration of systemically administered cytotoxic drugs across the blood-brain barrier. While this phenomenon was first recognized in children with acute lymphoblastic leukemia, a similar pattern is also occurring in breast cancer, ovarian cancer, and small cell lung cancer. Recognition of the limitations of standard systemic antileukemic therapy for the treatment of meningeal disease led to the development of new therapeutic strategies targeted directly at the CNS. These include intralumbar therapy u

Malignant mesothelioma in children: report of seven cases and review of the literature.

Seven children with malignant mesothelioma have been seen at Memorial Hospital since 1953. In six, the origin was at the pleura and in one at the peritoneum. None of the patients related a history of exposure to asbestos. Two patients lived more than five years. The other five patients died within two years of the diagnosis. Distant metastases were seen in four of the patients, including three who had metastases to brain. Surgery or radiotherapy were not effective in controlling the disease in most of the patients. One patient had a complete response to a combination of Adriamycin, cyclophosph

Nonmyeloablative and reduced-intensity conditioning for allogeneic hematopoietic stem cell transplantation in sickle cell disease: a narrative review of safety, engraftment, and graft-versus-host-disease outcomes in adults and high-risk populations.

Allogeneic hematopoietic stem cell transplantation (HSCT) is the only widely available cure for sickle cell disease (SCD), yet its use in adults has historically been limited by transplant-related toxicity. Non-myeloablative (NMA) and reduced-intensity conditioning (RIC) regimens have addressed this by maintaining durable engraftment at considerably lower risk. The NIH NMA protocol, alemtuzumab plus 300 cGy total body irradiation, achieves overall survival (OS) above 90% in matched sibling transplants, with no chronic graft-versus-host disease (GVHD) and preserved fertility. Expert consensus for prevention, diagnosis and management of persistent chemotherapy-induced alopecia.

BACKGROUND: Persistent chemotherapy-induced alopecia (pCIA) is a distressing side effect of antineoplastic agents, imposing significant psychological burdens on cancer survivors. Despite its impact, there are no standardized guidelines for diagnosis, prevention or management. OBJECTIVES: To establish consensus-based definitions, diagnostic criteria, grading systems and management recommendations for pCIA. METHODS: A two-round Delphi method was conducted with 15 international experts in supportive oncodermatology and hair diseases from both Europe and the Americas. Statements were rated on a

Brentuximab-vedotin maintenance following chemotherapy without irradiation for primary intracranial embryonal carcinoma in down syndrome.

BACKGROUND: Germ cell tumors (GCT) are the most common central nervous system (CNS) tumors in individuals with Down syndrome. Patients with Down syndrome treated with CNS irradiation are at increased risk of developing cerebrovascular complications such as moyamoya disease. Embryonal carcinoma components are recognized to be more resistant to conventional chemotherapy and radiotherapy and confer a very poor prognosis. CD30 is a member of the tumor necrosis factor-receptor superfamily. CD30+ has a limited expression in normal cells but is the defining marker for embryonal carcinoma. Brentuximab
