Pigeon chest: comparative analysis of surgical techniques in minimal access repair of pectus carinatum (MARPC).

BACKGROUND: After minimally invasive repair for pectus excavatum (MIRPE), similar procedures for pectus carinatum were developed. This study aimed to analyse the various published techniques of minimal access repair for pectus carinatum (MARPC) and compare the outcomes. DATA SOURCES: Literature was reviewed on PubMed with the terms "pectus carinatum", "minimal access repair", "thoracoscopy" and "children". RESULTS: Twelve MARPC techniques that included 13 articles and 140 patients with mean age 15.46 years met the inclusion criteria. Success rate of corrections was n = 125, about 89% in cumu

Surgical site infection related to use of elastomeric pumps in pectus excavatum repair. Lessons learned from root cause analysis.

BACKGROUND: Pectus excavatum repair (PEX) is among the most painful thoracic procedures performed. Continuous peripheral nerve blockade (CPNB) is known to be efficacious in optimizing pain control while limiting narcotic use in adult thoracic procedures. It was introduced in May 2015 as a bridge to oral pain control in children undergoing PEX. Consequently, the surgical site infection (SSI) rate increased from 2.7% to 27.7%. METHODS: SSI surveillance followed national guidelines. The abrupt increase prompted root cause analysis and cessation of CPNB use. A dynamic systems model of SSI in PEX

Retrospective study of two-stage surgery in the treatment of scoliosis exceeding 100 degrees - assessment including spinal balance evaluation.

BACKGROUND: Scoliosis exceeding 100 degrees remains an important problem in spinal orthopaedics. The choice of an optimal surgical technique is crucial, not only because of the degree of correction needed, but also because of the need to minimize the number of complications and avoid imbalance after surgery. The aim of this work is to analyse the outcomes of a two-stage surgical regimen for scoliosis exceeding 100 degrees consisting of anterior release, cranio-femoral traction, and posterior fusion with derotational instrumentation. MATERIAL AND METHODS: Twelve patients with thoracic scoliosi

Surgical Outcomes of Double Compression and Complete Fixation Bar System in Pectus Excavatum.

BACKGROUND: Minimally invasive repair of pectus excavatum is a widely used technique for correction of pectus excavatum. Yet despite the advancement in the surgical techniques, it is still associated with various complications, including bar displacement leading to reoperation. To overcome this problem, we developed the double compression and complete fixation bar (DCCF) system that consists of 2 metal bars that are inserted above and below the sternum and compressed to correct pectus excavatum. METHODS: Patients who underwent pectus excavatum correction surgery at this center between April 2

Risk factors of infection of implanted device after the Nuss procedure.

PURPOSE: The Nuss procedure is a minimally invasive procedure for the correction of pectus excavatum. It involves insertion of a substernal metal bar. A feared complication of any implantation procedure is infection, which often requires removal of the implanted device. This report describes the authors' experience with infectious complications after the Nuss procedure. METHODS: The study included 195 patients diagnosed with pectus excavatum. We performed the Nuss procedure under thoracoscopic control on all the patients. Factors analyzed for all patients included bar infection, sex, age, num

Necrotizing fasciitis of the chest wall.

Necrotizing fasciitis is a highly morbid soft tissue infection that rarely involves the upper torso. An extremely unusual case of necrotizing fasciitis of the chest wall is reported, including the method by which an open thoracic cage was managed. This represents the second reported case of a patient surviving necrotizing fasciitis of the chest wall requiring rib resection and chest wall reconstruction.

Final Fusion After Growing-Rod Treatment for Early Onset Scoliosis: Is It Really Final?

BACKGROUND: Final fusion is thought to be the end point for patients with early onset scoliosis following treatment with the use of growing rods. But is it? The purpose of this study was to determine the incidence and cause of any reoperation after final fusion. METHODS: A multicenter database of patients with early onset scoliosis was retrospectively analyzed to identify patients treated with growing rods with a minimum of 2 years of follow-up after final fusion. All reoperations were recorded. Reoperation was defined as a return to the operating room for any complication related to the fina

Double-curve synchronous derotation with convex correction: a new corrective technique for adolescent idiopathic scoliosis with double curves.

STUDY DESIGN: Report of a new technique. OBJECTIVE: We describe a new technique of double-curve synchronous derotation with convex correction for double-major adolescent idiopathic scoliosis (AIS) and is highlighted through illustrative cases. SUMMARY OF BACKGROUND DATA: Rotational deformity is a significant component of scoliosis, which causes a secondary rib hump, and adversely affects patients' psychological and social well-being, and long-term functional results. For AIS, direct vertebral rotation is confirmed as a better and effective method for apical rotational correction and rib hump

Surgical Treatment Strategies for Severe and Neglected Spinal Deformities in Children and Adolescents without the Use of Radical Three-Column Osteotomies.

Background: Severe spinal deformity manifests as a pronounced deviation from the normal curvature of the spine in the frontal, sagittal, and horizontal planes, where the coronal plane curvature exceeds 90 degrees and may coincide with hyperkyphotic deformity. The most severe deformities exhibit rigidity, with flexibility below 30%. If left untreated or improperly treated, these deformities can result in serious complications associated with progression of the curvature. A combination of surgical techniques is frequently employed to attain optimal outcomes and minimize the risk of complications

Do health beliefs affect pain perception after pectus excavatum repair?

PURPOSE: The pain experience is highly variable among patients. Psychological mindsets, in which individuals view a particular characteristic as either fixed or changeable, have been demonstrated to influence people's actions and perceptions in a variety of settings including school, sports, and interpersonal. The purpose of this study was to determine if health mindsets influence the pain scores and immediate outcomes of post-operative surgical patients. METHODS: As part of a multi-institutional, prospective, randomized clinical trial involving patients undergoing a minimally invasive pectus

Elevation of serum lactate dehydrogenase in patients with pectus excavatum.

INTRODUCTION: Pectus excavatum is the most common congenital chest wall deformity and the depression of the anterior chest wall, which compresses the internal organs. The aim of the present study is to investigate the effects of pectus excavatum on blood laboratory findings. MATERIAL AND METHODS: From March 2011 to December 2011, 71 patients with pectus excavatum who visited Seoul Saint Mary Hospital for Nuss procedure were reviewed and analyzed. The blood samples were routinely taken at the day before surgery and pectus bar removal was usually performed in 2 to 3 years after Nuss procedure.

Congenital scoliosis with associated rib fusions: nursing care of patients following VEPTR insertion.

Congenital scoliosis with associated rib fusions, flail chest syndrome and hypoplastic thorax may create a progressive 3-dimensional deformity of the thorax, thereby inhibiting normal lung growth and respiratory function leading to respiratory insufficiency. The recently coined term for this condition is thoracic insufficiency syndrome (TIS). Traditional spine-based surgical procedures to correct congenital scoliosis with rib fusions inhibit the growth of the spine further, potentially worsening TIS and leading to respiratory problems during young adulthood. The dynamic interaction between the

Costochondral changes in the chest wall after the Nuss procedure: ultrasonographic findings.

PURPOSE: The Nuss procedure is one of the most popular surgeries for correcting pectus excavatum. However, little is known regarding stress and strain on the deformed ribs after inserting the pectus bar. We used ultrasonography to investigate costochondral changes before and after the Nuss procedure. METHODS: Ninety-five patients underwent the Nuss procedure between July 2007 and February 2008 at 2 institutions. Chest ultrasonography, specifically of the bones and cartilage, was performed 1 day before and 1 week after the operation. RESULTS: Postoperatively, all patients showed various degre

Muscle Flaps and Thoracomyoplasty as a Re-redo Procedure for Postoperative Empyema.

BACKGROUND: The role of muscle flaps and thoracomyoplasty in the treatment of postoperative empyema is controversial. The major difficulty is given by the sectioning of the muscular masses during the previous thoracotomy/thoracotomies, resulting in a limitation of the volume and mobility of the available neighborhood flaps. MATERIALS AND METHODS: Between January 1, 2004, and January 1, 2012, we used muscle flaps and thoracomyoplasty as a re-redo procedure in seven patients having a history of at least two major procedures performed through thoracotomy (without considering tube thoracostomy an
