Optical coherence tomography: imaging of the choroid and beyond.

Seventy percent of the blood flow to the eye goes to the choroid, a structure that is vitally important to the function of the retina. The in vivo structure of the choroid in health and disease is incompletely visualized with traditional imaging modalities, including indocyanine green angiography, ultrasonography, and spectral domain optical coherence tomography (OCT). Use of new OCT modalities, including enhanced depth imaging OCT, image averaging, and swept-source OCT, have led to increased visualization of the choroidal anatomy. The correlation of these new anatomical findings with other im

High-sensitivity anastigmatic imaging needle for optical coherence tomography.

We present a high-optical-quality imaging needle for optical coherence tomography (OCT) that achieves sensitivity and resolution comparable to conventional free-space OCT sample arms. The side-viewing needle design utilizes total internal reflection from an angle-polished fiber tip, encased in a glass microcapillary. Fusion of the capillary to the fiber provides a robust, optical-quality output window. The needle's focusing optics are based on an astigmatism-free design, which exploits the "focal shift" phenomenon for focused Gaussian beams to achieve equal working distances (WDs) for both axe

Optical coherence tomography detection of neurodegeneration in multiple sclerosis.

The pathophysiology of multiple sclerosis (MS) is typically characterised by inflammation and demyelination leading to neurodegeneration, which is associated with disability and the progressive stages of MS. The visual system is a valuable tool for studying neurodegeneration and potential neuroprotection in the central nervous system due to its ease of accessibility. Optical coherence tomography (OCT) is a non-invasive tool, which can be used to measure the thickness of the retinal nerve fibre layer (RNFL). The thickness of RNFL is reduced following the development of MS and optic neuritis and

Ultrathin side-viewing needle probe for optical coherence tomography.

We present the smallest reported side-viewing needle probe for optical coherence tomography (OCT). Design, fabrication, optical characterization, and initial application of a 30-gauge (outer diameter 0.31 mm) needle probe are demonstrated. Extreme miniaturization is achieved by using a simple all-fiber probe design incorporating an angle-polished and reflection-coated fiber-tip beam deflector. When inserted into biological tissue, aqueous interstitial fluids reduce the probe's inherent astigmatism ratio to 1.8, resulting in a working distance of 300 μm and a depth-of-field of 550 μm with beam

Spontaneous coronary artery dissection evaluated by optical coherence tomography.

A 42-year-old woman underwent coronary angiography for acute coronary syndrome. It showed a progressive narrowing from the middle segment of the left anterior descending artery. Optical coherence tomography (OCT) confirmed the angiographic suspicion of spontaneous coronary dissection. OCT also revealed different patterns of subjacent disease, imperceptible to the angiography: areas of intramural hematoma with blood stasis, progressive compromise of the true lumen, and areas with active flow through the false lumen. It also detected the distal guidewire going out from the true to the false lume

[Clinical forms of macular glaucoma using optical coherence tomography].

Glaucomatous damage of the macular area is varied: it can be sector-based or diffused, possibly with peripheral damage. The author has developed an original manual method for mapping the retinal ganglion layer as well as a 1200μm circular section around the fovea, using a Topcon 3D OCT 1000, which reveals the glaucomatous structural damage of the macula. Several examples show that the most reliable tool for early examination of the condition of the macula is the OCT of the ganglion cell complex at the perifoveolar raised edge. It is much more reliable than the papillary OCT of optical fibers,

Spectral-domain optical coherence tomography in healed ocular toxoplasmosis.

OBJECTIVES: Chronic inflammation and abnormal cholesterol metabolism are involved in the pathogenesis of gallbladder stone disease (GSD) and that of prostate cancer in experimental studies. We assessed the association between GSD and prostate cancer in this population-based study. RESULTS: The cumulative incidence of prostate cancer (log-rank test: P <.001) and the risk of prostate cancer (1.64 vs 1.14 per 10 000 person-y, adjusted hazard ratio [aHR] = 1.30, 95% confidence interval [CI] = 1.22-1.39) were greater in the patients with GSD than in those without GSD. Furthermore, the risk of pros

Laparoscopic optical coherence tomography imaging of human ovarian cancer.

OBJECTIVES: Ovarian cancer is the fourth leading cause of cancer-related death among women in the US largely due to late detection secondary to unreliable symptomology and screening tools without adequate resolution. Optical coherence tomography (OCT) is a recently emerging imaging modality with promise in ovarian cancer diagnostics, providing non-destructive subsurface imaging at imaging depths up to 2 mm with near-histological grade resolution (10-20 microm). In this study, we developed the first ever laparoscopic OCT (LOCT) device, evaluated the safety and feasibility of LOCT, and character

Optical coherence tomography for bile and pancreatic duct imaging.

Optical coherence tomography (OCT) is an optical imaging modality introduced in 1991 that performs high-resolution, cross-sectional, subsurface tomographic imaging of the microstructure in materials and biologic systems by measuring backscattered or backreflected infrared light. OCT has been used for biomedical applications where many factors affect the feasibility and effectiveness of any imaging technique. The highly scattering and absorbing living tissues greatly limit the application of optical imaging modalities. In the last decade, OCT technology has evolved from an experimental laborato

Torpedo maculopathy: in-vivo histology using optical coherence tomography.

PURPOSE: Recent reports of torpedo maculopathy have disclosed underlying irregularities within the chorioretinal tissue that have not been shown to induce disturbances in visual function. Optical coherence tomography (OCT) provides a unique means of assessing the chorioretinal organization and integrity of torpedo lesions in an in vivo setting that has yet to be explored. In agreement with reported cases, torpedo maculopathy appears to be a benign, non-progressive condition that subtends the horizontal raphe in the juxtafoveal region and imparts no disturbance in visual function. Diagnosis of oral submucous fibrosis with optical coherence tomography.

The epithelium (EP) thickness and the standard deviation (SD) of A-mode scan intensity in the laminar propria (LP) layer are used as effective indicators for the diagnosis of oral submucous fibrosis (OSF) based on the noninvasive clinical scanning of a swept-source optical coherence tomography (OCT) system of approximately 6 mum in axial resolution (in tissue) and 103 dB in sensitivity. Compared with the corresponding parameters in healthy oral mucosal mucosa, in OSF mucosa, the EP thickness becomes smaller and the SD of A-mode scan intensity in the LP layer (LP SD) also becomes smaller. [Morphologic classification of Reinke's edema through optical coherence tomography].

OBJECTIVE: Several classification systems for Reinke's edema have been proposed in the past, which are somewhat less than morphologically ideal. The objective of the present study is to evaluate the ability of optical coherence tomography (OCT) to attain a reproducible graduation of Reinke's edema. MATERIAL AND METHODS: In a prospective study, 30 consecutive patients underwent endolaryngeal, microsurgical resection of their Reinke's edema. Graduation was performed through OCT based on morphologic criteria, where each result was compared with that of other classification systems. RESULTS: In
