Endoscopic vocal fold lateralization using the lichtenberger needle holder in neonates and infants with bilateral vocal fold paralysis: A single-center case series.

BACKGROUND: Bilateral vocal fold paralysis (BVFP) is a rare but potentially life-threatening cause of respiratory distress in newborns and infants. Although tracheotomy has traditionally been the standard treatment for severe airway obstruction, less invasive surgical alternatives have been developed to avoid associated morbidity. Endoscopic vocal fold lateralization (EVFLA) represents one such approach. OBJECTIVE: To evaluate the safety and efficacy of EVFLA using the Lichtenberger needle holder as an alternative to tracheotomy in neonates and infants with respiratory distress caused by BVFP

[Suture lateralisation of the vocal fold in a patient with bilateral vocal cord paralysis].

The treatment of bilateral recurrent laryngeal paralysis is usually tracheotomy or various forms of irreversible surgery with removal of parts of the larynx. Since recurrent laryngeal paralysis is reversible in 33% of cases, tracheotomy is usually the treatment of choice. Here, in this case, we present an alternative reversible solution in a patient where a tracheostomy tube was a suboptimal solution.

Modified slide tracheoplasty technique in the management of a long-segment tracheoesophageal fistula.

Post-tracheotomy tracheoesophageal fistula remains a challenge because it frequently involves a considerable length of the trachea. We report a case in which a modified slide tracheoplasty was successfully used to repair a long-segment, subglottic post-tracheotomy tracheoesophageal fistula. This technique not only shortened the length of the resected tracheal segment but also maintained the patency of the tracheal lumen.

A standardized decannulation protocol improves outcomes in neurorehabilitation of critically ill patients: a quasi-experimental pre-post design.

PURPOSE: Clinical practice guidelines recommend tracheotomy decannulation protocols for neurorehabilitation of critically ill patients. However, evidence supporting their effectiveness remains limited. This study aimed to analyze the effect of a standardized decannulation protocol on decannulation rates in tracheotomized patients undergoing neurorehabilitation. METHODS: Participants were recruited from inpatients in the rehabilitation units of a university-affiliated hospital in Sichuan, China. Tracheotomized patients undergoing neurorehabilitation were assigned to the observation and control

Perinatal management of unanticipated congenital laryngeal atresia.

Unless ventilation is achieved within minutes of delivery, patients with congenital laryngeal atresia will not survive. There are 2 settings in which survival is more likely: a tracheotomy may be immediately performed in the delivery room, or a communication may exist between the airway and the pharynx, allowing for air exchange. In the latter case, there are no characteristic findings on prenatal sonography to suggest the diagnosis and to ensure that preparations for immediate tracheotomy are made. We describe a neonate with unanticipated laryngeal atresia and a high tracheoesophageal fistula

Selection of pediatric patients for use of the Passy-Muir valve for speech production.

A potential side effect of tracheotomy in the pediatric population is poor speech development. It has been well documented that children with tracheotomies have delays in expressive and receptive language out of proportion to the child's degree of intellectual functioning. While numerous methods of augmentative devices have been proposed, none are ideal for the child with a tracheotomy. Of the choices available, the Passy-Muir valve is best suited for use in the pediatric population. We present a method of selection of patients for Passy-Muir valve placement currently in use at Blythedale Chil

Continuous intra-arterial nimodipine infusion as rescue treatment of severe refractory cerebral vasospasm after aneurysmal subarachnoid hemorrhage.

Severe refractory cerebral vasospasm (CV) is a major cause of disability and death in patients with aneurysmal subarachnoid hemorrhage (SAH). One rescue therapy in selected patients is intra-arterial nimodipine, either given as a single shot or as continuous infusion. To evaluate treatment efficacy, we analyzed outcome factors such as the incidence of craniectomy, ventriculo-peritonial (VP) shunting, and tracheotomy after intra-arterial nimodipine infusion. We retrospectively analyzed the rates of cerebral infarction, decompressive craniectomy, VP shunting, and tracheotomy in patients with sev

Outcome of endoscopic management of extensive laryngotracheal stenosis.

OBJECTIVES: When performing endotracheal intubation or tracheotomy in unconscious patients in emergent situations, one should consider the possibility of the later complication of laryngotracheal stenosis, which can result in difficulties with decannulation. We analyzed the clinical features of laryngotracheal stenosis to search for its possible etiologic factors and its proper preventive methods. METHODS: The medical records of 249 cases of laryngotracheal stenosis out of 2,208 patients who underwent tracheotomy in our hospital during the past 12 years were retrospectively reviewed regarding

Serial dilation compared to elective laryngo-tracheoplasty in the treatment of acquired subglottic stenosis in children.

Acquired incomplete subglottic stenosis in children has historically been managed by serial bouginage of the strictured region sometimes accompanied by injection or systemic administration of corticosteroid medications. The forceful stretching of the scar whilst awaiting growth of the region to compensate for scar contracture, however, dooms that child to a prolonged period of waiting for an unpredictable result with a tracheotomy in place. Recently, laryngo-tracheoplasty has been undertaken in younger age groups and in a review of 30 children, with very similar types of stenosis, it was noted

Tracheostomy-Assisted Foreign Body Removal: Insights From a Case Series and Literature Review.

BACKGROUND: Foreign body aspiration into the trachea is a common occurrence in children. While most foreign bodies in the airways can be removed using rigid tracheoscopy, there are instances where tracheostomy or tracheotomy is needed. This article presents a series of cases in which tracheostomy was necessary to remove foreign bodies, and summarizes the relevant experiences and lessons learned. METHODS: We reviewed a total of 7 cases in which tracheostomy and rigid tracheoscopy were combined for foreign body removal out of a larger cohort of 1559 cases at Shenzhen Children's Hospital in Guan
