Surgical Technique for Revision of the Distally Migrated Fassier-Duval Femoral Rod in Osteogenesis Imperfecta: A Case Report.

Background/Objectives: Managing long bone fractures and deformities in osteogenesis imperfecta (OI) with telescoping rods is a common but challenging procedure. A rare complication is the distal migration of the rod's proximal female component, which complicates standard revision surgery. This article aims to describe a surgical technique for the revision of a distally migrated Fassier-Duval (FD) femoral rod. Methods: We present the case of an 8-year-old girl with OI type IV who experienced distal migration of her right femoral FD rod-the surgical technique involved extracting the rod retrogra

The longest documented survival after cranial trepanation in the Renaissance: The thirteen years' exceptional survival in Vespasiano Gonzaga Colonna.

This essay focuses on the Vespasiano Gonzaga Colonna (1531-1591), a remarkable example of long-term life following cranial trepanation during the Renaissance. Gonzaga suffered from syphilis and frequent migraines; therefore, in 1578, his court surgeon used a scraping procedure to perform cranial trepanation. The right parietal bone had obvious indications of healing, and osteological data from his tomb validated the surgery. One of the longest reported survivors following cranial trepanation in the Italian archaeological record, Gonzaga, lived for 13 years after the treatment. This instance ad

Dual endoscopic approach of intradiploic epidermoid cyst of the maxilla.

Intradiploic epidermoid cysts are relatively rare tumors that arise between the 2 tables of cranial bones. We report a very rare case of intradiploic epidermoid cyst of the maxilla, which was managed through dual endoscopic approach. We recommend the dual endoscopic approach, maxillary anterior wall trephination, and middle meatal antrostomy for removal of intradiploic epidermoid cyst of the maxilla. The endoscopic approach through maxillary anterior wall trephination and middle meatal antrostomy allows for good visualization, removal of the intradiploic epidermoid cyst, prompt spontaneous dra

[Chinese expert consensus on femtosecond laser-assisted keratoplasty (2022)].

Corneal transplantation, which can be divided into therapeutic keratoplasty and optical keratoplasty, is the main method to restore vision for patients with corneal blindness. The keys to optical keratoplasty are the regular excision and preparation of donor and recipient corneas, as well as well-developed suturing skills to reduce surgically induced astigmatism. Traditional keratoplasty mainly relies on trephination to prepare the corneal graft and the recipient bed, but this procedure lacks precision, which is an important factor affecting postoperative vision recovery. In this regard, femto

Femtosecond laser-assisted repeat keratoplasty: a case series.

PURPOSE: To evaluate the feasibility of femtosecond laser-assisted repeat penetrating keratoplasty (PK) in eyes having undergone corneal graft, thus presenting intracorneal scar tissue. METHOD: Repeat PK was performed with femtosecond laser (Intralase FS TM System; Abbott Medical Optics, Inc, Santa Ana, CA) creating a "top-hat" or "mushroom" configuration. RESULTS: Four patients were included in this study. Indications for repeat PK included recurrent keratoconus (n = 2), endothelial graft failure (n = 1) after conventional PK, and low visual acuity after lamellar keratoplasty (n = 1). Eyes

Healing results of prolonged acute frontal sinusitis treated with endoscopic sinus surgery.

In a marked part of patients suffering from acute frontal sinusitis, the symptoms do not resolve after initial therapy (antibiotics and/or trephination). The prolongation of the healing or recurrences may be caused by persistent inflammation or by structural abnormalities at the nasofrontal connection. During the last decade endoscopic surgery (FES) has become a very useful method to restore the frontonasal drainage although long term results of this kind of population are rare. We evaluated the results of endoscopic surgery in two patient groups (total number of 50) both operated after prolon

Reversible monoparesis following decompressive hemicraniectomy for traumatic brain injury.

OBJECT: The "syndrome of the trephined" is an uncommon and poorly understood disorder of delayed neurological deficit following craniectomy. From the authors' extensive experience with decompressive hemicraniectomy for traumatic brain injury (TBI), they have encountered a number of patients who developed delayed motor deficits, also called "motor trephine syndrome," and reversal of the weakness following cranioplasty repair. The authors set out to study motor function systematically in this patient population to define the incidence, contributing factors, and outcome of patients with motor tre

The surgical treatment of chondromalacia of the patella.

In a retrospective review of 184 knees treated for surgically diagnosed chondromalacia of the patella, the average follow-up period was 3.9 years. The treatment rendered included trephine and drilling in 71 knees, facetectomy in 57 knees, shaving in 35 knees, and a combination procedure in 21 knees. Knees were graded as 2, 3, or 4, depending on the depth and severity of the lesion. The best results were in Grades 2 and 3 disease treated by trephine and drilling or facetectomy. Six patients required further patellar surgery during the follow-up period. The purpose of surgical treatment of the p

Surgical Technique: Core Decompression of Femoral Head Osteonecrosis with Bone Marrow Aspirate Concentrate.

Osteonecrosis of the femoral head is a progressive and potentially debilitating disorder that is responsive to core decompression, especially in early-stage disease. This is typically accomplished through use of an 8 to 10mm trephine or multiple, small-diameter percutaneous drilling. Use of the large diameter trephine is associated with risk of fracture and may not allow healing across large gaps. Here, we present a technique for core decompression using percutaneous drilling that allows bone marrow aspiration concentrate to be introduced. We used the aspirate needle to decompress the femoral

Immunohistochemistry in bone marrow pathology: a useful adjunct for morphologic diagnosis.

Pathomorphological examination of trephine biopsies of the bone marrow (BM) represents a standard method for the diagnosis and staging of hematologic neoplasms and other disorders involving the BM. The increasing knowledge about the genetic basis and biology of hematologic neoplasms, as well as the recently proposed WHO classification system, provide the framework for an accurate diagnosis. Although conventional morphology remains the gold standard for paraffin-embedded BM trephines, immunohistochemical stainings have become an integral part of the diagnostic workup. A simplified approach for the reduction of specific closed anterior table frontal sinus fractures.

We describe a simplified approach to the management of closed anterior table fractures of the frontal sinus. Forty-seven patients were diagnosed with this type of fracture during a 3-year period. Of these, 6 had minimally-invasive periosteal raising of the anterior table through a small perieyebrow skin incision, and access to the fracture was through a small trephine or the edge of the fracture itself. We compared this with the more traditional approach of a coronal flap, which was associated with an increased duration of stay in hospital and the potential for neurological complications. Surg

Diagnosis and Treatment of a Swainson's Toucan (Ramphastos ambiguus swainsonii) With Rhinosinusitis.

A female Swainson's toucan (Ramphastos ambiguus swainsonii) was presented for examination with a 2-year history of mucoid nasal discharge and abnormal growth of the rhamphotheca. Nasal cytologic examination and culture results were consistent with mixed aerobic and anaerobic bacterial rhinitis. Radiographic and computed tomographic imaging demonstrated a deviated septum of the nasal diverticulum and multiple soft tissue densities in the caudal aspects of the maxillary and mandibular infraorbital diverticula of the infraorbital sinus. Results of rhinoscopy and biopsy confirmed bacterial rhiniti

Cortical bridging venous segment protruding into the inner skull depression: a potential site of hemorrhage at external ventriculostomy.

PURPOSE: The cortical bridging venous segment protruding into the inner skull depression (CBVISD), a distinctive structure found in the calvarial convexity, may be a potential risk factor for hemorrhagic complications resulting from trephination. However, this subject has not been documented. In the present study, we explore the CBVISD using magnetic resonance imaging (MRI). METHODS: A total of 94 patients underwent contrast-enhanced MRI. Distributions of the CBVISDs were recorded in two symmetrical regions of the frontal calvarial convexity. The lateral dimensions on both hemispheres were de

Choroidal hemorrhage, preplaced sutures, and host-over-donor keratoplasty.

An old and currently seldom used technique, true preplaced sutures, is described in a case that was high risk for choroidal hemorrhage (CH). A penetrating keratoplasty technique, termed graft-over-host technique, was used allowing the surgeon to expeditiously close the eye in the event of CH. After partial-thickness trephination, true preplaced sutures through the corneal host tissue were passed. After complete trephination, the corneal donor graft was placed under the host and sutured in a controlled fashion. The preplaced sutures had been drawn immediately, giving a broad secure cover, allow

The Eyebrow Approach for the Management of Pediatric Frontal Epidural Abscesses Secondary to Diffuse Sinusitis.

BACKGROUND: Minimally invasive approaches to the anterior cranial fossa have evolved over the past few decades. The management of frontal epidural abscesses (EDAs) secondary to diffuse sinusitis in the pediatric population using minimally invasive techniques is scarcely reported in the literature. Herein, we report the utilization of a minimally invasive eyebrow approach for multidisciplinary concurrent evacuation of frontal EDA secondary to diffuse sinusitis and trephination of the frontal sinus in three pediatric patients. CASE REPORTS: Three pediatric patients presented to the emergency ro
