Lower urinary tract dysfunction in patients with brain lesions.

Stroke and brain tumor are well-known brain diseases. The incidence of lower urinary tract dysfunction (LUTD) in these patients ranges from 14% to 53%, mostly overactive bladder (OAB), and is higher when the frontal cortex is involved. This presumably reflects damage at the prefrontal cortex, cingulate cortex, and other areas that regulate (mainly inhibit) the micturition reflex. White-matter disease (WMD) is a chronic, bilateral form of cerebrovascular disease, leading to a high prevalence of OAB (up to 90%). Since WMD is particularly common in the elderly, WMD may be one of the anatomic subs

The constant flow ventricular infusion test: a simple and useful study in the diagnosis of third ventriculostomy failure.

OBJECT: The evaluation of third ventriculostomy function in hydrocephalic patients is challenging. The utility of the constant flow infusion test in predicting response to shunt insertion in normal-pressure hydrocephalus, as well as in identifying shunt malfunction, has been previously demonstrated. The object of this study was to evaluate its usefulness in determining whether a revision CSF diversion procedure was indicated in patients presenting with recurring symptoms and persisting ventriculomegaly after endoscopic third ventriculostomy (ETV). METHODS: The authors conducted a prospective

Initial CSF total tau correlates with 1-year outcome in patients with traumatic brain injury.

OBJECTIVE: We investigated if tau, microtubular binding protein, in serum and ventricular CSF (vCSF) in patients with severe traumatic brain injury (TBI) during the initial posttraumatic days correlated to 1-year outcome. METHODS: Patients with severe TBI (n = 39, Glasgow Coma Scale score <or=8) were included. We measured serum and vCSF total tau on days 0 to 14, using ELISA. vCSF total tau correlated to 1-year Extended Glasgow Outcome Scale (GOSE), the NIH Stroke Scale (NIHSS) neurologic status, and the Bartel Daily Living Index. Patients (n = 20) with normal pressure hydrocephalus (NPH) ser

Third Ventricle Diameter Is Inversely Related to Thalamic Massa Intermedia Thickness in Hydrocephalus Caused by Congenital Aqueductal Stenosis.

BACKGROUND AND PURPOSE: In fetuses with lateral ventriculomegaly and normal posterior fossa cerebrospinal spaces, third ventricle distention is a compelling clue that supports a diagnosis of aqueductal stenosis. However, this association assumes normal ventricular anatomy. Structural constraints can impair pressure-induced compliance. We aimed to determine how thalamic massa intermedia size alterations may impact the size of the third ventricle in the setting of congenital aqueductal stenosis. MATERIALS AND METHODS: This retrospective study was performed at a single academic pediatric hospita

White matter hyperintensity mediating gait disorders in iNPH patients via neurofilament light chain.

PURPOSE: This study aimed to analyze the differences in regional white matter hyperintensities (WMH) volume and cerebrospinal fluid biomarker levels between idiopathic normal pressure hydrocephalus (iNPH) patients with or without gait disorder. METHODS: Forty-eight iNPH patients undergoing bypass surgery and 20 normal senile individuals were included. The LST toolkit was used to segment all MRI fluid attenuation inversion images and quantify the WMH volume in each brain region. Cerebrospinal fluid was collected from all individuals and measured for concentrations of Aβ, t-tau, p-tau, and neur

Useful outcome measures in INPH patients evaluation.

INTRODUCTION: Idiopathic normal pressure hydrocephalus (INPH) is a neurological disorder that is potentially reversible and clinically characterized by a specific triad of symptoms, including gait disturbance, cognitive disorders, and urinary incontinence. In INPH assessment, the most commonly used test is the Timed Up and Go test (TUG), but a more comprehensive assessment would be necessary. The first aim of the present study is to verify the sensitivity of a protocol with both clinical and instrumental outcome measures for gait and balance in recognizing INPH patients. The second aim is to v

Computer-assisted system for diagnosing degenerative dementia using cerebral blood flow SPECT and 3D-SSP: a multicenter study.

PURPOSE: Due to increasing numbers of patients with dementia, more physicians who do not specialize in brain nuclear medicine are being asked to interpret SPECT images of cerebral blood flow. We conducted a multicenter study to determine whether a computer-assisted diagnostic system Z-score summation analysis method (ZSAM) using three-dimensional stereotactic surface projections (3D-SSP) can differentiate Alzheimer's disease (AD)/dementia with Lewy bodies (DLB) and non-AD/DLB in institutions using various types of gamma cameras. METHOD: We determined the normal thresholds of Z-sum (summed Z-s

Reduced subarachnoid fluid diffusion in enlarged subarachnoid spaces of infancy.

Background and purpose Enlargement of the subarachnoid spaces in infancy (ESSI) is a common cause of macrocephaly without proven explanation. We have observed subarachnoid diffusion to be decreased in these patients. We aim to quantify the diffusivity of ventricular and subarachnoid cerebrospinal fluid in ESSI patients, to determine if diffusion characteristics deviate from normocephalic infants, and to propose a unique mechanism for ESSI. Materials and methods 227 consecutive brain magnetic resonance exams from different macrocephalic children were retrospectively reviewed after institutional

Dementia following stroke.

Strokes can be due to ischemic or hemorrhagic vascular disorders. Ischemic strokes outnumber hemorrhagic strokes approximately 4:1. Although the mode of presentation and pathophysiology are different in the two conditions, the outcome is really dependent on the extent and location of brain injury. A CT scan helps in this regard and reveals surgically correctable lesions such as a subdural hematoma or normal pressure hydrocephalus. Effective rehabilitation of the stroke patient is dependent on motivation and cognitive ability even more than on remaining motor or sensory function. A team approac

Complementary roles of radionuclide and computed tomographic imaging in evaluating trauma.

For 6 consecutive months all triage acceptable emergency room patients at the St. Francis Medical Center with blunt or penetrating trauma involving the abdomen were included in this review. Computed tomography and radionuclide imaging of the abdomen and urinary tract was performed in addition to the usually indicated diagnostic procedures. In our hospital the Radiology Department is adjacent to the Emergency Department. Close cooperation of the trauma team, the imaging physician and the technologists, has provided a stable, reproducible pattern in the performance and evaluation of nuclear medi

Adult organotypic brain slice cultures recapitulate extracellular matrix remodeling in hemorrhagic stroke.

Haemorrhagic stroke is a devastating condition characterized by vessel rupture and free blood within the brain parenchyma or cerebrospinal fluid (CSF) filled spaces. Across the major subtypes of hemorrhagic stroke (subarachnoid, intracerebral, and intraventricular hemorrhages), the presence of blood in the CSF generates significant tissue damage in the first 72 h after the event, known as early brain injury (EBI). EBI includes neuroinflammation, blood-brain barrier breakdown and dysregulation of extracellular matrix (ECM) dynamics. ECM dysfunction has been shown to trigger fibrosis of the cort

Early Prediction of External Ventricular Drain Weaning Trial Outcome in Patients with SAH Based on ICP Pulse Morphological Similarity: A Single-Center Retrospective Cohort Analysis.

BACKGROUND: External ventricular drain (EVD) weaning trials assess the ability of patients with nontraumatic subarachnoid hemorrhage (SAH) to maintain normal intracranial pressure (ICP) without external drainage. Early weaning outcome prediction may optimize clinical management. Existing studies rely on intermittent ICP measurements or static electronic health record data, failing to capture dynamic ICP information. We hypothesize that similar mean ICP produces similar pulse morphology under homeostatic conditions, and acute ICP dynamics changes alter this pattern. METHODS: We retrospectively

Treatment and outcomes of concomitant parasellar meningiomas and anterior circulation aneurysms: a systematic review and perspective in the endovascular era.

The coexistence of parasellar meningiomas and anterior circulation aneurysms is rare, and their anatomical proximity complicates diagnosis and treatment, making management challenging and risky. This study aims to assess the literature on the treatment strategies, clinical-surgical outcomes, and complications in such complex cases. We searched Pubmed, Embase, Scopus, and Web of Science databases following PRISMA guidelines for studies reporting clinical and/or surgical outcomes related to concomitant parasellar meningiomas and anterior circulation aneurysms. Thirteen case reports and one case

Profiling extracellular vesicles from cerebrospinal fluid for classification of intradural spinal tumors.

Extracellular vesicles (EVs) transport biomolecules that could serve as biomarkers for disease diagnosis and monitoring. The clinical utility of EVs derived from cerebrospinal fluid (CSF) in patients with intradural spinal tumors (IST) has not yet been investigated. Here, we obtained EVs from CSF of adult patients with intraspinal ependymoma (n = 9), meningioma (n = 9), hemangioma (n = 4) and schwannian tumors (n = 7), as well as comparison group ('CG', normal pressure hydrocephalus, n = 7), by ultrafiltration. CSF-EVs were characterized by electron microscopy and nanoparticle tracking analysi

EG-Unet: Edge-Guided cascaded networks for automated frontal brain segmentation in MR images.

Accurate segmentation of frontal lobe areas on magnetic resonance imaging (MRI) can assist in diagnosing and managing idiopathic normal-pressure hydrocephalus. However, frontal lobe segmentation is challenging due to the complexity of the degree and shape of damage and the ambiguity of the boundaries of frontal lobe sites. Therefore, to extract the rich edge information and feature representation of the frontal lobe, this paper designs an edge guidance (EG) module to enhance the representation of edge features. Accordingly, an edge-guided cascade network framework (EG-Net) is proposed to segme

Immunohistochemical visualization of lymphatic vessels in human dura mater: methodological perspectives.

BACKGROUND: Despite greatly renewed interest concerning meningeal lymphatic function over recent years, the lymphatic structures of human dura mater have been less characterized. The available information derives exclusively from autopsy specimens. This study addressed methodological aspects of immunohistochemistry for visualization and characterization of lymphatic vessels in the dura of patients. METHODS: Dura biopsies were obtained from the right frontal region of the patients with idiopathic normal pressure hydrocephalus (iNPH) who underwent shunt surgery as part of treatment. The dura sp

Glymphatic System as a Gateway to Connect Neurodegeneration From Periphery to CNS.

The classic concept of the absence of lymphatic vessels in the central nervous system (CNS), suggesting the immune privilege of the brain in spite of its high metabolic rate, was predominant until recent times. On the other hand, this idea left questioned how cerebral interstitial fluid is cleared of waste products. It was generally thought that clearance depends on cerebrospinal fluid (CSF). Not long ago, an anatomically and functionally discrete paravascular space was revised to provide a pathway for the clearance of molecules drained within the interstitial space. According to this model, C

Quantitative Evaluation of Gait Disturbance on an Instrumented Timed Up-and-go Test.

Although the 3-m timed up-and-go test (TUG) is reliable for evaluating mobility, TUG time is insufficient to evaluate mild gait disturbance; we, therefore aimed to investigate other measurements with instrumented TUG (iTUG) using a free smartphone application. Our inclusion criterion in this study is only that participants can walk without any assistance. This study included three heterogeneous groups; patients who underwent a tap test or shunt surgery, 29 inpatients hospitalized for other reasons, and 87 day-care users. After the tap test, 28 were diagnosed with tap-positive idiopathic normal

Rapid amyloid-β oligomer and protofibril accumulation in traumatic brain injury.

Deposition of amyloid-β (Aβ) is central to Alzheimer's disease (AD) pathogenesis and associated with progressive neurodegeneration in traumatic brain injury (TBI). We analyzed predisposing factors for Aβ deposition including monomeric Aβ40, Aβ42 and Aβ oligomers/protofibrils, Aβ species with pronounced neurotoxic properties, following human TBI. Highly selective ELISAs were used to analyze N-terminally intact and truncated Aβ40 and Aβ42, as well as Aβ oligomers/protofibrils, in human brain tissue, surgically resected from severe TBI patients (n = 12; mean age 49.5 ± 19 years) due to life-threa
