Massive subchorionic thrombohaematoma (Breus' mole) causing postpartum haemorrhage: a case series and management strategy.

BACKGROUND: Breus' mole (massive subchorionic thrombohaematoma, MST) is a rare placental disorder that can cause catastrophic postpartum haemorrhage due to acute rupture of a high-tension haematoma. However, the mechanism of this haemorrhage remains insufficiently understood in clinical practice. CASE PRESENTATION: Case 1: A 31-year-old woman, G4P1, was admitted at 26⁺¹ weeks of gestation for intrauterine foetal death. She had a history of chronic hypertension and obesity. Serial ultrasound revealed progressive placental thickening (39→87→97 mm) with a massive subchorionic haematoma. Her haem

Omphalocele with a patent vitelline duct: intraoperative identification of an uncommon presentation.

BACKGROUND: Omphalocele with a concomitant patent vitelline duct is a rare presentation, with very few cases reported in the literature. Most reported cases present with visible bowel prolapse, meconium drainage or mucosal exposure at birth. Presentations without obvious intestinal herniation or mucosal exposure are extremely uncommon and pose diagnostic challenges. CASE PRESENTATION: A male neonate was born to a G3P2 mother at 38 + 6/7 weeks via spontaneous vaginal delivery after a pregnancy notable for a 20-week prenatal ultrasound concerning for omphalocele. Upon delivery, the omphalocele m

Within- and between-country variation in gestational age thresholds for retinopathy of prematurity screening in Latin America.

BACKGROUND/OBJECTIVES: Most Latin American countries have national retinopathy of prematurity (ROP) screening guidelines, but how consistently these thresholds are applied and whether the metric used affects cross-country comparability is poorly documented. SUBJECTS/METHODS: We analysed data from the SPROP (Sociedad Panamericana de Retinopatía del Prematuro) database (2021-2024), including 333 of 343 registrants from 17 countries who provided gestational age (GA) and/or birth weight (BW) screening cutoffs. We assessed between- and within-country variation, guideline concordance, and relative

Comparison of microdrop administration using a bottle adapter of 2.5% phenylephrine and 0.8% tropicamide with standard drop for ROP examination: a randomized controlled noninferiority trial.

BACKGROUND: Pharmacologic mydriasis is essential for retinopathy of prematurity (ROP) screening; however, conventional ophthalmic drops substantially exceed neonatal conjunctival sac volume, leading to systemic absorption and potential adverse effects. Microdrop administration may reduce drug exposure while preserving adequate pupillary dilation. This study aimed to determine whether microdrop administration of phenylephrine and tropicamide is noninferior to standard drops in achieving adequate mydriasis for ROP screening via a bottle adapter. METHODS: This single-center, randomized, parallel

Recalculation of the late fetal death rate in Mexico, 2013-2023 using ICD-11 criteria applied to national vital statistics.

BACKGROUND: Cross-national comparability of fetal mortality indicators is limited by heterogeneous definitions and denominators. Mexico's official fetal death indicators include all gestational ages and use live births as the denominator, diverging from international standards. We recalculated late fetal death rates using ICD-11 criteria and compared them with published national and international data. METHODS: We conducted a retrospective analysis of national fetal death data from Mexico (2013-2023). Publicly available statistics on fetal deaths and registered live-birth datasets were used.

Challenges in implementing optimal perinatal regionalization: a scoping review.

BACKGROUND: Perinatal regionalization is a healthcare model designed to ensure that high-risk mothers and newborns receive care in facilities equipped with appropriate expertise and technology. While evidence supports its effectiveness in improving outcomes, the implementation of regionalized systems remains inconsistent and vulnerable to a range of clinical, structural, and financial barriers. This scoping review aimed to identify, categorize, and synthesize the reported challenges that hinder the effective functioning of perinatal regionalization systems across diverse settings. METHODS: A

Safety and efficacy of double plastic wrap in reducing insensible water loss in preterm infants in first week of life - a pilot randomized controlled trial from a low-to-middle-income country.

BACKGROUND: Preterm infants experience excessive insensible water loss in postnatal period. Established practices like cling film might not be sufficient alone in reducing this loss. Being expensive, humidified incubators might not be affordable in developing countries. Hence, we tried to explore double plastic wrap (cling film attached to the bassinet wall and occlusive plastic wrap covering the baby) as a low-cost solution of increased insensible water loss in postnatal period. METHODS: In this pilot trial 63 inborn infants of less than 32 weeks of gestation or birth weight less than 1200 g

Meta-analysis of maternal and neonatal outcomes of cannabis use in pregnancy current to March 2024.

IMPORTANCE: Following expansive legalization of cannabis in many parts of the United States, cannabis use in pregnancy has increased several fold. There is a pressing need to understand the maternal and neonatal outcomes associated with this exposure. OBJECTIVE: To quantify the maternal and neonatal outcomes of mothers using cannabis during pregnancy. DATA SOURCES: We searched five databases for all relevant observational studies, from each database's inception until March 1st 2024. STUDY SELECTION: Two reviewers separately screened the studies in duplicate. Our initial search yielded 5184

Anterior abdominal wall agenesis associated with cardiac exstrophy and macrocephaly in a gold mining area of the Democratic Republic of the Congo: management and prognosis in a resource-limited area.

BACKGROUND: Congenital malformations, which are responsible for thousands of neonatal deaths every year, are prevalent in low-resource countries. Severe birth defects are mainly multifactorial but their occurrence may be exacerbated by low socioeconomic conditions and environmental factors, especially in mining areas. CASE PRESENTATION: A male neonate delivered, at term, by caesarean section from a 23-year-old woman presented multiple malformations (cardiac exstrophy, abdominal agenesis) and died 12 h after birth. The (non-consanguineous) father was an artisanal gold miner. No genetic or furth

Determinants of neonatal near miss among newborns admitted to SOS Mother & Child Hospital, Benadir region, Somalia: a case-control study.

BACKGROUND: While the birth of a newborn is often a moment of great joy, it can be overshadowed by life-threatening complications that endanger survival in the early days of life. Neonatal near-miss (NNM) cases are infants who survive severe complications, offer a valuable lens for evaluating the quality of neonatal care. Somalia continues to have one of the highest neonatal mortality rates globally, with about 37 deaths per 1,000 live births, highlighting significant gaps in maternal and child health services. This study aimed to identify the determinants of neonatal near miss among neonates

Fear of child birth and its associated factors among pregnant women in Ethiopia: systematic review and meta-analysis.

BACKGROUND: Fear of childbirth (FOCB) significantly contributes to labor abnormalities, needless caesarean sections, and postpartum psychotic illnesses that occur throughout pregnancy, childbirth, and the postpartum period. This directly or indirectly increases unfavorable maternal and neonatal outcomes. However, there was a paucity of data regarding the scope and contributing factors of birthing fear in Ethiopia. OBJECTIVE: To assess the pooled prevalence of fear of childbirth and its associated factors among pregnant women in Ethiopia. METHODS: This systematic review and meta-analysis adhere

Maternal awareness of newborn danger signs before discharge: a secondary analysis of baseline data from a quasi-experimental study in Uganda.

BACKGROUND: The neonatal period is the most vulnerable time for children under 5 years. Neonatal mortality contributes to almost one-half of all under-5 deaths. Developing one or more newborn danger signs increases the mortality risk in the first 28 days of life. Understanding maternal awareness of newborn danger signs is essential in promoting early newborn care-seeking for better outcomes. In Uganda, there is a paucity of evidence on maternal awareness of newborn danger signs (NDS) immediately after delivery. This study aimed to determine the level and factors associated with maternal awaren

Impact of non-communicable diseases on maternal and perinatal outcomes in a low resource setting.

BACKGROUND: Non-communicable diseases (NCDs) are increasingly prevalent among women of reproductive age and may pose significant risks to maternal and perinatal health. Despite their growing burden, data on their impact in low-resource settings remains limited. This study aimed to investigate the impact of pre-pregnancy NCDs on severe maternal and adverse perinatal outcomes among pregnant women admitted at Mbarara Regional Referral Hospital (MRRH) in southwestern Uganda. METHODS: We prospectively enrolled pregnant women admitted to the maternity ward of MRRH from July 2022 to October 2023. We

Is severe maternal morbidity a risk factor for postpartum hospitalization with mental health or substance use disorder diagnoses? Findings from a retrospective cohort study in Maryland: 2016-2019.

BACKGROUND: Perinatal mental health conditions and substance use are leading causes, often co-occurring, of pregnancy-related and pregnancy-associated deaths in the United States. This study compares odds of hospitalization with a mental health condition or substance use disorder or both during the first year postpartum between patients with and without severe maternal morbidity (SMM) during delivery hospitalization. METHODS: Data are from the Maryland's State Inpatient Database and include patients with a delivery hospitalization during 2016-2018 (n = 197,749). We compare rate of hospitaliza

Association between social relationship of mentors and depressive symptoms in first-time mothers during the transition from pregnancy to 6-months postpartum.

BACKGROUND: First-time motherhood is characterized by high psychosocial distress, which untreated, has serious consequences. Informal social support provided by specially trained mentors may be protective against postpartum depressive symptoms but may vary by women's social relationship with the mentor. The objective of this study was to evaluate the association of types of mentors on women's depressive symptoms between late pregnancy to 6-months postpartum and the characteristics of women associated with mentor type. METHODS: This study was a secondary analysis of data from a community sampl
