Redefining the Paradigm: Advancing Evidence-Based Return-to-Sport Criteria Following Isolated Posterior Cruciate Ligament Reconstruction: A Scoping Review.

INTRODUCTION: Acute posterior cruciate ligament (PCL) injuries occur during high-impact sports including American Football, soccer, and skiing. While many of these injuries can be managed nonsurgically, surgery is necessary in some cases, particularly in cases of instability. Clear and standardized return-to-sport (RTS) criteria during rehabilitation following PCL reconstruction (PCL-R) are lacking. OBJECTIVES: This scoping review aims to identify and analyze existing RTS guidelines for PCL-R and propose improvements on the basis of frameworks from anterior cruciate ligament reconstruction, l

Physical and mental health scores worsen over division I collegiate sports season and decline further following mid-season injury or illness.

BACKGROUND/OBJECTIVES: Collegiate athletes commonly report higher physical function and lower pain interference than the average population. However, limited information is available regarding how patient-reported outcome scores change across a sports season or following a mid-season injury or illness. It was hypothesized that collegiate athletes would report higher scores in Patient-Reported Outcomes Measurement Information System (PROMIS) physical, mental, and social health domains compared to the general population, and mid-season injury/illness would be associated with worsened post-season

Injury and Illness Surveillance at the 2024 Hockey Australia U/18 National Championships.

BACKGROUND: There is a paucity of research examining injury in field hockey athletes compared to other sports such as Australian football and soccer. The injury profile of pre-elite Australian hockey athletes is unknown. PURPOSE: The primary aims of this research were to (1) determine the prevalence of injuries/illnesses at the start of a tournament and following an 8-day tournament; (2) describe injuries/illnesses and injury incidence rates at the 2024 U/18 Hockey Australia Championships (HAC). Secondary aims were to (1) assess whether the drag flicking hockey skill is correlated with injury

Surgically Treated Ischial Tuberosity Avulsion Fractures in Adolescents: Risks and Outcomes of 3 Fixation Constructs.

INTRODUCTION: Avulsion fractures of the ischial tuberosity (AFIT) are uncommon injuries that sometimes require operative fixation with screws, suture anchors, or cortical suspensory buttons. This study reviewed a series of surgically managed AFITs at a single institution and compared outcomes among fixation strategies. We hypothesized there would be no difference in outcomes between fixation types. METHODS: We conducted a retrospective review of all patients treated operatively at a single institution from 2010 to 2022 for AFIT identified by CPT code (27215). We collected patient demographics

Predictors of Anterior Cruciate Ligament Tears in Adolescents and Young Adults.

BACKGROUND: Anterior cruciate ligament (ACL) injury in youth can lead to expensive treatment, lengthy rehabilitation, and long-term impairment. Injuries to the ACL are more common in adolescents who participate in organized sports. PURPOSE: To examine whether there is an association between age, physical activity patterns, pubertal timing (Tanner stage), or body mass index (obesity/overweight status) and the risk of developing an ACL injury in youth. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: Data from the Growing Up Today Study (GUTS), a prospective cohort study of youth th

Effect of fall direction on the lower hip fracture risk in athletes with different loading histories: A finite element modeling study in multiple sideways fall configurations.

Physical loading makes bones stronger through structural adaptation. Finding effective modes of exercise to improve proximal femur strength has the potential to decrease hip fracture risk. Previous proximal femur finite element (FE) modeling studies have indicated that the loading history comprising impact exercises is associated with substantially higher fracture load. However, those results were limited only to one specified fall direction. It remains thus unclear whether exercise-induced higher fracture load depends on the fall direction. To address this, using magnetic resonance images of

High return to sport rate and few re-ruptures at long term in professional footballers after anterior cruciate ligament reconstruction with hamstrings.

PURPOSE: Anterior cruciate ligament (ACL) ruptures are considered high burden injuries in sports with high pivotal activity, especially for professional footballers. A lack of evidence exists about long-term follow up of professional elite athletes who underwent ACL reconstruction. The purpose of the study is to analyze the return to play and the career of professional footballers who underwent ACL reconstruction with hamstrings, to evaluate re-rupture and reoperation at either indexed and contralateral knee, and to assess the long-term clinical subjective outcomes and satisfaction. METHODS:

EPIDEMIOLOGY of NECK INJURIES ACCOMPANYING SPORT CONCUSSIONS in YOUTH OVER a 13-YEAR PERIOD IN a COMMUNITY-BASED HEALTHCARE SYSTEM.

BACKGROUND: The same trauma that produces concussion may also produce neck injury. The signs of concussion and neck injury are similar, and symptoms after acceleration-deceleration trauma to the head-neck complex do not accurately discriminate between them. Research on the epidemiology of neck injury among sport-concussed youth is sparse. PURPOSE: The purpose of this study was to investigate the epidemiology of diagnosed neck injury in non-sport-related concussion (Non-SRC) versus sport-related concussion (SRC) in youth by age, sex, and sport. STUDY DESIGN: Cross-sectional epidemiologic stud

Surgical excision of posttraumatic ossifications at the proximal hamstrings in young athletes: technique and outcomes.

BACKGROUND: Proximal hamstring tears can be associated with chronic disability related to the unusual formation of heterotopic ossifications. The role for operative intervention in these circumstances has not been clearly defined. PURPOSE/HYPOTHESIS: The purpose of this study was to describe the surgical management of young athletes who had chronic disability related to proximal hamstring ossifications after eccentric load injuries. The hypothesis was that after surgical excision of posttraumatic heterotopic ossifications at the proximal hamstrings with concomitant repair of the tendons to th

Four-Compartment Fasciotomy for Chronic Exertional Compartment Syndrome.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is characterized by pain related to exercise that resolves with a period of rest. Along with these symptoms, CECS is commonly diagnosed based on an increase in intercompartmental pressure during exercise. A negative magnetic resonance imaging (MRI) is helpful to rule out commonly presenting symptoms. While fasciotomy remains the gold standard for patients who require more than conservative treatment, sex-specific outcome data require further investigation. INDICATIONS: This case presents a 17-year-old female adolescent with a 2-year h

Midterm outcomes from a 20-year experience treating functional popliteal artery entrapment syndrome in athletes.

BACKGROUND: Functional popliteal artery entrapment syndrome (FPAES) is a rare condition affecting young, active individuals presenting with claudication, swelling, or paresthesias, and often impairing athletic performance. Our standardized approach to this disease process involves testing with arterial duplex/digit plethysmography and provocative computed tomography angiography followed by surgical debulking consisting of popliteal artery release and resection of the anterolateral quadrant of the medial head of the gastrocnemius muscle and four-compartment fasciotomy. We sought to determine ri

First MTP joint instability - Expanding the concept of "Turf-toe" injuries.

BACKGROUND: Although the classic definition of the Turf-toe injury refers to a very specific clinical and pathological picture, we think that the concept can be broadened to encompass a wide variety of traumatic lesions of the first metatarsophalangeal joint (MTP1). These are lesions typically difficult to diagnose, with a large variation of outcomes and a potential to impair patients' functional performance on a definitive and perennial basis. The objective of this study is to present the result obtained by treating 24 cases of traumatic injuries to the MTP1 joint in a midterm follow-up time.

Persistent cough in an adolescent.

Jessica, a 14-year-old girl with a history of asthma, went to her pediatrician's office because of a persistent cough. She had been coughing for at least 3 months with occasional cough-free periods of less than a few days. The cough was nonproductive and was not accompanied by fever, rhinorrhea, or facial or chest pain. Jessica and her mother observed that the cough increased with exercise and typically was not present during sleep. She has used two metered-dose inhalers--albuterol and cromolyn--without any change in the cough pattern. For the past 5 years, Jessica has had mild asthma responsi

Theoretical Study of Reactivity Indices and Rough Potential Energy Curves for the Dissociation of 59 Fullerendiols in the Gas Phase and in Aqueous Solution with an Implicit Solvent Model.

Buckminsterfullerene, C60, has not only a beautiful truncated icosahedral (soccer ball) shape but also simple Hückel calculations that predict a 3-fold degenerate lowest unoccupied molecular orbital, which can accommodate up to six electrons, making it a good electron acceptor. Experiments have confirmed that C60 is a radical sponge, and it is now sold for use in topical cosmetics. Further medical uses require functionalization of C60 to make it soluble, and one of the simplest functionalizations is to make C60(OH)n fullerenols. A previous article [Adv.
