Surgery of the footplate may cause a number of possible complications such as cochlear hearing defects or even complete loss of hearing. This has made it necessary to look for improved techniques. When a laser is used for stapedotomy, the energy transmitted to the cochlea must be reduced to the lowest possible level. We thus investigated the carbon dioxide laser to determine whether it would prove to be more advantageous than the argon laser. Animal experiments showed that CO2 laser irradiation is well tolerated. Tests performed on isolated petrous bone resulted in the development of a new sur

The pathology of congenital hearing loss is as varied as the etiologies. Severe external ear anomalies, such as microtia, and middle ear defects have a significant incidence of accompanying inner ear aplasias, more often of the vestibular than of the cochlear end-organs. Classic inner ear aplasias are discussed, including Michel, Mondini, Alexander and Scheibe. A case illustrating close resemblance between a genetically induced Scheibe malformation and rubella inner ear deformities is present. A variety of ear disorders acquired during intrauterine or neonatal life, including cytomegalic inclu

An oscilloscopic plus a graphic method of recording of the stapedius muscle reflex has revealed peculiar modifications in the latency, threshold, and shape of development of the reflex in cases of brain-stem impairment. Many subjects have been examined who were affected by nucleo-reticular vestibular syndrome, vascular insufficiency, disseminated sclerosis, tumours, etc. and a comparison has also been made in normal subjects and in the same subjects during a temporary impairment of the brain-stem structures following barbiturate administration. The greatest semeiological value is attributed to

INTRODUCTION: Carhart's notch (CN) is a false depression of bone conduction (BC) thresholds at 2-4 kHz initially described in cases of stapes fixation. This study was designed to estimate the incidence and assess the clinical significance of CN in cases of otitis media with effusion (OME) in children.
PATIENTS AND METHODS: Clinical records of 50 patients of OME that showed CN were analysed, retrospectively. First 24 were identified as seen in outpatients and 26 were found out of 100 consecutive cases of OME. The criteria of CN were a minimum 10 dB depression in BC at any frequency 500-4000 Hz

Vestibular atelectasis (VA) is a rare clinical entity characterized by a collapse of the endolymphatic space resulting in vestibular loss with the possible onset of positional and/or sound/pressure-induced vertigo. It could be idiopathic or secondary to other inner-ear diseases including Meniere's disease (MD). A collapse of the membranous labyrinth involving the semicircular canals (SCs) and the utricle represents its distinctive histopathological feature. While specific radiological patterns consistent with VA have been described on contrast-enhanced MRI with delayed acquisitions, an impairm

BACKGROUND: Although clinical otosclerosis is considered a rare disease, it is the common cause of hearing loss in patients with an intact tympanic membrane. Also, the hearing loss is amenable to other non-surgical remedies. However, stapes surgery is currently the preferred treatment modality. This study aimed to assess the short-term hearing results in adults after primary stapedotomy, and find, if any, the effects of some variables on post operative hearing.
MATERIALS AND METHODS: This was a retrospective review of the clinical records of all consecutive patients who had primary stapedotom

BACKGROUND: Currently, the optimum surgical approach for treating adherent otitis media is debatable. The traditional treatment is usually performed by microscopic tympanoplasty combined with temporal myofascial tympanic tube placement. In recent years, the application of whole ear endoscopic surgery in the treatment of middle ear diseases has gradually increased, otoendoscopy has been used in the operation of adhesive otitis media, but its safety and effectiveness are still controversial.
METHODS: This study retrospectively analyzed 17 patients with adhesive otitis media treated by endoscopi

OBJECTIVE: Little is known about the hearing characteristics in patients with congenital round window atresia (CRWA). This study aimed to investigate hearing characteristics in patients with CRWA by comparing them with two relatively common congenital middle ear anomalies: congenital stapedial fixation (CSF) and congenital ossicular discontinuity (COD).
METHODS: Literature searches yielded five patients with surgically confirmed CRWA (seven ears), who were included in the CRWA group, along with one of our patients. Air and bone conduction thresholds; air-bone gap (ABG); and presence and depth

The aim of this study was to evaluate the postoperative consequences of chorda tympani reclining during middle ear surgery for otosclerosis. Electrogustometric taste thresholds were measured at 11 loci on the tongue and the soft palate in 14 patients before surgery, and 8 d, 1 month and (in some cases) 6 months after surgery. A significant increase in thresholds was observed on the ipsilateral side of the tongue after surgery. The extent of the deficit and the recovery time course depended on tongue locus. The tip of the tongue displayed a limited deficit, suggesting bilateral chorda tympani i

CONCLUSIONS: The most likely cause of lesions with an absence of the long process of the incus was congenital origin because many cases were associated with the stapedial anomaly. However, the possibility of a potential inflammation of this lesion could not be completely excluded, especially in cases with only an absence of the long process of the incus.
OBJECTIVE: To clarify whether the absence of the long process of the incus is caused by congenital anomaly or a potential inflammation.
SUBJECTS AND METHODS: A total of 21 patients (5 bilateral cases, 16 unilateral cases) with absence of the

OBJECTIVE: The purpose of this study was to determine whether revision stapedectomy could precipitate sensorineural hearing loss in the contralateral ear.
METHODS: We conducted a retrospective review of 148 patients undergoing primary or revision stapedectomy at an academic, tertiary care facility.
RESULTS: Stapedectomy resulted in significant improvements in mean air conduction thresholds in the ipsilateral ear. Overall, there were no significant changes in either the air or bone conduction values of the contralateral ear. Two patients demonstrated a >20-dB decline in speech reception thres

HYPOTHESIS: Histopathologic examination of temporal bones from patients who had undergone stapedectomy may provide information concerning the causes of both residual and recurrent conductive hearing loss (CHL).
BACKGROUND: Although closure of the air-bone gap to within 10 dB occurs in approximately 90% of primary stapedectomies, a residual CHL occurs in approximately 10% and recurrent CHL may occur in up to 35% of cases. Putative causes of failure of surgery as determined during revision include erosion of the incus, bony regrowth at the oval window, and displacement of the prosthesis. Most r

Fragments of otospongiotic and otosclerotic footplates were investigated by immunohistochemical methods. Antibodies IgG, IgA, IgM were found to be bound to the vascular connective tissue of the resorption lacunae, IgG also to osteocytes. The application of antibodies against mumps, measles and rubella antigens showed the expression of the relevant viral antigens in the large cells of the resorption lacunae, in the vascular connective tissue, and in osteocytes, osteoclasts and chondrocytes, present in or around the otospongiotic areas. In the sclerotic stage only the perivascular connective tis

Hearing results are presented for 627 ears with chronic otitis media that were operated on radically and obliterated (Palva flap) and in which a tympanoplasty was performed. The ears were examined annually for five to 14 years (mean, 8.8 years). The long-term improvement (five to 14 years after the surgery) was the greatest in ears with an intact ossicular chain and in ears with ossicular reconstruction using autograft or homograft ossicles or autogenous cortical bone columellae. As a whole, the early (one year after the operation) improvement in the air-bone gap was 8.0 dB when compared with
