OBJECTIVE: To analyze optimal placement of recording-needle electrodes surrounding the eye and lip for facial nerve monitoring by identifying the maximum compound muscle action potential (CMAP) recorded by electrode pairs of different spatial configurations.
STUDY DESIGN: Prospective clinical trial.
SETTING: Ambulatory surgery at a tertiary care center.
PATIENTS: Thirty adults undergoing chronic ear surgery such as tympanoplasty, mastoidectomy, ossicular chain reconstruction, stapedectomy, and cochlear implantation.

A new prosthesis for malleus-grip stapedectomy is presented: the Clip(R) Piston MVP according to Häusler. The titanium piston is equipped with a self-fixing clip mechanism for automatic fixation of the prosthesis on the proximal malleus handle as well as a ball and socket articulation allowing easy introduction of the piston at an optimal angle into the oval window as well as adjustment of the insertion depth. OBJECTIVE: To determine whether the outcome of stapedotomy changes with experience.
STUDY DESIGN AND SETTING: Retrospective case review of an academic otologic practice of a fellowship-trained otologist that consisted of 50 consecutive patients who underwent primary stapedotomy. The main outcome measures were preoperative and postoperative audiograms for both the operated ear and the opposite ear (control).

OBJECTIVES: To analyze outcome after otosclerosis surgery with stamedeotomy with blood clot sealing.
PATIENTS AND METHODS: Otosclerosis surgery was performed in 150 adult patients between 1997 and 1999 by five surgical teams (70% of the procedures were performed by senior surgeons) and followed for 18 months. Bilophila wadsworthia is an obligately anaerobic, gram-negative rod, which was first described in 1989. The natural habitat of this species seems to be the lower intestinal tract where it was recovered from about 60% of the persons investigated. Eighty-two children with facial palsy aged less than 6 years were examined. Sixty-four cases, excluding patients with congenital and traumatic palsy, were distributed through the ages, but a predominant tendency to symptomatic palsy for cases aged less than 2 years was observed. BACKGROUND: Although clinical otosclerosis is considered a rare disease, it is the common cause of hearing loss in patients with an intact tympanic membrane. The usefulness of endoscopic examination for the diagnosis of idiopathic perilymphatic fistula (IPLF) was investigated. Eight patients presenting with unilateral sensorineural hearing loss and vertigo underwent endoscopic examination by the transtubal or transtympanic approach. Perhaps the most devastating complication in otologic surgery is that of inadvertent injury to the facial nerve. A review of 22 patients who had sustained an iatrogenic facial nerve injury was conducted. Although the most common procedure being performed during the injury was mastoidectomy (55%), a surprising number of patients had injury during tympanoplasty (14%) or during removal of exostoses (14%).
