Severe primary hyperparathyroidism masked by asymptomatic celiac disease.

OBJECTIVE: To present a case that illustrates the importance of considering asymptomatic celiac disease (CD) as a possible underlying cause in some cases of normocalcemic primary hyperparathyroidism. METHODS: We describe the clinical presentation, management, and outcome of a case of severe primary hyperparathyroidism manifesting with normocalcemia, likely attributable to the presence of asymptomatic CD. RESULTS: A 24-year-old woman presented with a 5-year history of generalized weakness, which had progressed until use of a wheelchair became necessary. She also had sustained low-trauma fragi

Gonadotroph adenoma in multiple endocrine neoplasia type 1.

OBJECTIVE: To describe a case of an in vivo follicle-stimulating hormone (FSH)-secreting gonadotroph adenoma in a man with multiple endocrine neoplasia type 1 (MEN 1) syndrome. METHODS: We present a retrospective case description and a discussion of the related literature. RESULTS: A 48-year-old man had progressively deteriorating visual acuity and bitemporal hemianopia, found to be attributable to a macroadenoma encircling both carotid arteries and compressing the optic chiasm. His libido and erectile function had been reduced for 2 years. The serum FSH level was substantially elevated at 3

Diagnosis by serendipity: Cushing syndrome attributable to cortisol-producing adrenal adenoma as the initial manifestation of multiple endocrine neoplasia type 1 due to a rare splicing site MEN1 gene mutation.

OBJECTIVE: To report a case that highlights the potential for Cushing syndrome to be the first manifestation of multiple endocrine neoplasia type 1 (MEN 1) syndrome and to describe the rare underlying genetic mutation and the heterogeneous manifestations of the syndrome within the same family. METHODS: We present a case report including biochemical and radiologic findings, review family data, and discuss the results of genetic analyses. RESULTS: A 16-year-old girl who was not known to have any medical illness and had no known family history of MEN 1 syndrome presented with Cushing syndrome a

Primary hyperparathyroidism and jaw tumor syndrome: a novel mutation of the HRPT2 gene.

OBJECTIVE: To discuss a case of hyperparathyroidism-jaw tumor syndrome (HPT-JT) and its clinical course, as well as describe a new mutation within HRPT2, the gene associated with HPT-JT. METHODS: We describe the clinical course, laboratory data, and diagnostic imaging of a patient with HPT-JT, including the mutation analysis of the HRPT2 gene. A review of the related literature is also presented. RESULTS: A 22-year-old man presented with progressive bone pain and weakness, and investigation revealed a serum calcium level of 17.6 mg/dL, a phosphorus concentration of 1.8 mg/dL, and an intact p

Pheochromocytoma in patients suspected of harboring adrenal metastasis: management and clinical predictors.

OBJECTIVE: To study clinical management of patients with suspected adrenal metastasis and to assess whether there are clinical predictors of pheochromocytoma in this patient population. METHODS: In this retrospective cross-sectional study, we reviewed medical records of patients who had adrenalectomy for adrenal lesions or had adrenal biopsy performed between January 1997 and July 2007 in a large academic hospital. Patients who harbored adrenal masses that were suspected of being metastases were identified on clinical findings. Pathologic diagnosis, demographic data, clinical history, imaging

Prevalence of valvular heart disease in a cohort of patients taking cabergoline for management of hyperprolactinemia.

OBJECTIVE: To determine the prevalence of valvular heart disease in a cohort of patients taking cabergoline for the management of hyperprolactinemia. METHODS: A retrospective review of medical records identified patients with hyperprolactinemia who underwent evaluation at Vanderbilt University Medical Center between January and June 2007. The medical records of those patients who were prescribed cabergoline and who underwent elective echocardiography were reviewed for details pertaining to cardiac valvular abnormalities and cabergoline use. RESULTS: Forty-five patients (mean age, 41 +/- 10 y

Association of low prostate-specific antigen concentration and panhypopituitarism.

OBJECTIVE: To report the association of undetectable or very low prostate-specific antigen (PSA) concentrations with hypopituitarism. METHODS: We present a case series of 4 patients with low or undetectable PSA concentrations and associated panhypopituitarism and summarize the clinical presentation and pertinent laboratory and radiologic findings. We review related literature and discuss possible mechanisms explaining the described association. RESULTS: Four men with PSA values below the second percentile of a healthy population had hypopituitarism. In 3 men, low PSA concentrations were note

Status of bone mineral density in patients selected for cardiac transplantation.

OBJECTIVE: To determine the prevalence and correlates of low bone mineral density (BMD) in ambulatory outpatients with end-stage heart failure who were awaiting cardiac transplantation. METHODS: Fifty-five cardiac transplant candidates with end-stage heart failure were enrolled in this study. Bone mineral density at the lumbar spine and proximal femur was determined by dual-energy x-ray absorptiometry. Laboratory studies included serum alkaline phosphatase, calcium, intact parathyroid hormone, and 25-hydroxyvitamin D. RESULTS: The mean proximal femur and lumbar spine Z scores were 0.3 +/- 1.

OBJECTIVE: To report the case of a patient with a virilizing ovarian Leydig cell tumor and subclinical Cushing syndrome attributable to an adrenal adenoma. METHODS: Detailed clinical, laboratory, radiologic, and pathologic findings are presented, and the pertinent literature is reviewed. RESULTS: A 49-year-old woman was referred for evaluation of a left adrenal mass (3.0 by 2.4 cm), which had been diagnosed by computed tomographic scan 4 years previously during a work-up for hirsutism. On examination, she had central obesity, facial hirsutism, and male pattern baldness. Work-up showed elevat

Amenorrhea, pregnancy, and pregnancy outcomes in women following spinal cord injury: a retrospective cross-sectional study.

OBJECTIVE: To assess the prevalence of amenorrhea and pregnancy as well as pregnancy outcomes following spinal cord injury (SCI) in women. METHODS: In this retrospective cross-sectional study, women with SCI were interviewed regarding demographic data, details about the timing and type of SCI they sustained, whether the neurologic deficit was complete or incomplete, and reproductive history. The study was initiated in September 2001 and lasted 3 years. Measures were compared for significance with the level of injury and extent of neurologic deficit using descriptive statistics, analysis of va

Thyroid autoimmunity and recurrent spontaneous abortion in Iran: a case-control study.

OBJECTIVE: To determine the association of thyroglobulin antibodies (TG-Ab) and thyroid peroxidase antibodies (TPO-Ab) with recurrent spontaneous abortion in a euthyroid, nonpregnant population of women in Iran. METHODS: In this case-control study conducted between November 2003 and September 2006 in Tehran, Iran, nonpregnant women with a history of 3 or more consecutive pregnancy losses and age-matched, healthy parous women without a history of reproductive problems were assessed. Thyroid function tests were performed, which included assessment of thyroid-stimulating hormone, triiodothyronin
