Surgical and medical treatment of hearing loss in mixed otosclerosis.

OBJECTIVES: We assessed the effect of using preoperative sodium fluoride (NaF) on the difficulty of working with the footplate during stapedectomy and its effect on the postsurgical hearing gain in patients with mixed otosclerosis (i.e., otosclerosis and/or otospongiosis). METHODS: There were 2 groups of patients with mixed otosclerosis. The experimental group was made up of patients with an active focus of the disease; they received NaF for 6 months and then underwent stapedectomy. The control group was made up of patients with an inactive focus of the disease; they underwent stapedectomy wi

Hearing loss with stapedotomy and treated otitis media.

OBJECTIVE: To determine if treated Pseudomonas aeruginosa otitis media (PA OM) increases the risk of sensorineural hearing loss (SNHL) with stapedotomy. STUDY DESIGN AND SETTING: PA OM was induced in 28 guinea pigs. Half of the animals underwent unilateral stapedotomy before antibiotic therapy (untreated), and half underwent stapedotomy after treatment (treated). Control animals underwent saline injections followed by stapedotomy. Electrocochleographic thresholds were measured before and after stapedotomy. Temporal bones were analyzed histomorphologically. RESULTS: Less SNHL was seen after s

Effect of cochlear reserve on postoperative outcome in otosclerosis.

Patients with elevated bone conduction (BC) thresholds are not considered a good candidate for otosclerosis surgery. Sometimes, it might be difficult to decide to operate these patients considering relatively poor cochlear function. However, viewpoints may vary among otologists. This study was undertaken to compare hearing outcome following otosclerosis surgery in patients who had bone conduction (BC) thresholds >or= 30 dB, and to investigate whether BC thresholds >30 dB has a negative impact on hearing outcome. Medical records of 111 patients who had undergone otosclerosis surgery were review

Audit of stapedectomy in the north west of England for 1996 and an analysis of the criteria used to describe success.

A questionnaire was used to collect information related to stapedectomy from all the NHS Trusts in the North West region. The data collected included the identity and rank of the surgeon, the age and sex of the patients, the side of the operation, the audiometric values of the air conduction audiogram preoperatively and postoperatively, and an account of any complications. The results were analysed and compared to national and international standards. The criteria for analysis of success in surgery for conductive hearing loss were reviewed.

Prospective study of resident-performed stapedectomy.

Because of the diminishing number of patients diagnosed with otosclerosis, the adequacy of residency training in stapedectomy techniques remains controversial. A prospective study, conducted from 1986 to 1991, assessed whether or not surgical outcome obtained by residents, under close supervision by otologic faculty, could be improved using a single stapedectomy technique. Comparison of pre- and postoperative hearing results (pure-tone averages of 500, 1000, and 2000 Hz) from 49 cases revealed closure of the air-bone gap to within 10 dB in 68 percent of procedures. Complications included failu

Perilymphatic fistula: a New Hampshire experience.

Thirty-five patients with perilymphatic fistulas (PLFs) are presented. Of this group of 35 patients (39 ears), 4 patients did not have fistulas that could be observed with certainty but were presumed to have fistulas by virtue of their excellent response to surgical repair. Our case reports provide examples of the great variety and possible classifications of presentations and symptom complexes that lead one to suspect the diagnosis of perilymphatic fistula. Comments on diagnostic and therapeutic modalities and on postoperative care and counseling are included. The age range of patients in our

Effect of angiotensin II on inflammation pathways in human primary bone cell cultures in otosclerosis.

INTRODUCTION: The aim of this study was to assess the expression and production of inflammation mediators in basal condition and after angiotensin II (AngII) in otosclerosis. MATERIALS AND METHODS: Human stapedial cell cultures (6 otosclerosis and 6 controls) were incubated with AngII (10(-7)M, 24 h) or vehicle. Cytokines and their mRNA expression were assessed by antibody and cDNA arrays. RESULTS: In basal conditions, otosclerotic cultures produced higher amounts of interleukin (IL)-1β and interferon-inducible protein 10, and smaller amounts of tissue inhibitor of metalloproteinase 2. AngII

[Glasgow Benefit Plot for evaluation of stapedectomy result].

In a group of 153 patients, the result of unilateral stapedectomy was evaluated using Glasgow Benefit Plot. Pure tone average at frequencies 0.5; 1; 2; and 3 kHz of operated and non-operated ear were used to distribute patients to pre- and post-operative groups. In 26 (79%) of 33 patients with unilateral hearing loss, bilateral normal hearing was achieved. Thirty one (46%) of 68 patients with asymmetric bilateral hearing loss and 37 (71%) of 52 patients with symmetric bilateral hearing loss had unilateral normal hearing after the operation. Twenty (29%) patients of the group III had bilateral

Experience with stapedectomy in a developing country: a review of 200 cases.

Since stapedectomy was first performed in 1956, many innovations have been described. We retrospectively analyzed 200 cases of stapedectomy that had been performed with standard otologic instruments. Of the 200 patients, 54 (27.0%) had significant sensory deafness associated with footplate fixation. In most cases, the footplate was partially removed (stapedotomy group); in the remainder, the footplate was removed in its entirety or subtotally (stapedectomy group). Of the 200 patients, 163 were available for short-term follow-up. The air-bone gap was closed to within 10 dB in roughly 83% of bot

Computed tomographic image analysis of ears with otosclerosis.

To evaluate the usefulness of quantitative analysis of computed tomographic images in diagnosing stapedial otosclerosis, high-resolution temporal bone computed tomography (CT) of 32 ears with otosclerosis and 19 control ears was used. The images on the section containing the oval window were digitized using a scanner, and the ratio of the mean gray scale value of 5 areas in the optic capsule including the area anterior to the oval window to that of the cortical bone was calculated. The mean value of the ratio of the area anterior to the oval window was significantly lower in the otosclerosis g

Otosclerosis: a measles virus associated inflammatory disease.

The etiology of otosclerosis is still unknown. Immunohistochemical studies have revealed the characteristics of chronic inflammation in the otospongiotic area. Paramyxoviral structures have been identified by electron microscopy and the expression of measles virus antigen has been observed by immunohistochemistry in active otosclerotic tissue. By use of the polymerase chain reaction, measles virus related sequences have been detected in otosclerotic bone tissue but not in control specimens. The aim of our study was i) to detect measles virus genome in affected patients using a more sensitive P

Cochlear and vestibular lesions in capsular otosclerosis as seen in microdissection.

In 24 temporal bones from patients with otosclerosis prepared by the method of microdissection and surface preparations, otosclerotic foci could be clearly seen during removal of the otic capsule. The state of activity of each focus was estimated on the basis of its consistency and vascularity. Small anterior foci constituted the most common form of involvement of the otic capsule. All were judged to be inactive, and none of them appeared to have caused obvious sensorineural degeneration. No cases of "pure cochlear otosclerosis" were seen. Sensorineural degeneration was associated with large a

Tarsal-carpal coalition syndrome: importance of early diagnosis.

Tarsal-carpal coalition syndrome is a progressive condition involving synostosis of the wrist, ankle and digits. We describe a mother and her newborn that have this rare inherited condition where the diagnosis was made only after the baby's birth. The baby's condition was suspected on antenatal scanning, and he was born with reduced range of motion of his digits, elbows and ankles. The mother's condition has progressed to involve a fixed flexion deformity of her bilateral elbows, synostoses of her second to fifth digits and extensive coalition of her tarsal and carpal bones. She has required r
