Bacterial induction of Snail1 contributes to blood-brain barrier disruption.

Bacterial meningitis is a serious infection of the CNS that results when blood-borne bacteria are able to cross the blood-brain barrier (BBB). Group B Streptococcus (GBS) is the leading cause of neonatal meningitis; however, the molecular mechanisms that regulate bacterial BBB disruption and penetration are not well understood. Here, we found that infection of human brain microvascular endothelial cells (hBMECs) with GBS and other meningeal pathogens results in the induction of host transcriptional repressor Snail1, which impedes expression of tight junction genes. Moreover, GBS infection also

Reconsidering the Current Preterm Premature Rupture of Membranes Antibiotic Prophylactic Protocol.

OBJECTIVE: The purpose of our study was to determine whether the current antibiotic regimen for preterm premature rupture of membranes (PPROM) is adequate for covering the current causative agents and sensitivities of chorioamnionitis and early-onset neonatal sepsis. STUDY DESIGN: During a 3-year period, we retrieved the results from placental and amniotic membrane cultures obtained at delivery in cases of maternal fever, chorioamnionitis, and PPROM, and from blood cultures obtained from neonates with early-onset sepsis (EOS) in three participating hospitals. Sensitivity of pathogens to antim

Invasive group B streptococcal infections in a tertiary care hospital between 1998 and 2007 in Japan.

OBJECTIVE: To clarify clinical and microbiological features of invasive group B Streptococcus (GBS) disease in Japan. METHODS: This was a retrospective review for the period 1998-2007 of patients across all age groups in Nishi-Kobe Medical Center. Invasive GBS disease was defined as GBS isolation from a normally sterile site or skin/soft tissues. RESULTS: Six infant and 52 adult cases of invasive infection were identified. Diagnosis was limited to bacteremia and meningitis in infants, but varied widely in adults with skin/soft tissue infections and bacteremia being common. The overall fatali

Infectious diseases among food handlers.

trichiura was the most common infection in the studied group. Continuous follow up and health education of food handlers have led to avoidance of outbreaks of infections among food consumers in the hospital. This study was carried out to find the prevalence and associated factors of communicable diseases especially parasitic infections among food handlers in King Fahd Hospital, Jeddah, Saudi Arabia, for early detection and proper management. This study was conducted as a preemployment examination on 52 new kitchen male workers with a mean age of 26.77+/-5.03 years. They were subjected to compl

The NOD2 receptor does not play a major role in the pathogenesis of Group B Streptococcus in mice.

Group B Streptococcus (GBS) capsular type III is an important agent of life-threatening invasive infections. It has been previously shown that encapsulated GBS is easily internalized by dendritic cells (DCs) and this internalization has an impact on cytokine production. The intracellular receptors or pathways underlying this response are not well understood. In this work, we investigated the role of NOD2 in the pathogenesis of GBS using a mouse model of infection. NOD2(-/-) mice showed similar levels of survival and bacteremia than control mice. Interestingly, ex vivo analysis of total spleen

Cerebellar injury in term infants: clinical characteristics, magnetic resonance imaging findings, and outcome.

Although cerebellar injury in the premature infant is an increasingly recognized form of neonatal brain injury, its structural and functional outcomes remain poorly defined in the term infant. The objective of this cross-sectional study was to characterize the clinical and magnetic resonance imaging features and neurodevelopmental outcome in children, born at term, with cerebellar injury. Over a 5-year period, 20 infants were identified with ischemic (n = 3) or hemorrhagic (n = 17) cerebellar injury. Lesions were small (<1 cm) in 12 cases, and large in 8 cases. Prenatal and intrapartum factors

Maternal and neonatal colonisation of group B streptococcus at Muhimbili National Hospital in Dar es Salaam, Tanzania: prevalence, risk factors and antimicrobial resistance.

BACKGROUND: Group B streptococcus (GBS), which asymptomatically colonises the vaginal and rectal areas of women, is the leading cause of septicemia, meningitis and pneumonia in neonates. In Tanzania no studies have been done on GBS colonisation of pregnant women and neonates. This study was conducted in Dar es Salaam, Tanzania to determine the prevalence of GBS colonisation among pregnant women, the neonatal colonisation rate and the antimicrobial susceptibility, thus providing essential information to formulate a policy for treatment and prevention regarding perinatal GBS diseases. METHODS:

Impact of intrapartum antibiotics on the care and evaluation of the neonate.

BACKGROUND: Management of infants whose mothers receive intrapartum antibiotic prophylaxis (IAP) is controversial. In 1996 consensus guidelines for prevention of neonatal Group B streptococcal disease included an algorithm for management of infants whose mothers received IAP. To assess practices for testing and treatment of infants, we evaluated a population-based sample of deliveries to see whether excessive evaluation and treatment occurs after IAP. METHODS: Medical records for 869 deliveries in Connecticut during 1996 were sampled. IAP was administered in 96 full term deliveries. We exclud

Emergence of group B streptococci in obstetric and perinatal infections.

A retrospective study of obstetric and perinatal illness due to group B streptococci during 1972-4 based on bacteriological referrals from Aberdeen Maternity Hospital and Special Nursery disclosed (1) a wide spectrum of maternal morbidity, particularly associated with amniotomy and a prolonged rupture-delivery interval, and (2) the emergence of the group B streptococcus as a major cause of serious neonatal infection in infants of low birth weight, often in the absence of maternal pyrexia. The group B isolates appeared to show a previously undocumented increased resistance to the aminoglycoside

Group B streptococcus alters properties of vaginal epithelial cells in pregnant women.

BACKGROUND: Group B streptococcus (GBS) infection in pregnancy is a major cause of maternal and neonatal morbidity. An understanding of the mechanisms responsible for GBS persistence in the genital tract, as well as recognition of host defenses employed to combat its presence, are crucial to our efforts to reduce maternal GBS colonization and prevent the acquisition of neonatal infections. However, alterations in vaginal immunity in response to GBS colonization in pregnant women remain incompletely defined. Whether GBS modulates autophagy, a major host defense mechanism and contributor to the

Severe Infections in HIV-Exposed Uninfected Infants Born in a European Country.

BACKGROUND: Several studies indicate that HIV-exposed uninfected (HEU) children have a high infectious morbidity. We previously reported an increased incidence of group B streptococcus (GBS) infections in HEU infants born in Belgium. METHODS: This study was undertaken to evaluate the incidence and risk factors of all cause severe infections in HEU infants born in Belgium between 1985 and 2006, including the pre-antiretroviral (ARV) prophylaxis era (1985 to 1994). The medical charts of 537 HEU infants followed in a single center were reviewed. RESULTS: The incidence rate of severe infections

Group B streptococcus colonization of pregnant women and their children observed on obstetric and neonatal wards of the University Hospital in Krakow, Poland.

The study was arranged to assess the actual rates of colonization of pregnant women and their children with group B streptococcus (GBS) in a Polish university hospital. Resistance of these cocci to macrolides and clindamycin was also tested and routes of transmission of GBS were followed in some cases using molecular typing. Colonization with GBS was checked in 340 pregnant women living in the south-eastern region of Poland (Małopolska) in the years 2004-2006. Women with a complicated pregnancy were more often colonized than those with a normal pregnancy (20.0 % versus 17.2 %). Moreover, women
