Relation of otosclerosis and osteoporosis: A bone mineral density study.

OBJECTIVE: Otosclerosis and osteoporosis are metabolic bone diseases. In this paper, we investigated presence of osteoporosis with bone mineral density test in patients who had surgery for otosclerosis. METHODS: We included 27 patients who had conductive hearing loss and diagnosed with otosclerosis during an exploratory tympanotomy, and 30 healthy controls into this study. Bone mineral densitometry test was used for analysis of osteoporosis. T-scores of the patients and the controls obtained from L1-L4 vertebrae and femur neck were compared. In addition, the relations of duration of the disea

Cerebrospinal fluid otorrhea and recurrent bacterial meningitis in a pediatric case with Mondini dysplasia.

Mondini dysplasia is a congenital malformation of the inner ear, which is characterized by a short and large cochlear canal of 1.5 turn rather than 2.5 turns and an apical region with cystic dilatation. Patients present with congenital deafness, when both cochlea are affected. Unilateral disease may cause recurrent meningitis, otorrhea or rhinorrhea. In this article, we report a three-year-old pediatric case with a history of meningitis and cerebrospinal fluid otorrhea following tympanostomy tube placement for serous otitis media.

Persistent and recurrent conductive deafness following stapedotomy.

BACKGROUND: Stapedotomy produces the best hearing results of any otological operation. However, in a small number of cases the air-bone gap is not successfully closed, or conductive hearing loss recurs. OBJECTIVE: To investigate the proportion of cases in the senior author's series which required revision surgery and to determine the medium to long term success rate of surgery, taking into account the results of revision surgery. METHODS: Review of a series of 233 primary stapedotomy operations and 100 revision procedures carried out by one surgeon over a 20-year period. RESULTS: Closure of

[Vestibular disorders in patients with otosclerosis: prevalence, diagnostic and therapeutic options].

A total of 177 patients with otosclerosis were selected for this study. Vestibular disorders were diagnosed in 40 (22.6%) of them. The patients were allocated to 2 groups comprising 11 and 18 subjects respectively. Patients of group 1 were given surgical treatment during the current hospitalization. Patients of group 2 were treated by conservative therapy following stapedoplasty performed 1 year or more earlier. The study included analysis of vestibular symptoms and evaluation of clinical effectiveness of the combined treatment in the form of therapy with betahistine dihydrochloride (betaserk)

Cochlear implantation in otosclerotic deafness.

The otosclerotic process commonly involves the otic capsule and may cause quite widespread demineralisation which leads to a progressive and often profound bilateral sensorineural hearing loss. In this situation cochlear implantation may be the only effective treatment. This chapter considers the pathology of this hearing loss, the effects of cochlear obliteration on implantation, and the effects of demineralisation of the otic capsule on placement of the electrode and nonauditory stimulation. A study is reported from 4 cochlear implant centres in the UK and the Netherlands of 53 patients with

Stapedotomy with microdrill or carbon dioxide laser: influence on inner ear function.

OBJECTIVES: A prospective randomized audiological analysis of 336 otosclerosis operations was conducted to compare the evolution of bone conduction thresholds after primary stapedotomy with two different techniques to open the footplate: microdrill and carbon dioxide laser stapedotomy. METHODS: To monitor the inner ear function, we compared the preoperative bone conduction thresholds with the postoperative levels at day 2, week 2, week 6, and month 6. Evolution of the bone conduction was compared for the two studied groups (laser versus microdrill). RESULTS: An average bone conduction loss o

Long-term results in otosclerotic patients operated by stapedectomy or stapedotomy.

Stapedectomy and stapedotomy are the techniques currently used in the surgical treatment of otosclerosis. During the period 1977-81, a total of 60 consecutive patients were subjected to surgery for otosclerosis according to the method of Schuknecht (Gelfoam and wire prosthesis). Another 55 consecutive otosclerotic patients were operated on during 1987-91with stapedotomy, according to the method advocated by Fisch (Fisch-type piston). Independent of the surgical technique used, the maximal hearing gain was obtained 1 year after surgery. In the frequency range 0.5-3 kHz, the mean value of the av

Cystic fibrosis and hearing loss.

The mucosal epithelium of the middle ear and Eustachian tube is in direct continuity with the upper respiratory tract. Since the otolaryngological aspects of cystic fibrosis (CF) are dominated by involvement of the paranasal sinuses, it might be assumed that children with CF would be expected to have a higher than usual incidence of middle ear disease. Eighty patients who were afflicted with CF had audiological evaluations, which consisted of hearing threshold levels (250 to 8,000 Hz) and speech and impedance audiometry. We found no greater incidence of a conductive or sensorineural hearing lo

Cochlear implantation of patients with far-advanced otosclerosis.

OBJECTIVE: This current single-subject, repeated-measures study was to describe our experience with 30 patients who had been diagnosed with "far-advanced otosclerosis" and who were included in our program of cochlear implants. We analyzed the history of the patients and their families before implantation, the surgical findings, and the performance over a follow up of 3 years. MATERIAL AND METHODS: All patients met one or more of the after criteria: 1) previous surgical intervention as a treatment of their otosclerosis; 2) signs of pericochlear hypodensities in high resolution computed tomogra

Benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo (BPPV) has received renewed clinical interest. At the Johns Hopkins Otological Vestibular Laboratory, 781 patients were tested on more than one occasion between September 1976 and November 1992. Of these patients, 187 (24%) were found to have positional nystagmus consistent with a diagnosis of BPPV. A retrospective analysis of these patients' records, including vestibular test and eye movement reports, audiograms, questionnaires, and hospital charts was performed. After this review, the nystagmus, initially diagnosed as due to BPPV, was believed to be a man
