Ibuprofen, aspirin and acetaminophen toxicosis and treatment in dogs and cats.

Toxicosis to 3 commonly available analgesics--ibuprofen, aspirin and acetaminophen--occurs in dogs and cats after acute ingestion or repetitive administration of therapeutic or excessive doses. Whereas in acute exposure, where the clinical course of an overdose to all 3 drugs is predictable in relation to the amount ingested, in chronic exposure to therapeutic doses of aspirin and ibuprofen, the development of gastric ulcers and analgesic nephropathy is unpredictable. Ibuprofen is not recommended for prolonged treatment in dogs and cats due to the likelihood of ulcer formation. Although gastri

[A case of rheumatoid lung exacerbated by acetaminophen-induced pneumonitis].

A 57-year-old man was admitted with a high fever, dry cough, severe dyspnea and an interstitial shadow bilaterally on chest roentogenogram. Although his illness was not diagnosed, he was treated with a high dose of methylprednisolone (1 g/day for 3 days) for acute interstitial lung disease. As the 3-day treatment was not effective, high-dose methylprednisolone therapy was repeated. Subsequently, he was treated with prednisolone (60 mg/day), after which his condition improved. After 8 months, the patient caught cold for which he was treated. Subsequently his previous lung disease appeared again

Plasma-kallikrein clearance by the liver of acetaminophen-intoxicated rats.

The liver synthesizes prokallikrein and is the main organ to clear the active enzyme (plasma-kallikrein) from circulation. This clearance, a receptor-mediated endocytosis, is calcium-independent and not affected by the blockade of Kupffer cells. The effects of endothelial cells blockade and of acetaminophen intoxication on the clearance of 10 nM rat plasma-kallikrein (RPK) by the isolated, exsanguinated and perfused rat liver are now reported. Endothelial cells blockade obtained by the addition of large excess (30 uM) of formaldehyde-treated serum albumin to the perfusion fluid does not affect

Acetaminophen and analogs as cosubstrates and inhibitors of prostaglandin H synthase.

Previous studies have shown that acetaminophen (APAP) is converted by prostaglandin H synthase (PGHS) to both one-electron oxidized products and the two-electron oxidized product, N-acetyl-p-benzoquinone imine (NAPQI). The present study further characterizes this reaction and shows that relatively low concentrations (20-200 microM) of APAP stimulate PGHS activity in ram seminal vesicle microsomes, whereas high concentrations (greater than 10 mM) inhibit the conversion of arachidonic acid (AA) to 15-hydroperoxy-9,11-peroxidoprosta-5,13-dienoic acid (PGG2). Stimulatory and inhibitory activities

Childhood fever: correlation of diagnosis with temperature response to acetaminophen.

Many people believe that temperature response to antipyretics in febrile children varies according to diagnosis. To evaluate the validity of this premise, we prospectively studied the temperature response to acetaminophen of febrile children who came to an urban pediatric emergency and walk-in facility. The study group consisted of 1,559 patients between the ages of 8 weeks and 6 years whose temperatures when seen were greater than 38.4 degrees C and who had not received antipyretic treatment within the previous four hours. Acetaminophen (15 mg/kg) was administered to each child and repeat tem

Free radicals of acetaminophen: their subsequent reactions and toxicological significance.

The oxidation of acetaminophen to the corresponding phenoxyl free radical and N-acetyl-p-benzoquinone imine by mammalian peroxidases is discussed. The acetaminophen free radical is very reactive--forming dimers, and, ultimately, melanin-like polymeric products. A model compound, leading to more stable metabolites, can be obtained by introduction of methyl groups next to the oxygen, to produce 3,5-dimethylacetaminophen. The electron spin resonance spectrum of this free radical could be completely analyzed. The phenoxyl radical of the dimethyl analog does not form polymers or bind with nucleophi

Aging effects on acetaminophen toxicity and glutathione status in the mosquito.

Previously we found that a decrease in glutathione (GSH) concentration is a general phenomenon in aging organisms. This suggested our hypothesis that the capacity for detoxification involving GSH will decrease in senescence. Thus the present objectives were to determine the toxicity of acetaminophen (APAP) and its relationship to GSH status in the aging mosquito. Different concentrations of APAP in Aedes Ringer were injected into adult mosquitoes of different ages through the life-span. The results showed that the LD50 values were highest in the young adult, plateaued in the mature, and decrea

Pharmacokinetic study of the enterohepatic circulation of acetaminophen glucuronide in rats.

A pharmacokinetic model of the enterohepatic circulation of acetaminophen glucuronide was investigated in rats with particular attention to a lag time between biliary excretion and reabsorption. The plasma drug data obtained after acetaminophen glucuronide injection into the various sites of the gut confirmed that there is a lag time in the enterohepatic circulation and that the lag time is due to the intestinal transit period of the conjugate to the site of the hydrolysis. The value of the lag time was fairly close to that reported previously in the rat. Based on the result, a compartment mod

Decreased toxicity of the N-methyl analogs of acetaminophen and phenacetin.

The N-methyl analogs of p-hydroxyacetanilide (acetaminophen) and p-ethoxyacetanilide (phenacetin) were prepared and tested for toxicity. N-Methylacetaminophen was found to cause no hepatic necrosis in mice, rats, or hamsters in doses that caused massive hepatic necrosis in the same animals when acetaminophen was administered. Neither acetaminophen nor its N-methylated analog caused methemoglobinemia at these dose levels. Fischer rats that were administered large doses of acetaminophen (900 mg/kg s.c.) sustained necrosis in the proximal renal tubules, whereas N-methylacetaminophen caused no ren

Safety of acetaminophen use in pregnancy: review of existing evidence.

Acetaminophen is the most widely used antipyretic and analgesic medication in pregnancy, but recent concerns have challenged its reproductive safety. Review articles and regulatory advisories have highlighted associations between prenatal acetaminophen exposure and adverse neurodevelopmental outcomes, including autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD); reproductive issues; and endocrine abnormalities. This review examines the biological plausibility, epidemiological evidence, and methodological limitations underlying observed associations. Systematic r

Emergency department diagnosis and management of acetaminophen or salicylate toxicity.

Acetaminophen and salicylate are among the most widely used nonprescription analgesics and are common causes of toxic medication overdose. Diagnostic and management approaches differ depending on whether toxicity results from a single acute ingestion or chronic supratherapeutic use. Although acetaminophen and salicylate toxicity are well studied, controversies remain regarding management. This review presents a systematic approach to the emergency department management of patients with acetaminophen or salicylate toxicity, including discussion of the tools used to assess severity, guide treatm

The potential of curcumin in mitigating acetaminophen-induced liver damage.

Acetaminophen (APAP) is a widely used over-the-counter medication for pain and fever, but its overuse can lead to liver toxicity, hepatocyte apoptosis, and necrosis. Despite therapeutic advances in drug-induced hepatotoxicity, APAP-induced liver damage still poses a medical challenge. Recently, natural products have emerged as potential options for mitigating the effects of APAP hepatotoxicity. Curcumin, a natural compound with antioxidant and anti-inflammatory properties, has shown promising results in drug-induced hepatotoxicity. However, further investigations are needed to assess the clini

The Role of Mechanistic Biomarkers in Understanding Acetaminophen Hepatotoxicity in Humans.

Our understanding of the fundamental molecular mechanisms of acetaminophen (APAP) hepatotoxicity began in 1973 to 1974, when investigators at the US National Institutes of Health published seminal studies demonstrating conversion of APAP to a reactive metabolite that depletes glutathione and binds to proteins in the liver in mice after overdose. Since then, additional groundbreaking experiments have demonstrated critical roles for mitochondrial damage, oxidative stress, nuclear DNA fragmentation, and necrotic cell death as well. Over the years, some investigators have also attempted to transla

Autophagy impairment as a key feature for acetaminophen-induced ototoxicity.

Macroautophagy/autophagy is a highly conserved self-digestion pathway that plays an important role in cytoprotection under stress conditions. Autophagy is involved in hepatotoxicity induced by acetaminophen (APAP) in experimental animals and in humans. APAP also causes ototoxicity. However, the role of autophagy in APAP-induced auditory hair cell damage is unclear. In the present study, we investigated autophagy mechanisms during APAP-induced cell death in a mouse auditory cell line (HEI-OC1) and mouse cochlear explant culture. We found that the expression of LC3-II protein and autophagic stru

Paracetamol (acetaminophen): A familiar drug with an unexplained mechanism of action.

Paracetamol (acetaminophen) is undoubtedly one of the most widely used drugs worldwide. As an over-the-counter medication, paracetamol is the standard and first-line treatment for fever and acute pain and is believed to remain so for many years to come. Despite being in clinical use for over a century, the precise mechanism of action of this familiar drug remains a mystery. The oldest and most prevailing theory on the mechanism of analgesic and antipyretic actions of paracetamol relates to the inhibition of CNS cyclooxygenase (COX) enzyme activities, with conflicting views on the COX isoenzyme

A case of Dressler's syndrome successfully treated with colchicine and acetaminophen.

The incidence of Dressler's syndrome after myocardial infarction (MI) has decreased in the reperfusion therapy era. Although guidelines recommend high-dose aspirin for treatment based on evidence from the pre-percutaneous coronary intervention (pre-PCI) era, bleeding and thrombotic concerns occurred upon aspirin administration after coronary stenting. A 69-year-old man with recent MI was admitted to our hospital. The patient presented with chest pain 1 week before admission.
