Bone-Anchored Hearing Aid Effects on Vestibular Function: A Preliminary Report.

OBJECTIVE: Cochlear receptors are sensitive to vibratory stimuli. Based on this sensibility, bone-anchored hearing aids have been introduced to correct unilateral or bilateral conductive or mixed hearing loss and unilateral deafness. The vestibular system is also sensitive to the vibratory stimulus and this type of response is used in clinics to test its functionality. Being aware of this double separated sensibility, we wondered whether bone vibration, which activates the acoustic receptors of patients with bone conduction aids, can also influence the functionality of the vestibular system.

Sense and Sensibility: A Review of the Behavioral Neuroscience of Tinnitus Sound Therapy and a New Typology.

Tinnitus Sound Therapy is not a single strategy. It consists of many different sound types, targeting many different mechanisms. Therapies that use sound to cover, reduce attention to, or facilitate habituation of tinnitus are among the most common tinnitus treatment paradigms. Recent history has seen a proliferation of sound therapies, but they have each been criticized for having limited empirical support. In this review, Sound Therapy's modern history will be described, and a typology will be introduced and discussed in light of current behavioral neuroscience research. It will be argued th

A functional evaluation of ansa cervicalis nerve transfer for unilateral vocal cord paralysis: future directions for laryngeal reinnervation.

There are a variety of methods for treating unilateral vocal cord paralysis, but to date there are few objective studies that evaluate the functional results of nerve transfer from the ansa cervicalis. Six dogs underwent unilateral recurrent laryngeal nerve section with immediate reanastamosis to the sternothyroid branch of the ansa cervicalis. After 5 to 6 months, measurements of vocal efficiency and acoustic parameters, videolaryngoscopy, videostroboscopy, and evoked electromyography were performed. Identical measurements were made in eight control dogs during normal electrically induced pho

Intraoperative monitoring of facial nerve antidromic potentials during acoustic neuroma surgery.

The present paper presents monopolar recording of facial nerve antidromic potentials as an alternative technique to facial electromyography for the continuous monitoring of the facial nerve during acoustic neuroma surgery. The investigation involved 22 patients undergoing acoustic neuroma surgery via a retrosigmoid approach (tumour sizes ranging from 5 to 28 mm). Bipolar electrical stimulation of the marginalis mandibulae was performed to elicit facial nerve antidromic potentials. Stimulus intensity ranged from 2 to 6 mA with a delivery rate of 7/sec. A silver wire monopolar electrode position

[Brain stimulation for the selective treatment of schizophrenia symptom domains : Non-invasive and invasive concepts].

Given that one third of patients with schizophrenia (SZ) only show limited response to established treatments, alternative therapeutic strategies such as non-invasive/invasive brain stimulation approaches have emerged as an adjunctive treatment option for distinct SZ symptom domains (e.g. acoustic hallucinations, negative/positive symptoms and cognitive impairment). Taking comparative interventional studies and standardized technical parameters into consideration, current meta-analyses indicate that adjunctive electroconvulsive therapy, repetitive transcranial magnetic stimulation and transcra

[Intracranially recorded trigeminal compound action potential: with special reference to trigeminal neuralgia].

To elucidate the origin of the early component of the trigeminal sensory evoked potential (TSEP) we reported in 1985, we recorded evoked potentials on the trigeminal root and the brain stem surface intracranially when suboccipital craniectomy was done for exploration of the C-P angle. Patients consisted of 4 who had idiopathic trigeminal neuralgia with microvascular decompression, 1 with glossopharyngeal neuralgia who also had microvascular decompression of the left glossopharyngeal nerve, and 5 patients with different tumors, specifically, Meckel's cave meningioma, two epidermoid cysts of the

Treatment of tinnitus with electrical stimulation.

The purpose of this study was to evaluate the treatment of tinnitus with electrical stimulation. Five hundred patients with tinnitus were treated with probe electrical stimulation. Causes of tinnitus were sensorineural hearing loss (303 patients), Meniere's disease (88), infection (25), head trauma (39), acoustic trauma (25), ototoxicity (4), and chemotherapy (2). Treatment involved 6 to 10 transcutaneous treatment electrical stimulation sessions biweekly. Fifty-three percent of patients showed decreases in their tinnitus as measured by a subjective rating scale. With a 3-month follow-up, 72%

Bimodal Auditory Electrical Stimulation for the Treatment of Tinnitus: Preclinical and Clinical Studies.

Tinnitus, or the phantom perception of sound, arises from pathological neural activity. Neurophysiological research has shown increased spontaneous firing rates and synchronization along the auditory pathway correlate strongly with behavioral measures of tinnitus. Auditory neurons are plastic, enabling external stimuli to be utilized to elicit long-term changes to spontaneous firing and synchrony. Pathological plasticity can thus be reversed using bimodal auditory plus nonauditory stimulation to reduce tinnitus. This chapter discusses preclinical and clinical evidence for efficacy of bimodal s

Electromyographic facial nerve monitoring during resection for acoustic neurinoma under moderate to profound levels of peripheral neuromuscular blockade.

The facial nerve is monitored intra-operatively using electromyography to identify and prevent damage during the excision of an acoustic neurinoma. In order to determine whether a profound level of peripheral neuromuscular blockade could be achieved without compromising facial electromyographic monitoring, 11 patients undergoing resection of acoustic neurinoma were studied. After induction of anaesthesia, an infusion of atracurium was administered to reduce the integrated electrically evoked muscle potential (EEMP) of the hypothenar eminence by 70-100%. The facial nerve was directly stimulated

Precision Induction and Distinction of Coughing and Sneezing Reflexes in Mice.

Coughing and sneezing, as vital respiratory defensive reflexes, have seen research into their mechanisms constrained by the nonspecific stimulation inherent in traditional models. Existing methods, such as capsaicin or citric acid nebulization, can simultaneously activate coughing (mediated by tracheal C fibers) and sneezing (mediated by nasal trigeminal nerves), leading to confounded behavioral observations. Although previous studies have attempted to distinguish coughing and sneezing by analyzing respiratory patterns or acoustic signatures, these studies lack models that can specifically sti

Intermittent theta burst stimulation over right somatosensory larynx cortex enhances vocal pitch-regulation in nonsingers.

While the significance of auditory cortical regions for the development and maintenance of speech motor coordination is well established, the contribution of somatosensory brain areas to learned vocalizations such as singing is less well understood. To address these mechanisms, we applied intermittent theta burst stimulation (iTBS), a facilitatory repetitive transcranial magnetic stimulation (rTMS) protocol, over right somatosensory larynx cortex (S1) and a nonvocal dorsal S1 control area in participants without singing experience. A pitch-matching singing task was performed before and after i

Cathodic-leading pulses are more effective than anodic-leading pulses in intracortical microstimulation of the auditory cortex.

OBJECTIVE: Intracortical microstimulation (ICMS) is widely used in neuroscientific research. Earlier work from our lab showed the possibility to combine ICMS with neuronal recordings on the same shank of multi-electrode arrays and consequently inside the same cortical column in vivo. The standard stimulus pulse shape for ICMS is a symmetric, biphasic current pulse. Here, we investigated the role of the leading-phase polarity (cathodic- versus anodic-leading) of such single ICMS pulses on the activation of the cortical network. APPROACH: Local field potentials (LFPs) and multi-unit responses w

Monaural Neonatal Deafness Induces Inhibition among Bilateral Auditory Networks under Binaural Activation.

Worldwide, almost 500 million people are hearing impaired, making hearing loss the most common sensory impairment among humans. For people with single-sided deafness (SSD), cochlear implants (CIs) can be enormously beneficial by providing binaural information. However, binaural benefits in CI users have been only incompletely realized. Overcoming these limitations requires a better knowledge of how neuronal circuits adapt to SSD and how unilateral CI stimulation can compensate a deaf ear. We investigated effects of neonatal SSD on auditory brainstem circuitry using acoustic (AS), electric (ES)

Effects of pulsatile electrical stimulation of the round window on central hyperactivity after cochlear trauma in guinea pig.

Partial hearing loss induced by acoustic trauma has been shown in animal models to result in an increased spontaneous firing rate in central auditory structures. This so-called hyperactivity has been suggested to be involved in the generation of tinnitus, a phantom auditory sensation. Although there is no universal cure for tinnitus, electrical stimulation of the cochlea, as achieved by a cochlear implant, can result in significant reduction of the tinnitus percept. However, the mechanism by which this tinnitus suppression occurs is as yet unknown and furthermore cochlear implantation may not

Hemifacial spasm resulting from facial nerve compression near the internal acoustic meatus--case report.

A 61-year-old female presented with a rare case of hemifacial spasm (HFS) resulting from facial nerve compression near the internal acoustic meatus. She underwent a first surgery for microvascular decompression at the root entry zone of the facial nerve, but this did not achieve resolution of the HFS. During the second surgery, the meatal loop of the anterior inferior cerebellar artery (AICA) was found to be the offending artery near the internal acoustic meatus. When the AICA was dissected and separated from the facial nerve, abnormal muscle responses of the mentalis muscle due to electrical

BAEP and autopsy findings in Wallenberg syndrome.

BAEPs were recorded in a 70-year-old man who had developed symptoms indicative of Wallenberg syndrome. He died, of extracerebral causes, shortly after hospitalization and an autopsy study was done on the brainstem lesions. Right ear stimulation disclosed BAEP abnormalities, but gave no indication of the level of the ischemic disorder. Histopathological examination showed a typical right dorsolateral medullary infarct. A detailed study of the acoustic pathways demonstrated their anatomical integrity. These findings confirm the uncertainty that still exists as to the exact location and type of g
