A Paradoxical Increase in Ventricular Arrhythmia After a Left Stellate Ganglion Block: A Case Report.

Ultrasound-guided left-sided or bilateral stellate ganglion block (SGB) has emerged as an effective bedside intervention for malignant ventricular arrhythmias (VA) refractory to conventional therapies, with demonstrated safety and efficacy in multiple studies. We report a medically complex patient with presumed genotype-negative Long QT Syndrome (LQTS) who experienced a sudden, transient increase in arrhythmic events shortly after an uncomplicated left-sided ultrasound-guided SGB. To our knowledge, this is the first documented case of increased VA burden following left-sided SGB, highlighting

Stellate Ganglion Block for Treating Post-COVID Impairment of Taste and Smell.

BACKGROUND: The loss of smell and taste are some of the chronic complications that have impacted and continue to impact patients who suffered a COVID-19 infection. Impairment of smell and taste have significant impact on patients' lives. Current treatment modalities are very limited. CASE REPORT: We present a 40-year-old patient who completely lost smell and taste following a COVID-19 infection. The patient was treated with bilateral stellate ganglion blocks (SGBs) where each side was blocked at separate visits. At the 6-month follow-up, taste and smell both improved by 90%. CONCLUSIONS: SGB

Early stellate ganglion block for improvement of postoperative cerebral blood flow velocity after aneurysmal subarachnoid hemorrhage: results of a pilot randomized controlled trial.

OBJECTIVE: Stellate ganglion block (SGB) is a commonly used sympathetic nerve block technique that may have benefits for patients with aneurysmal subarachnoid hemorrhage (aSAH) in the early stage. Cerebral vasospasm (CVS), one of the most common complications of aSAH, is accompanied by an abnormal increase in cerebral blood flow velocity (CBFV) and neurological dysfunction. In this pilot study the authors sought to determine the feasibility of early SGB for CVS in aSAH patients by observing the incidence of symptomatic CVS. METHODS: Prior to receiving surgical treatment, patients with aSAH we

Fluoroscope-guided celiac plexus block in a pregnant patient: a case report.

A 28-weeks gestation pregnant patient with chronic pacreatitis and severe abdominal pain leading to impaired nutrition is presented. After detailed consideration of the case, fluoroscopic-guided celiac plexus neurolysis was performed. The block was performed using the transaortic approach as it reduces radiation exposure.

[Endoscopic ultrasound-guided coeliac plexus neurolysis to reduce pain in patients with pancreatic cancer].

Pain is among the most common symptoms in patients with pancreatic cancer and up to 80% require analgesics, most often as opioids. Unfortunately the analgesic effect is frequently insufficient, and increasing doses are required, resulting in unpleasant side effects. Endoscopic ultrasound-guided neurolysis is a well established method to alleviate or reduce pain due to pancreatic cancer with a documented effect in 80% of patients. The aim of this review is to draw attention to endoscopic ultrasound-guided neurolysis and to discuss its potential which may not be fully utilized.

Recognizing Horner's syndrome.

Horner's syndrome is a dramatic finding identified by perianesthesia nurses after regional anesthesia. This article describes the relationship between Horner's syndrome and regional anesthesia while explaining the signs and symptoms as they relate to blockade of the sympathetic nervous system.

Critical evaluation of chemical neurolysis of the sympathetic axis for cancer pain.

BACKGROUND: Patients with pain caused by cancer frequently experience visceral pain. In addition to oral pharmacologic therapy to manage pain, neurolytic blocks of the sympathetic axis are also effective in controlling visceral cancer pain. METHODS: Four types of neurolytic blocks (interpleural phenol, celiac plexus, superior hypogastric plexus, and ganglion impar) used in the treatment of visceral cancer pain are reviewed. RESULTS: Several studies have documented the efficacy of neurolytic blocks in reducing pain intensity and opioid consumption. However, the narrow risk-benefit ratio assoc

Thermal grill illusion and complex regional pain syndrome type I (reflex sympathetic dystrophy).

BACKGROUND AND OBJECTIVES: In normal humans, placing a hand on a thermal grill containing warm elements separated by cool ones produces a burning sensation. In this case report, responses to a thermal grill in a patient with neuropathic pain were examined. METHODS: The responses of a 31-year-old woman with complex regional pain syndrome type I (reflex sympathetic dystrophy) to a thermal grill were evaluated before and after stellate ganglion block. RESULTS: The patient experienced a burning sensation when the unaffected hand was placed on the grill and could distinguish which element was war

[Treatment of chronic retinal artery obstruction with stellate ganglion block and electric acupuncture].

A 56-year-old man (160 cm, 64 kg) having right visual field defect due to obstruction of the central retinal artery for 6 months was treated with stellate ganglion block and electric acupuncture. His visual field improved slightly immediately after preliminary treatment with stellate ganglion block and electric acupuncture judging from perimeter recording and his complaints. Stellate ganglion block on every day and electric acupuncture on every other day were performed for 10 days and there after these treatments were done two times a week. Twenty days after starting the treatment, his visual

Algodystrophy (reflex sympathetic dystrophy) complicating unilateral acrodermatitis continua.

A 65-year-old female suffered trauma to her left hand on which acrodermatitis continua (psoriasis of Hallopeau) developed. It is suggested that the persistent pain of this skin condition, its poor response to treatment and her allied mental state led to the development of a severe algodystrophy with marked underlying osteoporosis.

[Effectiveness of sympathetic block using various technics].

Blocking of sympathetic conduction aims at permanent or temporary elimination of those pain pathways conducted by the sympathetic nervous system. In order to provide an objective evaluation of sufficient blocking effect, earlier inquiries referred to parameters such as: (1) observation of clinical signs such as Horner's syndrome, Guttman's sign, anhidrosis, extended venous filling; (2) difference in skin temperature of at least 1.5 degrees C between blocked and unblocked side; (3) increase in amplitude of the pulse wave; and (4) depression of the psychogalvanic reflex (PGR) on the blocked side

Effect of autonomic blockade on tracheobronchial blood flow.

Tracheobronchial blood flow increases two to five times in response to cold and warm dry air hyperventilation in anesthetized tracheostomized dogs. In this series of experiments we have attempted to attenuate this increase by blockade of the autonomic nervous system. Four groups of anesthetized, tracheostomized, open-chest dogs were studied. Group 1 (n = 5) were hyperventilated for 30 min with 1) warm humid [approximately 26 degrees C, 100% relative humidity, (rh)] air followed by bilateral vagotomy, 2) warm humid air, 3) cold (-22 degrees C, 0% rh) dry air, and 4) warm humid air. Groups 2, 3,

[Treatment of frostbite of the extremities by combined axillary plexus and peridural anesthesia: a case report including blood levels of bupivacaine].

To enhance peripheral perfusion after local frostbite, temporary or definitive interruption of the sympathetic activity in the region of injury is useful as an adjunct to systemic treatment. The common catheter techniques of axillary or epidural block provide the advantage of continued sympathicolysis without the risks of serial punctures or irreversible surgical alterations of the autonomic nervous system. bupivacaine into the three catheters (Fig. 2). A very satisfactory clinical result was observed. There was no loss of fingers
