Ethical Issues in Dysphagia Management.

Dysphagia management is complex and requires balancing individuals' preferences, quality of life, and medical consequences. Ethical challenges are not uncommon given the complexity of dysphagia. Professionals must engage in ethical reflection and shared decision-making when managing dysphagia. Recognizing one's own presuppositions and beliefs may be fundamental to ensuring an ethical approach. The goal of this article is to apply principles of ethics using hypothetical case studies of dysphagia. To this end, we will describe the challenges of working with the disorder of dysphagia; the influen

Two infants, same prognosis, different parental preferences.

A central principle of justice is that similar cases should be decided in similar ways. In pediatrics, however, there are cases in which 2 infants have similar diagnoses and prognoses, but their parents request different treatments. In this Ethics Rounds, we present such a situation that occurred in a single NICU. Three physician-ethicists analyze the issues.

The art, science and philosophy of newborn care.

Neonates truly constitute the foundation of a nation and no sensible government can afford to neglect their needs and rights. In the last 50 y, technology has revolutionized neonatology and we have moved from an exceedingly passive or "hands-off" philosophy to an extremely aggressive or mechanistic approach. Deaths during first 28 d of life account for over 60 % of all infant deaths and 40 % of all deaths of under-5 children. If we have to further reduce infant mortality rate in our country we must focus our strategies to improve health and survival of newborn babies. There should be equitable

Doping, fairness, and unequal responsiveness: A response to Lavazza.

In a thought-provoking article in Bioethics, Andrea Lavazza defends the view that for reasons of fairness, those who cannot benefit from the use of performance-enhancing methods such as transcranial direct current stimulation (tDCS) should receive compensation for their inability. First, we argue that Lavazza's proposal to compensate athletes who are non-responsive to tDCS is practically unfeasible. Second, the compensation principle-which he appeals to in his defense of his compensation scheme-is false, as it is incoherent to focus only on the compensation of athletes who respond less well to

Ethics Students Go to the Jail.

This article describes an educational initiative in which clinical ethics students, who were either in a bioethics master's degree program or in the fourth year of medical school, spent two days observing health care in an urban jail. Students submitted reflections about their experience, in which they drew attention to concerns about privacy, physical restriction, due care, drug addiction, mistrust, and the conflicting expectations that arise when incarcerated people become patients. The rotation was of great value to the students both because it exposed them to many of the ethical issues tha

The Case of Ms D: A Family's Request for Posthumous Procurement of Ovaries.

The MedStar Washington Hospital Center clinical ethics team became involved in a case when the family requested the posthumous removal of a patient's ovaries for future reproductive use. This case presents a novel question for clinical ethicists, since the technology for posthumous female reproduction is still in development. In the bioethics literature, the standard position is to refuse to comply with such a request, unless there is explicit consent or evidence of explicit conversations that demonstrate the deceased would have wanted this option pursued. Ms D's case, we suggest, offers an ex

Social responsibility and global health: lessons from the Rio Olympics Zika controversy.

BACKGROUND: Identification of antigens by immunoblotting techniques, using epidermal and dermal extracts, is regarded as essential for making a definitive diagnosis in autoimmune bullous diseases (AIBDs). These procedures involve epidermal-dermal separation for subsequent protein extraction, which may result in partial loss of some antigenic polypeptides and changes in the conformational epitopes targeted by autoantibodies in AIBDs. It may therefore be necessary to use different substrates for consistent results. Objectives To evaluate the usefulness of human placental amnion extract as a subs

'There is a time to be born and a time to die' (Ecclesiastes 3:2a): Jewish perspectives on euthanasia.

Reviewing the publications of prominent American rabbis who have (extensively) published on Jewish biomedical ethics, this article highlights Orthodox, Conservative and Reform opinions on a most pressing contemporary bioethical issue: euthanasia. Reviewing their opinions against the background of the halachic character of Jewish (biomedical) ethics, this article shows how from one traditional Jewish textual source diverse, even contradictory, opinions emerge through different interpretations. In this way, in the Jewish debate on euthanasia the specific methodology of Jewish (bio)ethical reason

[First French data on sperm donation requests from female couples].

OBJECTIVE: The new bioethics law, enacted on August 2, 2021, has expanded access to medically assisted reproduction (MAR) with sperm donation to unmarried women and female couples. The implementation of this law led to a significant influx of sperm donation requests at French sperm banks. This major development in the field of ART in France, and in French society as a whole, prompted us to focus specifically on the requests from female couples. METHOD: A retrospective, single-center study was conducted at a gamete donation center in Paris. The data of women in same-sex couples who attended a

Social and ethical implications of AI-based facial technologies in medicine and healthcare: a scoping review.

BACKGROUND: AI-based facial technologies are increasingly explored in medicine and healthcare for purposes ranging from diagnosis and monitoring to identification. While ethical debates on AI-in-health are well developed, the specific social and ethical implications of facial AI in medical and healthcare contexts remain fragmented. METHODS: We conducted a scoping review following PRISMA-ScR guidelines to map the social and ethical implications of AI-based facial technologies reported in the literature and to examine how these implications are framed across disciplinary domains. Third-party reproductive assistance around the Mediterranean: comparing Sunni Egypt, Catholic Italy and multisectarian Lebanon.

The article examines religious and legal restrictions on third-party reproductive assistance in three Mediterranean countries: Sunni Egypt, Catholic Italy and multisectarian Lebanon. In Egypt, assisted reproduction treatments are permitted, but third parties are banned, as in the rest of the Sunni Islamic world. Italy became similar to Egypt with a 2004 law ending third-party reproductive assistance. In multisectarian Lebanon, however, the Sunni/Catholic ban on third-party reproductive assistance has been lifted, because of Shia rulings emanating from Iran. Today, third-party reproductive assi

A foundation for public health ethics at Tuskegee University in the 21st century.

This commentary is a reflection on Tuskegee University's National Center for Bioethics in Research and Health Care on the health and ethical challenges of the 21st century. The Center has dedicated the last 10 years to addressing the unresolved biomedicine and public health issues and/or the ethical dilemmas that plague the nation's health. The authors believe that health disparities continue to worsen because the approach under-appreciates the ethical dilemma that plagues health policy and health disparities. The authors discuss synergies and the paradigmatic differences between science and m

Fertility preservation and sperm donation in transgender individuals: The current situation within the French CECOS network.

BACKGROUND: Many studies reported that reproductive desire could be high among transgender individuals. In France, fertility preservation and sperm donation were very little proposed to transgender individuals until recently, mainly because the Bioethics Law allows the use of assisted reproductive technologies only in infertile couples and prohibits surrogacy. OBJECTIVES: To evaluate the distribution of care on the French territory concerning fertility preservation and sperm donation in transgender individuals. MATERIALS AND METHODS: A multicentric national survey was carried out between Jan

Sickle cell trait screening in athletes: pediatricians' attitudes and concerns.

BACKGROUND: As part of a legal settlement in 2010, the National Collegiate Athletic Association (NCAA) adopted a recommendation that all Division I athletes be screened for sickle cell trait (SCT) or sign an exemption waiver. Pediatricians' attitudes about this policy are unknown. OBJECTIVE: We queried 3 specialty sections of the American Academy of Pediatrics (AAP)--the Section on Adolescent Health, the Council on Sports Medicine and Fitness (COSMF), and the Section on Bioethics-to determine attitudes about and knowledge of SCT testing of athletes. METHODS: Three e-mail surveys were sent to

[Triploid cloned human embryos: ethical, social, and legal aspects].

This work attempts to place the experiment within the scientific and social framework of pluripotent-stem-cell research and offer reflections of an ethical and (to a lesser extent) legal nature on the results obtained by this research group. To these ends, the work is divided into two parts. The first part describes the most important aspects of Noggle and Egli's announcement and the biotechnological and media context in which it was made. The second part is concerned with the bioethical issues raised by the experiment. There are basically four issues, which relate to: (1) the nuclear transfer

Gestational Surrogacy in Brazil: A Position Statement of the Brazilian Association of Assisted Reproduction (SBRA).

Surrogacy includes traditional surrogacy, in which the carrier contributes her own oocyte, and gestational surro-gacy, in which the embryo is derived exclusively from the gametes of the intended parents or donors. In Brazil, only gestational surrogacy is legally recognized. It is governed by resolutions of the Federal Council of Medicine and the national health surveillance agency in the absence of spe-cific federal legislation, and it requires altruistic, unpaid participation and compliance with carrier eligibility crite-ria. This Position Statement, developed by the Strategic Committee on Ge
