Neglected tropical disease as a 'biographical disruption': Listening to the narratives of affected persons to develop integrated people centred care in Liberia.

BACKGROUND: Integrated disease management, disability and inclusion (DMDI) for NTDs is increasingly prioritised. There is limited evidence on the effectiveness of integrated DMDI from the perspective of affected individuals and how this varies by differing axes of inequality such as age, gender, and disability. We used narrative methods to consider how individuals' unique positions of power and privilege shaped their illness experience, to elucidate what practical and feasible steps could support integrated DMDI in Liberia and beyond. METHODS: We purposively selected 27 participants affected

Membrane perturbing properties of toxin mycolactone from Mycobacterium ulcerans.

Mycolactone is the exotoxin produced by Mycobacterium ulcerans and is the virulence factor behind the neglected tropical disease Buruli ulcer. The toxin has a broad spectrum of biological effects within the host organism, stemming from its interaction with at least two molecular targets and the inhibition of protein uptake into the endoplasmic reticulum. Although it has been shown that the toxin can passively permeate into host cells, it is clearly lipophilic. Association with lipid carriers would have substantial implications for the toxin's distribution within a host organism, delivery to ce

Epidemiology of Mansonella perstans in the middle belt of Ghana.

BACKGROUND: Mansonellosis was first reported in Ghana by Awadzi in the 1990s. Co-infections of Mansonella perstans have also been reported in a small cohort of patients with Buruli ulcer and their contacts. However, no study has assessed the exact prevalence of the disease in a larger study population. This study therefore aimed to find out the prevalence of M. perstans infection in some districts in Ghana and to determine the diversity of Culicoides that could be potential vectors for transmission. METHODS: From each participant screened in the Asante Akim North (Ashanti Region), Sene West a

Proposing BCG Vaccination for Mycobacterium avium ss. paratuberculosis (MAP) Associated Autoimmune Diseases.

Bacille Calmette-Guerin (BCG) vaccination is widely practiced around the world to protect against the mycobacterial infection tuberculosis. BCG is also effective against the pathogenic mycobacteria that cause leprosy and Buruli's ulcer. BCG is part of the standard of care for bladder cancer where, when given as an intravesicular irrigant, BCG acts as an immunomodulating agent and lessens the risk of recurrence. Mycobacterium avium ss. paratuberculosis (MAP) causes a fatal enteritis of ruminant animals and is the putative cause of Crohn's disease of humans. MAP has been associated with an incre

Leveraging wastewater-based epidemiology to monitor the spread of neglected tropical diseases in African communities.

Neglected tropical diseases continue to cause a significant burden worldwide, with Africa accounting for more than one-third of the global burden. Over the past decade, progress has been made in eliminating, controlling, and eradicating these diseases in Africa. By December 2022, 47 out of 54 African countries had eliminated at least one neglected tropical disease, and more countries were close to achieving this milestone. Between 2020 and 2021, there was an 80 million reduction in people requiring intervention. However, continued efforts are needed to manage neglected tropical diseases and ad

Community awareness, symptom recognition, and perceived causes of skin neglected tropical diseases in selected regions of Ghana.

BACKGROUND: Leprosy and other skin neglected tropical diseases, including Buruli ulcer and yaws, remain significant public health challenges in Ghana. Despite control efforts, these conditions continue to cause disability, stigma, and social exclusion, particularly in rural and underserved communities. Misconceptions rooted in cultural and spiritual beliefs further delay care-seeking and reinforce discrimination. OBJECTIVE: This study assessed community awareness, symptom recognition, and perceived causes of leprosy and other skin NTDs across four regions of Ghana, and examined how these vari

[Atypical mycobacteriosis of the skin caused by Mycobacterium abscessus in an immunocompetent woman].

Mycobacterium abscessus is the most pathogenic of the fast-growing mycobacteria, and it is resistant to most of the antimicrobial and tuberculostatic drugs available. This non-tuberculous mycobacterium is significant in medicine because it can contaminate post-traumatic wounds and be a causative agent in chronic skin and soft tissue infection after surgical procedures.A 60-year-old immunocompetent woman was suffering from chronic ulcers and abscesses on the heels and malleoli of both feet. Histological examination revealed a granulomatous inflammation with detection of acid-fast rods, albeit w

Pharmacokinetics of extended-release clarithromycin in patients with Mycobacterium ulcerans infection.

Clarithromycin extended-release (CLA-ER) was used as companion drug to rifampicin (RIF) for Mycobacterium ulcerans infection in the intervention arm of a WHO drug trial. RIF enhances CYP3A4 metabolism, thereby reducing CLA serum concentrations, and RIF concentrations might be increased by CLA co-administration. We studied the pharmacokinetics of CLA-ER at a daily dose of 15 mg/kg combined with RIF at a dose of 10 mg/kg in a subset of trial participants, and compared these to previously obtained pharmacokinetic data. Serial dried blood spot samples were obtained over a period of ten hours, and

Domestic animals infected with Mycobacterium ulcerans-Implications for transmission to humans.

BACKGROUND: The environmental pathogen, Mycobacterium ulcerans (MU) can infect both humans and animals and cause Buruli ulcer (BU) disease. However, its mode(s) of transmission from the colonized environment to human/animal hosts remain unclear. In Australia, MU can infect both wildlife and domestic mammals. Till date, BU-like lesions have only been reported in wildlife in Africa. This warrants a thorough assessment of possible MU in domestic animals in Africa. Here, we screened roaming domesticated animals that share the human microhabitat in two different BU endemic sites, Sedje-Denou in Ben

Genetic dissection of immunity to mycobacteria: the human model.

Humans are exposed to a variety of environmental mycobacteria (EM), and most children are inoculated with live Bacille Calmette-Guérin (BCG) vaccine. In addition, most of the world's population is occasionally exposed to human-borne mycobacterial species, which are less abundant but more virulent. Although rarely pathogenic, mildly virulent mycobacteria, including BCG and most EM, may cause a variety of clinical diseases. Mycobacterium tuberculosis, M. leprae, and EM M. ulcerans are more virulent, causing tuberculosis, leprosy, and Buruli ulcer, respectively. Remarkably, only a minority of ind

One-step loop-mediated isothermal amplification system for Mycobacterium marinum detection.

UNLABELLED: Mycobacterium marinum is the etiological agent of cutaneous nontuberculous mycobacteriosis, inducing granuloma formation in human skin. Other mycobacterial species, including Mycobacterium ulcerans, Mycobacterium chelonae, and Mycobacterium fortuitum, also cause cutaneous infections, with M. ulcerans and M. ulcerans subsp. shinshuense showing close genetic relatedness to M. marinum. M. ulcerans is a recognized causative agent of Buruli ulcer, and numerous molecular detection and diagnostic methods have been established. However, there is a lack of specific detection methods for M.

Efficacy of Rifampin Plus Clofazimine in a Murine Model of Mycobacterium ulcerans Disease.

Treatment of Buruli ulcer, or Mycobacterium ulcerans disease, has shifted from surgical excision and skin grafting to antibiotic therapy usually with 8 weeks of daily rifampin (RIF) and streptomycin (STR). Although the results have been highly favorable, administration of STR requires intramuscular injection and carries the risk of side effects, such as hearing loss. Therefore, an all-oral, potentially less toxic, treatment regimen has been sought and encouraged by the World Health Organization. A combination of RIF plus clarithromycin (CLR) has been successful in patients first administered R

[Overview of infectious and non-infectious diseases in French Guiana in 2022].

Source of many myths, French Guiana represents an exceptional territory due to the richness of its biodiversity and the variety of its communities. The only European territory in Amazonia, surrounded by the Brazilian giant and the little-known Suriname, Ariane 6 rockets are launched from Kourou while 50% of the population lives below the poverty line. This paradoxical situation is a source of health problems specific to this territory, whether they be infectious diseases with unknown germs, intoxications or chronic pathologies. Some infectious diseases such as Q fever, toxoplasmosis, cryptococc

Toponymous diseases of Australia.

Names are more than just labels used to identify diseases. They can be windows into the discovery, characteristics and attributes of the disease. Toponymous diseases are diseases that are named after places. Hendra, Ross River, Bairnsdale, Murray Valley and Barmah Forest are all examples of Australian places that have had diseases named after them. They all have unique and interesting stories that provide a glimpse into their discovery, history and culture. Because of perceived negative connotations, the association of diseases with placenames has sometimes generated controversy.

Patient journeys for neglected tropical diseases in rural sub-Saharan Africa: a scoping review.

BACKGROUND: Patient journeys highlight evolving processes of care seeking from patient perspectives over the course of time and disease progression. Patient journeys for neglected tropical diseases (NTDs) in rural sub-Saharan Africa (SSA) are poorly understood. This review aims to identify studies including patient journeys for NTDs in rural SSA. METHODS: Systematic search of six scientific databases from inception to 18 November 2024. All studies were required to include patient journeys for NTDs, defined as the continuous arc of the patient care seeking experience at multiple time points wh

Community-based geographical distribution of Mycobacterium ulcerans VNTR-genotypes from the environment and humans in the Nyong valley, Cameroon.

BACKGROUND: Genotyping is a powerful tool for investigating outbreaks of infectious diseases and it can provide useful information such as identifying the source and route of transmission, and circulating strains involved in the outbreak. Genotyping techniques based on variable number of tandem repeats (VNTR) are instrumental in detecting heterogeneity in Mycobacterium ulcerans (MU) and also for discriminating MU from other mycobacteria species. Here, we describe and map the distribution of MU genotypes in Buruli ulcer (BU) endemic communities of the Nyong valley in Cameroon. We also tested th

Effectiveness of self-care interventions for integrated morbidity management of skin neglected tropical diseases in Anambra State, Nigeria.

BACKGROUND: Lymphatic filariasis (LF), Buruli ulcer (BU) and leprosy are neglected tropical diseases (NTDs) of the skin co-endemic in some communities in Nigeria. Not enough is known about the effectiveness of integrated morbidity management and disability prevention in people with these conditions. An integrated self-care intervention was carried out for people with these skin NTDs in two endemic communities of Anambra state, Nigeria. The objective of the study was to assess the effectiveness of self-care practices on costs of care, disability status and health-related quality of life. METHO

Strengthening access to people-centred mental healthcare for people affected by skin neglected tropical diseases in Nigeria: a pilot study.

OBJECTIVES: Neglected tropical diseases (NTDs) contribute to the global burden of mental health conditions, largely mediated through mechanisms of stigma and social exclusion. Within this group, skin NTDs are particularly prominent, as visible deformities often drive stigma reinforced by cultural myths, fear of contagion, social isolation, and associated poverty. The WHO recommends a collaborative cross-sectoral approach in its NTD Road Map 2021-30, integrating care for people with NTDs. This study was conducted as a pilot to assess the feasibility, acceptability, and impact of a person-centre

The psychosocial impacts of skin-neglected tropical diseases (SNTDs) as perceived by the affected persons: A systematic review.

BACKGROUND: Neglected Tropical Diseases (NTDs) disproportionately affect marginalised groups within impoverished communities, conferring devastating physical, financial and psychosocial effects. Skin-NTDs (SNTDs) are uniquely stigmatising due to their visible nature, rendering affected individuals vulnerable to psychosocial risk and the associated decline in social participation, quality of life and mental health. In response to knowledge gaps identified by current global efforts for integrated control of SNTDs this review gathers existing evidence on the psychosocial effects of SNTDs, with co

Rapid test for Mycobacterium leprae infection: a practical tool for leprosy.

BACKGROUND: Detection of infection with Mycobacterium leprae allows timely prophylactic treatment, thereby reducing transmission as well as the risk of permanent, leprosy-associated nerve damage. However, since there is no worldwide-implemented standard test for M. leprae infection, detection of infection in asymptomatic individuals remains a major challenge for control programs in endemic areas. In previous studies, we developed and field-tested a lateral flow assay (LFA) quantitatively detecting human IgM against M. leprae-specific phenolic glycolipid I (anti-PGL-I), a marker for both active
