[Development of a portable fall risk index for elderly people living in the community].

AIM: To develop a portable risk index for falls. METHODS: Risk factors were chosen from previously established factors then we added several environmental factors to the risk index; previous falls in the last 12 month, trippig or stumbling, inability to ascend or descend stairs without help, decreased walking speed, inability to cross a road within the green signal interval, inability to walk 1km without a break, inability to stand on one leg for 5 seconds (eyes open), using a cane, inability to wring out a towel, dizziness or faintness, stooped or rounded back, knee joint pain, visual distur

An evaluation of needs in elderly continuing-care settings.

This article describes the met and unmet needs of elderly residents of nursing care (NC) and residential care (RC) settings. Thirty-four residents of an RC home and 40 residents of two NC settings were assessed. Each resident and a respective staff member were interviewed using the Camberwell Assessment of Need for the Elderly (CANE) to indicate the resident's current met and unmet needs. The Clifton Assessment Procedure for the Elderly-Behaviour Rating Scale (CAPE-BRS) was completed by the staff member to indicate the participant's current level of dependency. In addition, the Mini-Mental Sta

Bilateral Early Knee Osteoarthritis Treated With Unilateral Proximal Tibial Osteotomy And Contralateral Non-Surgical Technique.

Some non-surgical techniques that decrease the mechanical load of the knee may be effective in treating early primary knee osteoarthritis. Thirtysix consecutive patients with bilateral early primary knee osteoarthritis and genu varum were treated with unilateral proximal tibial osteotomy in the more degenerated knee. After the osteotomized bone healed, ambulation with protected weight bearing using a cane in the contralateral hand was advised continuously for at least three months or until knee pain subsided bilaterally. Thirty-one patients were followed for an average of 4.6 years (range, 2.1

Upper-limb pain in long-term poliomyelitis.

BACKGROUND: Late functional deterioration is common in long-term poliomyelitis patients. While upper-limb pain in individual functional regions is common, its overall prevalence and pattern in long-term poliomyelitis is poorly documented. AIM: To assess the nature of upper-limb pain in these patients and examine its association with dependence on mobility aids (and therefore upper limb 'overuse'). DESIGN: Questionnaire-based survey of patients attending a specialist unit. METHODS: Questionnaires were sent to 139 patients with known long-term poliomyelitis (mean 49.8 years post polio), atten

Fracture of an 18-mm Diameter Extensively Porous-Coated Femoral Stem 17.9 Years After Primary Total Hip Arthroplasty: A Case Report.

CASE: A 79-year-old man presented with progressive right thigh pain over 6 months and reliance on a cane. He had undergone primary total hip arthroplasty (THA) with a metal-on-metal bearing in 2007. In 2025, radiographs revealed a fracture of his 18-mm diameter extensively porous-coated, cobalt-chrome alloy femoral stem (Prodigy, DePuy Orthopaedics), prompting revision THA. To the best of our knowledge, this case represents the largest diameter fracture of this type of component reported to date. CONCLUSION: While fractures of smaller diameter stems are well-documented, clinicians should be a

A biological invasion modifies the dynamics of a host-parasite arms race.

By imposing novel selection pressures on both participants, biological invasions can modify evolutionary 'arms races' between hosts and parasites. A spatially replicated cross-infection experiment reveals strong spatial divergence in the ability of lungworms (Rhabdias pseudosphaerocephala) to infect invasive cane toads (Rhinella marina) in Australia. In areas colonized for longer than 20 years, toads are more resistant to infection by local strains of parasites than by allopatric strains. The situation reverses at the invasion front, where super-infective parasites have evolved. Invasion-induc

[Functional and surgical complications after bariatric surgery for severe obesity].

Bariatric surgery, the most effective treatment for obesity, can lead to long-term complications. These may be of functional nature (dumping, hypoglycemia, reflux) or of surgical nature. Acute or chronic abdominal pain is not unusual after gastric bypass. Often unrelated to surgery, it can be caused by an acute surgical problem (internal hernia, obstruction, intussusception), which can lead to extensive bowel necrosis if not treated quickly. Other complications (candy cane, anastomotic ulcer, reflux) can also develop. An « obstructed sleeve » can develop early or late after sleeve gastrectomy.

Skin diseases in musicians.

Instrumental musicians are a risk group for skin diseases. A systematic review was performed on Pubmed database and in the musical literature. Most publications on dermatoses in musicians are case reports. The exact prevalence of skin diseases in musicians is unknown but high rates have been reported. The most at-risk musicians are percussionists, string and wind instrumentalists. Repeated physical trauma is a frequent cause of skin conditions in musicians (callosities, fiddler's neck syndrome…). The allergens most often reported in musicians' allergic contact dermatitis are metals (nickel, di

[Human leptospirosis outbreak in the district of Ciego de Avila, Cuba].

Forty leptospirosis outbreaks occurred in Ciego de Avila province from 1980 to 1995. The main events involved in the outbreaks were floods, bathing in rivers and activities related to sugar cane and banana cultivation. The number of cases increased after June, with higher incidences reported in October and November. The most affected age groups were 10-14 years, 15-19 years and 30-34 years. Men were more affected than women. Students, people residing in the urban zone and farmers were the most affected groups. A total of 21 outbreaks out of 40 were confirmed by the microagglutination test and

Prevalence and pathogenicity of Entamoeba histolytica in three different regions of Pernambuco, northeast Brazil.

Parasitological examinations were carried out on 663 individuals of three different cities of Pernambuco State, Northeastern Brazil: Recife, Palmares and Bodocó. The population from a drought area of Pernambuco State, Bodocó, was investigated for amoebiasis and compared with Recife, metropolitan city (about 1.3 million of inhabitants) and another inland community, Palmares, located inside of the sugar-cane plantation region of the State. No evidence of invasive strains of E. histolytica were found in these inhabitants, provided that the isolated zymodemes I, III, IV, VIII, IX, X, XVII and XVII

Peroneal nerve stimulator in rehabilitation of hemiplegic patients.

Of nine hemiplegic patients having foot-drop who were test-treated with the Philips functional electronic peroneal stimulator, only three who tolerated continued use of the stimulator for five weeks had improvement in function. Findings in these cases indicate that to be selected for trial use of a stimulator a hemiplegic patient should fulfill the following criteria: (1) be in the hospital, (2) be ambulant with or without a cane and/or short leg brace, (3) have no severe contracture of the ankle joint, (4) have no severe spasticity, (5) have no obvious lower motor neuron lesions, (6) be coope

Diagnosis of Guillain-Barré Syndrome After Total Hip Arthroplasty: A Case Report.

CASE REPORT: A 67-year-old man presented with signs of acute periprosthetic infection after total hip arthroplasty (THA). Surgical debridement, antibiotics, and a head and liner exchange were performed. After showing no improvement, a single-stage revision was conducted. Postoperatively, he developed back pain and lower extremity weakness. Electrodiagnostic studies showed a Guillain-Barré syndrome (GBS) variant. Intravenous immunoglobulin was administered to halt disease progression. After 1 year, he still demonstrated neuromuscular deficits and required a cane for ambulation. CONCLUSION: Thi

Salvage Reconstruction With the Masquelet Technique Following Wide Resection for Chondrosarcoma of the Proximal Femoral Metaphysis: A Case Report.

BACKGROUND: Bone reconstruction following a wide resection for a malignant musculoskeletal tumor remains challenging, especially for an intercalary defect following the resection of a metaphyseal lesion. CASE REPORT: Here, we describe a surgical procedure using the Masquelet technique for the biological reconstruction of a huge subtrochanteric bone defect following failed pasteurized autologous bone grafting for a conventional chondrosarcoma of the proximal femoral metaphysis with a subtrochanteric pathological fracture. The patient, a 43-year-old Japanese male, was able to walk without a can

Allograft reconstruction for extensor mechanism rupture after total knee arthroplasty: 4.8-year follow-up.

Three patients who experienced extensor mechanism rupture after total knee arthroplasty underwent reconstruction using an allograft consisting of quadriceps tendon, patella, patellar tendon, and tibial tubercle. All patients who failed initial attempts to repair the extensor mechanism disruption achieved pain relief at average 4.8-year follow-up. Patients were able to extend their knee actively against resistance. Ambulation with full weight bearing was possible in all patients; however, one patient required a cane. Few reports have been published on allograft reconstruction of the extensor me

Combined Hip Procedure in a 74-Year-Old Patient with Acetabulum Fracture.

In elderly individuals, low-energy trauma can cause acetabular fractures. Many surgical options have been described, including open reduction and internal fixation (ORIF), arthroplasty, and combined hip procedure. We present a 74-year-old man with a fall-related acetabular fracture with an unusual pattern, which was managed by ORIF and total hip arthroplasty. The patient experienced a hip dislocation at three weeks post-operation that was reduced in the emergency department. At 12 months post-operation, the patient resumed the same level of function he had before falling, using a cane while ou

Trunnion fracture of a cobalt-chrome fully porous-coated femoral stem: A case report.

Modular femoral stem-head systems are used increasingly due to their ease of application and offset adjustment. However, this modular ity has brought complications of trunnion wear and breakage. Although very rarely encountered, trunnion fracture is a catastrophic com plication that requires challenging revision surgery. This report presents a trunnion fracture of a cobalt-chrome alloy, fully porous-coated femoral stem. Following single-stage revision surgery, full weight-bearing was achieved in six weeks, and the patient was painlessly mobile with a single cane and had a Hip Score of 81 in th

Epizootiology of arenaviral infections in the white-throated woodrat (Muridae: Sigmodontinae) and other woodrats in Arizona.

The purpose of this study was to extend and refine our knowledge of the geographical distribution and natural host relationships of the arenaviruses associated with woodrats indigenous to Arizona. Antibody to a Tacaribe serocomplex virus was found in 112 of 1,250 white-throated woodrats, five of 208 Mexican woodrats, one of 114 Stephen's woodrats, and none of 862 other rodents captured at 51 sites in 10 counties in Arizona. Of the 112 antibody-positive white-throated woodrats, 109 (97.3%) were captured within extensive, dense patches of prickly pear cactus and cane cholla in three counties in

Occult bilateral acetabular fractures associated with high-energy trauma and osteoporosis.

Acetabular fractures that are radiographically occult are associated with osteoporosis, low-energy trauma, and advanced age. We present a case of bilateral occult acetabular fractures in a 65-year-old woman who presented with an open tibia-fibula fracture and compartment syndrome and thoracolumbar spine fractures sustained in a motor vehicle accident. Initial radiographs of the pelvis and a pelvic CT scan did not show any bony injuries when the films were reviewed prospectively. At 6 weeks after injury, a CT scan and plain radiographs showed bilateral healing transverse acetabular fractures wi

Cervical myelo-radiculopathy in athetoid cerebral palsy.

Cervical myelopathy complicating athetoid cerebral palsy has not been adequately highlighted in the literature. We report two cases of patients with athetoid cerebral palsy and long histories of involuntary movements who developed cervical myelo-radiculopathy. Dystonic athetoid neck movements may cause excessive axial neck rotation as well as flexion and extension movements of the spine. These repetitive exaggerated movements may result in early degenerative changes of the vertebrae which may enhance the myelo-radiculopathy. The two patients were treated with combined anterior and posterior fu
