MR colonography without bowel cleansing or water enema: a pilot study.

OBJECTIVES: The value of screening for colorectal cancer has been well demonstrated, and national programmes are being implemented. At present the primary imaging modality is colonoscopy, which has an associated morbidity and mortality. CT colonography (CTC) has been proposed as an alternative, but this is associated with radiation exposure and generally requires bowel preparation. The aim of this pilot study was to assess the feasibility of MR colonography (MRC) without bowel preparation or water enema for the detection of colorectal neoplasia. METHODS: Patients scheduled for colonoscopy wer

Colon cancer screening in 2010: an up-date.

Colorectal cancer is among the most common cancers worldwide. The prognosis for limited disease is excellent; however, it becomes poor for more advanced disease. The majority of colorectal cancers arise from premalignant adenomatous polyps. This makes the detection of polyps and early carcinoma an attractive screening strategy. This article will review the current tests available for screening for colorectal cancer. These include stool based tests (guaiac-based fecal occult blood tests, immunochemical fecal tests, stool DNA panel), radiologic tests (double contrast barium enema and computed to

Screening and diagnosis for colorectal cancer: present and future.

AIMS AND BACKGROUND: Colon cancer is the third most common cause of cancer diagnosed in the United States, and the second leading cause of cancer death. Although rates of the disease have been going down in recent years, results can be further improved. About 90% of people whose colon cancer is caught before it has spread to nearby lymph nodes or organs survive more than 5 years after diagnosis. However, only 10% of patients whose cancer has spread to distant parts of the body survive 5 years. Diagnosis at an early stage aims to reduce the incidence of tumors in an advanced stage and hence mor

Findings on optical colonoscopy after positive CT colonography exam.

BACKGROUND & AIMS: The aim of this study is to evaluate the findings on optical colonoscopy (OC) after a positive CT colonography (CTC) exam and characterize the type of polyps seen on OC but not reported by CTC. METHODS: Over an 18-month period a total of 159 asymptomatic adults had polyps seen on computed tomography colonography examination and subsequently underwent planned therapeutic optical colonoscopy. The colonoscopists were aware of the findings on CT colonography prior to further evaluation of the colon. Characteristics of polyps and adenomas seen on subsequent optical colonoscopy b

Minimal preparation computed tomography instead of barium enema/colonoscopy for suspected colon cancer in frail elderly patients: an outcome analysis study.

AIM: To evaluate the efficacy of minimal preparation computed tomography (MPCT) in diagnosing clinically significant colonic tumours in frail, elderly patients. MATERIALS AND METHODS: A prospective study was performed in a group of consecutively referred, frail, elderly patients with symptoms or signs of anaemia, pain, rectal bleeding or weight loss. The MPCT protocol consisted of 1.5 l Gastrografin 1% diluted with sterile water administered during the 48 h before the procedure with no bowel preparation or administration of intravenous contrast medium. Eight millimetre contiguous scans throug

Magnetic resonance imaging based colonography for diagnosis and assessment of diverticulosis and diverticulitis.

BACKGROUND AND AIMS: MRI-based colonography is a new minimally invasive imaging modality to assess the colon and abdomen. This new method which is applied mainly for polyp screening could be an integrative approach for colonic diverticulitis assessment. This study evaluated the feasibility of MRI-based colonography to assess diverticulosis or diverticulitis. PATIENTS AND METHODS: Fourteen consecutive patients with clinically suspected diverticulitis were examined by MRI colonography on a 1.5-T scanner. All patients underwent abdominal CT as gold standard. N-Butyl-scopalamin was given intraven

Colon cancer: comprehensive evaluation with 64-section CT colonography using water enema as intraluminal contrast agent-a pictorial review.

Sixty-four-section CT colonography with water enema combines intracolonic neutral contrast agent with high-resolution CT images of the abdomen. Owing to submillimeter isotropic voxels, high-quality reformatted images are obtained. High-resolution images offer added value for the detection and localization of colonic lesions, evaluation of the local extent of the disease, and depiction, if any, of synchronous colorectal lesions and distant metastases. Sixty-four-section CT colonography with water enema has a major role in the evaluation of patients with colon cancer before planning therapy. It

Screening for Colorectal Cancer: US Preventive Services Task Force Recommendation Statement.

IMPORTANCE: Colorectal cancer is the second leading cause of cancer death in the United States. In 2016, an estimated 134,000 persons will be diagnosed with the disease, and about 49,000 will die from it. Colorectal cancer is most frequently diagnosed among adults aged 65 to 74 years; the median age at death from colorectal cancer is 68 years. OBJECTIVE: To update the 2008 US Preventive Services Task Force (USPSTF) recommendation on screening for colorectal cancer. EVIDENCE REVIEW: The USPSTF reviewed the evidence on the effectiveness of screening with colonoscopy, flexible sigmoidoscopy, co

Expert commentary--virtual colonoscopy: utility as a screening test for colorectal cancer?

Virtual colonoscopy represents a noninvasive test for the examination of the colonic lumen that involves the generation of both 2-dimensional and 3-dimensional views of the colon and rectum using data derived from computed tomography. This imaging modality has been explored as an alternative to conventional endoscopic colonoscopy, in particular as an alternative screening tool for colorectal cancer. Although virtual colonoscopy does not require sedation and requires less time for completion, the study requires the same bowel-cleansing preparation as conventional colonoscopy as well as gas insu

Computed tomography colonography in routine clinical practice.

OBJECTIVE: To describe the experience of a radiology unit in using open access computed tomography (CT) colonography instead of double-contrast barium enema in patients who refused or had an incomplete first-attempt colonoscopy. METHODS: All consecutive patients who underwent CT colonography from December 1998 to August 2001 were recalled and evaluated. Patients in whom CT colonography showed intraluminal growths were sent for colonoscopy, performed using deep sedation if the first attempt failed. RESULTS: A total of 463 consecutive CT colonography examinations were performed: 304 patients w

Robotic Colonoscopy and Beyond: Insights into Modern Lower Gastrointestinal Endoscopy.

Lower gastrointestinal endoscopy is considered the gold standard for the diagnosis and removal of colonic polyps. Delays in colonoscopy following a positive fecal immunochemical test increase the likelihood of advanced adenomas and colorectal cancer (CRC) occurrence. However, patients may refuse to undergo conventional colonoscopy (CC) due to fear of possible risks and pain or discomfort. In this regard, patients undergoing CC frequently require sedation to better tolerate the procedure, increasing the risk of deep sedation or other complications related to sedation. Accordingly, the use of CC

CT Colonography and Colorectal Carcinoma: Current Trends and Emerging Developments.

Colorectal carcinoma is the third most malignant and second leading cause of death from cancer. The cruelty of this entity is that it takes decades to be symptomatic and is known to be detected late in its timeline by a screening technique. The fatality of this carcinoma only means heightened importance of screening guidelines to be laid down and strict follow-ups by the healthcare providers. A novel method, a potential competitor that could now replace the present screening techniques for colorectal carcinoma, is computed tomographic colonography (CTC) or virtual colonoscopy. Though it first

Colorectal cancer: a summary of the evidence for screening and prevention.

Colorectal cancer causes significant morbidity and mortality in the United States. The incidence of colorectal cancer can be reduced with increasing efforts directed at mass screening of average-risk adults 50 years and older. Currently, fecal occult blood test and flexible sigmoidoscopy have the highest levels of evidence to support their use for colorectal cancer screening. Colonoscopy does not have a proven colorectal cancer mortality benefit, but it does have the greatest single-test accuracy, and it is the final test in the pathway to evaluate and treat patients with other abnormal screen

Colorectal Cancer Screening and Prevention.

Colorectal cancer is a common cause of morbidity and mortality in the United States. Most colorectal cancers arise from preexisting adenomatous or serrated polyps. The incidence and mortality of colorectal cancer can be reduced with screening of average-risk adults 50 to 75 years of age. Randomized controlled trials show evidence of reduced colorectal cancer-specific mortality with guaiac-based fecal occult blood tests and flexible sigmoidoscopy. There are no randomized controlled trials on the effectiveness of colonoscopy to reduce colorectal cancer-specific mortality; however, several random
