Scaling community-based care for conductive hearing loss in low- and middle-income settings.

PURPOSE OF REVIEW: This review examines recent advances in community-based care for conductive hearing loss in low- and middle-income countries (LMICs), focusing on developments that have materially shifted the feasibility of decentralized service delivery across prevencommution, screening and diagnosis, medical management, and rehabilitation. RECENT FINDINGS: World Health Organization guidance now explicitly endorses task-sharing for primary ear and hearing care, including community health worker-facilitated hearing aid provision. Long-term follow-up from pneumococcal vaccination trials demo

Is the money following the clients with learning disabilities?

For the past decade patients with learning disabilities living in long stay mental handicap hospitals have been resettled in the community. Local authorities have also taken on the care of new patients who would once have been long stay residents. The imperfect data that are available suggest that in England about half the residents in mental handicap hospitals in 1981 are now the responsibility of local authorities; the figures for Wales and Northern Ireland are 38% and 33%. Data on revenue suggest that the savings to the health service are much less--perhaps 9% in Northern Ireland and 3.6% i

A qualitative study on the current status and problems of pharmacists in home healthcare from the viewpoint of care managers in medically underpopulated areas in Japan.

BACKGROUND: Unlike in urban areas, community healthcare in medically underpopulated areas in Japan is constantly challenged because of the uncertainty in effectively using the limited resources. However, no study has focused on human resources or identified the actual state of pharmacists' support in the community. Therefore, our study identified the actual status and problems of pharmacists involved in home medical care in medically underpopulated areas and discussed the roles required of pharmacists and specific methods of support. METHODS: The content of semi-structured interviews with car

Community Health Worker Intervention to Address Social Determinants of Health for Children Hospitalized With Asthma.

BACKGROUND: Social determinants of health (SDOH) contribute to racial disparities in asthma outcomes. Community health worker (CHW) programs represent a promising way to screen for SDOH and connect patients to resources, but the impact of CHW programs in the inpatient pediatric setting has been examined in few studies. In this study, we aimed to evaluate a CHW program for children hospitalized with asthma in a predominantly Hispanic community by examining rates of SDOH and social resource navigation. METHODS: This pilot study involved a CHW intervention to improve pediatric asthma care. Patie

Code Social: Integrating SDoH into Emergency Resident Education.

AUDIENCE: This small group curriculum is geared toward medical students and residents of all levels of training in emergency medicine programs. INTRODUCTION: Emergency departments serve as critical first points of contact for patients with acute health needs, often exacerbated by social determinants of health (SDoH). Incorporating SDoH awareness into emergency medicine practice is essential for addressing immediate health concerns, preventing future crises, and reducing the burden on healthcare systems. By identifying and addressing social factors, emergency providers can break cycles of recu

PlanoUp!: A pilot program for the identification and treatment of depression for youth in low-income secondary schools.

Rates of depression in youth are continuing to increase at a steady rate, yet these youth often do not receive mental health services (Bertha & Balázs, 2013; Thomas et al., 2011). Schools are an ideal setting to connect youth to mental health services; however, many barriers exist with respect to schools having adequate resources and access to the appropriate levels of services (Duong et al., 2021; Owens & Peltier, 2002). Schools may collaborate with local community providers with available resources to address these gaps. The current article describes the pilot of a school-based mental health

Lessons Learned from Implementation of the Food Insecurity Screening and Referral Program at Kaiser Permanente Colorado.

Traditionally, health care systems have addressed gaps in patients' diet quality with programs that provide dietary counseling and education, without addressing food security. However, health care systems increasingly recognize the need to address food security to effectively support population health and the prevention and management of diet-sensitive chronic illnesses. Numerous health care systems have implemented screening programs to identify food insecurity in their patients and to refer them to community food resources to support food security. This article describes barriers encountered

Community pharmacy chlamydia and gonorrhea test-to-treat program: Development of an implementation toolkit.

BACKGROUND: Sexually transmitted infection (STI) surveillance showed more than 2.5 million cases of chlamydia, gonorrhea, and syphilis nationally in the United States in 2022. Individuals often seek out nonemergency medical care at pharmacies. This makes community pharmacies well-positioned to address rising STI rates by offering services to screen and treat common STIs. A local health department, an independent pharmacy, and a school of pharmacy in Pennsylvania partnered to implement a test-to-treat service for chlamydia and gonorrhea within a pharmacy. This pilot program utilized the followi

Community Readiness for Children's Oral Health Improvement Across Three Cities in England.

OBJECTIVES: Childhood caries is a widespread global public health issue, with its prevalence closely reflecting underlying health and social inequalities. Planning effective oral health programmes tailored to the needs of the community is of primary importance. The Community Readiness Model (CRM) provides a methodological approach to quantify the degree to which a community is willing to engage with programmes and thus inform adaptations to maximise engagement and impact. This study aimed to apply the CRM to explore the factors influencing community readiness to improve children's oral health.

Empowering communities: action research through healthy cities.

The Healthy Cities process uses action research to empower communities to take action for health. Five concepts that link community empowerment and action research are: focus on community, citizen participation, information and problem solving, sharing of power, and quality of life. Two city examples from Healthy Cities Indiana, a pilot program of CITYNET Healthy Cities, provide illustrations of these concepts. The dynamics of community participation in action research and the successes and barriers to community participation are presented. Outcomes that empowered the community are suggested:

Barriers and facilitators for community surgery in general practice / family medicine settings: a scoping review.

BACKGROUND: Rising healthcare demand and costs have prompted policymakers to explore strategies for delivering appropriate care closer to patients. In Ireland, Sláintecare supports the transfer of suitable hospital activity to community-based providers, while the United Kingdom's Fit for the Future: 10-Year Health Plan promotes a shift towards community-based care. Community surgery may contribute to this transition, but successful implementation requires appropriate resources, training, governance, and collaboration. This scoping review aimed to synthesise existing evidence on barriers and fa

[The Promotion of Resources Integration in Long-Term Care Service: The Experience of Taipei City Hospital].

The home-based medical care integrated plan under Taiwan National Health Insurance has changed from paying for home-based medical care, home-based nursing, home-based respiratory treatment, and palliative care to paying for a single, continuous home-based care service package. Formerly, physician-visit regulations limited home visits for home-based nursing to providing medical related assessments only. This limitation not only did not provide practical assistance to the public but also caused additional problems for those with mobility problems or who faced difficulties in making visits hospit

Community groups as 'critical enablers' of the HIV response in Zimbabwe.

BACKGROUND: The Investment Framework for a more effective HIV response has become integral to discussions on how best to respond to the HIV epidemic. The Framework calls for greater synergy and attention to factors that serve as 'critical enablers' and optimise HIV programmes. In this paper we argue for recognition of informal and indigenous community groups as 'critical enablers' of the HIV response. METHODS: This qualitative study was conducted in Matobo district of the Matabeleland South province in Zimbabwe. It draws on 19 individual in-depth interviews and 9 focus group discussions condu

Animal shelters: managing heartworms in resource-scarce environments.

Animal shelters must frequently make difficult decisions regarding the allocation of limited resources to appropriately care for the millions of dogs and cats that enter their doors annually. Insufficient staffing, expertise, and guidance on heartworm management in animal shelters creates significant confusion on how these facilities should appropriately address heartworm infection in dogs and cats. The American Heartworm Society (AHS) issues comprehensive guidelines for the diagnosis, prevention, and management of heartworm infection in pets, but shelters are often unable to fully comply with
