A Probabilistic Method to Model Progressive Metatarsal Displacement and Stiffness During Fatigue Testing.

To better understand the mechanisms of bone stress injuries (BSI) in metatarsals, we developed an algorithm that adapts finite element (FE) models of metatarsals to simulate fatigue displacements through progressive stiffness loss. Twenty-two human metatarsals were imaged using computed tomography (CT) and then cyclically loaded in uniaxial compression until failure. CT images were used to generate specimen-specific FE models, and a custom program was developed to iteratively simulate cyclic loading and progressive stiffness loss associated with microdamage accumulation. Probability was incorp

Interpretable vertebral fracture quantification via anchor-free landmarks localization.

Vertebral body compression fractures are early signs of osteoporosis. Though these fractures are visible on Computed Tomography (CT) images, they are frequently missed by radiologists in clinical settings. Prior research on automatic methods of vertebral fracture classification proves its reliable quality; however, existing methods provide hard-to-interpret outputs and sometimes fail to process cases with severe abnormalities such as highly pathological vertebrae or scoliosis. We propose a new two-step algorithm to localize the vertebral column in 3D CT images and then detect individual verteb

Anterior hip pain in the adult: an algorithmic approach to diagnosis.

The adult patient who complains of anterior hip pain is a dilemma frequently encountered by the primary care physician. Detailed history taking, physical examination, and plain x-ray films are indicated for the initial evaluation. Anterior hip pain is often diagnosed as musculoskeletal strain/sprain and treated with a conservative regimen represented by the acronym NICER (nonsteroidal anti-inflammatory drugs, ice, compression, elevation, and rest) with or without physical therapy. On occasion, this therapy fails to eradicate the symptoms. When these symptoms are refractory to diagnosis by conv

Brain-stem auditory evoked responses as indicators of early brain insult.

The relationship between cranial ultrasonograms (SONOs) and brain-stem auditory evoked responses (BAERs) was evaluated in 2 independent samples of newborn infants at risk for brain injury (n = 113 and 203). Features of the BAER wave forms subjected to stepwise linear discriminant analysis formed the basis of an algorithm used to detect and follow early brain injury. Using this algorithm, information derived from BAERs reliably predicted SONO abnormalities at least 82.3% of the time in the initial study which was replicated with the second sample (77.3%). The wave I component latency (CL) and t

A cloud-edge adaptive lightweight network with dynamic inference enables real-time ultrasound image segmentation.

Ultrasound image segmentation plays an important role in clinical diagnosis, but existing deep learning methods struggle to balance accuracy and efficiency, limiting practical deployment in resource-constrained medical scenarios. This study proposes a Cloud-Edge Adaptive Lightweight Network (CEA-Net) with dynamic inference, implementing parameter compression through a lightweight encoder constructed with depthwise separable convolutions and inverted residual structures, introducing serial channel-spatial attention mechanisms to enhance lesion feature representation, and designing intelligent r

A narrative review of the role of AI and machine learning in enhancing cauda equina syndrome diagnosis.

INTRODUCTION: Cauda Equina Syndrome (CES) is a neurological emergency requiring rapid diagnosis. Traditional diagnostic methods face challenges due to variable presentations. This review evaluates the clinical performance, limitations, and validation needs of artificial intelligence and machine learning (AI/ML) compared to traditional CES diagnostic approaches. METHODS: A systematic PRISMA 2020 search of PubMed and ScienceDirect (2019-2025) using "cauda equina" yielded 520 articles. After removing 200 duplicates and 284 non-diagnostic papers, 36 diagnostic studies were identified. Ultimately,

Ten Years After SINS: Role of Surgery and Radiotherapy in the Management of Patients With Vertebral Metastases.

The objective of the different types of treatments for a spinal metastasis is to provide the best oncological and functional result with the least aggressive side effects. Initially created in 2010 to help clinicians in the management of vertebral metastases, the Spine Instability Neoplastic Score (SINS) has quickly found its place in the decision making and the treatment of patients with metastatic spinal disease. Here we conduct a review of the literature describing the different changes that occurred with the SINS score in the last ten years. After a brief presentation of the spinal metasta

Current management of thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) is a condition caused by compression of the neurovascular structures leading to the arm passing through the thoracic outlet. There are three distinct types of TOS: neurogenic (95%), venous (4%-5%), and arterial (1%). Treatment algorithms depend on the type of TOS. Although statistically the most common type, neurogenic TOS can often be the most difficult to diagnose and treat. We have good follow-up data indicating that appropriately selected patients benefit from surgical intervention. Arterial and venous TOS often present more urgently with arterial or venous t

Evaluation of Hemodynamic Data in an Ovine Model to Estimate Optimal Time for Compressor Change during Neonatal Resuscitation.

INTRODUCTION: Survival from cardiac arrest is directly and consistently linked to the quality of chest compressions (CC). Although compressor fatigue is well established, neonatal resuscitation guidelines (NRP) do not offer specific recommendations on rotation intervals beyond suggesting provider changes every 2-5 min. By comparison, Pediatric Advanced Life Support recommends a strict 2-min rotation to prevent fatigue and maintain high-quality CC. METHODS: Prospective observational data from resuscitation of asphyxiated cardiac-arrested neonatal lambs to evaluate changes in chest compression

Degree of distal trigeminal nerve atrophy predicts outcome after microvascular decompression for Type 1a trigeminal neuralgia.

OBJECT: Trigeminal neuralgia is often associated with nerve atrophy, in addition to vascular compression. The authors evaluated whether cross-sectional areas of different portions of the trigeminal nerve on preoperative imaging could be used to predict outcome after microvascular decompression (MVD). METHODS: A total of 26 consecutive patients with unilateral Type 1a trigeminal neuralgia underwent high-resolution fast-field echo MRI of the cerebellopontine angle followed by MVD. Preoperative images were reconstructed and reviewed by 2 examiners blinded to the side of symptoms and clinical out

Experience with a DICOM-compatible digital pediatric echocardiography laboratory.

A digital pediatric echocardiography laboratory, without videotape redundancy was established at Children's Hospital of Wisconsin in December 1998. To characterize the experience, 1198 consecutive patient studies were reviewed-50% from the first 2 months after establishing the digital protocol and 50% from the last 2 months available. Each study was stored using a protocol that was based on capture of single beat clips of relevant two-dimensional/color Doppler imaging and static frame spectral Doppler tracings. Studies were digitally compressed using a DICOM-compatible JPEG algorithm at 20:1 a

Determination of elastic moduli from measured acoustic velocities.

Methods are evaluated in solution of the inverse problem associated with determination of elastic moduli for crystals of arbitrary symmetry from elastic wave velocities measured in many crystallographic directions. A package of MATLAB functions provides a robust and flexible environment for analysis of ultrasonic, Brillouin, or Impulsive Stimulated Light Scattering datasets. Three inverse algorithms are considered: the gradient-based methods of Levenberg-Marquardt and Backus-Gilbert, and a non-gradient-based (Nelder-Mead) simplex approach. Several data types are considered: body wave velocitie

Composition and micromechanical properties of the femoral neck compact bone in relation to patient age, sex and hip fracture occurrence.

Current clinical methods of bone health assessment depend to a great extent on bone mineral density (BMD) measurements. However, these methods only act as a proxy for bone strength and are often only carried out after the fracture occurs. Besides BMD, composition and tissue-level mechanical properties are expected to affect the whole bone's strength and toughness. While the elastic properties of the bone extracellular matrix (ECM) have been extensively investigated over the past two decades, there is still limited knowledge of the yield properties and their relationship to composition and arch
