Sinus Bradycardia in Carriers of the SCN5A-1795insD Mutation: Unraveling the Mechanism through Computer Simulations.

The SCN5A gene encodes the pore-forming α-subunit of the ion channel that carries the cardiac fast sodium current (INa). The 1795insD mutation in SCN5A causes sinus bradycardia, with a mean heart rate of 70 beats/min in mutation carriers vs. 77 beats/min in non-carriers from the same family (lowest heart rate 41 vs. 47 beats/min). To unravel the underlying mechanism, we incorporated the mutation-induced changes in INa into a recently developed comprehensive computational model of a single human sinoatrial node cell (Fabbri-Severi model). The 1795insD mutation reduced the beating rate of the mo

The influence of simulator input conditions on the wear of total knee replacements: An experimental and computational study.

Advancements in knee replacement design, material and sterilisation processes have provided improved clinical results. However, surface wear of the polyethylene leading to osteolysis is still considered the longer-term risk factor. Experimental wear simulation is an established method for evaluating the wear performance of total joint replacements. The aim of this study was to investigate the influence of simulation input conditions, specifically input kinematic magnitudes, waveforms and directions of motion and position of the femoral centre of rotation, on the wear performance of a fixed-bea

Sinus irrigations before and after surgery-Visualization through computational fluid dynamics simulations.

OBJECTIVES/HYPOTHESIS: Topical sinus irrigations play a critical role in the management of sinonasal disease, and the improvement in irrigant penetration into the sinuses postoperatively greatly contributes to the success of endoscopic sinus surgery. Prior investigations on postoperative sinus irrigations have been mostly limited to cadaver studies, which are labor intensive and do not capture the full dynamics of the flows. A pilot study was conducted to investigate the impact of surgery on sinus irrigation through computational fluid dynamics (CFD) simulations. STUDY DESIGN: Retrospective c

Hemodynamics in stented vertebral artery ostial stenosis based on computational fluid dynamics simulations.

Hemodynamic factors may affect the potential occurrence of in-stent restenosis (ISR) after intervention procedure of vertebral artery ostial stenosis (VAOS). The purpose of the present study is to investigate the influence of stent protrusion length in implantation strategy on the local hemodynamics of the VAOS. CTA images of a 58-year-old female patient with posterior circulation transient ischemic attack were used to perform a 3D reconstruction of the vertebral artery. Five models of the vertebral artery before and after the stent implantation were established. Model 1 was without stent impl

Flow disturbances in stent-related coronary evaginations: a computational fluid-dynamic simulation study.

AIMS: Angiographic ectasias and aneurysms in stented segments have been associated with late stent thrombosis. Using optical coherence tomography (OCT), some stented segments show coronary evaginations reminiscent of ectasias. The purpose of this study was to explore, using computational fluid-dynamic (CFD) simulations, whether OCT-detected coronary evaginations can induce local changes in blood flow. METHODS AND RESULTS: OCT-detected evaginations are defined as outward bulges in the luminal vessel contour between struts, with the depth of the bulge exceeding the actual strut thickness. Evagi

Recent advances in 3D computed tomography techniques for simulation and navigation in hepatobiliary pancreatic surgery.

A few years ago it could take several hours to complete a 3D image using a 3D workstation. Thanks to advances in computer science, obtaining results of interest now requires only a few minutes. Many recent 3D workstations or multimedia computers are equipped with onboard 3D virtual patient modeling software, which enables patient-specific preoperative assessment and virtual planning, navigation, and tool positioning. Although medical 3D imaging can now be conducted using various modalities, including computed tomography (CT), magnetic resonance imaging (MRI), positron emission tomography (PET)

Comparing the structural properties of human and rat islet amyloid polypeptide by MD computer simulations.

Conformational properties of the full-length human and rat islet amyloid polypeptide 1-37 (amyloidogenic hIAPP and non-amyloidogenic rIAPP, respectively) were studied at 310 and 330 K by MD simulations both for the cysteine (reduced IAPP) and cystine (oxidized IAPP) moieties. At all temperatures studied, IAPP does not adopt a well-defined conformation and is essentially random coil in solution, although transient helices appear forming along the peptide between residues 8 and 22, particularly in the reduced form. Above the water percolation transition (at 320 K), the reduced hIAPP moiety prese

Validation of computational fluid dynamics methodology used for human upper airway flow simulations.

An anatomically accurate human upper airway model was constructed from multiple magnetic resonance imaging axial scans. This model was used to conduct detailed Computational Fluid Dynamics (CFD) simulations during expiration, to investigate the fluid flow in the airway regions where obstruction could occur. An identical physical model of the same airway was built using stereo lithography. Pressure and velocity measurements were conducted in the physical model. Both simulations and experiments were performed at a peak expiratory flow rate of 200 L/min. Several different numerical approaches wit

Self-assembly of peptide scaffolds in biosilica formation: computer simulations of a coarse-grained model.

The self-assembly of model peptides is studied using Brownian dynamics computer simulations. A coarse-grained, bead-spring model is designed to mimic silaffins, small peptides implicated in the biomineralization of certain silica diatom skeletons and observed to promote the formation of amorphous silica nanospheres in vitro. The primary characteristics of the silaffin are a 15 amino acid hydrophilic backbone and two modified lysine residues near the ends of the backbone carrying long polyamine chains. In the simulations, the model peptides self-assemble to form spherical clusters, networks of

Development of computer-based environment for simulating the voluntary upper-limb movements of persons with disability.

Upper-limb orthotic systems have been designed for restoring the upper-limb functions of individuals with disabilities resulting from spinal cord injury (SCI), stroke and muscular dystrophy. These systems employ either functional electrical stimulation or external power. It is proposed that, instead of time-consuming and complicated monitoring using sensors and motion analysis, a software simulator with both angular displacement and acceleration parameters can facilitate the design of a control strategy for an orthosis. Reaching movements of three cervical SCI subjects are used to verify the s

Importance of accurate geometry in the study of the total cavopulmonary connection: computational simulations and in vitro experiments.

Previous in vitro studies have shown that total cavopulmonary connection (TCPC) models incorporating offset between the vena cavae are energetically more efficient than those without offsets. In this study, the impact of reducing simplifying assumptions, thereby producing more physiologic models, was investigated by computational fluid dynamics (CFD) and particle flow visualization experiments. Two models were constructed based on angiography measurements. The first model retained planar arrangement of all vessels involved in the TCPC but incorporated physiologic vessel diameters. The second m

Doublets in motor unit activity of human forearm muscle during simulated computer work.

Motor unit firing pattern and characteristics were analyzed during voluntary contractions as they occur during computer work. For comparison controlled standard ramp contractions were performed. Highly diverse firing patterns were identified while the MU characteristics were similar. Of special notice is the frequent occurrence of double discharges during computer like work but not during ramp contractions.

A computer model for describing the effect of urethral afferents on simulated lower urinary tract function.

A computer model of mechanical properties of the bladder, the urethra and the rhabdosphincter, as well as their neural control is presented in this paper. The model has a rather simple design and processes sensory information from both the bladder wall tension and urethral stretch. It is assumed that afferent signals from the urethra are involved in a sacral excitatory reflex and a supraspinal inhibitory reflex. Pressure and flow signals that resemble experimentally measured normal human behaviour could be simulated with this model. From these simulations the relation between the neural contro

Computational simulation of turbulent signal loss in 2D time-of-flight magnetic resonance angiograms.

Time-of-flight magnetic resonance (MR) angiography is currently limited in the evaluation of arterial stenoses by flow-induced signal loss. This signal loss has been attributed to phase dispersion and to phase misregistration. We have developed a fluid mechanics model of 2D time-of-flight MR angiograms to study the amount of signal loss caused by random turbulence. The simulations were created by stochastic analysis of particle pathlines determined by computational fluid dynamics for turbulent flow. The images obtained by the model compare well to actual MR images of flow in stenoses. By selec

Oto-traumatic effects of computer simulated magnetic coil impulse noise: analysis of mechanisms.

The brief impulse noise artifacts of 1.0 ms or less generated by some magnetic coils used in extracranial magnetic stimulation may induce acoustic trauma. We investigated the effects of these magnetic coil acoustic artifacts (MCAA) on the inner ear by exposing rabbits to computer stimulated impulse noise designed to mimic the impulse noise of the coil in spectrum and acoustic energy. The simulated impulse noise stimuli (50 impulses) were varied in maximum peak sound pressure (160, 157, and 155 dB re: 20 muPascal), rise-time (100 microseconds and 1,000 microseconds) and duration. The frequency

Delineating the structural distortions in hIAPP protofibrils by rationally designed hexapeptides using computer simulations.

Disease-associated fibrillation of human islet amyloid polypeptide (hIAPP), also referred to as human amylin, plays a pivotal role in the etiology of type 2 diabetes (T2D). The hIAPP fibrillation leads to the generation of cytotoxic β-sheet-rich oligomers and protofibrils that contribute to the apoptosis of pancreatic β-cells. Potter et al. reported hIAPP fragment peptide ANFLVH (residues 13-18, designated as AF6) as a potent inhibitor of hIAPP fibrillation. Notably, AF6 blocked IAPP fibrillation in vitro and in human islet cell cultures, which noticeably enhanced the viability of islet cells.
