Radiographic evaluation of the patient with lung cancer: surgical implications of imaging.

Lung cancer is the leading cause of cancer deaths in the United States. Despite many advances in treatment, surgery remains the preferred treatment modality for patients presenting with early stage disease. Imaging is critical in the preoperative evaluation of these patients being considered for a curative resection. Advanced imaging techniques provide valuable information, including primary diagnostics, staging, and intraoperative localization for suspected lung cancer. Knowledge of surgical implications of imaging findings can aid both radiologists and surgeons in delivering safe and effecti

A Kohonen neural network description of scoliosis fused regions and their corresponding Lenke classification.

PURPOSE: Surgical instrumentation for adolescent idiopathic scoliosis (AIS) is a complex procedure where selection of the appropriate curve segment to fuse, i.e., fusion region, is a challenging decision in scoliosis surgery. Currently, the Lenke classification model is used for fusion region evaluation and surgical planning. Retrospective evaluation of Lenke classification and fusion region results was performed. METHODS: Using a database of 1,776 surgically treated AIS cases, we investigated a topologically ordered self organizing Kohonen network, trained using Cobb angle measurements, to d

CBCT-based adaptive radiotherapy and automated radiotherapy decision-making systems: development and current status.

Cone-beam computed tomography (CBCT)-based adaptive radiotherapy (ART) has emerged as a promising strategy for addressing inter-fractional anatomical and physiological changes during treatment. By incorporating on-treatment imaging, ART enables treatment plans to be adapted to the anatomy of the day, with the potential to improve target coverage and reduce dose exposure to organs at risk (OARs). However, routine implementation remains constrained by the limited image quality of CBCT, uncertainties in dose calculation, labour-intensive workflows and the lack of reliable criteria for determining

Meeting housing needs of child welfare-involved families: Policy insights from simulation modeling.

BACKGROUND: Inadequate housing contributes to the risk of family separation in nearly one-quarter of child maltreatment investigations. Child welfare struggles to identify and address the demand for housing assistance. A range of housing interventions shows promise for stabilizing families. Still, aid remains difficult to access, and little evidence exists for prioritizing households to interventions. Inefficient decisions about who to serve with scarce housing resources threaten to diminish resources and unintentionally place children at greater risk. OBJECTIVE: The present study leverages c

Optimizing multiple diagnostic modalities for small bowel evaluation: a strategic approach.

Over the past two decades, the small bowel, once considered a diagnostic "blind spot," has become fully and comprehensively accessible. This review summarizes recent advances in the main diagnostic modalities: capsule endoscopy (CE), device-assisted enteroscopy (DAE), computed tomography enterography (CTE), magnetic resonance enterography (MRE), and intestinal ultrasound (IUS). CE provides a noninvasive assessment of mucosal abnormalities, whereas DAE offers the added benefits of tissue sampling and therapeutic intervention. CTE and MRE provide complementary cross-sectional information on mura

Computer vision for automatic identification of blastocyst structures and blastocyst formation time in In-Vitro Fertilization.

Embryo selection is an indispensable step to ensure the success of In-Vitro Fertilization; however, this decision is a time-consuming, laborious, and highly subjective task for embryologists. In the best scenarios, when implanting an embryo of the best quality, the probability of pregnancy rate is just 34.1%. Automatic segmentation provides detailed, quantitative, and objective information about the embryo, reducing the workload of embryologists and enhancing the success rate of embryo implantation. As such, it represents a valuable resource in the field of assisted reproduction. Towards that

Dexmedetomidine-Assisted Anesthetic Management of Empyema in a High-Risk Parturient With Cardiopulmonary Failure: A Case Report.

Empyema, characterized by purulent fluid collection in the pleural space, presents treatment challenges. Video-assisted thoracoscopic surgery (VATS) and computed tomography (CT)-guided chest tube drainage are the main options, each with distinct anesthetic considerations. We describe the case of a 33-year-old pregnant woman (G8P8) at 29 weeks of gestation with a large left-sided empyema, cardiopulmonary failure, and a history of complex obstetric complications, including seven previous cesarean sections, placenta accreta, and placenta previa. The use of fuzzy logic to select which curves need to be instrumented and fused in adolescent idiopathic scoliosis: a feasibility study.

Selection of the appropriate curve fusion levels for surgery in adolescent idiopathic scoliosis (AIS) is a complex and difficult task. Despite numerous publications on this subject, the decision as to which spinal curve, that is proximal thoracic, main thoracic or lumbar, needs to be instrumented and included in the fusion relies mostly on each surgeon's past experience, although recently published data have revealed a high variability of spinal instrumentation configurations among spinal surgeons in AIS. This situation exists because of ambiguity and vagueness in the decision process. Our obj

Control theory as a conceptual framework for intensive care monitoring.

Monitoring patients hospitalized in hemato-oncology departments to undergo clinical protocols of therapy is a complex task. The main difficulty arises in the management of the oncology protocol as well as in the management of critical episodes of acute illness which frequently occur due to high toxicity of the used antimitotics. This problem of controlling a patient's condition can be conceptualized within the control theory paradigm as the task of controlling a process whose state changes over time and can deviate unacceptably from a normal range. Upon request of a French clinical department

Ventricular geometry-regularized QRSd predicts cardiac resynchronization therapy response: machine learning from crosstalk between electrocardiography and echocardiography.

Up to one-third of patients selected by current guidelines do not respond to cardiac resynchronization therapy (CRT), the aim of this study was to find out novel analytical approaches to improve pre-implantation CRT response prediction. Among 31 pre-implantation features of clinical, laboratory, electrocardiography (ECG), and echocardiography variables in a consecutive cohort of patients receiving a first-time CRT device (CRT-pacemaker or CRT-defibrillator), we developed a machine learning (ML) model with three classification algorithms (support vector machines (SVM), K nearest neighbors, and

Mapping cognitive activity from electrocorticography field potentials in humans performing NBack task.

Objective. Advancements in data science and assistive technologies have made invasive brain-computer interfaces (iBCIs) increasingly viable for enhancing the quality of life in physically disabled individuals. Intracortical microelectrode implants are a common choice for such a communication system due to their fine temporal and spatial resolution. The small size of these implants makes the implantation plan critical for the successful exfiltration of information, particularly when targeting representations of task goals that lack robust anatomical correlates.Approach. GuideNav: User-Informed Development of a Vision-Only Robotic Navigation Assistant for Blind Travelers.

While commendable progress has been made in user-centric research on mobile assistive systems for blind and low-vision (BLV) individuals, references that directly inform robot navigation design remain rare. To bridge this gap, we conducted a comprehensive human study involving interviews with 26 guide dog handlers, four white cane users, nine guide dog trainers, and one O&M trainer, along with 15+ hours of observing guide dog-assisted walking. After de-identification, we open-sourced the dataset to promote human-centered development and informed decision-making for assistive systems for BLV pe

Patient navigation to promote lung cancer screening in a community health center for people experiencing homelessness: Protocol for a pragmatic randomized controlled trial.

BACKGROUND: Lung cancer is a major cause of death among people experiencing homelessness, with mortality rates more than double those in the general population. Lung cancer screening (LCS) with low-dose computed tomography (LDCT) could reduce lung cancer deaths in this population, although the circumstances of homelessness present multiple barriers to LCS LDCT completion. Patient navigation is a promising strategy for overcoming these barriers. METHODS: The Investigating Navigation to Help Advance Lung Equity (INHALE) Study is a pragmatic randomized controlled trial of patient navigation for
