Hyperbaric treatment of air or gas embolism: current recommendations.

Gas can enter arteries (arterial gas embolism, AGE) due to alveolar-capillary disruption (caused by pulmonary over-pressurization, e.g. breath-hold ascent by divers) or veins (venous gas embolism, VGE) as a result of tissue bubble formation due to decompression (diving, altitude exposure) or during certain surgical procedures where capillary hydrostatic pressure at the incision site is subatmospheric. Both AGE and VGE can be caused by iatrogenic gas injection. AGE usually produces stroke-like manifestations, such as impaired consciousness, confusion, seizures and focal neurological deficits. S

Patent foramen ovale (PFO): is there life before death in the presence of PFO?

Patent foramen ovale (PFO) is an embryologic remnant with incomplete postnatal adhesion of the cardiac atrial septum primum and secundum. After birth, the prevalence of PFO decreases from about 35% at young to approximately 20% at old age. PFO has been associated with numerous conditions such as decompression illness in divers, migraine, high-altitude pulmonary oedema, cerebrovascular and coronary ischaemia, and obstructive sleep apnoea syndrome. PFO is the cause of intermittent atrial right-to-left shunt, and it can be the source of cardiac paradoxical embolism. So far, randomized controlled

[Breath-hold diving--an increasing adventure sport with medical risks].

Breath-hold diving as a recreational and competitive sports activity is on the increase. In this review physiological limitations and medical risks associated with breath-hold diving are discussed. Specific topics include hypoxia, ascent blackout, hyperventilation, squeeze or barotrauma of descent including effects on the pulmonary system, glossopharyngeal breathing, and decompression illness. It is also concluded that the health requirements for competitive breath-hold diving should follow essentially the same standards as used for SCUBA-diving.

Massive portal venous gas embolism after scuba diving.

INTRODUCTION: Portal venous gas from a diving injury is an infrequent finding and only a few cases are described. We report a case of severe decompression sickness (DCS) associated with a massive amount of gas in the portal and mesenteric veins. CASE REPORT: A 49-year-old man suffered from DCS after two deep dives on the same day. He presented with cutaneous, neurological and pulmonary symptoms associated with hypoxaemia. He had no abdominal pain. A computed tomography (CT) scan showed large quantities of hepatic and portal venous gas and excluded other explanations for its presence. All symp

Serum tau concentration after diving - an observational pilot study.

INTRODUCTION: Increased concentrations of tau protein are associated with medical conditions involving the central nervous system, such as Alzheimer's disease, traumatic brain injury and hypoxia. Diving, by way of an elevated ambient pressure, can affect the nervous system, however it is not known whether it causes a rise in tau protein levels in serum. A prospective observational pilot study was performed to investigate changes in tau protein concentrations in serum after diving and also determine their relationship, if any, to the amount of inert gas bubbling in the venous blood. METHODS: S

Suspected arterial gas embolism after glossopharyngeal insufflation in a breath-hold diver.

INTRODUCTION: Many competitive breath-hold divers employ the technique of glossopharyngeal insufflation in order to increase their lung gas volume for a dive. After a maximal inspiration, using the oral and pharyngeal muscles repeatedly, air in the mouth is compressed and forced into the lungs. Such overexpansion of the lungs is associated with a high transpulmonary pressure, which could possibly cause pulmonary barotrauma. CASE REPORT: We report a case of transient neurological signs and symptoms occurring within 1 min after glossopharyngeal insufflation in a breath-hold diver. He complained

The indigenous fisherman divers of Thailand: in-water recompression.

BACKGROUND: The Urak Lawoi, part of the Sea Gypsies of Thailand, have been diving using surface-supplied compressed air for more than 30 years. Their dive sites range from one hour to several days from their villages. Similar to other indigenous fisherman divers, the Urak Lawoi suffer from a high incidence of decompression illness. Their methods of in-water recompression were investigated. METHODS: In December 1998, available divers in two Urak Lawoi villages were asked if they had ever been treated using in-water recompression following decompression illness. If the divers responded positive

Decompression at 1.3 versus 1.6 bar and Nitrogen Elimination and Venous Gas Emboli: A Randomized Controlled Trial.

INTRODUCTION: The optimal depth for decompression stops is unclear. We hypothesize that a decompression stop at 1.3 bar, compared with 1.6 bar, decreases post-dive whole-body nitrogen washout volumes and venous gas emboli (VGE). METHODS: In this randomized crossover trial, divers performed wet air dives of 40 minutes at 3.4 bar (340 kPa) with a seven-minute-long decompression stop at either 1.3 bar (Deco 1.3) or 1.6 bar (Deco 1.6) in randomized order. The primary outcome was the difference in post-dive whole body nitrogen washout volume, analyzed using multilevel linear regression. The second

Acute coronary syndrome and decompression illness: a challenge for the diving physician.

Decompression illness (DCI) is a syndrome with diverse clinical manifestations but in which cardiac symptoms are rare. In the presence of cardiac symptoms, the necessity to rule out an acute coronary syndrome (ACS) which requires prompt treatment may result in delay to appropriate recompression treatment. We describe three cases with cardiologic symptoms referred to our centre by the Emergency Department (ED) of our facility. The first was a 48-year-old woman who lost consciousness during a dive and required cardiopulmonary resuscitation. The final diagnosis was acute myocardial infarction and

The ultrastructural morphology of air embolism: platelet adhesion to the interface and endothelial damage.

The pathogenesis of the ill effects following air embolism cannot be attributed solely to the space occupying and surface tension effects of the air bubbles altering the normal flow of blood through the vasculature. Decompression sickness was induced in rats and the following features of this process observed by electron microscopy in the vessels of the mesentery: imprisonment of blood elements (especially platelets) took place within the various enclosures created by the boundaries set up by different sized air bubbles between the layer of blood and the vessel walls, and the air/blood interfa

[Clinical value of hyperbaric oxygen therapy for the treatment of chronic wounds].

The physical and biological effects of hyperbaric oxygen therapy (HBOT) are used to treat various medical conditions, such as decompression sickness after diving accidents or in cases of gas embolism or CO-poisoning. Another, generally less well-known indication for HBOT is the use of HBOT in supporting the healing of chronic wounds. HBOT corrects tissue hypoxia, stimulates angiogenesis and collagen synthesis, reduces environmental oedema and has anti-inflammatory and anti-microbial effects. Various clinical studies have shown a positive effect of HBOT on wound healing, particularly in diabeti

Neurological complications of underwater diving.

The diver's nervous system is extremely sensitive to high ambient pressure, which is the sum of atmospheric and hydrostatic pressure. Neurological complications associated with diving are a difficult diagnostic and therapeutic challenge. They occur in both commercial and recreational diving and are connected with increasing interest in the sport of diving. Hence it is very important to know the possible complications associated with this kind of sport. Complications of the nervous system may result from decompression sickness, pulmonary barotrauma associated with cerebral arterial air embolism

Gynecomastia following spinal cord disorder.

Gynecomastia, an excessive development of the mammary glands in men, is a known phenomenon among patients with spinal cord disorder, yet in the last 50 years it has not been fully described in relation to spinal cord disorder. Over a period of 2 years, six patients with spinal cord disorder (4 secondary to a traumatic injury, 1 to decompression sickness, and 1 to transverse myelitis) manifested gynecomastia. The onset of gynecomastia occurred between 1 to 6 months after injury. These patients are presented along with a review of the possible causes for gynecomastia and a suggested workup routi
