The utility and limitations of FRAX: A US perspective.

The FRAX calculator is a major achievement in terms of our understanding of measuring fracture risk. Along with being an easily accessible web-based tool, it is the only model based on extensive data on multiple cohorts. FRAX will help clinicians identify individuals who need osteoporosis treatments, while also screening out those who do not require osteoporosis treatments. However, FRAX is limited by a number of factors. Although it is web based, few physicians have the means to access it. It also assumes that body mass index and mortality are constant across different racial and ethnic group

ULK1 mediated autophagy in airway cells during Aspergillus infection.

INTRODUCTION: Aspergillus fumigatus is a major airborne fungal pathogen that causes invasive aspergillosis in immunocompromised individuals. This study aims to elucidate the autophagic mechanisms activated following the internalization of A. fumigatus conidia in human bronchial epithelial cells. METHODS: Specifically, we investigated the role of ULK1 and the autophagic processes in Beas2B cells upon A. fumigatus conidia internalization. The Beas2B cell line was used to assess the protein expression of ULK1, phosphorylated ULK1, and LC3-I/II via Western blotting. Autophagosome structures were

Microbiota-derived self-adjuvanting exopolysaccharide-based codelivery system for potent cancer immunotherapy.

Cancer vaccines hold great promise by eliciting antigen-specific T cell responses against tumors; however, their efficacy remains limited by poor coordination between antigen delivery and innate immune activation. Here, microbiota-derived metabolites (exopolysaccharides, EPSs) are presented as a self-adjuvanting codelivery system that simultaneously delivers the Toll-like receptor 7/8 (TLR7/8) agonist R848 and ovalbumin (OVA). This bi-adjuvant nanovaccine (EPS@R848/OVA) rapidly accumulates in draining lymph nodes (dLNs) as early as 6 h after administration and significantly enhances the uptake

B Cell Recognition of Candida albicans Hyphae via TLR 2 Promotes IgG1 and IL-6 Secretion for TH17 Differentiation.

Candida albicans is usually a benign member of the human gut microbiota, but can become pathogenic under certain circumstances, for example in an immunocompromised host. The innate immune system, in particular neutrophils and macrophages, constitutes a crucial first line of defense against fungal invasion, however adaptive immunity may provide long term protection and thus allow vaccination of at risk patients. While TH1 and TH17 cells are important for antifungal responses, the role of B cells and antibodies in protection from C. albicans infection is less well defined. In this study, we show

P2Y6 signaling in alveolar macrophages prevents leukotriene-dependent type 2 allergic lung inflammation.

Antagonists of the type 1 cysteinyl leukotriene receptor (CysLT1R) are widely used to treat asthma and allergic rhinitis, with variable response rates. Alveolar macrophages express UDP-specific P2Y6 receptors that can be blocked by off-target effects of CysLT1R antagonists. Sensitizing intranasal doses of an extract from the house dust mite Dermatophagoides farinae (Df) sharply increased the levels of UDP detected in bronchoalveolar lavage fluid of mice. Conditional deletion of P2Y6 receptors before sensitization exacerbated eosinophilic lung inflammation and type 2 cytokine production in resp

Auricularia auricula's Polysaccharides Confer Protection in Murine Sepsis via Immune Modulation.

Sepsis is a life-threatening condition characterized by severe organ dysfunction resulting from an uncontrolled host response to infection. Sepsis treatment poses a challenge due to its complexity and the need for effective therapeutic approaches. The objective of this study was to evaluate the immunomodulatory and antimicrobial effects of exopolysaccharides (EPS) from Auricularia auricula in a murine model of lethal sepsis. Our findings demonstrated that treatment with EPS significantly enhances the proliferation of circulating lymphocytes and granulocytes, which is associated with improved h

Exopolysaccharides of Ganoderma lucidum Activate the NLRP3 Inflammasome, Modulating Phagocytes and the Response of Mice to Infection with Cryptococcus neoformans.

Interleucin-1β (IL-1β) is a pivotal cytokine in pro-inflammatory response induction, and it is regarded as protective in various diseases. IL-1β secretion is mainly associated with activation of the inflammasome complex, which normally is tightly regulated, depending on two signals for significant activity. The water insoluble fraction of the mushroom Ganoderma lucidum was extracted and co-cultivated with murine bone-marrow macrophages. We demonstrated that G. lucidum exopolysaccharides (EPS) were able to induce IL-1β production and release in murine macrophages. The mechanisms underlying EPS-

The Major Chromoblastomycosis Etiologic Agent Fonsecaea pedrosoi Activates the NLRP3 Inflammasome.

Fonsecaea pedrosoi is the main etiologic agent of chromoblastomycosis (CBM), one of the most prevalent subcutaneous mycosis in tropical and subtropical countries. CBM is a poorly characterized chronic infection that commonly starts after transcutaneous inoculation of conidia and saprophytic hyphae of F. pedrosoi. Recently, we have shown that unlike conidia, hyphae and muriform cells (the parasitic morphotype) of F. pedrosoi promotes an intense inflammatory response pattern in vivo, which comprises the production of an inflammasome-derived cytokine, IL-1β. Nonetheless, the mechanisms underlying

MyD88-dependent signaling drives host survival and early cytokine production during Histoplasma capsulatum infection.

The ability of the innate immune system to trigger an adaptive T cell response is critical to resolution of infection with the fungal pathogen Histoplasma capsulatum. However, the signaling pathways and cell types involved in the recognition of and response to this respiratory pathogen remain poorly defined. Here, we show that MyD88, an adaptor protein vital to multiple innate immune pathways, is critically required for the host response to Histoplasma. MyD88-deficient (MyD88-/-) mice are unable to control the fungal burden and are more sensitive to Histoplasma infection than wild-type, Dectin

MyD88 is dispensable for resistance to Paracoccidioides brasiliensis in a murine model of blood-borne disseminated infection.

We have studied the role of MyD88, an adaptor protein of Toll-like receptors (TLRs), in murine defenses against Paracoccidioides brasiliensis in a model of blood-borne disseminated infection. Wild-type (WT) and MyD88-deficient mice infected intravenously with P. brasiliensis yeast cells showed an equivalent fungal burden, as well as similar levels of proinflammatory IL-1beta, IL-6, IL-12p70, tumor necrosis factor (TNF)-alpha and MIP-2, T-helper type 1 (Th1) (IFN-gamma) and Th2 cytokines (IL-4) in tissue homogenates. Ethanol modulates the effector functions of human monocyte-derived macrophages in response to Paracoccidioides brasiliensis yeast cells.

We aimed to investigate the effects of ethanol and its metabolites (β-hydroxybutyrate and sodium acetate) in the effector functions of macrophages in response to Paracoccidioides brasiliensis yeast cells and to determine their influence in the development of the adaptive response. Purified peripheral blood monocytes were differentiated into macrophages and were treated with ethanol, β-hydroxybutyrate, and sodium acetate, and stimulated with P. brasiliensis yeast cells and evaluated for their phenotypic characteristics, functional activity, and capability to induce T cells activation/differenti

Small but mighty: the human mycobiome as an immunopharmacological regulator of inflammation and autoimmunity.

The human mycobiome comprising commensal fungi such as Saccharomyces, Candida, Malassezia and others constitutes less than 0.1 percent of total gut microbes yet exerts outsized influence on host immune homeostasis. Fungal cell-wall components (β-glucans, mannans, chitin) engage C-type lectin receptors (Dectin-1/2), Toll-like receptors and NOD-like receptors to initiate Syk-CARD9, NF-κB and inflammasome signaling. These pathways shape both pro-inflammatory Th17/IL-22 responses that reinforce mucosal barrier integrity and anti-inflammatory IL-10/Treg circuits that promote immunotolerance. Commen

β-Glucan-Modified Liposomes for Microfold Cell-Targeted Oral Delivery of Food-Derived Angiotensin I-Converting Enzyme Inhibitory Peptides Using Microfluidics.

This study presents a novel oral delivery system for food-derived angiotensin I-converting enzyme (ACE) inhibitory peptides by integrating β-glucan-functionalized liposomes with microfluidic encapsulation and receptor-mediated targeting. Two peptides, RADHPFL and YAEERYPIL derived from egg white proteins, were encapsulated into liposomes (composed of lecithin, cholesterol, and DSPE-β-glucan) via a controlled microfluidic self-assembly process. Encapsulation significantly improved the gastrointestinal stability of the peptides and preserved their ACE inhibitory activity compared to free peptide

The inhibitory effect of oral administration of lentinan on DSS-induced inflammation is exerted by the migration of T cells activated in the ileum to the colon.

Oral administration of lentinan ameliorated dextran sulfate sodium (DSS)-induced colitis through Dectin-1 receptor on intestinal epithelial cells. However, it is unclear where lentinan affects in the intestine to prevent the inflammation. We found that the administration of lentinan has induced migration of CD4+ cells from the ileum to the colon by using Kikume Green-Red (KikGR) mice in this study. This result suggests that the oral lentinan treatment could accelerate the migration of Th cells in lymphocyte from ileum into the colon during lentinan intake. Then, C57BL/6 mice were administered

Incorporating the Detection of Single Nucleotide Polymorphisms Associated With Invasive Aspergillosis Into the Clinic.

Exposure to fungi is inevitable, yet only a small number of patients with significant clinical risk develop invasive aspergillosis (IA). While timing of exposure in relation to immune status, environmental and occupational factors will influence the probability of developing IA, factors specific to the individual will likely play a role and variation in the host's genetic code associated with the immunological response to fungi have been linked to increased risk of developing IA. Screening for SNPs in genes significantly associated with IA (e.g. Pentraxin-3, Toll-like receptor 4, Dectin-1, DC-

Investigation of Aspergillus flavus in animal virulence.

Aspergillus flavus is a common fungal pathogen of plants, animals and humans. Recently, many genes of A. flavus have been reported involving in regulation of pathogenesis in crops, but whether these genes are involved in animal virulence is still unknown. Here, we used a previous easy-to-use infection model for A. flavus based on mouse model by intravenous inoculation of A. flavus conidia. The outcome of infections in mice model showed that A. flavus NRRL3357 and laboratory strain CA14 PTS were both in dose dependent manner and highly reproducible. The progress of disease could be monitored by

Cellular and cytokine changes in the alveolar environment among immunocompromised patients during Pneumocystis jirovecii infection.

Limited data exist on the cytokine and cellular changes in the alveolar environment in immunocompromised patients during Pneumocystis jirovecii infection. A cellular and a cytokine analysis were performed on bronchoalveolar lavage (BAL) samples from three groups of patients, i.e., an initial study group of 64 immunocompromised P. jirovecii-positive individuals and two control groups of P. jirovecii-negative patients who had been or not immunosuppressed (65 patients). The results were related to alveolar dilution as determined by urea measurement. Compared with non-infected groups, P. jirovecii

Th17 cells, Job's syndrome and HIV: opportunities for bacterial and fungal infections.

PURPOSE OF REVIEW: Patients with hyper IgE syndrome (HIES) share with HIV patients a predisposition to infections, including candidiasis in autosomal dominant HIES (AD-HIES) and molluscum contagiosum and other viral infections in other disorders of elevated IgE with infectious predilections. This review highlights the underlying pathogenesis of these diseases and their relevance to HIV infection. RECENT FINDINGS: Patients with mutations in STAT3, who lack Th17 cells, develop AD-HIES, whereas other disorders of elevated IgE may be caused by mutations in Tyk2 or DOCK8, the latter of which is as

D-galacto-D-mannan-mediated Dectin-2 activation orchestrates potent cellular and humoral immunity as a viral vaccine adjuvant.

INTRODUCTION: Conventional foot-and-mouth disease (FMD) vaccines have been developed to enhance their effectiveness; however, several drawbacks remain, such as slow induction of antibody titers, short-lived immune response, and local side effects at the vaccination site. Therefore, we created a novel FMD vaccine that simultaneously induces cellular and humoral immune responses using the Dectin-2 agonist, D-galacto-D-mannan, as an adjuvant. METHODS: We evaluated the innate and adaptive (cellular and humoral) immune responses elicited by the novel FMD vaccine and elucidated the signaling pathwa

Zymosan-Induced Murine Peritonitis Is Associated with an Increased Sphingolipid Synthesis without Changing the Long to Very Long Chain Ceramide Ratio.

Sphingolipids are key molecules in inflammation and defense against pathogens. Their role in dectin-1/TLR2-mediated responses is, however, poorly understood. This study investigated the sphingolipidome in the peritoneal fluid, peritoneal cells, plasma, and spleens of mice after intraperitoneal injection of 0.1 mg zymosan/mouse or PBS as a control. Samples were collected at 2, 4, 8, and 16 h post-injection, using a total of 36 mice. Flow cytometry analysis of peritoneal cells and measurement of IL-6, IL-1β, and TNF-α levels in the peritoneal lavages confirmed zymosan-induced peritonitis. The co
