Can we learn from the clinically significant face processing deficits, prosopagnosia and Capgras delusion?

This review describes two clinically significant face processing deficits, prosopagnosia and Capgras delusion, and provides new knowledge about the face recognition process by a convergence of empirical findings. These empirical findings are structured around two questions that are reviewed from the perspectives of the two deficits. First is the question of hemispheric specificity, which inquires into the degree of each hemisphere's contribution to the face recognition process. Second is the question of dual neural pathways, which addresses the possibility that the face recognition process pro

An objective study of relationships and discontinuities between paranoid schizophrenia and Kretschmer's syndrome of sensitive delusions of reference.

The nosological status of Kretschmer's syndrome of sensitive delusions of reference (Sensitiver Beziehungswahn) has been a matter of discussion over many years. In an attempt to resolve the matter, a series of 147 consecutive patients who had been diagnosed as schizophrenic were reviewed. A group of 13 patients met specified criteria for Kretschmer's syndrome of sensitive delusions of reference, and these patients were compared with the remainder in a variety of ways. The patients with the syndrome of sensitive delusions of reference differed from the remainder in their antecedents (family his

A Rare Case of Cotard's Delusion Treated With Oral and Long-Term Injectable Antipsychotics: A Case Report.

Cotard's delusion is a relatively infrequent neuropsychiatric condition that presents as a form of extreme and abnormal thinking where an individual believes that they are dead, do not exist, or have lost parts of their body or internal organs. The primary descriptions of Cotard's delusion were associated with severe depressive illness; however, Cotard's delusion may also present in individuals with schizophrenia. Because Cotard's delusion often appears as part of an array of psychotic symptoms, it can be misdiagnosed or underdiagnosed in hospitals. As such, many reported cases of Cotard's del

A Case Report of Delusions in a Patient Receiving Cabergoline Therapy for Prolactinoma: Pathophysiology and Proposed Treatment With Aripiprazole.

Cabergoline is a dopamine 2 receptor agonist used as first-line treatment of pituitary prolactinomas. Here, we describe the case of a 32-year-old woman with a pituitary prolactinoma who was treated with cabergoline for 1 year, during which time she developed delusions. We also discuss the use of aripiprazole to mitigate the psychotic symptoms, while maintaining the efficacy of cabergoline treatment.

The role of culture on the phenomenology of hallucinations and delusions, explanatory models, and help-seeking attitudes: A narrative review.

AIM: Culture has been posited to be involved in the formation and maintenance of delusions and hallucinations. The extent of these differences and how they affect explanatory models of psychosis and help-seeking attitudes remains to be understood. This review aims to present a cultural formulation to account for psychosis onset, symptom maintenance, and help-seeking attitudes. METHODS: A narrative review was conducted to summarize the existing evidence base regarding cross-cultural differences in hallucinatory and delusional prevalence, explanatory models, and help-seeking attitudes in First

Mirror self recognition as a product of forward models; implications for delusions of body image and visual neglect.

Forward models allow individuals to learn to predict the sensory consequences of their own behavior. Social forward models have been proposed as an extension of forward models, allowing individuals to learn to predict the response of another to the individual's own behavior. This article proposes similarly that an individual who treats their reflection as another may learn to predict the behavior of their reflection, offering a new perspective on mirror self-recognition and a potential framework through which to investigate visual delusions. Specifically this article investigates this framewor

The role of expressive suppression in hallucinatory-like and delusion-like experiences. Findings in a non-clinical sample.

INTRODUCTION: Previous works concerning emotion regulation in psychosis highlighted the relationship between expressive suppression and negative psychotic symptoms, as well as between expressive suppression and auditory hallucinations. However, a direct association between suppression and delusions has not been observed. In the study we examined whether expressive suppression is associated with psychotic-like experiences and whether it may influence delusion-like experiences by changing the impact of cognitive biases on delusional beliefs. METHODS: Two hundred and twenty-one healthy individua

[Etiopathogeny of the delusion of pregnancy using a literature review: Role of hyperprolactinemia and application of the theory of abductive inference].

INTRODUCTION: Delusions of pregnancy are not well known. The delusion of pregnancy is defined as the belief of being pregnant despite factual evidence to the contrary. The clinical picture is heterogeneous (duration, mechanisms, topics and pre-existing psychiatric disorders). Several causes have been proposed to explain the occurrence of the delusions of pregnancy: cenesthetic theory, hyperprolactinemia, polydipsia and psychodynamic conflicts. Hyperprolactinemia is an interesting hypothesis (physiologic increase during pregnancy and similar manifestations in the course of gestation). The abduc

Use of modified electroconvulsive therapy in a case of polymyositis presenting with delusion of nihilism of proxy (Odysseus syndrome).

We present a case of psychotic depression with polymyositis presenting with the distinct phenomenon of nihilism by proxy, which was treated with electroconvulsive therapy (ECT). A female patient with polymyositis was initially treated with pharmacotherapy. After initial response, there was deterioration in her mental state and hence, after careful consideration, neurological, and anaesthetic consultations, modified ECT was given with close monitoring. The mental state of the patient improved with a course of ECT, which proceeded without any complications. Her depressive symptoms including the

Cognitive Behavioural Therapy for Nightmares for Patients with Persecutory Delusions (Nites): An Assessor-Blind, Pilot Randomized Controlled Trial.

OBJECTIVE: Nightmares are relatively common in patients experiencing psychosis but rarely assessed or treated. Nightmares may maintain persecutory delusions by portraying fears in sensory-rich detail. We tested the potential benefits of imagery-focused cognitive behavioural therapy (CBT) for nightmares on nightmare severity and persecutory delusions. METHOD: This assessor-blind parallel-group pilot trial randomized 24 participants with nightmares and persecutory delusions to receive CBT for nightmares delivered over 4 weeks in addition to treatment as usual (TAU) or TAU alone. Assessments wer

Clinical characteristics and course of oral somatic delusions: a retrospective chart review of 606 cases in 5 years.

OBJECTIVE: Oral cenesthopathy is characterized by foreign body sensations without medical and dental evidence for them. It is thought to be a rare disease in psychiatry, but many patients are visiting dental clinics seeking treatment to remove a foreign body. Even though the features of oral cenesthopathy might be different between a psychiatric clinic and a dental clinic, there has been no clinic-statistical study from dentists. In this study, we report a clinico-statistical study of patients with oral cenesthopathy in dentistry. METHODS: This is a retrospective chart review of 606 outpatien

Psychotic phenomena in 257 young children and adolescents with bipolar I disorder: delusions and hallucinations (benign and pathological).

OBJECTIVES: In contrast to studies of adult bipolar I disorder (BP-I), there is a paucity of data on psychotic phenomena in child BP-I. Therefore, the aim of this work was to describe delusions and hallucinations in pediatric BP-I. METHODS: Subjects were 257 participants, aged 6-16, in either of two large, ongoing, NIMH-funded studies, 'Phenomenology and Course of Pediatric Bipolar Disorders' or 'Treatment of Early Age Mania (TEAM)'. All subjects had current DSM-IV BP-I (manic or mixed phase) with a Children's Global Assessment Scale score <or=60 (definite clinical impairment), and all had ca

Persecutory beliefs, attributions and theory of mind: comparison of patients with paranoid delusions, Asperger's syndrome and healthy controls.

Schizophrenia patients with persecutory delusions and patients with Asperger's syndrome were compared using two measures of theory of mind (ToM; the ability to infer mental states in other people), the Hints task, and the Reading the Mind in the Eyes task, and a new measure of attributional style (style of inferring the causes of important events), the Attributional Style Structured Interview (ASSI). Paranoid beliefs were measured using Fenigstien and Vanable's Paranoia Scale (PS). The deluded group had the highest scores on the Paranoia Scale but the scores of the Asperger's group's were high

Hallucinations and delusions are associated with elevated tau PET signal independent of age, clinical severity, and amyloid burden.

INTRODUCTION: Psychosis in Alzheimer's disease (AD) is associated with worse outcomes, yet no established biomarkers exist for early diagnosis and intervention. We compared tau positron emission tomography (PET) burden across older individuals with and without psychotic symptoms. METHODS: [18F]AV1451 tau PET binding was compared between 32 Alzheimer's Disease Neuroimaging Initiative (ADNI) subjects with psychotic symptoms (delusions and/or hallucinations) and 32 ADNI subjects without psychotic symptoms, matched for age, sex, race/ethnicity, and clinical severity. Effects of Cognitive Behavioral Therapy for Worry Versus Befriending Therapy in Individuals with Persecutory Delusions: A Randomized Trial.

INTRODUCTION: Worry is common and distressing in psychotic disorders, contributing to the onset and persistence of persecutory delusions. A previously developed 8-week manualized cognitive behavioral therapy (CBT)-worry intervention has shown efficacy in treating persecutory delusions, compared with standard care. We aimed to test CBT-worry's impact on persecutory delusions at 8 weeks, as previously reported, delusions at 24 weeks, and secondary clinical outcomes, relative to an active comparison therapy. MATERIALS AND METHODS: A two-arm, assessor-blinded, randomized controlled trial was cond

Fragile self, mechanical world: mechanistic delusions and ego fragility in schizotypal-affective spectrum disorder-a CARE case report.

Categorical nosological systems frequently fall short when confronted with patients whose presentations cross established diagnostic boundaries. We report M.S., a 44-year-old Brazilian man involuntarily admitted to a psychiatric inpatient unit who presented with systematized persecutory ideation of mechanistic-technological content (chip implantation, satellite-based surveillance), structural ego fragility, absent insight, and progressive social and occupational deterioration. While prior diagnoses of bipolar disorder and provisional schizophrenia had been considered, neither fully captured th

Neural correlates of referential/persecutory delusions in schizophrenia: examination using fMRI and a virtual reality underground travel paradigm.

BACKGROUND: The brain functional correlates of delusions have been relatively little studied. However, a virtual reality paradigm simulating travel on the London Underground has been found to evoke referential ideation in both healthy subjects and patients with schizophrenia, making brain activations in response to such experiences potentially identifiable. METHOD: Ninety patients with schizophrenia/schizoaffective disorder and 28 healthy controls underwent functional magnetic resonance imaging while they viewed virtual reality versions of full and empty Barcelona Metro carriages. RESULTS: C

The effects of individually tailored formulation-based cognitive behavioural therapy in auditory hallucinations and delusions: a meta-analysis.

There is no meta-analysis of cognitive behavioural therapy for delusions and hallucinations separately. The aim of this meta-analysis is to evaluate the end-of-treatment effects of individually tailored case-formulation cognitive behavioural therapy on delusions and auditory hallucinations using symptom-specific outcome measures. A systematic search of the trial literature was conducted in MEDLINE, PSYCHINFO and EMBASE. Eighteen studies were selected with symptom specific outcome measures. Hedges' g was computed and outcomes were pooled meta-analytically using the random-effects model. Our mai
