Recent Advances in Pharmacotherapeutic Paradigm of Mild to Recalcitrant Atopic Dermatitis.

Atopic dermatitis (AD) is a common, chronic skin inflammatory disorder characterized by perivascular infiltration of immunoglobulin E (IgE), T lymphocytes, and mast cells. The key factors responsible for the pathophysiology of this disease are immunological disorders and defects in epidermal barrier properties. Pruritus, intense itching, psychological stress, deprived physical and mental performance, and sleep disturbance are the hallmark features of this dermatological disorder. Preventive interventions such as educational programs, avoidance of allergens, and exclusive care toward the skin c

Unmasking the underlying cause: A rash with deeper implications.

We present the case of a 9-year-old girl with a medical history of growth hormone deficiency, Hashimoto's thyroiditis, failure to thrive, and eosinophilic colitis, and with a new progressive rash. The rash began as erythematous, pruritic papules, which gradually spread and evolved into larger, well-circumscribed plaques. Despite treatment with topical corticosteroids and broad antifungal therapy, the rash persisted without improvement. The patient reported no systemic symptoms. On examination, she was well appearing and afebrile, with normal vital signs. Dermatologic findings included erythema

Concurrence of Acrodermatitis Enteropathica and Eczema Herpeticum in a Child with Atopic Dermatitis.

Acrodermatitis enteropathica (AcE) is a rare, autosomal recessive inherited disorder caused by mutation of the SLC39A4 gene coding for zinc transport protein (ZIP 4). The disease appears during childhood especially in breastfeeding or post-breastfeeding infant. Eczema herpeticum refers to a disseminated skin infection of herpes simplex virus that usually leads to vesicular eruptions commonly seen on a background of atopic dermatitis (AD). We describe an 11-year-old boy with periorificial erosions in periorbital, perinasal, perioral, perineal, and gluteal areas, accompanied with itchy vesicles,

The Nottingham Eczema Severity Score: preliminary refinement of the Rajka and Langeland grading.

A method for assessing disease severity of atopic dermatitis (AD) in children has been developed for population-based research. Based on an index first described by Rajka and Langeland in 1989, disease severity is determined by evaluating the three elements of clinical course, disease intensity and extent of examined AD. This paper describes development of the index for use in epidemiological studies based on a community-based study of 290 pre-school children (aged 1-5 years). Construct validity of the index was evaluated with respect to clinical severity assessment according to a dermatologis

Symptomatic acquired zinc deficiency in at-risk premature infants: high dose preventive supplementation is necessary.

Zinc is a cofactor for several enzymes involved in many metabolisms. Zinc deficiency induces various disorders such as acrodermatitis enteropathica, either inherited or acquired. We report three cases of premature infants (24-31 wks gestational age) with low birthweight (650 to 940 g) and enteropathy, two of whom presented with necrotizing enterocolitis. All infants were fed by total parenteral nutrition. At a chronological age ranging from 73 to 80 days, all infants developed a periorificial dermatitis. Before the onset of the first signs, they had received zinc supplementation ranging from 1

Use of Biobrane after laser resurfacing.

BACKGROUND: Laser skin resurfacing has become an ever more popular and effective technique for the treatment of photoaged skin. Often a prolonged postoperative healing period adversely affects patients and physician satisfaction. Erythema and a certain degree of patient discomfort are often believed to be inevitable as they are thought to reflect depth of wounding and consequently the efficacy of wrinkle removal. In addition, a high incidence of irritant contact dermatitis has been observed, complicating recovery. OBJECTIVE: To develop a simplified wound dressing protocol aimed at achieving m

Quantification of l-carvone in toothpastes available on the Swedish market.

BACKGROUND: Toothpastes have widespread use in the population, and contain flavours to give a pleasant and often minty aroma. Flavours are prevalent allergens in toothpastes, and adverse reactions often present as perioral dermatitis or stomatitis. l-Carvone, a mint flavour found in spearmint oil, is one of these allergens. There are few studies on contact allergy to l-carvone, and some of them have indicated a positive relationship with oral lichenoid lesions. OBJECTIVES: To qualitatively and quantitatively investigate the presence of l-carvone in commercially available toothpastes on the Sw

Dermatologists' Perspectives on Skincare Routine Trends.

BACKGROUND: Multi-step skincare routines have become increasingly popular, particularly among adolescents and young adults, driven by social media exposure and commercial marketing. Although individual skincare ingredients have demonstrated efficacy for specific dermatological conditions, evidence supporting the routine use of elaborate multi-step regimens in otherwise healthy skin remains limited. Concerns have been raised regarding potential adverse effects and misinformation, especially among younger users. OBJECTIVE: To explore dermatologists' perspectives on multi-step skincare routines,

Potassium Iodide for Cutaneous Inflammatory Disorders: A Monocentric, Retrospective Study.

OBJECTIVES: Potassium iodide (KI) is a medication that has been used for decades in dermatology and it is mentioned as a treatment option in all major dermatology textbooks. Yet, there is little recent information on its efficacy. In our study, we wanted to retrospectively evaluate the therapy response to KI in our patients. METHODS: The hospital information system was searched for patients treated with KI at the Department of Dermatology (University Hospital Zurich) in the last 20 years (January 1, 1998 to December 31, 2017). A total of 52 patients were found and, subsequently, 35 patients w

[Chronic bullous dermatoses of childhood in the differential diagnosis of bullous dermatitis in children].

We report a case of chronic bullous dermatosis of children, a benign dermatologic disease of infancy of unknown etiology, characterized, on IIF, by linear deposits of immunoglobulins in the Basement Membrane Zone of cutis, and clinically by a ring shaped disposition of bullae (rosette-like). This work concerns a ten year old girl admitted to the pediatric ward at E. Agnelli Hospital of Pinerolo (Turin) about one year ago, who started having perioral bullous eruption, subsequently spread all over the body skin, with general symptoms of fever and itching. Round erythematous plaques, crusts, and

[Staphylococcal Scalded Skin Syndrome in a Very Low Birth Weight Premature Infant].

INTRODUCTION: Staphylococcal scalded skin syndrome (SSSS) was often endemic in the past but is nowadays rare. The hematogeneous spread of exfoliative toxins A (ETA) or B (ETB) produced by specific Staphylococcus aureus strains causes a scald-like eruption with disseminated bullous lesions. CASE REPORT: A perioral impetigo lesion occurred on day 14 of life in a preterm male infant (1,065 g, 30 weeks of gestational age). Empiric antibiotic therapy with cefotaxime and vancomycin was given for 6 days and led to complete resolution. A Staphylococcus aureus strain was isolated. After a symptom-free

Glucocorticoids promote CCL20 expression in keratinocytes.

BACKGROUND: Glucocorticoids (GCs) are generally envisioned as immunosuppressive, but in conditions such as rosacea and perioral dermatitis they can lead to increased skin inflammation. In lung epithelia, GCs promote expression of the proinflammatory cytokine CCL20, which contributes to steroid-resistant asthma. In the skin, CCL20 stimulates inflammation by recruiting T helper 17 T lymphocytes and dendritic cells, and is elevated in papulopustular rosacea. OBJECTIVES: To understand if, and how, GCs affect CCL20 expression in human keratinocytes. CCL20 expression was assessed by quantitative re
