Journal of child and adolescent psychopharmacology2026eng
OBJECTIVE: Use measurement-based care to assess symptom change across diagnostic categories for adolescents receiving acute-care psychiatric inpatient treatment including both psychopharmacology and dialectical behavior therapy.
METHODS: Adolescents were aged 12-17 and hospitalized on a coeducational, acute-care inpatient unit within a psychiatric hospital in New York City. We conducted a retrospective chart review to analyze symptoms across two or more timepoints from the first 15 days of admission using mixed effects regression models to estimate linear change on five outcome measures inclu

BACKGROUND: Narcissism-as either clinical or subclinical-has long been discussed as a risk factor for suicide. Whereas traditional studies link narcissism with grandiosity, entitlement, and deficits in empathy, newer research has emphasized the intricate interactions among this construct and vulnerability to mental illness, emotion dysregulation, as well as the effects of social stress. The inconsistency in the impingement of narcissistic features on suicidal ideation highlights for us rethink and reformulate our conceptual understanding for therapies.

Symptoms of emotion dysregulation and dissociation are common among people with a history of psychological trauma, and can complicate treatment effectiveness. Phase-based approaches to treatments, which begin with a stabilization and skills phase before moving to trauma-oriented interventions allow for flexible, integrable, and effective approaches that can be applied across various therapeutic modalities and delivery contexts. This narrative review describes empirically supported methods (e.g., Dialectical Behavioral Therapy) for targeting emotion dysregulation within phase-based treatments f

Treatment of borderline personality disorder (BPD) with comorbid substance use disorder can be challenging due to symptom overlap and limited assessment methods. Preliminary evidence has shown promising effectiveness of dialectical behavioral therapy (DBT) for BPD with comorbid substance use disorders. The current study compared the benefits of a 28-day transitional DBT treatment program for individuals with BPD with and without substance use disorders through evaluating the changes in coping skills, generalized anxiety, and depression symptom scales at admission and discharge. A total of 76 p

Emotional Dysregulation is a frequent challenge in clinical child psychiatric contexts. The Stress-Arousal-Regulation-Treatment for Children aged six to twelve (START-Kids, Dixius u. Möhler, 2021a), was developed as low threshold treatment tool for children with emotional dysregulation. The program is based on principles of dialectic behavioral therapy. The present article focuses feasibility and a first evaluation of this innovative treatment tool for emotionally dysregulated children. 23 child psychiatric patients aged six to twelve years took part in this eight weeks program with two group

There are currently three major psychotherapeutic approaches to the management of borderline personality disorder (BPD): the psychodynamic, the cognitive-behavioral, and the supportive. There are special varieties within each: e.g., transference-focused psychotherapy (psychodynamic) or dialectic behavioral therapy (cognitive-behavioral). Though differing in basic conceptions and in methodology, all approaches aim at the amelioration of both the symptom-aspects that dominate the clinical picture at the outset, and the personality difficulties that remain apparent after the symptoms have been al

According to recent research, body positivity and self-compassion are key outcomes that are tied to better psychological and physical health. To date, it is unclear whether body positivity and self-compassion improve, stay constant, or deteriorate over the course of a weight management program, particularly one that addresses the psychological roots of behavior change. Additionally, beyond controlled settings, there are no studies on body positivity and self-compassion in individuals who choose to join a commercial weight management program. Therefore, this single-arm prospective study examine

Borderline personality disorder (BPD) is a severe mental illness marked by unpredictable feelings, behaviors, and relationships. Symptoms like emotional instability, impulsivity, and poor social connections are the basis for diagnostic criteria. A noteworthy discovery highlights the clinical overlap between BPD and several psychotic disorders by arguing that BPD and psychotic symptoms raise the risk of psychopathology. According to neuroimaging evidence, structural and functional brain changes, notably in regions controlling affective regulation and impulse control, are seen in BPD patients. A

Borderline personality disorder (BPD) is a complex personality disorder characterised by immense emotional dysregulation, impulsivity, aggression and substantial interpersonal difficulties. This review begins with examining DSM-5-TR diagnostic clusters for BPD, highlighting the importance of accurate classification. It provides an in-depth analysis of BPD, starting with its epidemiology, diagnostic subtypes, core symptoms, and the challenges these symptoms pose for patients and their support networks. The review explores common co-occurring conditions, such as mood disorders, anxiety disorders

BACKGROUND: Mindfulness skills training is a core component of Dialectical Behavior Therapy and aims to improve emotion dysregulation (ED) in people with Borderline Personality Disorder (BPD). However, the underlying mechanisms of change are not fully understood.
METHODS: A total of 75 BPD outpatients participated in a 10-week mindfulness skills training. Multilevel models with a time-lagged approach were conducted to examine the temporal dynamics between the proposed mechanisms and ED. Decentering, nonjudgment, body awareness and attention awareness as putative mechanisms and ED as outcome w

Emotion regulation deficits are associated with eating disorder (ED) symptoms, regardless of eating disorder diagnosis. Thus, recent treatment approaches for EDs, such as dialectical behavior therapy (DBT), have focused on teaching patients skills to better regulate emotions. The present study examined changes in emotion regulation among adult patients with EDs during DBT-oriented partial hospital treatment, and at follow-up (M[SD] = 309.58[144.59] days from discharge). Exploratory analyses examined associations between changes in emotion regulation and ED symptoms. Patients with anorexia nerv

Because little research has been conducted on which therapist-client interactions lead to intermediate and end-point improvements in Dialectical Behavior Therapy (DBT), we have a small evidence-base from which to specify what therapists must know and do in order to obtain good outcomes using DBT. As with other evidence-based practices, dissemination of DBT has defaulted to assumptions and methods from the "psychotherapy technology model," which emphasizes transfer of the validated treatment package from the research clinic to routine settings with high fidelity (Morgenstern & McKay, 2007). There is evidence from case studies suggesting that adapted dialectical behavior therapy (DBT) for borderline personality disorder (BPD) and eating disorders (ED) might improve disorder related complaints. Twenty-four women with BPD (9 with comorbid anorexia nervosa [AN] and 15 with bulimia nervosa [BN]), who already had failed to respond to previous eating-disorder related inpatient treatments were consecutively admitted to an adapted inpatient DBT program. Assessment points were at pre-treatment, post-treatment, and 15-month follow-up. At follow-up, the remission rate was 54% for BN, and 33%

OBJECTIVE: Child sexual abuse (CSA) is a public crime, prevalent worldwide and associated with various consequences, which are potentially destructive, both in the short- and the long-term. This systematic review aimed to critically describe and analyze the studies published about the treatments implemented with these victims.
METHOD: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, studies were identified through search on EBSCO, PubMed, and Web of Science. Of the 49 articles retrieved, 14 were retained for further analysis, and seven were consider

Individuals with co-occurring borderline personality disorder and narcissistic personality disorder often present with severe emotional dysregulation, interpersonal instability, identity disturbance, and entrenched patterns of shame, vulnerability, and defensiveness that challenge traditional treatment approaches. This article describes the treatment model of the Gunderson Residence, a highly specialized residential program that provides intensive, milieu-based care for individuals with complex personality pathology through an integrated, evidence-based framework. Grounded in empirically suppo

BACKGROUND: Justice-involved veterans demonstrate greater mental health and psychosocial needs relative to justice-involved nonveterans and veterans without a criminal history. Veterans treatment courts (VTCs) serve as an alternative to incarceration for veterans whose criminogenic risk is believed to be related to mental health symptoms. Despite observed improvements in functioning and recidivism risk following successful VTC completion, little is known about factors that impede engagement with VTCs. This paper describes a trauma-informed training program that included psychoeducation, skills

BACKGROUND: Suicide has become a first-order public health concern, especially following the negative impact of COVID-19 on the mental health of the general population. Few studies have analysed the effects of early psychotherapeutic interventions on subjects who have attempted suicide, and even fewer have focused on those hospitalized in non-psychiatric units after a Medically Serious Suicide Attempt (MSSA). The main aim of this study is to describe the protocol designed to evaluate the effectiveness of individual psychological treatment for patients hospitalized after an MSSA. The secondary

BACKGROUND: Emerging evidence indicates effectiveness of dialectical behaviour therapy (DBT) for people with intellectual disabilities (PWID) in forensic settings; however, little is known about "what works" facilitating engagement and change.
METHODS: Eleven interviews were conducted with nine service users across two secure inpatient services. Grounded theory was used to develop a model of perceived engagement and change.
RESULTS: The model provides insights into how change occurs during DBT delivered in forensic settings. DBT constitutes a challenging journey, yet provides the motivation

BACKGROUND: Non-suicidal self-injury (NSSI) is highly prevalent during adolescence and is consistently associated with suicidal ideation (SI) and suicide attempts (SA). Although numerous psychological, social, and neurobiological correlates of the NSSI-suicidal behavior relationship have been investigated, current evidence remains fragmented, and the extent to which these correlates are specific to NSSI rather than reflecting broader psychopathology remains unclear. This narrative review aimed to synthesize current evidence on the psychological, social, and neurobiological correlates of the NS

BACKGROUND: Posttraumatic stress disorder (PTSD) is a significant risk factor for suicide among veterans, but there is a critical knowledge gap about how to treat PTSD in the context of elevated acute suicide risk. Prior research suggests that a treatment that combines Dialectical Behavior Therapy (DBT) with the DBT Prolonged Exposure protocol (DBT PE) reduces PTSD and self-directed violence (SDV) among high-risk individuals while also being feasible, acceptable, and safe to deliver. However, a large-scale randomized controlled trial is needed. This study will compare the effectiveness of DBT
