[Drug-receptor interactions in single smooth muscle cells].

Smooth muscle tissues were contracted by excitation of each muscle cell. Single cells prepared from guinea pig taenia caecum and trachea were contracted by extracellular application of acetylcholine and/or carbachol, whose concentrations were the same as those in the tissues. The concentration-response curve was shifted in a parallel fashion by competitive antagonists. The pA2-values of the antagonists were in good agreement with those estimated using the intact tissue. The apparent dissociation constants of cholinergic drugs estimated from inhibition of the specific binding of [3H]QNB (quinuc

Oral contraceptives. Are drug interactions of clinical significance?

There is a large quantity of literature on drug interactions with oral contraceptive (OC) steroids although their incidence is not known. The potential clinical significance of some interactions makes it important for all prescribing doctors and dentists to have some knowledge of the topic. Interactions may be divided into those in which OC effectiveness is impaired, causing breakthrough bleeding or pregnancy, those in which OC activity is enhanced by other drugs and those in which OCs interfere with the metabolism or activity of other therapeutic agents. Consideration of their pharmacology in

Pharmacokinetic drug interactions with nonsteroidal anti-inflammatory drugs.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most widely used drugs. Drug interactions with this class of compounds are frequently reported and can be pharmacokinetic and/or pharmacodynamic in nature. The pharmacokinetic interactions can be divided into 3 classes: (1) drugs affecting the pharmacokinetics of an NSAID. (2) an NSAID interfering with the pharmacokinetics of another NSAID and (3) NSAIDs altering the pharmacokinetics of another drug. Although the pharmacokinetics of some NSAIDs may be significantly affected by the concurrent administration of certain other drugs (incl

Investigation of N-Acetyltransferase 2-Mediated Drug Interactions of Amifampridine: In Vitro and In Vivo Evidence of Drug Interactions with Acetaminophen.

Amifampridine is a drug used for the treatment of Lambert-Eaton myasthenic syndrome (LEMS) and was approved by the Food and Drug Administration (FDA) of the United States (US) in 2018. It is mainly metabolized by N-acetyltransferase 2 (NAT2); however, investigations of NAT2-mediated drug interactions with amifampridine have rarely been reported. In this study, we investigated the effects of acetaminophen, a NAT2 inhibitor, on the pharmacokinetics of amifampridine using in vitro and in vivo systems. Acetaminophen strongly inhibits the formation of 3-N-acetylamifmapridine from amifampridine in t

Physiologically Based Pharmacokinetic Model to Predict Drug-Drug Interactions With the Antibody-Drug Conjugate Trastuzumab Deruxtecan.

The topoisomerase I inhibitor payload (DXd) released from the human epidermal growth factor receptor 2-targeted antibody-drug conjugate (ADC) trastuzumab deruxtecan (T-DXd) is eliminated by hepatic uptake via OATP1B1/3, metabolism by CYP3A, biliary excretion via P-glycoprotein and breast cancer resistance protein, and urinary excretion. In a clinical drug-drug interaction (DDI) study between T-DXd and ritonavir (a strong cytochrome P450 [CYP] 3A/organic anion transporting polypeptide 1B inhibitor) or itraconazole (a strong CYP3A inhibitor), a 1.2-fold increase in DXd exposure was observed. Analysis of drug-drug interactions between psychiatric drugs in spontaneous adverse drug reaction reports from EudraVigilance.

In psychiatric care, polypharmacy is common practice. The more drugs are used, the higher the likelihood of drug-drug interactions (DDI) and, consequently, the occurrence of adverse drug reactions (ADR). The aim of our study was to identify DDI between psychiatric drugs and to analyze their associated factors in spontaneous ADR reports contained in the European ADR database EudraVigilance. Out of 9287 ADR reports from EudraVigilance, 1811 reports with potential DDI (pDDI) were detected by the ABDATA interaction analysis. During the individual case assessment of the most frequently identified p

Optimization of a drug transporter probe cocktail: potential screening tool for transporter-mediated drug-drug interactions.

AIMS: Previous pharmacokinetic characterization of a transporter probe cocktail containing digoxin (P-gp), furosemide (OAT1, OAT3), metformin (OCT2, MATE1, MATE2-K) and rosuvastatin (OATP1B1, OATP1B3, BCRP) in healthy subjects showed increases in rosuvastatin systemic exposure compared to rosuvastatin alone. In this trial, the doses of metformin and furosemide as putative perpetrators were reduced to eliminate their drug-drug interaction (DDI) with rosuvastatin. METHODS: In a randomized, open-label, single-centre, five-treatment, five-period crossover trial, 30 healthy male subjects received

Predictive Modeling of Pharmacokinetic Drug-Drug and Herb-Drug Interactions in Oncology: Insights From PBPK Studies.

Physiologically based pharmacokinetic (PBPK) modeling is increasingly used to anticipate, quantify, and strategically manage drug-drug (DDI) and herb-drug (HDI) interactions that can alter the exposure of chemotherapy agents together with co-administered phytochemicals or nutraceuticals. To evaluate current knowledge, we performed a comprehensive Google Scholar search (2003-2024) and selected studies that employed PBPK platforms, reported quantitative validation, and focused on chemotherapy-related interactions. From these reports, key modeling parameters, validation metrics, and clinically re

Physiologically based pharmacokinetic model to predict drug-drug interactions with the antibody-drug conjugate enfortumab vedotin.

Enfortumab vedotin is an antibody-drug conjugate (ADC) comprised of a Nectin-4-directed antibody and monomethyl auristatin E (MMAE), which is primarily eliminated through P-glycoprotein (P-gp)-mediated excretion and cytochrome P450 3A4 (CYP3A4)-mediated metabolism. A physiologically based pharmacokinetic (PBPK) model was developed to predict effects of combined P-gp with CYP3A4 inhibitor/inducer (ketoconazole/rifampin) on MMAE exposure when coadministered with enfortumab vedotin and study enfortumab vedotin with CYP3A4 (midazolam) and P-gp (digoxin) substrate exposure. A PBPK model was built f

Integrating Static and Dynamic Models to Predict Cannabidiol-Mediated Metabolic Drug-Drug Interactions.

Cannabidiol (CBD) inhibits multiple cytochrome P450 (CYP450) enzymes in vitro via reversible and time-dependent mechanisms, raising concerns for metabolic drug-drug interactions (DDIs). Clinical DDI data for CBD are limited, and the translation of in vitro derived inhibition parameters into in vivo DDI predictions remains uncertain. We implemented a stepwise, model-informed framework to evaluate CBD-mediated metabolic DDIs using (i) basic, (ii) mechanistic static (MSM), and (iii) physiologically based pharmacokinetic (PBPK) models. The basic model and MSM incorporated in vitro inhibition param

Potential Drug-Drug Interactions With Nirmatrelvir/Ritonavir Among Long-Term Care Facility Residents.

OBJECTIVES: Nirmatrelvir/ritonavir (Paxlovid) is an oral antiviral for COVID-19. Although effective, its use is complicated by clinically important potential drug-drug interactions (DDIs). Long-term care facility (LTCF) residents are especially vulnerable to both COVID-19 and the adverse effects of potential DDIs because of advanced age, multimorbidity, and polypharmacy. The objective of this study was to quantify the prevalence and duration of potential DDIs among US LTCF residents administered nirmatrelvir/ritonavir. DESIGN: We conducted a cohort study of LTCF residents using electronic med

Tramadol in drug-facilitated sexual assault: global case prevalence, pharmacology, and drug interactions.

Drug-facilitated sexual assault (DFSA) refers to sexual acts where the victim is incapacitated by the effects of psychotropic substances, causing impaired ability to prevent an attack and/or provide informed consent. To date, the level of understanding of the prevalence of the specific drugs involved in DFSA has been limited. This review examines global toxicological case data to assess the prevalence of tramadol, a widely prescribed opioid analgesic, in the context of DFSA. Tramadol was confirmed in reports across Africa, Oceania, Europe, the Middle East, and North America, with detection rat

Pharmacokinetic Drug-Drug Interactions of Imlunestrant in Healthy Female Participants of Nonchildbearing Potential.

Imlunestrant is a next-generation, oral, selective estrogen receptor degrader approved for treating adults with estrogen receptor-positive, human epidermal growth factor receptor 2-negative, ESR1-mutated advanced/metastatic breast cancer. Imlunestrant is metabolized partly by cytochrome P450 (CYP)3A4 oxidation and conjugation with glucuronic acid and sulfate; thus, pharmacokinetic and drug-drug interaction (DDI) studies for treatments utilizing similar pathways are essential for clinical development. Three open-label, phase 1 studies enrolled 182 healthy females of nonchildbearing potential to
