Beyond intraocular pressure: vascular and metabolic modifiers of optic nerve vulnerability in glaucoma.

PURPOSE: To critically integrate epidemiological, mechanistic, and clinical evidence linking systemic blood pressure dysregulation and diabetes mellitus with glaucomatous optic nerve injury, while distinguishing biological plausibility and association from demonstrated causality. METHODS: PubMed/MEDLINE was searched from database inception through 30 June 2026 using three prespecified blocks covering blood pressure exposures, diabetes and candidate antidiabetic therapies, and glaucoma subtypes. Backward citation tracking and targeted journal and publisher searches identified additional and re

Glancing at the role of Toxoplasma gondii in neuro-oncology: from risk factor to therapeutic target.

BACKGROUND: Cancer is a leading cause of morbidity and mortality, with brain tumors posing significant challenges due to their site and complexity. Therefore, exploring tumor pathogenesis and the discovery of novel therapeutic agents have drawn attention to the role of infectious agents. Toxoplasma gondii is a highly prevalent protozoan parasite that establishes a chronic latent infection in neural tissues. Several epidemiological studies have linked infection with Toxoplasma gondii to a higher risk of developing cancer, particularly brain tumors. However, research findings are controversial,

Feline heartworm disease in endemic settings: an integrated diagnostic approach.

Feline heartworm disease, caused by Dirofilaria immitis, is an emerging concern in endemic regions, although its epidemiology remains poorly defined due to diagnostic limitations. In Italy, available data on feline D. immitis infection are limited and largely based on serological investigations. The aim of the present study was to provide an epidemiological assessment of infection in cats across Sardinia, a region where canine dirofilariosis is endemic. A total of 141 cats were evaluated using a multimodal diagnostic approach including echocardiography, antigen detection, and the modified Knot

Global, regional, and national burden of lower extremity peripheral arterial disease: a systematic analysis of prevalence, incidence, deaths, and dalys from 1990 to 2021 with projections and dietary risk factors to 2036.

OBJECTIVE: This research aims to analyze the global, regional, and national burden of LE-PAD from 1990 to 2021 through the GBD 2021 database. We also assess the impact of dietary risk factors and use decomposition and frontier analyses to identify key drivers and disparities in disease burden. METHODS: We used data from the GBD 2021 study (covering 204 countries and territories) to analyze LE-PAD-related prevalence, incidence, mortality, and DALYs, and conducted trend analysis and projections via joinpoint regression and autoregressive integrated moving average (ARIMA) models. Decomposition a

Microplastics as an emerging driver of osteoarthritis: a translational synthesis of environmental exposure, patho-mechanisms, and public health implications.

BACKGROUND: Osteoarthritis (OA), a leading global cause of disability, has been increasingly associated with environmental microplastic (MP) exposure. MPs bioaccumulate in human tissues via the food chain and may disrupt joint homeostasis through inflammatory and oxidative pathways. However, their role in OA pathogenesis remains underexplored in translational contexts. METHODS: We conducted a synthesis of environmental microplastic contamination data from global aquatic ecosystems, integrated with molecular pathway analyses and epidemiological evidence. Microplastic sampling and polymer ident

Gastro-intestinal nematodes in goats in Bangladesh: A large-scale epidemiological study on the prevalence and risk factors.

Goats greatly influence the economic sustainability of rural communities. However, parasitic diseases, especially gastrointestinal nematodes (GINs) are a major constraint on profitable small ruminants' production worldwide. During July- 2015 to June- 2016, we conducted a cross sectional study within seven topographic zones of Bangladesh to explore the level of infection and associated risk factors of GINs infections of goats. The study followed standard flotation and modified McMaster techniques. Among 1998 samples from goats; 1241 (62.1%) were found to be infected with one or more species of

Nutritional epidemiology and cancer: A Tale of Two Cities.

Recently, Ioannidis questioned whether nutritional epidemiology could be reconciled with good scientific principles, and suggested that the field needs radical reform. One of the reforms he advocated was more randomized trials; though what diet would be tested and how it would be measured were unspecified, how adherence would be monitored was unclear or unimportant, and the length of time vaguely stated as "lifelong". The other reform was reanalysis of shared data, which actually already exists in a large number of cohort consortia of individual pooled data. The 2018 report analysis of diet an

Epidemiology of hypertensive disorders in pregnancy: prevalence, risk factors, predictors and prognosis.

Hypertensive disorders in pregnancy (HDP) represent some of the most important problems faced by public health because HDP is a major cause of maternal and prenatal morbidity and mortality. Several epidemiological studies have been performed to determine the prevalence and risk factors of HDP as well as its subtypes. The prevalences of HDP, gestational hypertension and preeclampsia are 5.2-8.2%, 1.8-4.4% and 0.2-9.2%, respectively. Body mass index, anemia and lower education appear to be modifiable risk factors for HDP. Maternal age, primiparous, multiple pregnancy, HDP in previous pregnancy,

A rotavirus vaccine for prevention of severe diarrhoea of infants and young children: development, utilization and withdrawal.

The importance of rotaviruses (RVs) as the single most important cause of severe diarrhoea of infants and young children is well recognized. At NIH, we developed a quadrivalent (tetravalent [TV]) vaccine to protect against the four epidemiologically important RV serotypes. It is comprised of live attenuated rhesus RV (RRV), a VP7 serotype G3 strain (the 'Jennerian' approach), and three reassortant RVs, each containing 10 RRV genes and one human RV gene that codes for the major outer protein, VP7, that determines serotype G1, G2 or G4 specificity (the 'modified Jennerian' approach). The vaccine

Ten years of freedom from smallpox: lessons and experiences. Dedicated to the tenth anniversary of worldwide freedom from smallpox.

Many lessons and experiences were learned during the global programme of smallpox eradication, the most important being those which could be generalized and applied to other health programmes. This does not mean imitating or implementing smallpox eradication techniques to other diseases, since each infection requires its own strategy. It is difficult to dissect out the single key element or to equate the various factors responsible for the success, as these always worked together, in combination, depending one on others. For global eradication, the element of essential importance was internati

High-Dose Ethanol-Induced Immunosuppression Modulates Sex-Specific Disease Outcomes in a Murine Model of Multiple Sclerosis.

Both epidemiological studies and prior work in animal models suggest that moderate-dose alcohol reduces disease severity across several autoimmune conditions, including multiple sclerosis (MS). However, the mechanisms underlying the potentially beneficial effects of alcohol and how these effects may change with alcohol dose in autoimmunity remain underexplored. In this study, we characterize the effects of chronic, high-dose ethanol consumption in experimental autoimmune encephalomyelitis (EAE), a murine model of MS, by examining EAE disease severity, gut microbial composition, and peripheral

The use of race and ethnicity in air pollution epidemiology and methodological recommendations: A scoping review focusing on California.

Air pollution remains a significant global health risk, and exposure to air pollutants can lead to multiple health issues across the life course. Air pollution is also an environmental justice issue and plays a crucial role in health disparities across racial and ethnic groups, through both differential exposure and differential vulnerability. Historical patterns of discriminatory siting of emission sources have led to differential exposure to air pollution among racial and ethnic groups. Differential vulnerability, influenced by social factors and community composition, further exacerbates he

Injury Epidemiology and Preventative Strategies in Triathletes: A Study of the 2022 Season.

CONTEXT: Triathlons are a popular endurance sport with an elevated injury risk. Prevalence for injuries in triathletes varies greatly and is thought to be related to racing distance. The most common injuries in triathletes occur in the lower-extremities and are considered overuse in nature. The primary purpose of this study was to examine injury patterns and prevalence among triathletes during the 2022 season. DESIGN: This study adopts an epidemiological approach using a retrospective survey to better understand the prevalence, nature, and patterns of injuries among triathletes. METHODS: A l

Identifying studies evaluating susceptibility factors for chemical health assessments: A case study focused on methylmercury developmental neurotoxicity.

Identifying susceptibility factors for adverse health effects from chemical exposures is an important aspect of characterizing human health impacts. However, to date, an efficient approach for identifying these factors has not been established. To address this limitation, two approaches were utilized to find studies that contained susceptibility information using methylmercury (MeHg) developmental neurotoxicity (DNT) as a case study. Both approaches start with a comprehensive literature search of 5 databases on MeHg followed by keyword filtering for potential epidemiology studies; however, the

Cigarette smoking knowledge and perceptions among students in four Italian medical schools.

INTRODUCTION: Tobacco smoking is the leading cause of premature death in the developed world. Advice and assistance by physicians help smokers quit, but little attention has been paid to the topic of tobacco dependence in the curricula of Italian medical schools. Consequently, few physicians follow the clinical practice guidelines for treating dependence. METHODS: This study was conducted on 439 students at 4 Italian medical schools in 2010. Students were asked to complete a 60-item questionnaire. Two scores were computed: Score 1 assessed knowledge of the epidemiology of smoking, risks assoc

Bioaerosols and sick building syndrome: particles, inflammation, and allergy.

PURPOSE OF REVIEW: Sick building syndrome is a poorly understood condition that can be vexing to clinicians and public health investigators alike. Concerns about possible causes have recently shifted to bioaerosols, especially indoor mold contamination. Recently, controversy over the health effects of indoor bioaerosols has intensified in the media and in medical forums. Allergists and other clinicians are increasingly being asked to evaluate cases of sick building syndrome attributed to bioaerosol exposure. Although allergy may play a role, it is unlikely to fully explain the nonspecific symp

Public health surveillance after a volcanic eruption: lessons from Cerro Negro, Nicaragua, 1992.

The eruption of the Cerro Negro volcano near León, Nicaragua, on 9 April 1992 distributed an estimated 1.7 million tons of ash over a 200 square kilometer area. An assessment was conducted to evaluate the health effects on approximately 300,000 residents, using routine data obtained by the national epidemiologic surveillance system. It was found that rates of visits to health care facilities for acute diarrheal and respiratory illnesses increased in two study communities, one within and one near the disaster zone. Specifically, visits for acute diarrhea were nearly 6 times more numerous than b

Spatiotemporally resolved air exchange rate as a modifier of acute air pollution-related morbidity in Atlanta.

Epidemiological studies frequently use central site concentrations as surrogates of exposure to air pollutants. Variability in air pollutant infiltration due to differential air exchange rates (AERs) is potentially a major factor affecting the relationship between central site concentrations and actual exposure, and may thus influence observed health risk estimates. In this analysis, we examined AER as an effect modifier of associations between several urban air pollutants and corresponding emergency department (ED) visits for asthma and wheeze during a 4-year study period (January 1999-Decemb

Antibiotic Exposure and Periodontal Susceptibility: A Risk-Modifying Hypothesis.

Systemic antibiotics are among the most widely prescribed therapeutic agents worldwide, and their effects on host-microbe equilibrium extend well beyond the infection for which they are intended. Periodontitis is conventionally framed as a biofilm-initiated, host-mediated inflammatory disease, although recent work has shifted this framework toward microbial homeostasis as a regulator of periodontal stability. We hypothesize that antibiotics are not direct etiologic agents of periodontitis but instead act as risk-modifying factors that lower the threshold at which plaque-mediated inflammation p

Identifying risk factors of young-onset dementia and evaluating evidence hierarchy: a meta-analysis and umbrella review.

BACKGROUND: Young-onset dementia (YOD) directly affects the working-age population. The premature onset of dementia intensifies peer caregiving responsibilities and diverts medical and nursing resources. While modifiable risk factors for late-onset dementia have been well established, uncertainty remains regarding the applicability of these findings to YOD. We aim to identify modifiable risk factors for YOD and evaluate the strength of evidence. METHODS: We searched PubMed, Embase, Web of Science, and Ovid Medline from inception to 22 May 2025 for epidemiological studies on non-genetic risk f
