We present a case of an elderly man with the persisting type II endoleak following endovascular aneurysm repair. In a view of the multiple comorbidities, the patient was denied an open elective repair. However, when presented with a ruptured aneurismal sac, he underwent an emergency open abdominal aortic aneurysm repair. Overall, the patient has made a good postoperative recovery.

Neck angulation (NA) is an important risk factor for type 1 proximal endoleaks following stenting of abdominal aortic aneurysms. The Aorfix (Lombard Medical, Oxon, UK) is a new flexible stent graft designed to overcome this issue. The aim of this study was to compare the endoleak flow rate (EFR) in relation to NA between the Aorfix and other manufactured stent grafts. A flow model with silicone proximal and distal necks was used. EFRs corresponding to 10 neck angles between 0 and 70 degrees were measured. Eight stent grafts were tested: Aorfix, Ancure (Guidant, Indianapolis, IN), Powerlink (En

Ruptured abdominal aortic aneurysms (rAAAs) are life-threatening and require emergent surgical therapy. Endovascular aortic repair for rupture (rEVAR) has become the leading strategy due to its minimal invasive approach with expected lower morbidity and mortality, especially in patients presenting with hemodynamic instability and relevant comorbidities. Following rEVAR, intraoperative angiography or early postinterventional computed tomography angiography have to exclude early type 1 or 3 endoleaks requiring immediate reintervention. Persistent type 2 endoleaks (T2ELs) after rEVAR, in contrast

Endovascular treatment of abdominal aortic aneurysms (endovascular aneurysm repair [EVAR]) has been shown to be effective, with lower perioperative mortality and morbidity than open surgical treatment in patients with compatible anatomy. This benefit is lost after 3 years of follow-up. This could be explained by the higher rate of reintervention in EVAR patients due to endoleaks. Around half of patients with a persistent type 2 endoleak (T2EL) have an increase in aneurysm size. Either endovascular or conventional surgery techniques have been described to treat this type of endoleak. The aim of

BACKGROUND: Type Ia endoleaks after endovascular aortic repair (EVAR) almost always mandate secondary percutaneous reinterventions. Several patients, however, will require conversion to open surgical repair with complete graft explant, which is associated with significant morbidity and mortality. We herein present 3 cases of hybrid surgical repair for type Ia endoleaks, using a limited open exposure for proximal stent graft edge revision to achieve graft preservation and effective aneurysm sac exclusion.
METHODS: Angiography was used to confirm type Ia endoleak in 3 patients (2 males) who had

PURPOSE: After an endovascular aortic aneurysm repair (EVAR), a follow-up at 1, 6 and every 12 months is recommended for remainder of the patient's life. The diagnostic standard methods for diagnosing endoleaks and visualization of aneurysms in EVAR-patients are: invasive digital subtraction angiography (DSA), contrast enhanced (CE) computed tomographic angiography (CE-CTA), and magnetic resonance angiography (CE-MRA). These techniques, however, require the use of iodine- or gadolinium-based contrast agents with rare, but possibly life threatening side effects such as renal impairment, thyroto

BACKGROUND: Billowing is a unique computed tomography (CT) finding of the Endologix stent graft where a rim of contrast is observed outside the stent's endoskeleton but still contained within the graft cover. The purpose of this study is to evaluate the incidence and clinical outcome of billowing following an Endologix stent-graft placement.
METHODS: All patients who underwent abdominal aortic aneurysm (AAA) repair with endovascular placement of an Endologix stent graft from January 2002 to September 2013 and who were followed with imaging were included in the study. The images were evaluated

BACKGROUND/OBJECTIVES: The natural history of type II endoleaks (T2ELs) is still not completely understood; however, it is widely accepted that those associated with aneurysmal sac growth are harmful. We aimed to review our experience with T2ELs in endovascular aneurysm repair (EVAR).
METHODS: We retrospectively reviewed electronic medical records of all patients who underwent EVAR for infrarenal-type abdominal aortic aneurysms (AAAs) at a single institution from August 2007 to November 2015. Demographic and clinical data were collected. Preoperative contrast computed tomography scans were re

Conventional revascularization of the lower extremities secondary to aortoiliac atherosclerotic vascular disease (ASVD) involves major intra-abdominal surgery with aortobifemoral bypass grafting. Complication rates are as high as 10% and perioperative mortality can be as high as 5%; therefore, careful patient selection is paramount. Endovascular aortobifemoral bypass repair with aortic bifurcation reconstruction is an emerging option in patients deemed unsuitable for conventional surgical management. Emerging techniques such as snorkel/chimney method are expanding the number of patients who ca

INTRODUCTION: Hostile neck anatomy (HNA) remains a major challenge to the technical success and durability of endovascular aneurysm repair (EVAR) for abdominal aortic aneurysm (AAA). This study investigates aortic neck characteristics and the risk of intraoperative type I endoleak (EL-I) following EVAR.
METHODS: All patients who underwent EVAR between 2015 and 2024 were identified from the Society for Vascular Surgery Vascular Quality Initiative database. Patients with a history of arterial bypass or aneurysm repair, those with iliac aneurysm, deemed unfit for open repair, or who required con

INTRODUCTION: Routine monitoring is necessary to detect endoleaks after treating abdominal aortic aneurysms (AAAs) with endovascular aneurysm repair (EVAR). Accurately diagnosing endoleak type is crucial to ensure a durable repair during secondary intervention. Computed tomography angiography (CTA) has been the standard for follow-up of patients following EVAR. Contrast-enhanced ultrasound (CEUS) became available to clinicians in the United States in 2021 and utilizes noniodinated contrast while minimizing radiation, which may prove beneficial for patients.
METHODS: Status post-EVAR patients

INTRODUCTION: Type II endoleaks (T2ELs) are a common finding after endovascular aortic repair (EVAR), often caused by retrograde flow from the inferior mesenteric artery (IMA). Although transarterial embolisation is standard, laparoscopic IMA ligation may provide a definitive alternative in selected cases. Indocyanine green fluorescence angiography (ICG-FA) adds real time vascular visualisation, helping the identification and confirmation of successful ligation. This report illustrates the potential benefit of Indocyanine green guided laparoscopic IMA clipping for managing T2ELs.

OBJECTIVE: Thoracic endovascular aortic repair (TEVAR) including a branch or fenestration offers a less invasive alternative to open surgery in the treatment of aortic arch pathologies. Nevertheless, the occurrence of a proximal type I endoleak (EL) in this specific setting may be particularly challenging to fix. This study presents the case of a type Ia endoleak after a zone 2 single branched TEVAR that was successfully treated with a proximal endovascular relining using a custom made double branch aortic arch graft.
CASE: A 74 year old male presented with a 70 mm thoracic aortic aneurysm an

This study aimed to develop and validate a novel nomogram risk assessment model to predict the possibility of type II endoleak (T2EL)-related re-intervention. The data of 455 patients with abdominal aortic aneurysms who underwent elective endovascular aneurysm repair (EVAR) procedures between January 2018 and December 2021 at our single center were retrospectively reviewed. Following the implementation of exclusion criteria, 283 patients were finally included and divided into T2EL-related re-intervention (n = 42) and non-T2EL (n = 241) groups. The overall T2EL-related re-intervention rate for

OBJECTIVE: The midterm results of endovascular abdominal aortic aneurysm repair (EVAR) with aortic side branch coil embolization during EVAR was evaluated.
METHODS: Our center began coil embolization for all patent inferior mesenteric artery (IMA) and lumbar artery (LA) with an inner diameter more than 2.0 mm during EVAR since June 2015. When four or more LA were patent, coil embolization for LA with inner diameter 2.0 mm or less was done. EVAR without aortic side branches coil embolization was performed for 59 patients prior to June 2015 (control group) and 79 patients underwent EVAR with co

Type II endoleak is a common complication that develops after endovascular aneurysm repair. Patients with type II endoleak, which has persisted for 6 months, have a significantly higher rate of aneurysmal sac enlargement, reintervention, and rupture. To date, several studies have examined the effectiveness of preoperative embolization of branch vessels for the prevention of type II endoleak. Particularly, the embolization of the large inferior mesenteric artery (IMA) seems to be a precise, safe, and effective method. IMA is a significant risk factor for type II endoleak. However, there is curr

Juxtarenal aortic aneurysms (JRAAs) are challenging to treat by endovascular aneurysm repair (EVAR) procedures. The chimney technique with EVAR (Ch-EVAR) is one of the feasible and less invasive treatments for JRAAs. However, the main concern of Ch-EVAR is the potential risk of "gutters," which can lead to type Ia endoleak (EL). Most type Ia ELs after Ch-EVAR procedures occurred intraoperatively, and these ELs could be treated using an endovascular technique. However, late-onset type Ia ELs could be extremely rare, which might have a fear of conservative treatment. Type Ia ELs are associated w

OBJECTIVE: Junctional component separation producing type IIIa endoleak after endovascular abdominal aortic aneurysm repair (EVAR) is an uncommon but serious complication requiring unanticipated reinterventions. This retrospective study analyzed main-body EVAR component uncoupling and type IIIa endoleaks encountered with Powerlink and AFX (Endologix Inc, Irvine, Calif) endografts during an 8-year period.
METHODS: Type IIIa endoleaks were identified from a database of secondary interventions and clinical surveillance. Operative reports, medical records, and computed tomography studies were rev

Endovascular treatment options have evolved in many ways and become an important alternative for open surgical repairs in various vascular territories. Herein, we present a case of an 18-year-old man with complex injury to the left subclavian artery and vein caused by a gunshot 4 months ago. After the gunshot, a high-flow fistula between the left subclavian artery and the vein occurred with pseudoaneurysm formation. This fistula led to a significant left subclavian steal phenomenon. A stent-graft was deployed along the injured left subclavian artery after embolization of the left vertebral art
