Random forest classification as a tool in epidemiological modelling: Identification of farm-specific characteristics relevant for the occurrence of Fasciola hepatica on German dairy farms.

Fasciola hepatica is an internal parasite of both human and veterinary relevance. In order to control fasciolosis, a multitude of attempts to predict the risk of infection such as risk maps or forecasting models have been developed. These attempts mainly focused on the influence of geo-climatic and meteorological features. Predicting bovine fasciolosis on farm level taking into account farm-specific settings yet remains challenging. In the present study, a new methodology for this purpose, a data-driven machine learning approach using a random forest classification algorithm was applied to a c

A Comparative Assessment of Epidemiologically Different Cutaneous Leishmaniasis Outbreaks in Madrid, Spain and Tolima, Colombia: An Estimation of the Reproduction Number via a Mathematical Model.

Leishmaniasis is a neglected tropical disease caused by the Leishmania parasite and transmitted by the Phlebotominae subfamily of sandflies, which infects humans and other mammals. Clinical manifestations of the disease include cutaneous leishmaniasis (CL), mucocutaneous leishmaniasis (MCL) and visceral leishmaniasis (VL) with a majority (more than three-quarters) of worldwide cases being CL. There are a number of risk factors for CL, such as the presence of multiple reservoirs, the movement of individuals, inequality, and social determinants of health. However, studies related to the role of

Establish a PrEP (Pre-Exposure Prophylaxis) Epidemiology, Modeling, and Surveillance (PREMISE) System to Analyze Trends in PrEP Uptake and the Impact of PrEP Programs and Policies: Protocol for a Natural Experiment and Modeling Study in the United States.

BACKGROUND: Pre-exposure prophylaxis (PrEP) is highly effective in preventing HIV transmission; yet, many people who would benefit from PrEP are not currently using it. Numerous programs and policies, including those provided under the US Ending the HIV Epidemic effort, have been implemented to increase PrEP use. Programs vary enormously, ranging from telemedicine PrEP support to electronic medical record prompts to social marketing and messaging campaigns. However, limited evidence exists regarding their relative impact on PrEP uptake. OBJECTIVE: The aims of the PREMISE (PrEP Epidemiology, M

Co-infection of multiple human papillomavirus genotypes and its effects on symptomatic risks and transmission dynamics: an integrated epidemiological and modelling study in Fujian, China.

INTRODUCTION: Global human papillomavirus (HPV) prevalence among women rose significantly from 14% (2019) to 24% (2024), underscoring the need to understand transmission dynamics and public health impact. Although multi-genotype infections are increasingly documented, evidence remains limited on their combined effect on cervical lesion severity and transmission, especially in regional populations. Using clinical datasets from Xiamen, China, this study evaluates co-infection patterns through integrated statistical and dynamical models to quantify associations with histological severity and tran

A mathematical model for evaluating the role of trypanocide treatment of cattle in the epidemiology and control of Trypanosoma brucei rhodesiense and T. b. gambiense sleeping sickness in Uganda.

BACKGROUND: Human and animal African trypanosomiases impose a large economic and health burden in their endemic regions. Large strides have been made in the control of human African trypanosomiasis (HAT), yet these efforts have largely focused on the non-zoonotic form of the disease. Using a mathematical model with a 10 year time horizon, we demonstrate the role of the cattle treatment with trypanocides in the epidemiology of zoonotic and non-zoonotic HAT in Uganda, and its potential implications on elimination and eradication of the disease. METHODOLOGY/PRINCIPAL FINDINGS: We created two com

The effects of post-exposure smallpox vaccination on clinical disease presentation: addressing the data gaps between historical epidemiology and modern surrogate model data.

Decades after public health interventions - including pre- and post-exposure vaccination - were used to eradicate smallpox, zoonotic orthopoxvirus outbreaks and the potential threat of a release of variola virus remain public health concerns. Routine prophylactic smallpox vaccination of the public ceased worldwide in 1980, and the adverse event rate associated with the currently licensed live vaccinia virus vaccine makes reinstatement of policies recommending routine pre-exposure vaccination unlikely in the absence of an orthopoxvirus outbreak. Consequently, licensing of safer vaccines and the

Epidemiologic natural history and clinical management of Human Papillomavirus (HPV) Disease: a critical and systematic review of the literature in the development of an HPV dynamic transmission model.

BACKGROUND: Natural history models of human papillomavirus (HPV) infection and disease have been used in a number of policy evaluations of technologies to prevent and screen for HPV disease (e.g., cervical cancer, anogenital warts), sometimes with wide variation in values for epidemiologic and clinical inputs. The objectives of this study are to: (1) Provide an updated critical and systematic review of the evidence base to support epidemiologic and clinical modeling of key HPV disease-related parameters in the context of an HPV multi-type disease transmission model which we have applied within

Integrating epidemiologic modeling and explainable machine learning to predict and identify factors associated with self-reported depression among adults in Tennessee, United States.

BACKGROUND: Depression is a major public health concern, with Tennessee ranking among the U.S. states with the highest prevalence. Despite its burden, many cases remain undetected due to limited screening and access to mental health services. This study integrated epidemiologic modelling and explainable ML techniques to predict self-reported depression and identify key risk factors among Tennessee adults using the Behavioral Risk Factor Surveillance System (BRFSS) 2023 data. METHODS: We conducted a cross-sectional analysis of 5596 adults from the 2023 Tennessee BRFSS, representing 5,569,707 w

Meeting report: CEPI workshop on Rift Valley fever epidemiology and modeling to inform human vaccine development, Nairobi, 4-5 June 2024.

Rift Valley fever (RVF) is a zoonotic viral disease that causes epidemics and epizootics among humans and livestock, resulting in substantial health and socioeconomic consequences. Currently, there are no RVF vaccines licensed for humans, but several candidates show promise in early-stage development. Existing gaps in RVF epidemiological data and challenges associated with predicting RVF outbreak risk complicate the planning of efficacy studies, making the pathway to licensure for promising candidates unclear. In June 2024, the Coalition for Epidemic Preparedness Innovations (CEPI) convened a

Effect of living in well-insulated warm houses on hypertension and cardiovascular diseases based on a nationwide epidemiological survey in Japan: a modelling and cost-effectiveness analysis.

INTRODUCTION: Cardiovascular diseases (CVDs) are more prevalent in colder homes, partly due to cold-induced high blood pressure (BP). While thermal insulation and heating are rational strategies to mitigate cold exposure, the high initial and running costs pose significant barriers. Therefore, this study aims to evaluate the cost-effectiveness of living in well-insulated warm houses. METHODS: An economic model was developed based on the indoor temperature-BP and BP-CVDs relationships. Five scenarios were outlined: a base scenario (Scenario 0: the most prevalent thermal insulation level (Grade

Estimating Amyotrophic Lateral Sclerosis and Motor Neuron Disease Prevalence in Portugal Using a Pharmaco-Epidemiological Approach and a Bayesian Multiparameter Evidence Synthesis Model.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a rare and progressive neurodegenerative disease involving the upper and lower motor neurons. It is also the most common and the one with the worst prognosis among the motor neuron diseases (MND). ALS invariably progresses to respiratory failure, which is an essential factor affecting the prognosis of this disease. Its prevalence in the world is heterogeneous and, in many countries, is even unknown, since national registries are not mandatory or comprehensive enough. Worldwide, the ALS/MND prevalence is estimated between 4 and 8 cases per 100,

Dose response models and a quantitative microbial risk assessment framework for the Mycobacterium avium complex that account for recent developments in molecular biology, taxonomy, and epidemiology.

Mycobacterium avium complex (MAC) is a group of environmentally-transmitted pathogens of great public health importance. This group is known to be harbored, amplified, and selected for more human-virulent characteristics by amoeba species in aquatic biofilms. However, a quantitative microbial risk assessment (QMRA) has not been performed due to the lack of dose response models resulting from significant heterogeneity within even a single species or subspecies of MAC, as well as the range of human susceptibilities to mycobacterial disease. The primary human-relevant species and subspecies respo
