Pentoxifylline increases extremity blood flow in diabetic atherosclerotic patients.

Pentoxifylline, a new trisubstituted methylxanthine derivative known for its hemorrheologic action, has been shown to improve exercise tolerance in atherosclerotic patients. We examined the responses of diabetic atherosclerotic patients to pentoxifylline administration, measured by Doppler waveform analysis and exercise tolerance. Standard exercise tolerance and Doppler waveform analytic studies of the lower extremity, specifically the right dorsalis pedis artery, were performed before and after three months of pentoxifylline administration (400 mg three times a day). The study group comprised

Exercise Intolerance in Heart Failure with Preserved Ejection Fraction.

Exercise intolerance represents a typical feature of heart failure with preserved ejection fraction (HFpEF), and is associated with a poor quality of life, frequent hospitalizations, and increased all-cause mortality. The cardiopulmonary exercise test is the best method to quantify exercise intolerance, and allows detection of the main mechanism responsible for the exercise limitation, influencing treatment and prognosis. Exercise training programs improve exercise tolerance in HFpEF. However, studies are needed to identify appropriate type and duration. This article discusses the pathophysiol

[Comparative study of the effectiveness of korinfar, izoptin and obzidan in patients with variant stenocardia combined with effort stenocardia].

Twelve patients with variant angina combined with angina of effort were examined. Repeated exercise tests prior to the administration of antianginal drugs revealed the spontaneous variability of the patients' tolerance to the exercise and of the pulse-pressure index at the exercise peak which may serve as an indirect sign of the angiospastic factor involvement in clinical manifestations of angina of effort. The treatment of such patients with corinfar and isoptin was associated with the elimination and significant reduction of spontaneous attacks of angina attended by a considerable increase i

Post-exercise hypoglycaemia in adults with cystic fibrosis compared to healthy control adults.

BACKGROUND: People with cystic fibrosis-related diabetes (CFRD) are at risk of hypoglycaemia related to insulin therapy; and hypoglycaemia is also prevalent in those without CFRD. Although people with other forms of diabetes mellitus are at risk of hypoglycaemia within 48 h after exercise, this has not been objectively measured in people with CF. This study investigated the prevalence of post-exercise hypoglycaemia in adults with CF (pre- and post-commencement of elexacaftor/tezacaftor/ivacaftor (ETI)), compared to healthy controls. METHODS: Participants completed a 20-min supervised, moderat

Supine cycling plus volume loading prevent cardiovascular deconditioning during bed rest.

There are two possible mechanisms contributing to the excessive fall of stroke volume (and its contribution to orthostatic intolerance) in the upright position after bed rest or spaceflight: reduced cardiac filling due to hypovolemia and/or a less distensible heart due to cardiac atrophy. We hypothesized that preservation of cardiac mechanical function by exercise training, plus normalization of cardiac filling with volume infusion, would prevent orthostatic intolerance after bed rest. Eighteen men and three women were assigned to 1) exercise countermeasure (n=14) and 2) no exercise countermea

Evaluation of Cardiac, Vascular, and Skeletal Muscle Function With MRI: Novel Physiological End Points in Cardiac Rehabilitation Research.

Impaired exercise tolerance is a major determinant of decreased quality of life and survival in individuals with cardiovascular disease. The relative contribution that abnormal cardiac, vascular, and skeletal muscle function plays in limiting exercise tolerance and its improvement with exercise training in patients with cardiovascular disease is not fully known. In this review, we provide an overview of the functional impairment of these systems as they relate to exercise capacity and the emerging role of magnetic resonance imaging as a comprehensive tool to evaluate mechanisms that may explai

Drug treatment of breathlessness: contrasting effects of diazepam and promethazine in pink puffers.

Fifteen out of 18 "pink and puffing" patients completed a double-blind, placebo-controlled cross-over trial of diazepam and promethazine for breathlessness and reduced exercise tolerance. Dosages were 25 mg and 125 mg daily, respectively, and each course lasted two weeks. Patients with psychiatric or other major medical histories were excluded. Of the three patients who did not complete the trial, one died during an exacerbation of breathlessness while taking diazepam, one was withdrawn because of mild hypercapnia while taking placebo, and one suffered intolerable drowsiness while taking diaze

Weightlifting outperforms voluntary wheel running for improving adiposity and insulin sensitivity in obese mice.

BACKGROUND: Exercise is an effective intervention for obesity and type 2 diabetes, with significant physiological benefits over pharmacological interventions. However, there is limited preclinical data available comparing endurance and resistance exercise for the impacts on obesogenic pathology and glycemic control. METHODS: Male mice were subjected to 8 weeks of diet-induced obesity (DIO) by high-fat diet (HFD) feeding concurrent with voluntary wheel running (endurance exercise (EEX)) or weightlifting (resistance exercise (REX)). Sedentary (SED) mice fed on normal chow (NC) or HFD were used

Dyspnoea and exercise intolerance during cardiopulmonary exercise testing in patients with univentricular heart. The effects of chronic hypoxaemia and Fontan procedure.

BACKGROUND: Patients with univentricular hearts have decreased exercise tolerance and may demonstrate exertional dyspnoea. It is not known if chronic hypoxaemia exacerbates exercise intolerance and contributes to symptomatic limitation. The extent to which surgical correction of a right-to-left shunt by a Fontan-type procedure can increase exercise tolerance by reducing arterial deoxygenation is not well documented. The cardiopulmonary exercise responses and the symptomatic status in two groups of univentricular patients, those who are cyanotic and those who are acyanotic with Fontan-type circ

Ergogenic effects of beetroot juice supplementation during severe-intensity exercise in obese adolescents.

Previous studies showed a higher O2 cost of exercise, and therefore, a reduced exercise tolerance in patients with obesity during constant work rate (CWR) exercise compared with healthy subjects. Among the ergogenic effects of dietary nitrate ([Formula: see text]) supplementation in sedentary healthy subjects, a reduced O2 cost and enhanced exercise tolerance have often been demonstrated. The aim of this study was to evaluate the effects of beetroot juice (BR) supplementation, rich in [Formula: see text], on physiological variables associated with exercise tolerance in adolescents with obesity

Effects of a mind-body exercise intervention on anxiety, depression and quality of life in elderly patients with chronic obstructive pulmonary disease: A systematic evaluation and META analysis.

BACKGROUND: Elderly patients with Chronic Obstructive Pulmonary Disease (COPD) commonly suffer from anxiety, depression, and impaired health-related quality of life (HRQOL). This review evaluated the effects of mind-body exercises on these outcomes and exercise tolerance in COPD patients aged ≥ 60 years. METHODS: Following PRISMA guidelines, we systematically searched PubMed, Embase, PsycINFO, Cochrane Library, and Web of Science (inception-January 2025) for randomized controlled trials (RCTs) comparing mind-body exercise versus usual care. Outcomes were anxiety, depression, HRQOL (St. George

[New perspectives for respiratory rehabilitation in COPD].

COPD is associated with some skeletal muscle dysfunction which contributes to a poor exercise tolerance. This dysfunction results from multiple factors: physical inactivity, corticosteroids, smoking, malnutrition, anabolic deficiency, systemic inflammation, hypoxia, oxidative stress. Respiratory rehabilitation is based on exercise training and allows patients with COPD to experience less dyspnoea, and to improve their exercise tolerance and quality of life. Not all patients, however, benefit from rehabilitation. Acknowledging the different factors leading to muscular dysfunction allows one to

Usefulness of exercise stress echocardiography for predicting cardiovascular events and atrial fibrillation in hypertrophic cardiomyopathy.

BACKGROUND: In hypertrophic cardiomyopathy (HCM), the determinants of exercise tolerance and the usefulness of exercise stress echocardiography (ESE) for predicting hard endpoints have not been fully investigated. We aimed to assess the key parameters of ESE for exercise tolerance and the factors predictive of cardiovascular events and new-onset atrial fibrillation (AF) in patients with HCM. METHODS: Seventy-four consecutive patients with HCM who underwent ESE and with an ejection fraction ≥50 % were enrolled. The primary endpoint was a composite of cardiovascular death, heart failure hospita

Exercise training alone or in combination with high-protein diet in patients with late onset Pompe disease: results of a cross over study.

BACKGROUND: Late onset Pompe disease (LOPD) is a lysosomal neuromuscular disorder which can progressively impair the patients' exercise tolerance, motor and respiratory functions, and quality of life. The available enzyme replacement therapy (ERT) does not completely counteract disease progression. We investigated the effect of exercise training alone, or associated with a high-protein diet, on the exercise tolerance, muscle and pulmonary functions, and quality of life of LOPD patients on long term ERT. METHODS: The patients were asked to participate to a crossover randomized study comprehend

Effect of Abdominal Binding on Diaphragmatic Neuromuscular Efficiency, Exertional Breathlessness, and Exercise Endurance in Chronic Obstructive Pulmonary Disease.

We tested the hypothesis that abdominal binding (AB) would reduce breathlessness and improve exercise tolerance by enhancing neuromuscular efficiency of the diaphragm during exercise in adults with chronic obstructive pulmonary disease (COPD). In a randomized, controlled, crossover trial, 20 adults with COPD (mean ± SD FEV1, 60 ± 16% predicted) completed a symptom-limited constant-load cycle endurance exercise test at 75% of their peak incremental power output with concomitant measures of the diaphragm electromyogram (EMGdi) and respiratory pressures without (CTRL) vs. with AB sufficient to in

Exercise in muscle glycogen storage diseases.

Glycogen storage diseases (GSD) are inborn errors of glycogen or glucose metabolism. In the GSDs that affect muscle, the consequence of a block in skeletal muscle glycogen breakdown or glucose use, is an impairment of muscular performance and exercise intolerance, owing to 1) an increase in glycogen storage that disrupts contractile function and/or 2) a reduced substrate turnover below the block, which inhibits skeletal muscle ATP production. Immobility is associated with metabolic alterations in muscle leading to an increased dependence on glycogen use and a reduced capacity for fatty acid ox

Increased cGMP improves microvascular exercise training adaptations in diet-induced obesity.

With the development of atherosclerosis, impaired microvascular function can result in diminished capacity for ambulation and is a risk factor for type 2 diabetes. Dynamic changes in vascular tone are determined, in large part, by the endothelial nitric oxide synthase (eNOS)/nitric oxide (NO)/cGMP axis. We used pharmacological gain of function of the eNOS/NO/cGMP axis in diet-induced obese (DIO) mice and reduced function in lean mice to test the hypothesis that functionality of this vascular control mechanism parallels the benefits of an exercise training regimen. DIO mice have 50% lower exerc
