Female genital tract congenital malformations and the applicability of the ESHRE/ESGE classification: a systematic retrospective analysis of 920 patients.

PURPOSE: Several classification systems for female genital tract anomalies exist but are of limited use in clinical practice. We, therefore, assessed the applicability and ease of use of the new ESHRE/ESGE classification, using only patient records. METHODS: This retrospective, single-center, proof-of-principle study systematically analyzed the surgical reports and other hospital records of 920 inpatients and outpatients treated for confirmed female genital tract congenital malformations at a major German university hospital during 2003-2013. Using only this information, a non-expert (medical

IBCB: Efficient Inverse Batched Contextual Bandit for Behavioral Evolution History.

Traditional imitation learning focuses on modeling the behavioral mechanisms of experts, which requires a large amount of interaction history generated by some fixed expert. However, in many streaming applications, such as streaming recommender systems, online decision-makers typically engage in online learning during the decision-making process, meaning that the interaction history generated by online decision-makers includes their behavioral evolution from novice expert to experienced expert. This poses a new challenge for existing imitation learning approaches that can only utilize data fro

DTREEv2, a computer-based support system for the risk assessment of genetically modified plants.

Risk assessment of genetically modified organisms (GMOs) remains a contentious area and a major factor influencing the adoption of agricultural biotech. Methodologically, in many countries, risk assessment is conducted by expert committees with little or no recourse to databases and expert systems that can facilitate the risk assessment process. In this paper we describe DTREEv2, a computer-based decision support system for the identification of hazards related to the introduction of GM-crops into the environment. DTREEv2 structures hazard identification and evaluation by means of an Event-Tre

The Burden of Alopecia Areata and Management with Baricitinib in the United Arab Emirates: A Narrative Review.

Alopecia areata is an inflammatory autoimmune disease that presents as non-scarring hair loss in adults and children and causes substantial psychological distress, economic burden, and reduced quality of life for those affected. Although steroids and immunosuppressants are common treatments for alopecia areata, results from studies of Janus kinase inhibitors in the systemic management of alopecia areata, including those from long-term efficacy and safety studies on the Janus kinase 1/2 inhibitor baricitinib, show promising results. Despite alopecia areata placing a burden on healthcare systems

ARTEMIS: a pilot study comparing AI-based and expert therapeutic decisions in simulated clinical cases of neuroendocrine neoplasms.

Neuroendocrine neoplasms (NENs) are rare and heterogeneous malignancies requiring multidisciplinary management. Large language models (LLMs) are emerging as decision-support tools, but their role in therapeutic decision-making is largely unexplored. ARTEMIS was a pilot cross-sectional study comparing three configurations-a baseline GPT, a customised GPT with static domain knowledge (GPTs), and a retrieval-augmented GPT (RAG)-against a panel of nine Italian NEN experts using twenty simulated, non-surgical cases. The primary endpoint was non-inferiority for systemic therapy recommendations; seco

Maintaining the value of influenza vaccination - the shift from quadrivalent to trivalent vaccines: an expert review.

INTRODUCTION: This review provides an expert perspective on the sustained value of seasonal influenza vaccines as they transition from quadrivalent to trivalent formulations, based on apparent elimination of the B/Yamagata strain from circulation and subsequent advice from the World Health Organization (WHO) to remove the B/Yamagata antigen from influenza vaccines. Influenza has a high clinical and economic burden globally. However, coronavirus disease 2019 has created new challenges for managing seasonal influenza by amplifying vaccine hesitancy. Understanding why influenza virus circulation

Computer-aided detection for tuberculosis and silicosis in chest radiographs of gold miners of South Africa.

BACKGROUND: For over one hundred years, the gold mining sector has been a considerable source of tuberculosis (TB) and silicosis disease burden across Southern Africa. Reading chest radiographs (CXRs) is an expert and time-intensive process necessary for the screening and diagnosis of lung disease and the provision of evidence for compensation claims. Our study explores the use of computer-aided detection (CAD) of TB and silicosis in CXRs of a population with a high incidence of both diseases.METHODS: A set of 330 CXRs with human expert-determined classifications of silicosis, TB, silcotubercu

Probabilistic T-spherical hesitant fuzzy decision support for selecting advanced tertiary wastewater treatment technologies using comprehensive distance based ranking method.

Pharmaceuticals, microplastics, per- and polyfluoroalkyl substances (PFAS) and endocrine-disrupting compounds are increasingly discharged in treated wastewater, are not effectively removed by conventional secondary treatment, and have important implications for the environment and human health. Advanced tertiary treatment is needed for safe reuse of urban non-potable water, but the choice among competing technologies is a complex decision involving technical, economic, environmental and regulatory considerations, as well as uncertainty and reluctance in expert evaluations. Exploring the Gaps in Programming for Men and Women with a Gambling Disorder in the Correctional System in Canada.

The prevalence of problem gambling in the correctional system is about 10 times higher than in the general population, but programming for gambling problems in the correctional system is scarce. Examined barriers to treatment for problem gamblers in the correctional system, sex or gender differences, and options for program design. We conducted interviews with 16 experts who had worked with problem gamblers in the correctional system and analyzed the data in terms of themes. For barriers, the experts discussed a lack of awareness about problem gambling, as well as stigmatization and lockdowns.

Breaking the Barrier of Brain Disease Therapeutics: Advocating Targeted Drug Delivery for Improved Neuro-Resident Interventions.

This paper provides an expert review of extant research into commercially utilized targeted drug delivery systems for brain therapeutics, with Glioblastoma Multiforme (GBM), Parkinson's Disease (PD), Alzheimer's Disease (AD) and Major Depressive Disorder (MDD), as case studies. While the paper highlights the progress made in developing effective drug delivery solutions, a common denominator across much of the research is the singular challenge posed by the Blood Brain Barrier (BBB) against the bulk of drug delivery options and treatments. The paper identifies critical pathways and transporting

Automated Diagnosis of Hepatitis B Using Multilayer Mamdani Fuzzy Inference System.

In this research, a new multilayered mamdani fuzzy inference system (Ml-MFIS) is proposed to diagnose hepatitis B. The proposed automated diagnosis of hepatitis B using multilayer mamdani fuzzy inference system (ADHB-ML-MFIS) expert system can classify the different stages of hepatitis B such as no hepatitis, acute HBV, or chronic HBV. The expert system has two input variables at layer I and seven input variables at layer II. At layer I, input variables are ALT and AST that detect the output condition of the liver to be normal or to have hepatitis or infection and/or other problems. The furthe

Carbamazepine extended-release capsules: a new treatment option for bipolar I disorder.

Carbamazepine has a long history in psychiatry and neurology. More recently, carbamazepine extended-release capsules have been approved by the US Food and Drug Administration for the treatment of manic and mixed episodes associated with bipolar I disorder. This decision represents a significant step forward for the agent, which was first described to have efficacy in the disorder in the early 1970s. Approval of carbamazepine extended-release capsules was aided by two large placebo-controlled, 3-week trials. Using nearly identical protocols, these trials demonstrated that monotherapy with carba

Using cognitive task analysis to facilitate the integration of decision support systems into the neonatal intensive care unit.

OBJECTIVE: New medical systems may be rejected by staff because they do not integrate with local practice. An expert system, FLORENCE, is being developed to help staff in a neonatal intensive care unit (NICU) make decisions about ventilator settings when treating babies with respiratory distress syndrome. For FLORENCE to succeed it must be clinically useful and acceptable to staff in the context of local work practices. The aim of this work was to identify those contextual factors that would affect FLORENCE's success. METHODS: A cognitive task analysis (CTA) of the NICU was performed. First,

[Artificial intelligence in sleep analysis (ARTISANA)--modelling visual processes in sleep classification].

We describe a novel approach to the problem of automated sleep stage recognition. The ARTISANA algorithm mimics the behaviour of a human expert visually scoring sleep stages (Rechtschaffen and Kales classification). It comprises a number of interacting components that imitate the stepwise approach of the human expert, and artificial intelligence components. On the basis of parameters extracted at 1-s intervals from the signal curves, artificial neural networks recognize the incidence of typical patterns, e.g. delta activity or K complexes. This is followed by a rule interpretation stage that i
