What is the place for non-operative management of neck of femur fractures: analysis of the Australia and New Zealand Hip Fracture Registry (ANZHFR).

BACKGROUND: Hip fractures cause significant morbidity and mortality worldwide, a burden expected to rise with population ageing. Operative management remains the mainstay of treatment; however, a small number of patients receive non-operative treatment. The Australian and New Zealand Hip Fracture Registry (ANZHFR) began recording reasons for non-operative management in 2021. The study aimed to identify factors influencing the decision for non-operative management, compare outcomes between operative and non-operative treatment and compare palliative patients who died ⩽30 days to those who survi

The Neck of Femur Fracture in an Elderly Patient with Schizophrenia and Parkinsonism Managed with Dual Mobility Total Hip Replacement- A Rare Case Report.

INTRODUCTION: Neck of femur fractures is the most common fractures in elderly people as they present with osteoporosis. There is a higher chance of proximal femur fracture in a schizophrenic patient as there is a loss of bone mineral density due to a drug-induced increase in the level of prolactin and immobility. Patients with Parkinson's disease were at higher risk for the development of hip fractures due to gait disturbances and instability. CASE REPORT: A 63-year-old male patient presented with complaints of the right arm pain, swelling, deformity of the right arm, and difficulty in extend

An Unexpected Complication of Residual Fibular Strut Graft While Performing A Press fit Hemiarthroplasty for A Secondary Nonunion Fracture Neck of the Femur.

INTRODUCTION: There have been fewer than 150 cases of Bizarre Parosteal Osteochondromatous proliferation (Nora Lesions) reported in the literature to date and no significant reports on angular deformities caused by this lesion. Nora's lesion can easily be misdiagnosed as osteochondroma or chondrosarcoma and therefore inappropriately managed. Although this condition classically appears in the second or third decade, we present to you a three year old boy who presented with multiple bony swellings over the hand and feet that caused an angular deformity of the involved digits. In the case report

Tensile type of stress fracture neck of femur: role of teriparatide in the process of healing in a high risk patient for impaired healing of fracture.

Neck of femur stress fractures, whilst representing only around 8% of stress fractures is associated with a high morbidity. Radiographically, stress fractures of neck of femur are classified into compression and tensile type. Tensile type is notorious for displacement and hence prophylactic surgery is indicated, unlike stable compression fracture. Currently recombinant Parathyroid Hormone (PTH) is used in the treatment of osteoporosis. More recently, many animal studies suggest that the recombinant PTH is helpful to achieve fracture union in normal model and in impaired fracture healing model.

Avascular necrosis of the femoral epiphysis complicating a minimally displaced fracture of solitary bone cyst of the neck of the femur in a child. A case report.

Reported herein is a rare case of a patient with a pathologic fracture through a cyst in the femoral neck that resulted in avascular necrosis of the femoral epiphysis. This type of necrosis is commonly understood to follow displaced fractures of the femoral neck in children, but has also been reported to follow undisplaced fractures.

Enhanced hip fracture management: use of statistical methods and dataset to evaluate a fractured neck of femur fast track pathway-pilot study.

The increasing elderly population and subsequent rise in total hip fracture(s) in this group means more effective management strategies are necessary to improve efficiency. We have changed our patient care strategy from the emergency department (ED), acute orthopaedic wards, operating theatre, post-operation and rehabilitation, and called it Fracture Neck of Femur Fast Track Pathway. All clinical data and actions were captured, integrated and displayed on a weekly basis using 'signalfromnoise' (SFN) software. The initial four months analysis of this project showed significant improvement in pa

Finite element analysis of sliding vs static configuration of partial threaded cannulated compression screws for Pauwels type III fracture neck of femur.

BACKGROUND: Cannulated Cancellous Screws fixation is done either as a sliding or static configuration according to the number, thread length and orientation of the screws. This study aims to compare the biomechanical properties of 3-CCS as sliding configuration and BDSF & 4-CCS (Alpha configuration) as static configuration, by finite element analysis for Pauwels type III fracture neck of femur. METHODS: DICOM format images of saw-bone were used to build the 3-D geometric models, which were hypermeshed to create models of Pauwels type III FNF with an angle of 65°. CAD models of three configura

Valgus-impacted subcapital neck of femur fractures: a systematic review, meta-analysis with cost analysis of fixation in-situ versus nonoperative management.

BACKGROUND: The management of the valgus-impacted neck of femur fracture (AO/OTA 31-B1) remains contentious. The objective of this study was to determine whether operative intervention is cost-effective. METHODS: We conducted a systematic review using electronic databases (Medline, Embase, Cochrane, Ebsco, Scholar) identifying studies published in the English language concerning valgus-impacted neck of femur fractures until June 2022. Additional studies were identified through hand searches of major orthopaedic journals, and bibliographies of major orthopaedic textbooks. MeSH terms (hip fract

Anterior hip dislocation with ipsilateral displaced fracture neck of femur treated by open reduction and internal fixation: case report and review of the literature.

Combined fracture neck femur with anterior dislocation of the head of femur is a very rare injury with little resources available in the literature. This case report describes a case that was treated by open reduction and internal fixation with 2-year follow-up showing good radiological and functional outcome. Functional evaluation was done according to Merle d'Aubigné score modified by Matta JM, which takes into account the presence of pain, ability to walk and joint range of motion. The measured functional score was 15 points out of a maximum of 18, signifying a good postoperative result. Ea

Meyer's Muscle Pedicle Bone Graft a Novel Procedure in Treatment of Neglected Fracture Neck of Femur in Pediatric Age Group-A Case Report.

Meyers muscle pedicle graft has been used in delayed presentation and non-union of neck femur fracture in adults with good results. Delayed presentation or non-union neck femur in children is not uncommon in children. Subtrochanteric valgus osteotomy with or without fixation has been suggested with varying results. We present meyers muscle graft being used in paediatric age group with excellent result at 3 years follow-up. There are limited report literature of meyers muscle pedicle graft being used in children.

Unusual presentation of altered bone metabolism with long-term antiepileptic therapy: pathological fracture of neck of femur due to brown tumor at calcer.

The antiepileptic drugs are known to alter the bone metabolism and result in various range of pathological conditions (osteoporosis, osteomalacia, vitamin D deficiency, secondary hyperparathyroidism). These clinical conditions developed either due to direct effect of these therapeutic agents on bone or due to alteration in the homeostasis of other regulators of bone metabolism like serum calcium, vitamin D, and parathyroid hormone. Long-term therapy with antiepileptic agents results in higher incidence of pathological fracture, which is a consequence of either osteoporotic or osteomalasic (sec

[Femoral neck fractures in adults: joint sparing operations. II. The significance of surgical timing and implant for development of aseptic femur head necrosis].

Statistical analysis of the results of the AO multi-center study (Part I) shows significant advantages of early (within 24 h) operative stabilization of the medial neck fracture (P < 0.05) and of use of the dynamic hip screw (DHS) as implant (P < 0.01). Secondary surgery necessitated by implant complications led to higher rates of aseptic femoral head necrosis (FHN) and non-unions (NU) (P < 0.05). According to the so-called logistic regression, the rate of NU is influenced most by the number of implant complications and the choice of implant, the rate of FHN by the interval time-lapse between

Does open or closed reduction with internal fixation reduces the incidence of complications in neck of femur fractures in pediatrics: a meta-analysis and systematic review.

The neck of femur fracture (FNF) in children is a rare injury with a high incidence of complications such as avascular necrosis (AVN), coxa-vara and nonunion. The aim of this review is to compare the incidence of complications between open reduction with internal fixation (ORIF) and closed reduction with internal fixation (CRIF) of FNF in children. Two independent reviewers searched EMBASE, MEDLINE, COCHRANE and PUBMED databases from inception until April 2022 according to the preferred reporting items for systematic reviews and meta-analyses guidelines. Studies included comparison of complica

International Normalized Ratio (INR) Sample Rejection in Neck of Femur Fracture Patients: A Retrospective Closed-Loop Study From a Major UK Trauma Centre.

Background Patients presenting with neck-of-femur (NOF) fractures often require urgent surgery, as prolonged delays beyond 36 hours are associated with increased morbidity, mortality, and length of hospital stay, whereas shorter time-to-surgery intervals have been shown to improve outcomes. Many of these patients are elderly and on anticoagulant therapy; therefore, making accurate International Normalized Ratio (INR) assessment is crucial for determining surgical readiness and anaesthetic safety. The INR reflects the extrinsic pathway of coagulation and is prolonged in patients on warfarin or
