Prospective assessment of incomplete ovarian suppression in breast cancer using LC-MS/MS and immunoassays for estradiol.

BACKGROUND: During gonadotrophin-releasing hormone agonist (GnRHa) therapy some patients do not adequately suppress estradiol, termed "ovarian escape" (OE). We prospectively measured estradiol using liquid chromatography-tandem mass spectrometry (LC-MS/MS), the gold standard to determine the prevalence of OE and assess discordance with estradiol immunoassays. METHOD: Patients receiving endocrine therapy plus GnRHa for ≥3 months were enrolled at two Dublin hospitals. Estradiol was measured by LC-MS/MS, Abbott Alinity i (Alinity) and Roche Cobas e 801 (Cobas) immunoassays. OE was defined as est

Identification of key genes modules linking brain aging signatures and COVID-19-associated cognitive impairment.

BACKGROUND: Emerging evidence indicates that SARS-CoV-2 infection is associated with aging, particularly in memory-related domains. However, the molecular mechanisms underlying COVID-19-associated aging, often referred to as "brain fog," remain poorly understood. This study aimed to identify key genes implicated in post-COVID cognitive dysfunction. METHODS: Transcriptomic data from the frontal cortex, obtained from the GEO dataset GSE188847 and GSE164332, were analyzed using differential expression analysis to identify differentially expressed genes (DEGs) associated with brain fog. Functiona

Real-world progression-free survival and overall survival in patients with HR+/HER2- advanced breast cancer treated in first-line with ribociclib, endocrine monotherapy or chemotherapy: Results from the observational RIBANNA study.

Cyclin-dependent kinase 4 and 6 inhibitors (CDK4/6i) combined with endocrine therapy are the preferred choice for first-line treatment of patients with HR+/HER2- locally advanced/metastatic breast cancer (aBC). The CDK4/6i ribociclib in combination with an aromatase inhibitor (AI) or fulvestrant (FUL) has demonstrated significant progression-free survival (PFS) and overall survival (OS) benefits for pre- and postmenopausal aBC patients who were enrolled in the three pivotal MONALEESA trials. Following the initial approval of ribociclib in 2017, the non-interventional RIBANNA study was initiate

A 5-year review of genomic medicine in breast cancer: insights from C-CAT data on 3776 Japanese patients.

BACKGROUND: In Japan, despite 5 years since CGP tests were covered by insurance in 2019, low drug accessibility rates remain a critical issue. We evaluated drug accessibility in 3776 breast cancer from the C-CAT database using two criteria: the proportion first linked to PMDA-approved drugs with phase III trial evidence for breast cancer through CGP tests but not existing Companion diagnostics [CDx] (*), and the proportion first linked to PMDA-approved drugs including based on phase I and II trial evidence (**). Additionally, cases linked to investigational drugs for non-PMDA-approved drugs we

Estrogen receptors as therapeutic targets in breast cancer.

The estrogen receptor alpha (ERalpha) has proven to be the single most important target in breast cancer over the last 30 years. The use of the selective ER modulator (SERM) tamoxifen for the treatment and prevention of breast cancer has changed therapeutics. The SERM raloxifene, approved for the treatment of osteoporosis, lacks tamoxifen's increased risk for endometrial cancer and is being evaluated for the prevention of breast cancer. Other SERMs approved or under development for use against breast cancer or osteoporosis include toremifene, GW5638, GW7604 (the active metabolite of GW5638), i

Estradiol Acts in Lateral Thalamic Region to Attenuate Varicella Zoster Virus Associated Affective Pain.

Varicella zoster virus (VZV) results in chicken pox and herpes zoster. Female rats show a higher level of herpes zoster associated pain than males, consistent with human studies. In this study, we addressed the novel hypothesis that sex difference in herpes zoster associated pain is due, in part, to estradiol modulating activity in the thalamus. To test this hypothesis a high and low physiological dose of estradiol was administered to castrated and ovariectomized rats and the affective pain response was measured after injection of VZV into the whisker pad. Thalamic infusion of the estrogen rec

Cajaninstilbene acid relaxes rat renal arteries: roles of Ca2+ antagonism and protein kinase C-dependent mechanism.

Cajaninstilbene acid (CSA) is a major active component present in the leaves of Cajanus cajan (L.) Millsp. The present study explores the underlying cellular mechanisms for CSA-induced relaxation in rat renal arteries. Vascular reactivity was examined in arterial rings that were suspended in a Multi Myograph System and the expression of signaling proteins was assessed by Western blotting method. CSA (0.1-10 µM) produced relaxations in rings pre-contracted by phenylephrine, serotonin, 9, 11-dideoxy-9α, 11α-epoxymethanoprostaglandin F(2α) (U46619), and 60 mM KCl. CSA-induced relaxations did not

Regulation of avian osteoclastic H+ -ATPase and bone resorption by tamoxifen and calmodulin antagonists. Effects independent of steroid receptors.

We used highly purified avian osteoclasts and isolated membranes from osteoclasts to study effects of tamoxifen, 4-hydroxytamoxifen, calmodulin antagonists, estrogen, diethylstilbestrol, and the anti-estrogen ICI 182780 on cellular degradation of 3H-labeled bone in vitro and on membrane HCl transport. Bone resorption was reversibly inhibited by tamoxifen, 4-hydroxytamoxifen, and trifluoperazine with IC50 values of approximately 1 microM. Diethylstilbestrol and 17-beta-stradiol had no effects on bone resorption at receptor-saturating concentrations, while ICI 182780 inhibited bone resorption at

Effects of Alcohol and Estrogen Receptor Blockade Using ICI 182,780 on Bone in Ovariectomized Rats.

BACKGROUND: Estrogen signaling is essential for the sexual dimorphism of the skeleton, is required for normal bone remodeling balance in adults, and may influence the skeletal response to alcohol. High levels of alcohol consumption lower bone mass in ovary-intact but not ovariectomized (ovx) rats. However, the extremely rapid rate of bone loss immediately following ovx may obscure the effects of alcohol. We therefore determined (i) whether heavy alcohol consumption (35% caloric intake) influences bone in sexually mature ovx rats with established cancellous osteopenia and (ii) whether ICI 182,7

Placenta defects and embryonic lethality resulting from disruption of mouse hydroxysteroid (17-beta) dehydrogenase 2 gene.

Hydroxysteroid (17-beta) dehydrogenase 2 (HSD17B2) is a member of aldo-keto reductase superfamily, known to catalyze the inactivation of 17beta-hydroxysteroids to less active 17-keto forms and catalyze the conversion of 20alpha-hydroxyprogesterone to progesterone in vitro. To examine the role of HSD17B2 in vivo, we generated mice deficient in Hsd17b2 [HSD17B2 knockout (KO)] by a targeted gene disruption in embryonic stem cells. From the homozygous mice carrying the disrupted Hsd17b2, 70% showed embryonic lethality appearing at the age of embryonic d 11.5 onward. The embryonic lethality was ass

Raloxifene acutely stimulates nitric oxide release from human endothelial cells via an activation of endothelial nitric oxide synthase.

Raloxifene is a selective estrogen receptor modulator (SERM) clinically effective for the prevention of postmenopausal osteoporosis. Estrogen's effect on cardiovascular diseases is mainly dependent on direct actions on the vascular wall. Since raloxifene has an endothelium-dependent relaxing effect, we studied the effects of this molecule on nitric oxide (NO) release from cultured human umbilical vein endothelial cells. Clinically effective concentrations of the compound triggered a rapid and dose-dependent release of NO from endothelial cells. Raloxifene-induced NO production was dependent on

Mutant NPM1-regulated estrogen signaling promotes leukemia cell survival by upregulating HGF and represents a therapeutic vulnerability.

Acute myeloid leukemia (AML) with mutated nucleophosmin 1 (NPM1) presents therapeutic challenge and exhibits a striking female predominance in clinical. However, the contribution of gender-specific factors, such as sex hormones to the pathogenesis and therapeutic implications in NPM1-mutated AML remains elusive. In the present study, we found that NPM1-mutated AML showed female bias, particularly among the young, pre-menopausal age (20-49 years) group, and that female exhibited inferior overall survival compared to males. Subsequently, 17β-estradiol (E2), the primary circulating estrogen in pr

Real-world treatment patterns and outcomes in Indian patients with HR+/HER2- advanced breast cancer treated with palbociclib.

BACKGROUND: Palbociclib, a CDK4/6 inhibitor, was approved by FDA in 2015 for the treatment of hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2-) advanced breast cancer (ABC). It was approved based on clinical benefits observed in PALOMA-1/TRIO-18 and PALOMA-2 Trial of Palbociclib. Despite its benefits in clinical studies, the data related to real-world outcomes are limited. This retrospective real-world observational study was designed to evaluate effectiveness of Palbociclib among Indian patients diagnosed with HR+/HER2- ABC. METHODS: The study was con

Using Deep Learning Models of Gene Regulation to Guide Drug Prioritization.

Background: Drug repurposing offers a cost-effective strategy to accelerate therapeutic discovery, but most computational approaches do not model noncoding genetic variation. Because over 90% of genome-wide association study (GWAS) risk variants reside in noncoding regions, linking regulatory variation to therapeutic hypotheses remains a major challenge. Methods: We developed an integrative deep learning framework that links allele-specific enhancer prediction to candidate therapeutics through two complementary prioritization strategies, a transcription factor (TF)-based and a gene-based appro
