Remimazolam besylate reduces hypoxemia during painless gastrointestinal endoscopy in elderly patients: study protocol for a multicenter, randomized, controlled trial.

BACKGROUND: Hypoxemia and circulatory depression are common complications in elderly patients undergoing procedure sedation anesthesia (PSA) for gastrointestinal (GI) endoscopy. Remimazolam besylate, a novel ultra-short-acting benzodiazepine, is postulated to offer a superior safety profile with reduced respiratory depressant effects compared to propofol. This study aims to compare the incidence of hypoxemia of remimazolam besylate versus propofol for painless GI endoscopy in elderly patients through a multicenter randomized controlled trial. METHODS: This study is a multicenter, double-blind

Reduced perioperative sensory impairment could lower postoperative delirium incidence: a before-and-after study in older patients with hip fracture.

BACKGROUND: Postoperative delirium (POD) is a common complication in older patients. Multicomponent prevention reduces POD but requires substantial resources. The effect of single interventions derived from multicomponent programs remains elusive, but it is essential to assess their effect to identify which components are meaningful. This study investigates whether a single intervention from a multicomponent program may reduce the incidence of POD. METHODS: This prospective, single-center before-and-after study evaluated older surgery patients at a university hospital's geriatric trauma cente

Preoperative nutritional evaluation of prostate cancer patients undergoing laparoscopic radical prostatectomy.

BACKGROUND AND OBJECTIVE: Prostate cancer (PCa) is one of the most common malignant tumors in men. Geriatric Nutritional Risk Index (GNRI) is an objective index for evaluating nutritional status of elderly people over 65 years old. The aim of the current study was to explore the correlation and predictive value between GNRI and postoperative recovery and complications in PCa patients undergoing laparoscopic radical prostatectomy (LRP). METHODS: Taking 98 as the GNRI boundary value, 96 PCa patients (aged≥65 y) undergoing LRP in the Department of Urology, Affiliated Hospital of North Sichuan Me

Does Postoperative Cognitive Decline Result in New Disability After Surgery?

OBJECTIVE: Establish whether POCD is associated with new disability after surgery, which would inform whether POCD impacts patient-centered outcomes. BACKGROUND: POCD is a decline in neuropsychiatric tests scores from presurgical baseline which occurs in approximately 15% of older patients 3 months after surgery. POCD is a research construct meant to investigate patient and family reports of older adults who were "never the same after surgery." However, many patients with POCD do not perceive difficulty with thinking and memory, and the question remains whether POCD impacts patient function.

Healthcare delivery to elderly and unfit patients with breast disease and comorbidities under an outpatient regime: A report of a personal surgical technique named "Cut&Sew".

INTRODUCTION: A growing need for proper geriatric assessment and short-stay surgical programs supported by the availability of less invasive approaches, even in ambulatory settings, is being recognized as a feasible option for breast cancer patients with comorbidities who are usually distressed after standard surgery with ordinary hospitalization. Few studies have been conducted in Italian breast centers with dedicated techniques and approach for frail patients with breast diseases due to a jeopardized approach to ambulatory surgery among institutions. METHODS: This study included 58 women di

Early Postoperative Actigraphy Poorly Predicts Hypoactive Delirium.

STUDY OBJECTIVES: Delirium is a postoperative complication accompanied by disturbances in attention, cognition, arousal, and psychomotor activity. Wrist actigraphy has been advocated to study inactivity and inferred sleep patterns during delirium. We hypothesized that altered patterns of motor activity or immobility, reflective of disordered sleep and wakefulness patterns, would serve as predictive markers of hypoactive postoperative delirium. METHODS: Eighty-four elderly surgical patients were classified into three groups based on the timing of hypoactive delirium following surgery: intact w

Comparison of pre- and intra-operative analgesia of fascia Iliaca compartment block in reducing post-operative delirium of elderly patients following hip fractures: a retrospective study.

BACKGROUND: Hip fractures significantly impact the physiological and psychological well-being of elderly patients. Post-operative delirium (POD) is a common complication after hip fractures in this population, severely affecting treatment outcomes and recovery. Effective pain management during the peri-operative period is crucial for reducing POD, yet the best analgesic approach remains debated. METHODS: A retrospective study (from January 2015 to December 2020) included 198 elderly hip fracture patients who received preoperative (PO-) or intraoperative (IO-) fascia iliaca compartment block (

A Comparison of the Clinicoradiological Outcomes of Intertrochanteric Fractures Treated Using Proximal Femoral Nail and Proximal Femoral Nail Anti-rotation.

Background Managing intertrochanteric fractures presents challenges for orthopedic surgeons, not only in fixing the fracture but also in preventing and managing associated complications, especially in the vulnerable geriatric population. Cephalomedullary nails are commonly used for surgical fixation due to their favorable functional profile, which preserves the hip's abductor lever arm and proximal femur anatomy. However, there's a lack of data comparing two major options: proximal femoral nail (PFN) and proximal femoral nail anti-rotation (PFNA). This study aimed to compare the radiological f

Patient screening and medical evaluation for implant and preprosthetic surgery.

Implant and preprosthetic surgeries aim to restore normal anatomic contours, function, comfort, aesthetics, and oral health. As such, they are not life-saving procedures. The prime concern must therefore be to not undermine the patient's overall health and safety. It is then that every step must be taken to select the appropriate treatment plan and maximize the longevity of the implanted system, including the overlying prostheses. One important category into which a number of possible complications may fall is the inadequate systemic screening of patients prior to implant and biomaterial inser

Remimazolam is associated with superior cerebral and pulmonary protection over propofol in elderly thoracic surgery: a real-world study validated by propensity score matching.

BACKGROUND: Selecting the optimal anesthetic regimen to mitigate postoperative cognitive decline and pulmonary complications in elderly patients undergoing thoracic surgery remains a clinical challenge. While propofol is widely used, remimazolam offers potential advantages due to its hemodynamic stability and rapid clearance. This study employed rigorous propensity score matching (PSM) to compare the real-world outcomes of these two intravenous anesthetics in a high-risk surgical cohort. METHODS: We conducted a retrospective cohort study of patients aged ≥65 years undergoing elective VATS lob

Utility of iliac vein stenting in elderly population older than 80 years.

BACKGROUND: The geriatric population aged 80 years and older with severe manifestations of chronic venous disease face diminishing therapeutic options. Self-applied compression is often not possible because of frailty or arthritis. Significant limb swelling diminishes mobility, affects independent living, and precipitates institutionalization. Limb ulceration and pain diminish quality of life at a time when it is paramount. Cellulitis uniquely poses septicemic risk in this age group. Family caregivers are often able to continue home care if the intensity of care can be reduced. Iliac vein sten

Are There Differences in Performance Among Femoral Stem Brands Utilized in Cementless Hemiarthroplasty for Treatment of Geriatric Femoral Neck Fractures?

BACKGROUND: For the vast majority of displaced femoral neck fractures in older patients, cemented femoral fixation is indicated because it is associated with a lower risk of periprosthetic fracture than cementless fixation. Nevertheless, cementless fixation continues to be utilized with high frequency for hip fractures in the United States. It is therefore helpful to understand the performance of individual cementless brands and models. Although prior studies have compared femoral stems by design type or stem geometry, there may still be a difference in revision risk according to femoral stem

Bilateral Intertrochanteric Femur Fractures in a Paraplegic Patient: A Case Report.

Bilateral intertrochanteric (IT) femur fractures are rare, and appropriate evaluation and treatment can vary depending on concurrent patient comorbidities. Even less has been described for patients with bilateral IT fractures with pre-existing paraplegia. This case report describes the unique case of a 72-year-old paraplegic female who presented with bilateral IT femur fractures due to a wheelchair accident. The patient was treated with single-stage bilateral cephalomedullary nail fixation so she could effectively transfer to and from the wheelchair with less pain and a greater chance of fract
