INTRODUCTION: Colorectal operations such as an extra-levator abdominoperineal (elAPE) excision for locally advanced or recurrent cancer create a significant perineal tissue deficit. Options for perineal reconstruction include bilateral pedicled gracilis muscle flaps (BPGMF). Fashioning the gracili into a 'weave' creates a muscular sling that supports pelvic contents and is a novel technique. Our series reports the outcomes of the BPGMF in 50 patients undergoing surgery for pelvic cancer.

We present a 78-year-old female who had undergone rectal excision due to Crohn's disease of the rectum with loss of fecal continence. This led to a chronic perineal sinus and vaginal fistula which was closed with a Martius procedure using a bulbocavernosus fat flap. The sinus healed uneventfully after 4 weeks. In our opinion, the gracilis muscle was too large for this defect. We suggest use of this flap in female patients with small persistent perineal sinuses.

Despite technical advances, the ability to restore motor function following a brachial plexus avulsion is limited. Twenty patients who suffered the loss of elbow flexion following a brachial plexus avulsion injury underwent a neurotization procedure in an attempt to restore that lost function. Facial paralysis following acoustic neuroma (AN) resection can be devastating, but timely and strategic intervention can minimize the resulting facial morbidity. BACKGROUND: Devastating extremity injuries are prevalent but most often survivable on the modern battlefield. The complexity of these injuries requires advanced methods of reconstruction. This study is designed to validate the feasibility of gracilis myocutaneous flap transplantation via microvascular free tissue transfer in a porcine model. The susceptibility to exercise-induced myopathy was studied by histological and biochemical methods in various skeletal muscles of mice 3-4 days after a single bout of prolonged running. Microsurgery is defined as surgery performed with the aid of ocular magnification. In Denmark, this is undertaken by four units. This review describes the history of microsurgery which evolved during the 1960s. Microsurgery in hand surgery is primarily replantation and revascularisation but also peripheral nerve surgery as well as brachial plexus surgery. Lymphoedema is being treated with super microsurgery on an experimental basis. Dynamic reconstruction of facial palsy is performed in a two-stage operation with cross-over nerve graft and a free microvascular muscle flap, typically gracilis.

Mutations in the skeletal muscle ryanodine receptor (RYR1) gene are associated with a wide range of phenotypes, comprising central core disease and distinct subgroups of multi-minicore disease. The diagnostic criteria for tendinitis of the knee are: (1) strict localization of the pain in the whole length of the tendon from its body to its end; (2) recrudescence of pain during movements that stretch or rub the tendon, and (3) positive local anaesthesia test if needed. Only this approach makes it possible not to miss a tendinitis of the extensor system in a so-called femoro-patellar syndrome, or to misdiagnose a tendinitis of the sartorius, gracilis and semitendinosus muscles when confronted with an osteoarticular pathology of the medial femoro-tibial compartment.

The transverse upper gracilis (TUG) flap is a free musculocutaneous (type II) flap consisting of a segment of the proximal gracilis muscle and a 25x10 cm skin paddle oriented transversely. The vascular pedicle of the TUG flap is the ascending branch of the medial circumflex femoral artery with two venae comitantes. The pedicle length is 6 cm and the diameter of the artery is 1.6 mm. In the year 2002, seven patients had breast reconstruction by the free TUG flap. There were three primary and four secondary reconstructions.
