Cardiac rupture, a clinically predictable complication of acute myocardial infarction: report of 70 cases with clinicopathologic correlations.

OBJECTIVE: To test the hypothesis that certain clinical events may precede free wall myocardial rupture and allow its prediction, we conducted a retrospective and prospective study of 70 patients with rupture. BACKGROUND: Rupture of the left ventricular free wall develops in approximately 10% of patients with fatal acute transmural myocardial infarction. Clinically, its occurrence has been considered precipitous and unexpected. Pathologically, however, rupture appears to be a stuttering, progressive process characterized in many instances by an infiltrating intramural hemorrhage and a thrombu

Transjugular approach percutaneous closure: a preferred solution for challenging surgical management of ventricular septal rupture.

BACKGROUND: Ventricular septal rupture (VSR) is a rare but life-threatening complication following myocardial infarction (MI). While traditional management typically involves surgical repair, percutaneous closure techniques are increasingly being considered, particularly in cases where surgery is challenging or patients are high risk. CASE PRESENTATION: We present the case of a 62-year-old male with anterior ST-segment elevation MI, complicated by a large VSR. Transthoracic echocardiography (TTE) revealed an 11.8-mm VSR, hypokinetic anteroseptal and anterior walls, and an ejection fraction of

Predictive Factors of Pericardial Effusion After a First Acute Myocardial Infarction and Successful Reperfusion.

The aim of the study was to identify the determinants of pericardial effusion (PE) after a first myocardial infarction (MI). Cardiac magnetic resonance enables early analysis of multiple post-MI parameters; 193 patients with a first ST-elevation MI admitted to the Angers University Hospital (France) were enrolled prospectively. Cardiac magnetic resonance was performed at baseline (median of 5 days [4 to 7]) and repeated at a 3-month follow-up to investigate left ventricular (LV) volumes, LV ejection fraction, infarct size, microvascular obstruction (MVO), systolic wall stress (SWS), and PE pre

[Transesophageal echocardiography in cardiogenic shock in acute posterior wall infarct with rupture of the papillary muscles].

A 63-year-old male patient suffered an acute myocardial infarction entirely confined to the posterior wall. As symptoms persisted and cardiogenic shock developed immediate heart catheterization was performed, which demonstrated an occlusion of the left circumflex branch. Right-heart catheterization demonstrated an enlarged v-wave. Thus, acute infarct associated mitral regurgitation was suspected. In the catheterization laboratory transesophageal echocardiography including Doppler- and contrast echocardiography was then performed, which revealed the diagnosis of posterior papillary muscle ruptu

[Encapsulated pseudoaneurysm post-left ventricular rupture: unusual presentation and follow-up].

External cardiac rupture with the development of a pseudoaneurysm is an uncommon complication of acute myocardial infarction (AMI) and do not always have typical presentations and can be challenging to diagnose. We present the case of a 59-year-old woman with no relevant cardiovascular history whose disease course was unusual. She initially presented with chest pain, dyspnea, and fever, and was diagnosed and treated for community-acquired pneumonia. Due to persistent symptoms, she went to the hospital where an electrocardiogram showed signs of an infarction. An echocardiogram revealed apical a

Cardiac Tamponade and Myocardial Infarction: A Case Report.

In recent times, there has been a concerning rise in the incidence of sudden death among individuals in middle age. Predominantly, cardiovascular diseases emerge as the leading cause behind such untimely fatalities. Myocardial infarction and its subsequent complications stand out as the most frequently encountered scenarios in these tragic events. Despite being a relatively uncommon occurrence, cardiac tamponade represents one of the rare yet fatal complications that can ensue following a myocardial infarction. This condition manifests when the pericardial cavity becomes filled with either blo

Endogenous interleukin-22 prevents cardiac rupture after myocardial infarction in mice.

Myocardial infarction (MI) can result in fatal myocardial rupture or heart failure due to adverse remodeling and dysfunction of the left ventricle. Although recent studies have shown that exogenous interleukin (IL)-22 shows cardioprotective effect after MI, the pathophysiological significance of endogenous IL-22 is unknown. In this study, we investigated the role of endogenous IL-22 in a mouse model of MI. We produced MI model by permanent ligation of the left coronary artery in wild-type (WT) and IL-22 knock-out (KO) mice. The post-MI survival rate was significantly worse in IL-22KO mice than

Clinical and echocardiographic presentation of postmyocardial infarction papillary muscle rupture.

BACKGROUND: Severe mitral regurgitation (MR) can occur following myocardial infarction (MI) with either partial or complete papillary muscle rupture (pPMR or cPMR). Although the incidence of this complication has significantly decreased, it is still associated with significant mortality. We sought to evaluate the different echocardiographic and clinical presentations of pPMR and cPMR. METHODS AND RESULTS: A review of all the urgent procedures for ischemic MR between January 2000 and June 2016 was performed to identify patients who underwent surgery for PMR. Surgical protocols and echocardiogr

Mitral valve repair after papillary muscle rupture through beating heart adjustment of artificial chordae length.

Acute papillary muscle rupture results in severe mitral regurgitation and hemodynamic instability, and it carries a poor prognosis with a high mortality rate. We present a case of an 86-year-old woman affected by mitral regurgitation due to an acute posterior papillary muscle rupture. The patient underwent a mitral valve repair with annuloplasty and artificial chordae implantation. The neochorda was sutured to the posterior mitral leaflet and fixed through the left ventricle wall on the epicardium. The neochorda length was determined on the beating heart under echocardiographic view.

Cardiac tamponade masking clinical presentation and hemodynamic effects of papillary muscle rupture after acute myocardial infarction.

A 67-year-old woman sustained an acute lateral-wall myocardial infarction and was treated with thrombolytic therapy. Postinfarction hypotension developed 3 days later. Clinical findings at that time were consistent with cardiac tamponade, and an echocardiographic study revealed a moderate-sized pericardial effusion. She underwent urgent pericardiocentesis with transient improvement in hemodynamics, followed by deterioration associated with the development of acute pulmonary edema. Follow-up transesophageal echocardiographic imaging revealed papillary muscle rupture with severe mitral regurgita

Delayed papillary muscle rupture following repair of post-infarction ventricular septal defect.

Ventricular septal defect (VSD) and papillary muscle rupture are severe complications of myocardial infarction which are usually observed individually although their simultaneous occurrence has been observed in pathological studies. An 82-year-old female underwent repair of a post-infarction VSD and 13 days later required mitral valve replacement due to delayed papillary muscle rupture. Mechanical complications of myocardial infarction can occur in the same patient even in delayed fashion and a two-stage correction may be successfully accomplished.

Cardiac rupture associated with thrombolytic therapy: impact of time to treatment in the Late Assessment of Thrombolytic Efficacy (LATE) study.

OBJECTIVES: This prospective ancillary study was conducted to determine the association between the time from symptom onset to treatment and cardiac rupture in patients with acute myocardial infarction. BACKGROUND: There is strong evidence that the time window for thrombolytic therapy should be extended to at least 12 h; however, many clinicians are concerned that late treatment may cause an excessive occurrence of death from cardiac rupture. Because up to 30% of patients with acute myocardial infarction arrive in the hospital > 6 h from symptom onset, resolving this issue is of paramount cli

Left ventricular free-wall rupture that occurred during a cardiopulmonary exercise test.

Although exercise testing has become a standard procedure before discharge for patients with acute coronary syndrome, a fatal accident during the test is extremely rare. A 60-year-old man was admitted for a non-ST-segment elevation myocardial infarction. A coronary angiogram showed stenosis at the distal lesion of the circumflex, and a balloon angioplasty was performed. His recovery was smooth, and a cardiopulmonary exercise test was performed 5 days after admission. At 2.5 metabolic equivalents, he suddenly went into cardiac arrest, and percutaneous cardiopulmonary support was initiated. Echo

Overview of mechanical circulatory support for the management of post-myocardial infarction ventricular septal rupture.

Post-myocardial infarction ventricular septal rupture (PIVSR) is becoming increasingly rare in the percutaneous coronary intervention era; however, the mortality rates remain high. Surgical repair is the gold standard treatment for PIVSR but is associated with surgical difficulty and high mortality. Therefore, the timing of surgery is controversial (i.e. either undertake emergency surgery or wait for resolution of organ failure and scarring of the infarcted area). Although long-term medical management is usually ineffective, several mechanical circulatory support (MCS) devices have been used t

Collagen denaturation in the infarcted myocardium involves temporally distinct effects of MT1-MMP-dependent proteolysis and mechanical tension.

Tissue injury results in profound alterations in the collagen network, associated with unfolding of the collagen triple helix, proteolytic degradation and generation of fragments. In the infarcted myocardium, changes in the collagen network are critically involved in the pathogenesis of left ventricular rupture, adverse remodeling and chronic dysfunction. We hypothesized that myocardial infarction is associated with temporally and spatially restricted patterns of collagen denaturation that may reflect distinct molecular mechanisms of collagen unfolding. We used a mouse model of non-reperfused

Single-cell and spatial transcriptomic profiling of cardiac fibroblasts following myocardial infarction.

Cardiac fibroblasts (CFs) are key mediators of heart repair following myocardial infarction (MI). A specific CF subpopulation, termed Reparative Cardiac Fibroblasts (RCFs), has been shown to orchestrate scar formation and prevent ventricular rupture after MI. However, the timing of RCF appearance and the molecular events underlying this transition remain largely undefined. Here, we present a multi-modal dataset capturing the transcriptional dynamics of CFs during the early phase post-MI. Our integrative dataset combines bulk RNA sequencing, RNAscope in situ hybridization, and spatial transcrip

Ischaemic heart disease. Pato-anatomic findings revealed by comprehensive autopsy technique.

Pato-anatomic findings in epicardial coronary vessels, myocardium and supraventricular parts of the conducting system were investigated by a comprehensive autopsy-technique (17) in a consecutive series of 63 patients dying from ischaemic heart disease. Two thirds of left ventricular acute myocardial infarctions (LV-AMI's) were transmural and one third was sub-endocardial. Coronary thrombosis was seen more often in transmural than in sub-endocardial LV-AMI (p less than 0,05) whereas severe arteriosclerotic stenosis (greater than or equal to 75%) of a least two major arteries was seen more often

Targeting the adaptive immune continuum in atherosclerosis and post-MI injury.

Atherosclerotic disease is a cholesterol-rich lipoprotein particle-driven disease resulting in the formation of atherosclerotic plaques in large and medium size arteries. Rupture or erosion of atherosclerotic plaques can trigger the formation of a thrombus causing the obstruction of the blood flow in the coronary artery and thereby leading to myocardial infarction (MI). Inflammation is a crucial pillar of the mechanisms leading to atherosclerosis and governing the cardiac repair post-MI. Dissecting the complex and sophisticated networks of the immune responses underlying the formation of ather

Cardiac pseudoaneurysm coupled with a rupture of the papillary muscle complicating myocardial infarction.

As a complication of myocardial infarction, dual rupture of the left ventricular myocardium and the papillary muscle is a rare condition. In such a case, the heart is predisposed to reduced output because of unloading of the ventricle during systole, resulting in the patient being in danger of deteriorating into a severe state or dying suddenly from cardiogenic shock. We report a rescued case of a 65-year-old woman, who had cardiogenic shock due to left ventricular pseudoaneurysm, coupled with partial rupture of the posteromedial papillary muscle three weeks after posterior myocardial infarcti

[Acute papillary muscle rupture and mitral valve insufficiency after myocardial infarction. Serious condition, but potentially curable through emergency surgery].

Acute papillary muscle rupture resulting in severe mitral regurgitation is an infrequent but extremely serious mechanical complication of myocardial infarction. The immediate and severe volume overload on the pulmonary circulation may cause pulmonary edema and circulatory shock with secondary organ failure. Diagnosis is preferably based on echocardiography, and urgent mitral valve replacement is the treatment of choice. This paper describes two such patients who were successfully treated by surgery despite cardiogenic shock. In addition, it illustrates the known fact that a papillary muscle ru
