New agents for the treatment of gout and hyperuricemia: febuxostat, puricase, and beyond.

The rising prevalence of gout has led the pharmaceutical industry to rediscover what it had considered a forgotten disease. In April 2009, the Food and Drug Administration (FDA) approved febuxostat (Takeda Pharmaceuticals; Deerfield, IL), the first new urate-lowering gout drug in more than 40 years. In August 2009, the FDA approved colchicine for the treatment of acute gout. Several other pharmaceutical companies are also conducting clinical trials to test new drugs for acute and chronic gout. This article reviews new drugs and drugs in development in the management of acute and chronic gout.

Hyperuricemia is a mediator of endothelial dysfunction and inflammation in renal allograft recipients.

BACKGROUND: Hyperuricemia is common in renal transplant recipients treated with calcineurin inhibitors. Uric acid induces glomerular hypertension, microvascular disease, and renal interstitial fibrosis and is an independent risk factor for cardiovascular complications. The mechanisms by which uric acid injures renal allografts and the cardiovascular system remain unclear. OBJECTIVE: To assess the influence of uric acid on biomarkers of endothelial dysfunction and inflammation in renal allograft recipients. PATIENTS AND METHODS: The study included 78 allograft recipients with normal allograft

Pharmacological basis for use of Pistacia integerrima leaves in hyperuricemia and gout.

ETHNOPHARMACOLOGICAL SIGNIFICANCE: Pistacia integerrima Stew ex. Brandis is an important component of commonly dispensed traditional dosage forms. We wished to determine whether polyphenolic constituents of this plant could be useful in oxidative stress and have potential to counter hyperuricemia. MATERIAL AND METHODS: Radical scavenging activity was determined by 1,1-diphenyl-2-picrylhydrazyl (DPPH) and xanthine oxidase (XO) inhibitory activity assay in vitro. Fructose (FRS) induced hyperuricemic animal model was used to asses the serum uric acid (UA) lowering effect by plant products. RESU

Urate oxidase in the prophylaxis or treatment of hyperuricemia: the United States experience.

Nonrecombinant urate oxidase (Uricozyme, Sanofi-Synthélabo, Inc, Paris, France) is a highly effective uricolytic agent, but its use is associated with hypersensitivity reaction manifested mainly by bronchospasm in approximately 5% of patients. Recently, several multi-institutional studies have evaluated the efficacy and safety of a recombinant urate oxidase (rasburicase). In a phase I/II study, all 131 patients with newly diagnosed acute lymphoblastic leukemia (ALL) or stage III/IV non-Hodgkin's lymphoma (NHL) experienced rapidly decreased plasma uric acid concentrations after receiving recomb

Urate oxidase in prevention and treatment of hyperuricemia associated with lymphoid malignancies.

Standard prophylaxis and treatment of malignancy-associated hyperuricemia in the USA has been allopurinol with vigorous hydration, urinary alkalinization and osmotic diuresis. Urate oxidase, the enzyme that converts uric acid to allantoin (a readily excreted metabolite that has 5- to 10-fold higher solubility than uric acid), is an alternative therapy; however, few published findings support this practice. Between February 1994 and December 1996, we administered non-recombinant urate oxidase (Uricozyme) to 126 children with newly diagnosed non-B cell acute lymphoblastic leukemia (ALL) during t

Renal handling of urate in two patients with hyperuricemia and primary hyperparathyroidism.

Two patients with primary hyperparathyroidism had hyperuricemia due to the decrease in urate clearance. In analysis by 4-component model system, the tubular secretion of urate commonly decreased without changes in either filtered urate or presecretory reabsorption of urate. Both patients had a reduction of urea clearance, and both parathyroidectomy in the former case and intravenous infusion of saline in the latter case could reduce the serum urate level associated with the increase in the ratio of urate clearance to creatinine clearance. It is of interest that the former case with a higher se

Pathogenesis, animal models and pharmacological treatments of hyperuricemia: A systematic review.

Hyperuricemia, a common metabolic disorder, arises primarily from excessive uric acid production or impaired excretion, resulting in abnormal elevation of uric acid in extracellular fluids and tissues. Sustained hyperuricemia is closely linked to the development of gout, kidney diseases, and cardiovascular disorders. To date, hyperuricemia pathogenesis remains incompletely elucidated, with key challenges in animal model selection, therapeutic drug characterization, and the development of emerging treatments. Notably, increasing research attention has focused on traditional Chinese medicine (TC

Effects of ABCG2 dysfunction on hyperuricemia progression in premenopausal and postmenopausal women.

Serum uric acid (SUA) levels are influenced by environmental and genetic factors, such as sex and ABCG2 variants. Their poorlyunderstood links to menstrual status prompted us to conduct this study to evaluate the effects of menstrual status-related ABCG2 function, detectable from the presence of its dysfunctional variants, on SUA levels. Recruited for the present study were 9244 Japanese individuals, comprising 4466 women and 4778 men. We evaluated the effect size on SUA and population attributable fractions (PAFs) for hyperuricemia progression based on factors, including ABCG2 function, BMI,

Advances in the study of gut microecology and mechanisms of hyperuricemia and gouty arthritis.

Gouty arthritis is a metabolic disorder caused by purine metabolism dysregulation, characterized by monosodium urate crystal deposition in and around joints, triggering acute articular inflammation via NLRP3 inflammasome activation and IL-1β-mediated inflammatory cascades. While hyperuricemia represents a critical biochemical prerequisite for gouty arthritis development, elevated serum urate levels do not invariably lead to the disease. Mounting evidence suggests a significant relationship between gut microbiota and the pathogenesis of both gouty arthritis and hyperuricemia. The gut microbial

Biomimetic bioreactor for potentiated uricase replacement therapy in hyperuricemia and gout.

INTRODUCTION: Uricase replacement therapy is a promising approach for managing hyperuricemia and gout but is hindered by challenges such as short blood circulation time, reduced catalytic activity, and excessive hydrogen peroxide (H2O2) production. These limitations necessitate innovative strategies to enhance therapeutic efficacy and safety. METHODS: We designed and synthesized RBC@SeMSN@Uri, a red blood cell-coated biomimetic self-cascade bioreactor, which encapsulates uricase (Uri) and a selenium-based nano-scavenger (SeMSN) within RBC membranes. This design aims to reduce immunogenicity,

[Features of diet in patients with cardiometabolic diseases with gout and hyperuricemia: A review].

Gout, like hyperuricemia, is often associated with serious cardiometabolic and renal comorbidities that lead to persistently elevated rates of premature mortality in the population. Traditional dietary recommendations given to patients to prevent purine load require revision. This article holistically reviews the relevant scientific rationale and available evidence to provide evidence-based dietary recommendations for the prevention and treatment of hyperuricemia and gout and its cardiometabolic comorbidities.

Current updates for hyperuricemia and gout in age-related macular degeneration.

The escalating prevalence of metabolic syndrome poses a significant public health challenge, particularly among aging populations, with metabolic dysfunctions contributing to pro-inflammatory states. In this review, we delved into the less recognized association between hyperuricemia (HUA), a manifestation of metabolic syndrome and a primary risk factor for gout, and age-related macular degeneration (AMD), a sight-threatening ailment predominantly affecting the elderly. In recent years, inflammation, particularly its involvement in complement pathway dysregulation, has gained prominence in AMD
