[Mitochondrial encephalomyopathies].

Mitochondrial encephalomyopathies include various syndromes involving both muscles and the nervous system. They are characterized by morphological and/or functional mitochondrial abnormalities. Relevant histological modifications in muscle are ragged-red fibers with or without cytochrome C oxidase (COX) activity. Neuropathological alterations in the brain are not specific. They consist of spongiosis with or without preferential involvement of territories of "system degeneration", neuronal loss, focal necrosis, capillary proliferation and mineral deposits. Their topographic patterns are charact

Steatocystoma multiplex with bilateral preauricular sinuses in four generations.

A rare case of steatocystoma multiplex and bilateral preauricular sinuses in four generations is presented along with a review of the literature. The cause, pathogenesis, and treatment of steatocystoma multiplex is discussed. Steatocystoma multiplex (SCM) is a rare clinical disorder characterized by numerous recurrent cutaneous cysts. Since the first case described by Bosellini in 1898, both nonfamilial and autosomal dominant inheritance patterns have been described. There have been at least 13 well-documented nonfamilial cases. However, since Noonjin and Reynolds in 1948 first described the a

The genetics of idiopathic clubfoot.

Idiopathic clubfoot is an uncommon congenital deformity that clusters in families but does not fit typical Mendelian inheritance patterns. Studies done on twins, different incidences in various ethnic groups, and transmission between generations all suggest a genetic component to clubfoot causation. Complex segregation analysis strongly supports the hypothesis that one major gene is responsible for a large portion of the risk for having a clubfoot. A few studies suggest an environmental causation of clubfoot, but are not persuasive. Clubfoot may be etiologically and/or genetically heterogeneou

Mathematical Model to Assess Mutational Burden in Retinal Dystrophy Patients Negative to Mendelian Genetic Tests and Carriers of Multiple Recessive Variants.

PURPOSE: Despite advancements in genetic testing, a significant number of patients with retinal dystrophy receive negative test results due to single-allele mutations in genes with autosomal recessive inheritance patterns. This study aimed to address this diagnostic challenge by proposing a preliminary mathematical model based on mutational burden analysis. This approach can identify unique digenic or oligogenic combinations, potentially prioritizing them for further investigation. METHODS: We retrospectively analyzed clinical and genetic data from 527 subjects comprising 453 patients with re

Transmission ratio distortion of NF1 variant alleles in familial neurofibromatosis type 1.

PURPOSE: Neurofibromatosis type 1 (NF1) is a common autosomal dominant tumor-predisposition syndrome (approximately 1:3000 worldwide), involving pigmentary, skeletal, and neurodevelopmental features, and lifelong tumor risk. Although NF1 has been traditionally assumed to follow Mendelian transmission, our analyses reveal a consistent deviation from this expectation. METHODS: We analyzed transmission patterns in 322 NF1 families across 4 well-characterized cohorts, applying strict inclusion criteria to minimize ascertainment bias and to exclude possible mosaic cases. Subsampling and large-scal

Mitochondrial cardiomyopathy: bridging molecular mechanisms and clinical frontiers.

Genetic cardiomyopathies caused by pathogenic variants in nuclear DNA (nDNA) that encodes contractile sarcomere proteins are among the best understood of all the cardiomyopathies. By contrast, mitochondrial cardiomyopathy is caused by a dysfunction in mitochondrial oxidative phosphorylation due to pathogenic variants in either nDNA or the maternal mitochondrial DNA (mtDNA). Unlike contractile protein defects, which generally follow predictable Mendelian inheritance patterns, mitochondrial cardiomyopathy is genetically complex as a result of the distinctive characteristics of the mitochondrial

Developing and assessing the utility of a You-Tube based clinical genetics video channel for families affected by inherited tumours.

We have designed and implemented the first worldwide You Tube channel with 22 videos covering common questions asked in familial cancer susceptibility clinics. We discuss the use of the videos including demographics of registered You Tube users, and what lessons have been learnt about how the general public uses medical information online. The most popular video on inheritance patterns has been watched on average 84 times per month. The mostly highly viewed videos include inheritance patterns, breast cancer screening and hereditary non-polyposis colorectal cancer. Registered viewers were more

Long telomere inheritance through budding yeast sexual cycles.

The ends of linear eukaryotic chromosomes are protected from being recognized as DNA double-strand breaks by telomeres, containing repetitive DNA sequences that bind specific proteins. In humans, mutations in telomere regulatory genes lead to short or long telomere syndromes. These syndromes often show genetic anticipation, where the disease has an earlier onset and a more severe manifestation in each new generation. Later generations inherit not only the mutation affecting telomere length, but also abnormal telomere length. Many aspects of telomere length homeostasis are conserved between mam

The Complexity of Familial Inheritance in Pectus Excavatum: A Ten-Family Exome Sequencing Analysis.

Background/Objectives: Pectus excavatum (PEx) is considered, at least partially, a familial disorder. A variety of inheritance patterns, associations with genetic syndromes, and pathogenic variants have been reported. However, the etiology of this condition is still not completely understood, and no known genes have been identified as definitive contributors. Methods: Family members with a confirmed PEx diagnosis (one proband and two first-degree relatives) and non-affected members were recruited into this study. Exome sequencing was performed on all affected familial PEx cases to systematical

The Genetic Landscape of Paediatric Cataract in Saudi Arabia: A Two-Decade Cohort with Novel Variants, Genotype-Phenotype Correlations, and Bioinformatic Analysis.

Background/Objectives: Paediatric cataract is among the most common treatable causes of childhood blindness, caused by a genetically diverse disorder with variable clinical features. Although genetic factors significantly contribute to the development of paediatric cataracts, recent data on their genetic makeup and genotype-phenotype relationships in Saudi Arabia is limited. This study aims to investigate the genetic spectrum, inheritance patterns, and genotype-phenotype correlations of paediatric cataract in a Saudi population over twenty years. Methods: We conducted a retrospective cohort st

Intracameral perfluoropropane for the management of acute hydrops following keratoconus recurrence in a corneal graft.

Keratoconus is an axial corneal ectasia of obscure etiology. It has been associated with many systemic and ocular diseases as well as with a variety of inheritance patterns. Penetrating keratoplasty (PK) has been highly successful in treating advanced cases. Hydrops within the transplanted material is unusual after PK. We report the two cases of hydrops postkeratoplasty in a keratoconus patient successfully managed with intracameral perfluoropropane injection.

Congenital absence of lacrimal puncta and of all major salivary glands: case report and literature review.

A 7-year-old girl had dry mouth and recurrent infections of the lacrimal fistulae with decreased lacrimal secretion. All four puncta were absent, and a Schirmer test showed decreased lacrimal secretion. Salivary gland imaging with sodium pertechnetate 99mTcO4 showed absence of all major salivary glands. Lower lip biopsy disclosed normal structure of the salivary gland. No evidence of abnormal inheritance patterns could be demonstrated.

Distinct genetic patterns and natural history of OPA1-related auditory neuropathy in Chinese population.

BACKGROUND: Auditory neuropathy (AN) represents a clinical manifestation of OPA1-related diseases. The diagnostic process of these diseases is challenging owing to the broad spectrum of intermediate phenotypes and diverse inherited patterns. The aim of this study was to comprehensively delineate the feature of OPA1-related patients in a Chinese AN cohort, encompassing the incident rate, inherited pattern, detailed audiological characteristics, and genotype-phenotype correlation. METHODS: Between 2003 and 2020, 452 unrelated probands with a diagnosis of AN were referred to our laboratory for m

Bilateral absence of the kidneys and ureters. Three cases reported in one family.

Three infant boys with bilateral absence of the kidneys and hypoplasia of the lungs are described. Two of the infants were brothers and the third was a first cousin. They were born to 2 sisters whose husbancs were unrelated to their wives and to each other. None of the parents had renal problems. The occurrence of this syndrome in 2 male sibs is suggestive of an autosomal recessive inheritance pattern which has been previously described. An additional male first cousin born to the mother's sister is sugesstive of sex-linked inheritance for this particular family, an inheritance pattern not pre

Autosomal recessive frameshift variant broadens HECW2-related disease spectrum.

The HECW2 gene encodes a HECT (homologous to E6-AP carboxy terminus)-type E3 ubiquitin ligase that plays a critical role in neurodevelopment. Pathogenic variants in HECW2 are associated with a neurodevelopmental disorder characterized by hypotonia, seizures, and absent language (NDHSAL; OMIM #617268), typically inherited in an autosomal dominant pattern. However, recent evidence suggests that some variants in HECW2 gene may cause the disease with autosomal recessive inheritance. Here, we report a 6-year-old female proband of consanguineous Afghan descent who presented with severe microcephaly,

Genetic study of Alport syndrome in Tunisia.

BACKGROUND: Alport syndrome is a genetic disorder affecting the kidneys, ears, and eyes, causing chronic kidney disease, sensorineural hearing loss, and ocular abnormalities. It results from pathogenic variants in the COL4A3, COL4A4, or COL4A5 genes, with different inheritance patterns: X-linked from COL4A5 variants, autosomal recessive from homozygous variants in COL4A3 or COL4A4, digenic from variants in both COL4A3 and COL4A4, and autosomal dominant from heterozygous variants in COL4A3 or COL4A4. METHODS: We analyzed 45 patients with Alport syndrome from 11 Tunisian families to determine t

Exposure to drugs of abuse induce effects that persist across generations.

Substance use disorders are highly prevalent and continue to be one of the leading causes of disability in the world. Notably, not all people who use addictive drugs develop a substance use disorder. Although substance use disorders are highly heritable, patterns of inheritance cannot be explained purely by Mendelian genetic mechanisms. Vulnerability to developing drug addiction depends on the interplay between genetics and environment. Additionally, evidence from the past decade has pointed to the role of epigenetic inheritance in drug addiction. This emerging field focuses on how environment
