[Evaluation of the degree of disability for the privately insured patient (author's transl)].

There are marked differences between the public health system and the private insurance system. This paper shows how to evaluate the degree of permanent disability according to "The General Rules for Private Insurances (AUVB 1965)". At the beginning the evaluation of the degree of permanent disability after the visus naturalis or the visus corrected and at unilateral and bilateral aphakia will be discussed. The paper gives detailed information on the degree of disability for: one injured eye, one injured eye with preexisting ocular lesion, bilateral injury, one injured eye with preexisting les

Spending Growth In A Large Massachusetts Insurer Driven By Volume And Intensity.

Claims data from Blue Cross Blue Shield of Massachusetts from 2018 through the first half of 2025 reveal that recent spending growth was driven primarily by utilization and prescription drug spending, especially for GLP-1 medications, rather than by growth in inflation-adjusted prices for medical services. Changes in service mix contributed little to spending growth, and coding intensity has been increasing.

In sickness and in health insurance: Gender in household benefit plan choice.

I document that households in the United States are more likely to choose health plans offered by the husband's employer than the wife's. This gap may reflect gender disparities in employment status, plan quality, or an inherent gender effect. Using a novel identification strategy that holds employment status and plan quality fixed, I find that the gender effect increases the probability of selecting the husband's plan by 6.3 percentage points.

"Is It Free?": Out-of-Pocket Costs for Annual Checkups Among the Insured.

Preventive services are intended to be provided without cost-sharing under the Affordable Care Act (ACA), yet many patients still incur out-of-pocket (OOP) costs during routine annual checkups where such services are typically delivered. This study examines the prevalence and predictors of OOP payments among insured adults and assesses how both perceived affordability and awareness of ACA preventive care provisions relate to financial experiences and future preventive care intentions. Using original survey data from a statewide sample of 912 insured adults in Pennsylvania who had received an a

Trends in Mental Health Diagnoses Among Publicly Insured Children.

IMPORTANCE: Children living in poverty are at increased risk of mental health and neurodevelopmental disorders. Little is known about the trends in diagnoses of these disorders among children enrolled in public insurance programs, such as Medicaid, which insure more than 1 in 3 US children. OBJECTIVE: To provide comprehensive, multistate estimates of changes in the percentage of publicly insured children with mental health and/or neurodevelopmental disorder diagnoses. DESIGN, SETTING, AND PARTICIPANTS: This serial, cross-sectional study used administrative claims data from 22 states to test

Changes in Colorectal Cancer Screening Modalities Among Insured Individuals.

IMPORTANCE: Several colorectal cancer (CRC) screening modalities (eg, colonoscopy, stool-based tests) are currently available. Yet, recent national patterns in screening using these modalities and potential differences across sociodemographic groups remain unclear. OBJECTIVE: To evaluate possible changes in the use of different CRC screening modalities among average-risk individuals aged 50 to 75 years in the US. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used deidentified claims data from 2017 to 2024. Participants included a dynamic cohort of commercially insured Bl

Insurance coverage and consistent pricing is needed for over-the-counter naloxone.

With the Food and Drug Administration's approval of the first over-the-counter naloxone nasal spray in 2023, it was expected that access to naloxone nasal spray would increase and that its cost would be reduced. However, the writers of this commentary found varying insurance coverage of naloxone during purchase attempts at local pharmacies. Failure to cover naloxone can reduce access and increase risk of overdose death. We suggest federal policy implementation that requires universal insurance coverage of at least one formulation of naloxone and to utilize national opioid settlement funds to p

Characterizing Medical Complications in Publicly Insured Youth With Eating Disorders.

PURPOSE: The present study describes the occurrence of eating disorder (ED)-related medical diagnoses in a publicly insured sample of youth with EDs. The study also compares ED medical diagnoses with other psychiatric disorders and identifies high-risk demographic groups. Improved screening practices are needed in public mental health systems where treatment is critical for youth with EDs. METHODS: Medicaid claims data were obtained from the state of California, including beneficiaries ages 7-18 who had at least one service episode between January 1, 2014, and December 31, 2016. From this pop

Polypharmacy among medicaid-insured children with and without documented obesity.

OBJECTIVE: Polypharmacy increases the risk of drug-drug interactions and adverse drug events. As obesity and rates of obesity-associated comorbid chronic conditions continue to rise, an improved understanding of whether children with obesity experience higher risk of polypharmacy is needed. This study aimed to compare chronic medication polypharmacy prevalence among children with and without a diagnosis of obesity. METHODS: We performed a cross-sectional examination of prescription data for children aged 2-18 years prescribed ≥1 chronic medication using the 2019 Marketscan Medicaid database.

Pulmonary exacerbations in insured patients with bronchiectasis over 2 years.

BACKGROUND: Patients with bronchiectasis experience persistent symptoms and frequent pulmonary exacerbations; this study investigated the frequency of exacerbations and all-cause hospitalisation. METHODS: This longitudinal, retrospective, claims database study (IBM® MarketScan®) identified patients aged ≥18 years from 1 July 2015 through 30 September 2018. Exacerbations were identified by bronchiectasis inpatient claim or a healthcare interaction, followed by antibiotic prescription within 7 days. Patients with ≥36 months of continuous health plan enrolment (12 months preceding the first bron

Previous Incarceration, Health Insurance, and the Affordable Care Act in the U.S.

INTRODUCTION: This study examines the association between prior incarceration and health insurance status and whether living in a state adopting the Affordable Care Act (ACA) Medicaid expansion moderates this relationship. METHODS: Data are from the National Longitudinal Study of Adolescent to Adult Health (Wave I [1993-1994], Wave IV [2008], and Wave V [2016-2018]; N=8,965). Multiple logistic regression with multiplicative interaction terms were performed to assess the relationship between previous incarceration and ACA Medicaid expansion on (1) being insured and (2) being on public health i

A review of American insurance coverage and criteria for conservative management of lymphedema.

OBJECTIVE: Lymphedema affects >1 in 1000 Americans, most often resulting from breast cancer surgery. Conservative treatment, such as compression garments, combined decongestive therapy (CDT), and pneumatic compression pumps, is the current standard of care. Despite the wide availability of these therapies, lymphedema has remained undertreated worldwide. We investigated whether third-party insurance coverage might be a barrier to obtaining conservative treatment in the United States. METHODS: We conducted a cross-sectional analysis of publicly accessible insurance policies. A total of 58 insur

Insurance Coverage of Prophylactic Mastectomies: A National Review of the United States.

BACKGROUND: Breast cancer is associated with a multitude of risk factors, such as genetic predisposition and mutations, family history, personal medical history, or previous radiotherapy. A prophylactic mastectomy (PM) may be considered a suitable risk-reducing procedure in some cases. However, there are significant discrepancies between national society recommendations and insurance company requirements for PM. MATERIALS AND METHODS: The authors conducted a cross-sectional analysis of insurance policies for a PM. One-hundred companies were selected based on the greatest state enrolment and m

Insurance coverage and financing landscape for HIV treatment and prevention in the USA.

In 2010, the US health insurance system underwent one of its most substantial transformations with the passage of the Affordable Care Act, which increased coverage for millions of people in the USA, including those with and at risk of HIV. Even so, the system of HIV care and prevention services in the USA is a complex patchwork of payers, providers, and financing mechanisms. People with HIV are primarily covered by Medicaid, Medicare, private insurance, or a combination of these; many get care through other programmes, particularly the Ryan White HIV/AIDS Program, which serves as the nation's

The epidemiology of stifle joint disease in an insured Swedish dog population.

BACKGROUND: Stifle joint diseases (SJD) are common in dogs and include a variety of diagnoses. The objective of the study was to provide an overview of the epidemiology of SJD in insured dogs. METHODS: An historical single cohort study of dogs insured in Agria Pet Insurance (2011-2016) in Sweden was performed. Incidence and relative risk (RR) of SJD was calculated for the whole dog population and for subgroups divided by breed, breed group and sex. RESULTS: The study population included almost 600,000 insured dogs (>1.7 million dog-years). Ninety-three different stifle joint diagnoses were r

The epidemiology of cruciate ligament rupture in an insured Swedish dog population.

Cruciate ligament rupture (CLR) is a common orthopedic disorder in dogs. The study objectives were to evaluate incidence rate (IR), cause-specific mortality rate (CSMR) and risk factors for CLR in insured dogs. A single cohort study of dogs insured in Agria Pet Insurance in Sweden (2011-2016) was performed. Age at diagnosis, IR, CSMR and relative risk (RR) for CLR was calculated overall and per breed. The cohort included just over 600,000 dogs. The IR of CLR was 23.8 (95% confidence interval, 23.1-24.6) cases per 10,000 DYAR. The breeds with highest RR of CLR were Boerboel and Dogo Canario, wh
