How useful are pharmacological treatments in eating disorders?

The eating disorders anorexia nervosa (AN) and bulimia nervosa (BN) are multifactorial syndromes of unknown origin which occur typically in female adolescents or young women. Nowadays, AN and BN are most often triggered by dietary restriction. Both are treatable conditions. As in other psychiatric disorders, a lower comorbidity, a shorter duration of illness, less familial psychopathology, and, in AN, a higher minimal weight have been shown to be associated with a better outcome. So far, no abnormalities specific to AN or BN that would shed light on their etiology have been identified. Control

The distinct roles of assessment method and theoretical framework in predicting functional impairment and alcohol use.

OBJECTIVE: The DSM-5 Alternative Model for Personality Disorders (AMPD) and Kernberg's object relations theory are prominent frameworks for assessing personality dysfunction. This study investigates their structural overlap and evaluates their specific capacity to predict clinical outcomes using a multi-method approach. METHODS: A sample of 189 Hungarian psychotherapy inpatients (127 females, 55 males, 7 undisclosed; mean age = 34.0 years) were assessed using four personality dysfunction measures (SCID-5-AMPD, STIPO-R, LPFS-BF, IPO) alongside measures of functional impairment (WSAS) and alcoh

Characteristics and treatment of women with antenatal and postpartum depression.

OBJECTIVE: Investigations of the efficacy of treatment for non-psychotic pregnancy-related mood disorders are scarce. Thus, a prospective, longitudinal study of six months duration, involving ninety-six index cases and forty-five healthy women as a reference group, was implemented to determine the response of mood, parenting stress and dyadic adjustment to an eclectic management. METHOD: In this naturalistic study, the index cases were offered treatment consistent with their symptoms, context, and level of compliance. All women received individual psychotherapy combining strategies from Inter

Assessment and Treatment of Attachment-Related Aspects in Adolescents with Personality Disorders In recent years the relevance of attachment-related aspects in psychotherapy research has increased. This approach offers an opportunity to test the effectiveness of specific psychotherapeutic procedures for personality disorders in adulthood. Although there are a number of novel psychotherapeutic interventions for personality disorders that were adapted for adolescents, there is still a lack of research on studies that consider attachment-related characteristics. The present study demonstrates att

[Stress and depression-a neurobiological perspective].

Depression is a common and often very debilitating disease causing a high number of years lost to disability worldwide. Mortality rates are high due to suicide and depression-associated somatic disorders, which seem to have a bidirectional connection. Depression is considered to be stress associated. Adverse life events such as losses, interpersonal conflicts, financial issues, unemployment, and loneliness are often found in the patient history. Also childhood maltreatment is a known risk factor. Chronic stress can cause maladaptive changes in different neurobiological systems and may contribu

Treatment of Hypoactive Sexual Desire Disorder Among Women: General Considerations and Pharmacological Options.

Hypoactive sexual desire disorder (HSDD) is a persistent or recurrent absence of sexual fantasies and desire for sexual activity, causing marked personal distress or interpersonal difficulties. HSDD affects 10% of U.S. women and is associated with depression and other negative emotional states. It is imperative that psychiatrists are competent to make this diagnosis and are aware of available treatment options. A full psychiatric and medical history are necessary to identify potential causes or contributing factors that may need to be addressed first. The authors discuss the diagnostic tools a

Giovanni Liotti (1945-2018): the Pied Noir of research in attachment and psychotherapy.

This article is a portrait of Giovanni Liotti, eminent psychiatrist and scholar of attachment theory who recently passed away. In this paper, we recall some fundamental steps in his professional and personal life: Liotti's encounter and friendship with Bowlby; Liotti's construction of a bridge between cognitive therapy (of which he was a pioneer) and attachment theory; the interest in attachment disorganization as a precursor of dissociative symptoms and syndromes in adolescence and adulthood; his appreciation for Janet's ideas (which Liotti helped bring back to the attention of clinicians and

The future of tic disorder treatment.

Competing theories on the etiology and treatment of chronic tic disorders and Tourette syndrome have long made the search for efficacious intervention more challenging for patients and families seeking to reduce functional impairment related to tic symptoms. These symptoms were historically posited to be either psychological in origin, leading to the long tradition of psychoanalytic psychotherapy for tics, or biological in nature, particularly since the advent of successful treatments using neuroleptic medications. Current thinking about the phenomenology of tic disorders comes from growing em

Brain and psyche in early Christian asceticism.

This study is an 11-part investigation of the psychology and neuropsychology of early Christian asceticism as represented by Evagrius Ponticus (AD 345-399), the tradition's first ascetical theologian and possibly its mosfinfluential. Evagrius's biography is reviewed in the first section. The living circuinstaii and perceptual consequences of desert asceticism are considered in the second. Penitence, dispassion, and the mysticism of "pure prayer" are discussed in the third. Austerities are addressed in the fourth section, particularly fasting, prostrations, and prolonged standing. Cognitive-behavioral therapy for bulimia nervosa: time course and mechanisms of change.

Cognitive-behavioral therapy (CBT) is an effective treatment of bulimia nervosa, but its mechanisms of action have not been established. In this study the authors analyzed the results of a randomized control trial comparing CBT with Interpersonal Psychotherapy (IPT) to identify possible mediators of change of CBT for BN and its time course of action. Reduction in dietary restraint as early as Week 4 mediated posttreatment improvement in both binge eating and vomiting. Measures of self-efficacy concerning eating behavior, negative affect, and body shape and weight at midtreatment were also sign

An interview study of people with learning disabilities' experience of, and satisfaction with, group analytic therapy.

A number of clinicians have developed psychodynamic treatments for people with learning disabilities, but there have been few studies assessing the impact of psychodynamic therapy with this population. In particular, there have been no studies in which investigators have asked clients about their experience of psychodynamic treatment. The current study is an attempt to rectify this by exploring people with learning disabilities' experiences of, and satisfaction with, two psychodynamic groups provided by an inner city service. Nine learning-disabled clients, four from a sexual offenders' group

A relational approach to group therapy for women with bulimia nervosa: moving from understanding to action.

This article explains how the psychology of women can inform group treatment by translating relational theory (RT) into practice within a short-term outpatient bulimia group. First, the article provides a brief overview of a relational understanding of women's psychological development, the etiology and maintenance of bulimia nervosa, and group psychotherapy. Then, clinical vignettes illustrate the application of RT in practice through discussion of four main healing factors at work in the different stages of the group. Through promoting validation, self-empathy, mutuality, and empowerment, th

Do therapists' subjective variables impact on psychodynamic psychotherapy outcomes? A systematic literature review.

BACKGROUND: Despite growing attention to the general therapist effects in a wide range of clinical settings, little is known about the individual, cross-situational, and therapy-nonspecific variables that impact on the differential effectiveness of clinicians. The current study is a systematic review of the evidence relating to the influence of therapist's subjective characteristics on outcomes of psychodynamic psychotherapies. METHOD: A multistage and systematic search of articles published between 1987 and 2017 identified 30 relevant studies, which were organized into 6 areas according to t

Do personality problems improve during psychodynamic supportive-expressive psychotherapy? Secondary outcome results from a randomized controlled trial for psychiatric outpatients with personality disorders.

Studies involving patients with personality disorders (PDs) have not focused on improvement of core aspects of the PD. The authors examined changes in quality of object relations, interpersonal problems, psychological mindedness, and personality traits in a sample of 156 patients with Diagnostic and Statistical Manual of Mental Disorders (4th ed.) PD diagnoses being randomized to either manualized or nonmanualized dynamic psychotherapy. Effect sizes adjusted for symptomatic change and reliable change indices were calculated. The authors found that both treatments were equally effective at redu

Therapeutic alliance and treatment adherence in two interventions for bulimia nervosa: a study of process and outcome.

The relationship between therapeutic alliance, therapist adherence to treatment protocol, and outcome was analyzed in a randomized trial of cognitive-behavioral therapy (CBT) and interpersonal psychotherapy for bulimia nervosa. Independent observers rated audiotapes of full-length therapy sessions. Purging frequency was the primary outcome variable. There were no significant therapist or Therapist x Treatment effects on outcome. Although results showed high levels of alliance and adherence across treatments, CBT was associated with greater adherence. Across treatments and time points, better a

The effect of Interpersonal and Social Rhythm Therapy (IPSRT) applied to individuals with bipolar disorder on biological rhythms and relapses: A randomized controlled study.

BACKGROUND: Research has shown that people with bipolar disorder often experience disruptions in their social rhythm patterns, leading to frequent episodes. Interpersonal Social Rhythm Therapy (IPSRT) is designed to help maintain these social rhythms and extend the periods without episodes. AIMS: This study aimed to assess the impact of IPSRT, grounded in Peplau's Interpersonal Relations theory, on biological rhythms, interpersonal relationships, and the frequency of relapses in individuals with bipolar disorder. MATERIALS AND METHODS: This randomized controlled trial utilized repeated asses

Yoga-based Psychotherapy Groups for Boys Exposed to Trauma in Urban Settings.

BACKGROUND: Children who experience abuse and neglect and are exposed to adverse life events are at risk of developing emotional and behavioral problems. They may display variable internalizing and externalizing symptoms, such as posttraumatic stress, depression, anxiety, low self-esteem, and aggression. Yoga may be able to regulate body-brain pathways that cause stress following traumatic experiences, thereby reducing adverse mental and physical sequelae. OBJECTIVE: The objective of this preliminary study is to examine changes in functioning following meetings of a yoga-based psychotherapy g

Clinical presentation of depression in the elderly.

Depressive syndromes and dementia are the most frequent psychiatric disorders in the elderly. Reactive depressive syndromes can be associated with the restriction of social competence due to increasing multi-morbidity, loneliness, social isolation, the consequences of retirement, interpersonal conflicts, and the loss of partners or close relatives. Endogenous depressive syndromes in the elderly are frequently combined with diffuse and changing somatic complaints, psychomotor restlessness and agitation. Furthermore, depressive syndromes can be caused by somatic illness, degenerative disorders,
