Diplopia after inferior alveolar nerve block: case report and related physiology.

Although inferior alveolar nerve block is one of the most common procedures performed at dental clinics, complications or adverse effects can still occur. On rare occasions, ocular disturbances, such as diplopia, blurred vision, amaurosis, mydriasis, abnormal pupillary light reflex, retrobulbar pain, miosis, and enophthalmos, have also been reported after maxillary and mandibular anesthesia. Generally, these symptoms are temporary but they can be rather distressing to both patients and dental practitioners. Herein, we describe a case of diplopia caused by routine inferior alveolar nerve anesth

[Value of visually evoked potentials in lesions of the optic nerve caused by a mucocele].

Mucoceles originating in accessory paranasal sinuses with orbit involvement are quite rare. The authors describe two cases of mucoceles, revealed by CT-Scan, the first in the ethmoid-sphenoidal-maxillary sinuses and the other in the ethmoid-frontal sinuses respectively. In both cases Visual Evoked Potentials were most important for assessing optic nerve involvement. They were also useful to check post surgical recovery. These case reports emphasize that early diagnosis of mucoceles involving the optic nerve is very important because prompt surgical treatment may lead to recovery of visual dist

Simultaneous Occurrence of Chicken Pox and Herpes Zoster with Facial Nerve Palsy in Immunocompetent Patient.

Patients who earlier suffered from chicken pox may develop herpes zoster (HZ) due to reactivation in dorsal root ganglia. The occurrence of both clinical conditions at the same time is sporadic. Unusual presentation and a belligerent course are seen in immunocompromised hosts. The presence of both HZ and chicken pox in immunocompetent patients is infrequent and incoherent with the present understanding of pathogenesis. Here, we present a case of HZ involving the maxillary division of the right trigeminal nerve with simultaneous occurrence of varicella-zoster and right peripheral facial nerve p

[The foramen rotundum. Anatomy and radiological explorations. Pathology].

The foramen rotundum is a small canal deeply situated in the base of the skull which represents the way of exit of the maxillary nerve, second branch of the trigeminal nerve. Its precise individualization and analyse is difficult and necessitates a precise and adapted technique as well as a precise knowledge of its anatomical relationships. Its represents a frontier area between the endo- and exocranial spaces. Its involvement which is preferentially related with tumoral pathologies (and particularly with retrograde perineural invasion) profoundly modifies the prognosis of the disease and so s

A rare variation of the infraorbital nerve, entrapping the infraorbital artery.

Anatomical variations are a common feature of the human anatomy. Variation can explain some pathological conditions and is important to keep them in mind during surgical procedures. The relations between nerves and their adjacent arteries have been proposed to play a role in the generation of peripheral trigger migraines. Close opposition between nerves and arteries can lead to vascular compression of the nerve that triggers episodes of pain. We did a routine dissection of the infratemporal fossa and orbital floor by opening the maxillary sinus. Here we report a case where the infraorbital ner

[A case of atypical Tolosa-Hunt syndrome with a contrast enhanced lesion of the oculomotor and trigeminal nerves on MRI].

A 30-year-old woman was admitted to our hospital because of intractable orbital pain and ptosis on the left side. On admission, she had left oculomotor, ophthalmic and maxillary nerves palsy. MRI revealed a contrast enhanced lesion of the left oculomotor and trigeminal nerves through the covernous sinus and orbita. We diagnosed her condition as Tolosa-Hunt syndrome. The orbital pain was resolved within 48-hours by the pulse therapy with intra-venous methylpredonisolone. The cranial nerve palsy was gradually improved, but never reached complete remission. The left oculomotor and trigeminal nerv

Perineural spread in head and neck malignancies.

Perineural tumor spread (PNS) of head and neck malignancies is a well-known form of metastatic disease in which a lesion can migrate away from the primary site along the endoneurium or perineurium. This pattern of spread may create a poor prognosis and require aggressive treatment when curable. Although representative histologies are squamous cell carcinoma and adenoid cystic carcinoma, other malignancies such as malignant lymphoma and sarcoma also can show such a specific pattern of extent. PNS can be insidious, often delaying diagnosis. Knowledge of anatomy of the nerves is crucial in the im

A Case Report on Contralateral Transient Diplopia After Regional Dental Anaesthesia: Do Anatomical Variations Play a Key Role?

One of the most common procedures in everyday dental surgical practice is the inferior alveolar nerve block anesthesia. The procedure is safe, though various complications may arise. Among them, ophthalmological complications such as temporary loss of vision, amaurosis, diplopia, or ophthalmoplegia are very rare, although they do occur. This case report highlights an inadvertent complication of contralateral temporary diplopia after inferior alveolar nerve block anesthesia that was administrated in a patient who was set to undergo root canal treatment. Anatomical variations of the middle menin

Perineural tumour spread from colon cancer, an unusual cause of trigeminal neuropathy--a case report.

Malignant trigeminal neuralgia due to perineural spread along the branches of the trigeminal nerve, is known to commonly occur secondary to squamous cell carcinomas, lymphomas and adenoid cystic carcinomas in the head and neck region. Rarely metastases to the trigeminal nerve have been reported in breast cancer, prostate cancer and colon cancer. To the best of our knowledge trigeminal neuropathy due to skull base metastases and perineural spread along the maxillary (V2) and mandibular (V3) branches of the trigeminal nerve, secondary to colon cancer, has not been previously reported. The diagno

Closed head injury combined with orbital blowout fracture and displacement of the eyeball into the maxillary sinus in a 14-year-old boy: a case report.

BACKGROUND: Trauma-induced orbital blowout fracture (OBF) with eyeball displacement into the maxillary sinus is rare. CASE PRESENTATION: We present the case of a 14-year-old with a closed head injury, OBF, and displacement of the eyeball into the maxillary sinus following a car accident. A prompt transconjunctival access surgery was performed for eyeball repositioning and orbital reconstruction in a single session, mitigating anaesthesia-related risks associated with multiple surgeries. At the 12-month follow-up, his visual acuity was 20/200. Despite limited eye movement and optic nerve atrop

The morbidity associated with fifty maxillary fractures treated by closed reduction.

Fifty patients with maxillary fractures treated by closed reduction over an 85 month period at a Level I trauma center were reviewed. Thirty patients (60%) experienced forty-one midfacial complications. The most frequent problems were infraorbital nerve paresthesia (12), enophthalmus (5), septal deviation (3), and altered vision (3). No relationship could be established between complications and age, sex, cause of injury, or the method of suspension used. Most complications were related to concomitant injury or the concurrent management of fractures of other facial bones. Only eight problems (

Trigeminal trophic syndrome: Strange evolution of maxillofacial surgery.

Trigeminal trophic syndrome (TTS) is a rare facial/cranial affection that arises in ulcerations, itch and paresthesia. Etiology is debated, however trigeminal nerve damage seems to be frequent in pathogenetic patterns. The disease may affect any region innervated by the trigeminal nerve, especially the maxillary branch. A case of TTS, trigged by allergic reaction to osteosynthetic materials and involving infraorbital nerve, was presented. The feature that makes this case one-off in the literature is the association with osteolytic lesion surrounding infraorbital nerve. Diagnosis and treatment

Horner's syndrome and trigeminal nerve palsy after lumbar epidural analgesia for labor and delivery.

This report highlights transient Horner's syndrome and trigeminal nerve palsy following labor epidural analgesia. A 29-year-old primigravida had a lumbar epidural catheter placed for analgesia in labor. The analgesia was maintained by infusion of a dilute local anesthetic/opioid mixture and turned off after achieving complete cervical dilation. Approximately 1 hour after delivery she complained of heaviness in her left eyelid, and was noted to have left-sided ptosis and paresthesia within the distribution of the ophthalmic and maxillary divisions of the trigeminal nerve, which resolved over th

Fasciculation in the early mouse trigeminal nerve is not ordered in relation to the emerging pattern of whisker follicles.

Detailed reconstructions of lengths of the embryonic mouse maxillary nerve were made from serial light and electron microscope sections to determine whether there is any correspondence between the arrangement of fasciculi in the developing nerve and the emerging pattern of whisker follicles on the snout. There was neither correspondence nor obvious pattern in the arrangement of fasciculi at either E11, when trigeminal nerve fibers first contact the presumptive whisker pad, or at E12, when the pattern of developing whisker follicles becomes apparent. Fasciculi merged and branched to form an int

Neurological Complications of Local Anesthesia in Dentistry: A Review.

Local anesthesia is a technique that temporarily desensitizes a specific body area, typically for a surgical procedure, dental work, or pain management. It is described as a sensation loss in a specific area of the body due to depression of excitation in the nerve endings or due to the inhibition of the conduction process within the peripheral nerves. It allows for safer and more comfortable medical procedures, reducing the need for general anesthesia and facilitating faster recovery. Local anesthesia is generally safe, but like any medical intervention, it carries potential risks and side eff

Injectate distribution patterns in posterior infrazygomatic and transoral approaches to the pterygopalatine fossa.

BACKGROUND: Injectate distribution patterns in the pterygopalatine fossa may differ based on the drug administration approach used. This study primarily aimed to assess and compare injectate distribution following the posterior infrazygomatic and transoral approaches. The secondary aim was to evaluate the safety of both approaches. METHODS: Injectate distribution patterns were evaluated in 13 cadaveric head specimens. The vessels were perfused with a gelatin-based solution containing an iodinated contrast agent. The ultrasound-guided posterior infrazygomatic approach and transoral approach we

Effect of maxillary sinus packing with epinephrine hydrochloride-soaked cotton pledgets on complications after canine fossa puncture.

CONCLUSION: Bleeding control with epinephrine hydrochloride-soaked cotton packing in the maxillary sinus after canine fossa puncture (CFP) during operation is a simple method for reducing complications, especially those associated with blood leakage from the maxillary sinus into the subcutaneous tissue. OBJECTIVE: Although CFP is a useful method for extensive maxillary sinus disease, complications can occur associated with nerve injuries or mucosal bleeding. We evaluated whether maxillary sinus packing with epinephrine hydrochloride-soaked cotton pledgets would reduce complications after CFP.
