Review article: Diagnosis and management of enlarged extraocular muscles.

Extraocular muscle (EOM) enlargement may be due to a variety of causes. These causes can be classified in three ways: according to pathogenesis and histopathological features, according to the site, and according to the clinical features. Diagnosis of the cause is dependent upon history, clinical examination, and investigations. Imaging with computed tomography or magnetic resonance imaging and muscle biopsy is typically necessary to make the correct diagnosis. Treatment of the patient must be directed toward the specific cause. This review emphasizes important clinical and pathological guidel

Spinal premotor interneurons controlling antagonistic muscles are spatially intermingled.

Elaborate behaviours are produced by tightly controlled flexor-extensor motor neuron activation patterns. Motor neurons are regulated by a network of interneurons within the spinal cord, but the computational processes involved in motor control are not fully understood. The neuroanatomical arrangement of motor and premotor neurons into topographic patterns related to their controlled muscles is thought to facilitate how information is processed by spinal circuits. Rabies retrograde monosynaptic tracing has been used to label premotor interneurons innervating specific motor neuron pools, with p

Coagulation of posterior cricoarytenoid muscles for abduction spasmodic dysphonia.

INTRODUCTION: Spasmodic dysphonia (SD), a form of focal dystonia, has been defined as a neurogenic, task-specific disorder characterized by abrupt spasms of intrinsic laryngeal muscles that result in phonatory breaks. There are three classic types of SD: adductor SD, abductor (ABSD) and mixed SD. Compared with the more common adductor form, treatment of ABSD with botulinum toxin injection is related to a shorter efficacy and improvement in only about 70% of patients [Blitzer et al. in Laryngoscope 108:1435-1441, 1998]. Moreover, due to the possible loss of efficacy over time or patient refusal

Bilateral absence: A rare anomaly of the gemelli muscles.

The superior and inferior gemelli are deep muscles of the gluteal region, that facilitate the maintenance of a dynamic stability of the hip joint. Their absence could increase the chances of failure in the management of total hip anthroplasty and column acetabular ring fractures. We present a case of an adult female cadaver, with bilateral absence of the superior and inferior gemelli muscles.

Evolution of the strikingly diverse submandibular muscles in Anura.

The most ventral muscles of the head (the mm. submentalis, intermandibularis, and interhyoideus) provide support to the gular region and lift the buccal floor during ventilation and feeding. These muscles show limited variation in most gnathostomes, but in Anura they exhibit a surprising diversity. The few studies that have explored this character system highlighted its potential as a source of phylogenetic information. In this paper we explored the diversity of this character system studying specimens of 567 anuran species and reviewing published data to cover a total of 1321 species, belongi

Fulminant myocarditis with myositis of ocular and respiratory muscles.

A 46-year-old Japanese woman visited a nearby hospital because of diplopia after flu-like symptoms. One month later, she presented with blepharoptosis and external ophthalmoplegia. Laboratory tests showed a high creatine kinase concentration (3146 U/L). She underwent intravenous immunoglobulin therapy; however, her symptoms did not improve, prompting transfer to our institute. On admission, transthoracic echocardiography revealed 30% of left ventricular ejection fraction and edema of the left ventricular wall. Coronary angiography showed no significant coronary stenosis. An endomyocardial biop

Autologous Dermal Mesh in Severe Postpregnancy Recti Muscles Diastasis.

INTRODUCTION: The severe postpregnancy diastasis of recti muscles causes a biomechanical alteration of the statics of the trunk and an unaesthetic bulging of the abdominal wall. This is always combined with cutaneous and subcutaneous excess, which is different on a case-by-case basis. There is not a consensus on the best way to treat it. The anterior sheath plication is the mostly used surgical technique, and many surgeons believe that the use of mesh is necessary in the cases of severe diastasis. The authors here report the outcomes of the use of dermal automesh in the retromuscular layer. M

Segmental scleral buckling without sling sutures around the muscles.

PURPOSE: To introduce a modified technique for segmental buckling through a small in situ conjunctival opening without rectus muscle traction for minimally invasive repair of uncomplicated rhegmatogenous retinal detachment. METHODS: This technique was performed in 15 eyes of 15 selected patients with primary retinal detachment. Conventional traction sutures placed beneath rectus muscles were replaced by an absorbable translimbal traction suture at lesion quadrant. The conjunctival incision was made 8-9 mm posterior and parallel to the limbus. After cryopexy, a minimal explant was fixed with o

Denervation Edema of Hamstring Muscles Following Acute Strain Injury.

A 20-year-old male American football player sustained a first-time, left hamstring strain injury (HSI) during competition. Magnetic resonance imaging indicated involvement of the biceps femoris long head (BFLH) and semitendinosus (ST). Following 3 weeks of rehabilitation, the athlete returned to competition without further incident. Fourteen weeks after injury, magnetic resonance imaging was repeated as part of an ongoing study monitoring HSI recovery. Despite full athletic function, imaging revealed atrophy and increased signal intensity of the BFLH and a portion of the ST. Further testing an

Magnetic resonance imaging of masticatory muscles in basset hounds.

OBJECTIVES: To describe low- and high-field MRI characteristics of occult masticatory muscle lesions in basset hounds. MATERIALS AND METHODS: Retrospective review of the MR images of the heads of 44 basset hounds that had been imaged for reasons unrelated to suspected muscle disease. RESULTS: In most basset hounds, there was reduced bulk of the superficial part of the temporalis and masseter muscles together with bilaterally symmetrical T2W and T1W hyperintense signal with reduced contrast enhancement compared to normal muscle. Interpretation of various pulse sequences suggested that the aff

Hip and trunk muscles activity during nordic hamstring exercise.

The nordic hamstring exercise (NHE) is a dynamic lengthening hamstring exercise that requires trunk and hip muscles activation. Thigh muscles activation, specifically hamstring/quadriceps contractions has been previously examined during NHE. Trunk and hip muscles activity have not been enough studied. The aim of this study was to analyze of hip and trunk muscles activity during NHE. Surface electromyography (EMG) and kinematic data were collected during NHE. Ten healthy men with the age range of 21-36 years performed two sets of two repetitions with downward and upward motions each of NHE. EMG

The hypaxial origin of the epaxially located rhomboid muscles.

In vertebrates, skeletal muscles of the body are made up of epaxial and hypaxial muscles based on their innervation and relative position to the vertebral column. The epaxial muscles are innervated by the dorsal branches of the spinal nerves and comprise the intrinsic (deep) back muscles, while the hypaxial muscles are innervated by the ventral branches of the spinal nerves including the plexus and consist of a heterogeneous group of intercostal, abdominal, and limb as well as girdle muscles. The canonical view holds that the epaxial muscles are derived from the medial halves of the somites, w

Imaging of respiratory muscles in neuromuscular disease: A review.

Respiratory muscle weakness frequently occurs in patients with neuromuscular disease. Measuring respiratory function with standard pulmonary function tests provides information about the contribution of all respiratory muscles, the lungs and airways. Imaging potentially enables the study of different respiratory muscles, including the diaphragm, separately. In this review, we provide an overview of imaging techniques used to study respiratory muscles in neuromuscular disease. We identified 26 studies which included a total of 573 patients with neuromuscular disease. Imaging of respiratory musc

Quantifying spasticity in individual muscles using shear wave elastography.

Spasticity is common following stroke; however, high subject variability and unreliable measurement techniques limit research and treatment advances. Our objective was to investigate the use of shear wave elastography (SWE) to characterize the spastic reflex in the biceps brachii during passive elbow extension in an individual with spasticity. The patient was a 42-year-old right-hand-dominant male with history of right middle cerebral artery-distribution ischemic infarction causing spastic left hemiparesis. We compared Fugl-Meyer scores (numerical evaluation of motor function, sensation, motio

Stiffness mapping of lower leg muscles during passive dorsiflexion.

It is challenging to differentiate the mechanical properties of synergist muscles in vivo. Shear wave elastography can be used to quantify the shear modulus (i.e. an index of stiffness) of a specific muscle. This study assessed the passive behavior of lower leg muscles during passive dorsiflexion performed with the knee fully extended (experiment 1, n = 22) or with the knee flexed at 90° (experiment 2, n = 20). The shear modulus measurements were repeated twice during experiment 1 to assess the inter-day reliability. During both experiments, the shear modulus of the following plantar flexors w

Isozymes of AMP-Deaminase in Muscles Myasthenia Gravis Patients.

Similar symptoms observed in Myasthenia gravis (MG) can be also detected in the case of skeletal muscle AMP-deaminase deficiency. We compared the activity and expression of AMP-deaminase (AMPD) products in skeletal muscles of MG patients and MG-free individuals. The activity of AMP-deaminase in the muscles of MG patients was significantly higher than in the controls and was 2.05 µmol/min/mg protein (±0.31). The two groups differ in level of AMPD product expression. Furthermore in MG-group molecular size of isoform AMPD1 is 90 kDa in contrast to MG-free group where is present 70 kDa isoform of

Central and peripheral motor drive to the palatal muscles.

AIM OF THE STUDY: To characterize the motor command of the soft palate muscles using a magnetic stimulation technique. MATERIAL AND METHODS: Motor evoked potentials (MEPs) were recorded in 10 right-handed and 5 left-handed subjects at the midline of the palate or on the right or left hemipalate to peripheral and cortical magnetic stimulation. RESULTS: Mean palatal MEP amplitude ranged from 0.06 to 0.26mV to peripheral stimulation and from 0.36 to 1.09mV to cortical stimulation. In hemipalate recordings, MEPs to peripheral stimulation had greater amplitude when recorded ipsilaterally to the s

Late cortical disinhibition in relaxed versus active hand muscles.

Recent research suggests that long-interval intracortical inhibition (LICI) is followed by a transitory period of late cortical disinhibition (LCD) that can even lead to a net increase in cortical excitability. The relationship between LICI/LCD and voluntary drive remains poorly understood. Our study aims at investigating the influence of index abduction on LICI and LCD in an actively engaged muscle and a neighboring muscle, while varying the intensity of the conditioning stimulus (CS). Motor-evoked potentials (MEPs) were recorded from the first dorsal interosseus (FDI) and abductor digiti min
