Pain Management in Mild Traumatic Brain Injury: Central Sensitization as a Multispecialty Challenge.

BACKGROUND: Several studies indicate that approximately two-thirds of individuals with mild traumatic brain injury (mTBI) will develop chronic pain, which is often debilitating and a primary factor in long-term disability. Patients with mTBI can suffer concurrently from multiple pain types, such as chronic neuropathic (central or peripheral), nociceptive, or nociplastic pain; however, the prevailing pain types in mTBI patients remain undetermined. This knowledge void limits the formulation of effective therapies for mTBI-related pain. OBJECTIVE: We aimed to identify the predominant pain mecha

Dorsal Root Ganglion Size in Patients With Complex Regional Pain Syndrome of the Lower Extremity: A Retrospective Pilot Study.

OBJECTIVE: Complex regional pain syndrome (CRPS) is a debilitating chronic condition characterized by severe, nociplastic pain along with various other symptoms. Neuromodulation, particularly electrical stimulation of the dorsal root ganglion (DRG), has emerged as a promising intervention for patients with CRPS unresponsive to conventional treatments. However, little is known about the anatomical characteristics of DRGs in patients with CRPS. This study aimed to assess DRG size in patients with CRPS compared with healthy controls. MATERIALS AND METHODS: A retrospective pilot study was conduct

Identification of a central CGRP circuit for trigeminal V1-mediated migraine-like pain in mice.

Migraine attacks can be provoked in people with underlying primary headache disorders by multiple peripheral mechanisms suggesting the possible convergence of trigeminal nociceptive inputs onto a common, unknown central circuit. As calcitonin gene-related peptide (CGRP) expressing cells in the parabrachial nucleus (PBN) have been implicated in nociplastic pains, we hypothesized that (a) PBN CGRP-expressing neurons and their projections to the central amygdala (CeA) may be an essential central relay of multiple peripheral inputs promoting injury-free migraine headache and (b) this pathway may p

Association of inflammatory indices with coronary artery calcium burden in fibromyalgia: A retrospective CT-based cross-sectional study.

Fibromyalgia (FM) may involve systemic abnormalities in addition to its characteristic nociplastic pain features. This study aimed to investigate the association between routinely available inflammatory indices, particularly the neutrophil-to-lymphocyte ratio (NLR), and coronary artery calcium (CAC) burden in patients with FM. A total of 142 patients with FM and 64 controls were included. Computed tomography-derived CAC scores were categorized as < 100 or ≥ 100, with CAC ≥ 400 examined as an exploratory threshold. Multiple inflammatory indices were evaluated, and multivariable logistic regress

Diagnostic Guidance for Chronic Complex Regional Pain Syndrome Type I and Type II from The American Society of Interventional Physicians (ASIPP).

BACKGROUND: Complex Regional Pain Syndrome (CRPS) is a challenging and often disabling condition marked by persistent pain, most commonly in a limb following injury or surgery. It presents with a wide array of symptoms, including intense pain, swelling, alterations in skin color and temperature, motor dysfunction, and trophic changes such as skin and tissue atrophy. While the precise cause of CRPS is not fully understood, it is thought to stem from abnormal nervous system activity, leading to heightened pain sensitivity and inflammatory responses. A thorough understanding of CRPS is essential

Persistent neuropathic-like pain phenotype after arthroscopic repair of massive rotator cuff tears: occurrence, associated factors, and a preoperative four-domain risk score.

BACKGROUND: Persistent neuropathic-like pain may limit recovery after arthroscopic repair of massive rotator cuff tears, but its postoperative occurrence, associated factors, and preoperative risk stratification remain insufficiently characterized. This study estimated the 6-month occurrence proportion, examined associated factors, and developed an internally validated preoperative four-domain risk score. METHODS: This single-center retrospective cohort screened 418 consecutive adults treated between January 2019 and December 2024; 346 met the eligibility requirements, including ascertainable

What to do when the first TNF inhibitor fails in rheumatoid arthritis: stratified expert recommendations from a scoping review and Delphi consensus.

BACKGROUND: Despite widespread use of tumor necrosis factor inhibitors (TNFi) as first-line therapy in rheumatoid arthritis (RA), up to 40% of patients fail initial treatment. Subsequent therapeutic choices remain poorly structured, with limited evidence-based guidance to inform individualized post-TNFi decision-making. OBJECTIVE: To develop evidence-informed, profile-based recommendations to guide treatment selection after inadequate response to a first TNFi in RA, combining evidence and expert consensus. DESIGN: Delphi-based consensus study informed by a PRISMA-guided scoping review (ScR)

Association of Vitamin B6 and Homocysteine-ACTH Coupling With Depressive and Anxiety Symptoms in Burning Mouth Syndrome: An Exploratory Case-Control Study.

BACKGROUND: Burning mouth syndrome (BMS) is a chronic, idiopathic and debilitating orofacial pain disorder, increasingly discussed within oral dysaesthesia and nociplastic pain frameworks, in which psychological distress and stress-related mechanisms are frequently implicated. Vitamin B6, homocysteine and adrenocorticotropic hormone (ACTH) reflect biologically interconnected micronutrient, one-carbon-metabolic and neuroendocrine pathways, but their relevance to BMS and particularly their interrelationships within patients remains unclear. OBJECTIVES: To compare plasma vitamin B6, homocysteine

The Missing Link: Integrating Interventional Pain Management in the Era of Multimodal Oncology.

Cancer-related pain (CrP) is one of the most frequent and debilitating issues that affect the quality of life of patients with cancer. Systemic analgesics, particularly opioids, have been the cornerstone of pain management. However, the following shortcomings of the mentioned therapies, such as side effects, tolerance, and inadequate relief in refractory cases, make implementing a more complete, multimodal treatment plan necessary. Interventional pain management (IPM) uses specific invasive procedures, with different degree of invasiveness, such as nerve blocks, neurolysis, neuromodulation, an

Guideline "diagnosis and non interventional therapy of neuropathic pain" of the German Society of Neurology (deutsche Gesellschaft für Neurologie).

2019 the DGN (Deutsche Gesellschaft für Neurology) published a new guideline on the diagnosis and non-interventional therapy of neuropathic pain of any etiology excluding trigeminal neuralgia and CRPS (complex regional pain syndrome). Neuropathic pain occurs after lesion or damage of the somatosensory system. Besides clinical examination several diagnostic procedures are recommended to assess the function of nociceptive A-delta and C-Fibers (skin biopsy, quantitative sensory testing, Laser-evoked potentials, Pain-evoked potentials, corneal confocal microscopy, axon reflex testing). First line

The correlation between pain and function in cancer survivors.

BACKGROUND: Function is critically important to cancer survivors, who are at risk of losing independence and mobility. But relatively, little is known about how pain, a widely prevalent symptom, contributes to functional decline. Understanding this may lead to earlier intervention and improved clinical outcomes. METHODS: Four hundred eighty-eight participants across five cancer centers with rehabilitation clinics completed the PROMIS Cancer Function Brief 3D profile assessment and the numeric pain rating scale (NRS) during ambulatory clinic visits. Key clinical and demographic factors, includ

The Role of the CCR5 Receptor in Neuropathic Pain Modulation: Current Insights and Therapeutic Implications.

Neuropathic pain, a chronic condition arising from injury or dysfunction of the somatosensory nervous system, is characterized by persistent hypersensitivity and spontaneous pain. The chemokine receptor CCR5 (C-C motif chemokine receptor 5) has recently been identified as a critical mediator in neuroinflammation and neuropathic pain signaling pathways. Expressed on immune cells and neurons, CCR5 regulates immune cell recruitment and activation, thereby contributing to neuronal sensitization and maintenance of pain states. This review examines the currently characterized molecular mechanisms th

Common Non-Rheumatic Medical Conditions Mimicking Fibromyalgia: A Simple Framework for Differential Diagnosis.

Fibromyalgia (FM) is a chronic non-inflammatory disorder mainly characterized by widespread musculoskeletal pain, fatigue, sleep disturbances, and a constellation of other symptoms. For this reason, delineating a clear distinction between pure FM and FM-like picture attributable to other common diseases can be extremely challenging. Physicians must identify the most significant confounders in individual patients and implement an appropriate diagnostic workflow, carefully choosing a minimal (but sufficient) set of tests to be used for identifying the most plausible diseases in the specific case
