Olfactory Cues and Patient Safety Behavior in Virtual Reality Medical Simulation: Controlled Trial.

BACKGROUND: Simulation-based medical education is essential for improving patient safety. In virtual reality (VR)-based simulation, immersion is primarily generated through visual and auditory cues, while other sensory modalities are typically absent. This sensory limitation may reduce the emergence of authentic safety-relevant behaviors. Olfaction plays an important role in clinical reasoning, risk perception, and self-protective behavior and is closely linked to memory and emotion. Although olfactory cues have been shown to influence hand hygiene behavior in real or simulated environments, t

Externalization Errors of Olfactory Source Monitoring in Healthy Controls-An fMRI Study.

Using a combined approach of functional magnetic resonance imaging (fMRI) and noninvasive brain stimulation (transcranial direct current stimulation [tDCS]), the present study investigated source memory and its link to mental imagery in the olfactory domain, as well as in the auditory domain. Source memory refers to the knowledge of the origin of mental experiences, differentiating events that have occurred and memories of imagined events. Because of a confusion between internally generated and externally perceived information, patients that are prone to hallucinations show decreased source me

Parosmia in Right-lateralized Semantic Variant Primary Progressive Aphasia: A Case Report.

Parosmia, defined as the distorted perception of an odor stimulus, has been reported to be associated with head trauma, upper respiratory tract infections, sinonasal diseases, and toxin/drug consumption. To date, little is known about parosmia in right-lateralized semantic variant primary progressive aphasia. A 60-year-old right-handed man presented with a 2-year history of parosmia and prosopagnosia. Brain magnetic resonance imaging demonstrated severe atrophy of the right anterior and mesial temporal lobe, particularly in the fusiform cortex and the regions known as the primary olfactory cor

Molecular Basis of Mammalian Odor Discrimination: A Status Report.

Humans have 396 unique, intact olfactory receptors (ORs), G-protein coupled receptors (GPCRs) containing receptor-specific binding sites; other mammals have more. Activation of these transmembrane proteins by an odorant initiates a signaling cascade, evoking an action potential leading to perception of a smell. Because the number of distinguishable odorants vastly exceeds the number of ORs, research has focused on mechanisms of recognition and signaling processes for classes of odorants. In this review, selected recent examples will be presented of "deorphaned" mammalian receptors, where the O

Prescription Medication Use and Phantom Odor Perception Among US Adults.

INTRODUCTION: Prescription medication use may be associated with phantom odor perception. We evaluated associations between number of prescription medications and their therapeutic class and phantom odor perception among U.S. adults. METHODS: Data were collected between 2011-2014 as part of the National Health and Nutrition Examination Survey (NHANES). A complex sampling design resulted in a nationally representative sample of 7,417 adults aged 40 years and older. During an in-home interview, participants were asked whether they had experienced an unpleasant, bad, or burning odor when nothing

Evaluation of predictive value of olfactory dysfunction, as a screening tool for COVID-19.

OBJECTIVES: At the end of 2019, SARS-CoV-2 was identified, the one responsible for the COVID-19 disease. Between a 5.1% and a 98% of COVID-19 patients present some form of alteration in their sense of smell. The objective of this study is to determine the diagnostic yield of the smell dysfunction as screening tool for COVID-19. METHODS: Cross-sectional, observational, and pro-elective study was performed in a tertiary care hospital from May 25th to June 30th, 2020. One hundred and thirty-nine patients were included in the study. Demographic characteristics were collected from anamnesis. A Sel

Exploring the mechanism of olfactory recognition in the initial stage by modeling the emission spectrum of electron transfer.

Olfactory sense remains elusive regarding the primary reception mechanism. Some studies suggest that olfaction is a spectral sense, the olfactory event is triggered by electron transfer (ET) across the odorants at the active sites of odorant receptors (ORs). Herein we present a Donor-Bridge-Acceptor model, proposing that the ET process can be viewed as an electron hopping from the donor molecule to the odorant molecule (Bridge), then hopping off to the acceptor molecule, making the electronic state of the odorant molecule change along with vibrations (vibronic transition). The odorant specific

Short-term plasticity in glomerular inhibitory circuits shapes olfactory bulb output.

Short-term plasticity is a fundamental synaptic property thought to underlie memory and neural processing. The glomerular microcircuit comprises complex excitatory and inhibitory interactions and transmits olfactory nerve signals to the excitatory output neurons, mitral/tufted cells (M/TCs). The major glomerular inhibitory interneurons, short axon cells (SACs) and periglomerular cells (PGCs), both provide feedforward and feedback inhibition to M/TCs and have reciprocal inhibitory synapses between each other. Olfactory input is episodically driven by sniffing. We hypothesized that frequency-dep

Severity of olfactory deficits is reflected in functional brain networks-An fMRI study.

Even though deficits in olfactory function affect a considerable part of the population, the neuronal basis of olfactory deficits remains scarcely investigated. To achieve a better understanding of how smell loss affects neural activation patterns and functional networks, we set out to investigate patients with olfactory dysfunction using functional magnetic resonance imaging (fMRI) and olfactory stimulation. We used patients' scores on a standardized olfactory test as continuous measure of olfactory function. 48 patients (mean olfactory threshold discrimination identification (TDI) score = 16

Amygdalar Gating of Early Sensory Processing through Interactions with Locus Coeruleus.

Fear- and stress-induced activity in the amygdala has been hypothesized to influence sensory brain regions through the influence of the amygdala on neuromodulatory centers. To directly examine this relationship, we used optical imaging to observe odor-evoked activity in populations of olfactory bulb inhibitory interneurons and of synaptic terminals of olfactory sensory neurons (the primary sensory neurons of the olfactory system, which provide the initial olfactory input to the brain) during pharmacological inactivation of amygdala and locus coeruleus (LC) in mice. Although the amygdala does n

Transcriptional regulation of olfactory receptor OR51B5 by the TBX6.

Olfactory receptors (ORs) are G protein-coupled receptors primarily expressed in olfactory tissue, facilitating the perception of odors. Interestingly, they have also been detected in nonolfactory tissues such as the skin, where they regulate processes like collagen synthesis. This study aimed to analyze the promoter of the OR family 51 subfamily B member 5 (OR51B5) and identify the transcription factors that bind to it to understand the potential regulatory mechanisms for OR51B5 expression. We examined the promoter region spanning 2,000 base pairs upstream of the transcription start site and

Olfactory Outcomes after Resection of Tuberculum Sella and Planum Sphenoidale Meningiomas via a Transcranial Approach.

Objectives  Controversy exists surrounding the optimal approaches to tuberculum sella meningioma (TSM) and planum meningioma (PM). Olfaction is infrequently considered within this context but is nonetheless an important quality of life measure. The evolution of olfactory outcomes following contemporary transcranial surgery remains unclear. This study reviews olfactory outcomes after supraorbital craniotomy for TSM or PM and defines temporal trends in its recovery. Study Design  A prospective study of a patients who underwent a minimally invasive supraorbital craniotomy for TSM or PM was conduc

Audio-visual and olfactory-visual integration in healthy participants and subjects with autism spectrum disorder.

The human capacity to integrate sensory signals has been investigated with respect to different sensory modalities. A common denominator of the neural network underlying the integration of sensory clues has yet to be identified. Additionally, brain imaging data from patients with autism spectrum disorder (ASD) do not cover disparities in neuronal sensory processing. In this fMRI study, we compared the underlying neural networks of both olfactory-visual and auditory-visual integration in patients with ASD and a group of matched healthy participants. The aim was to disentangle sensory-specific n

Patient-reported olfactory function following endoscopic sinus surgery with modified endoscopic Lothrop procedure / Draf 3.

OBJECTIVES/HYPOTHESIS: The Modified Endoscopic Lothrop procedure (MELP) or Draf 3 is a complex procedure, performed for chronic frontal sinusitis that is refractory to standard functional endoscopic sinus surgery. The procedure involves drilling of the frontal T (formed by the septum and middle turbinate`s attachment to the skull base) onto the olfactory fossa often with exposure of the first olfactory neuron and may affect olfactory function. This study was performed to assess patients` subjective sense of smell following this procedure. STUDY DESIGN: Prospective study of retrospective data.

Synaptic activation of T-type Ca2+ channels via mGluR activation in the primary dendrite of mitral cells.

Mitral cells are the primary output of the olfactory bulb, projecting to many higher brain areas. Understanding how mitral cells process and transmit information is key to understanding olfactory perception. Mitral dendrites possess high densities of voltage-gated channels, are able to initiate and propagate orthodromic and antidromic action potentials, and release neurotransmitter. We show that mitral cells also possess a low-voltage-activated T-type Ca(2+) current. Immunohistochemistry shows strong Cav3.3 labeling in the primary dendrite and apical tuft with weaker staining in basal dendrite

Coincidence of COVID-19 Infection and Smell-Taste Perception Disorders.

Many reports by physicians and patients during the 2019 to 2020 pandemic indicate that COVID-19 is associated with elevated levels of odor and taste perception disorders (anosmia, hyposmia, ageusia, and/or dysgeusia). Recent increase in olfactory dysfunction in patients referred to ear nose and throat clinics and COVID-19 infection at the same time encouraged us to examine anosmic/hyposmic patients to establish any association between these signs. It has been shown that the COVID-19 virus exploits the uses angiotensin-converting enzyme 2 receptor to obtain cell entry. This result increases the

Theoretical study for investigating three experimental dose-response curves of rosy odorants on human olfactory receptor OR2A25 via molecular docking calculations and statistical physics modeling.

Humans perceived rosy odorants, as well as other odorant molecules, through olfactory receptors that belong to the family of seven-transmembrane GPCRs. However, characterizing olfactory receptors remains a major challenge, largely due to the lack of X-ray crystallographic structures with high resolution. In this work, a theoretical statistical physics simulation and analysis, validated with molecular docking calculations, was applied to obtain deeper insight into human olfactory perception function by reducing the docking process to an adsorption one. Fitting results showed that human OR2A25 r

The influence of olfactory stimuli on social cognition, prejudice and emotions: a systematic review of the literature.

This review synthesizes research on how olfactory stimuli influence social cognition, emotions, and prejudice, an area that remains understudied despite the olfactory system's direct neural connections to limbic regions like the amygdala and hippocampus. Following PRISMA 2020 guidelines, the authors systematically searched PubMed and Web of Science (2000-2025), identifying 21 eligible studies from 569 records. The literature reveals three core mechanisms through which odors exert their effects: (1) early perceptual modulation of social stimuli, (2) affective priming via disgust and valence sys

The Nose Knows: Olfaction as a Metabolic Gatekeeper in Health and Disease.

Long regarded as an accessory sense, olfaction is now emerging as a metabolic architect, an active agent in energy homeostasis, appetite regulation, and systemic physiology. This review explores the converging lines of evidence positioning the olfactory system not as a passive enhancer of flavor but as a dynamic mediator between environment, behavior, and internal metabolic state. Food odors engage specific olfactory receptors (ORs), which are embedded in neural circuits that project to hypothalamic, limbic, and reward regions. These circuits modulate insulin release, lipid metabolism, thermog
