Vestibular evoked myogenic potential threshold and seasickness susceptibility.

Numerous attempts have been made to find physiological parameters that might predict susceptibility to seasickness. However, to date, no physiological index has been found that can serve as a diagnostic tool for individual susceptibility to motion sickness at the personnel selection stage. A number of studies have demonstrated that linear acceleration in the vertical plane is the most provocative stimulus of motion sickness. The main physiological sensory modality responsible for monitoring vertical acceleration is the saccule. Over the last decade, vestibular evoked myogenic potentials (VEMP)

Post-traumatic benign paroxysmal positional vertigo: mechanisms, clinical phenotypes, and a structured clinical pathway for management.

Post-traumatic benign paroxysmal positional vertigo (BPPV) is a common but frequently underrecognized cause of dizziness following trauma. Unlike idiopathic BPPV, trauma-related BPPV arises from diverse injury mechanisms and is often characterized by heterogeneous canal involvement, greater need for repeated treatment, and frequent coexistence with broader vestibular dysfunction. These features contribute to diagnostic delays and variable clinical outcomes, particularly in the trauma and emergency care settings. We conducted a structured literature search and synthesized clinical, epidemiologi

Why and when to refer patients for vestibular evoked myogenic potentials: A critical review.

Cervical and ocular vestibular evoked myogenic potentials (cVEMPs and oVEMPs respectively) are now used by an increasing number of laboratories to evaluate otolith inner ear function and their pathways through the central nervous system. However, the literature is incomplete or unclear as to what information both c- and oVEMPs can add beyond what a good clinical examination can provide, and what other paramedical tests can provide also, and the present review aims to clarify what is known so far. The following review will describe what is known with regards to both c- and oVEMPs and their use.

Vestibulo-ocular responses during static head roll and three-dimensional head impulses after vestibular neuritis.

This study aimed to investigate whether unilateral vestibular neuritis (VN) causes the same deficits of ocular counter-roll during static head roll (OCR(S)) and dynamic vestibulo-ocular reflex gains during head impulses (VOR(HI)) as unilateral vestibular deafferentation (VD). Ten patients with acute and 14 patients with chronic vestibular paralysis after VN were examined. The testing battery included fundus photography of both eyes with the head upright (binocular cyclorotation) and dual search coil recordings in a three-field magnetic frame. With one dual search coil on the right eye and the

Personality traits modulate subcortical and cortical vestibular and anxiety responses to sound-evoked otolithic receptor stimulation.

OBJECTIVE: Strong links between anxiety, space-motion perception, and vestibular symptoms have been recognized for decades. These connections may extend to anxiety-related personality traits. Psychophysical studies showed that high trait anxiety affected postural control and visual scanning strategies under stress. Neuroticism and introversion were identified as risk factors for chronic subjective dizziness (CSD), a common psychosomatic syndrome. This study examined possible relationships between personality traits and activity in brain vestibular networks for the first time using functional m

Central projections of lagenar primary neurons in the chick.

Perception of linear acceleration and head position is the function of the utricle and saccule in mammals. Nonmammalian vertebrates possess a third otolith endorgan, the macula lagena. Different functions have been ascribed to the lagena in arboreal birds, including hearing, equilibrium, homing behavior, and magnetoreception. However, no conclusive evidence on the function of the lagena in birds is currently available. The present study is aimed at providing a neuroanatomical substrate for the function of the lagena in the chicken as an example of terrestrial birds. The afferents from the lage

An effective maneuver of positional test by turning of the head and body together.

OBJECTIVE: To find the effective maneuver of a positional test. METHODS: We performed three different maneuvers of a positional test in 86 vertiginous patients in supine-lying position in the following order: (1) "body-only maneuver": a patient turned only his body to one side while keeping his head still; (2) "head-only maneuver": a patient turned only his head to one side while keeping the body still; (3) "head-and-body maneuver": a patient turned his head and body together to one side. Eye movement during the whole procedure was recorded by two-dimensional electronystagmography. RESULTS:

Motion sickness and vestibular hypersensitivity.

OBJECTIVE: Motion sickness is poorly understood, although it has been recognized for years as debilitating. Vestibular function is required for motion sickness to occur, but motion sickness can also be brought on without body motion. The aim of this study was to see if there was a correlation between caloric response and motion sickness susceptibility. DESIGN: One experiment was a prospective study carried out on 200 patients. A second prospective study was carried out on 121 patients. SETTING: Patients referred to our tertiary/quaternary care dizziness clinic. METHODS: In experiment 1, cal

Walk-rotate-walk test identifies patients responding to Lempert's maneuver, with benign paroxysmal positional vertigo of the horizontal canal.

Two hundred and fifteen patients were diagnosed and treated for benign paroxysmal positional vertigo of the horizontal canal (BPPV-HC). All patients were tested with conventional positional nystagmus tests lying supine and rotating head for geotropic nystagmus, registered with Frenzels glasses, and in 109 cases with ENG. The walk-rotate-walk (WRW) test, developed by one of us (T.R.) and described in the text, was applied to all patients. The immediate good treatment results with Lempert's maneuver verify the correct diagnosis of BPPV-HC. The WRW test is a more sensitive test for BPPV-HC than e

Autonomic reaction to vestibular damage.

The vestibular system provides inputs to many neurons in the brain stem that participate in autonomic control. This multiplicity of vestibular-autonomic connections plays a variety of roles. Whereas it has been known for decades that unilateral vestibular lesions can result in motion sickness, recent data suggest that the vestibular system participates in making adjustments in blood pressure and respiration that are necessary to maintain homeostasis during movement and changes in posture. Animals with bilateral vestibular lesions are more susceptible to posturally related hypotension than vest

Vestibular plasticity following orbital spaceflight: recovery from postflight postural instability.

Results of previous studies suggested that the vestibular mediated postural instability observed in astronauts upon return to earth from orbital spaceflight may be exacerbated by an increased weighting of visual inputs for spatial orientation and control of movement. This study was performed to better understand the roles of visual and somatosensory contributions to recovery of normal sensori-motor postural control in returning astronauts. Preflight and postflight, 23 astronaut volunteers were presented randomly with three trials of six sensory organization test (SOT) conditions in the EquiTes

Disorders of the inner-ear balance organs and their pathways.

Disorders of the inner-ear balance organs can be grouped by their manner of presentation into acute, episodic, or chronic vestibular syndromes. A sudden unilateral vestibular injury produces severe vertigo, nausea, and imbalance lasting days, known as the acute vestibular syndrome (AVS). A bedside head impulse and oculomotor examination helps separate vestibular neuritis, the more common and innocuous cause of AVS, from stroke. Benign positional vertigo, a common cause of episodic positional vertigo, occurs when otoconia overlying the otolith membrane falls into the semicircular canals, produc

Mapping the Sacculo-collic and Otolith-ocular Pathway Dysfunction in Individuals with Vestibular Migraine.

BACKGROUND: Vestibular migraine is the second most common cause of dizziness and is characterized by variability in migraine symptom presentation, duration, and temporal relationship to vestibular symptoms. Studies have reported alterations in amplitude and latency of vestibular-evoked myogenic potentials among individuals with vestibular migraine. But these reports have limitations in terms of sample size, number of tests, and confirmation of the diagnosis. METHODS: A prospective standard group comparison design was employed for this study. Seventy-six individuals with confirmed vestibular m

Otoconial loss or lack of otoconia - An overlooked or ignored diagnosis of balance deficits.

HYPOTHESIS: Lack of otoconia or otoconial loss may be the major reason for increasing imbalance with age, posttraumatic dizziness and residual dizziness as well as other so far unexplained imbalance affecting probably millions of people. BACKGROUND: It is written in every textbook that we need sensation of gravity for stable gait and stance, especially on two legs. Lack of otoconia is known to cause lifelong balance problems in animals. Loss of otoconia is happening in aging humans, like shown by increasing incidence of benign paroxysmal positional vertigo (BPPV) and in histological sections.

Otolith organ function according to subtype of benign paroxysmal positional vertigo.

OBJECTIVES/HYPOTHESIS: The clinical features and treatment outcomes of benign paroxysmal positional vertigo (BPPV) are known to be different depending on the type of and involved canal. This difference could be due to differences in the functional change of the otolith organ. STUDY DESIGN: Case series. METHODS: Forty-nine patients were diagnosed to primary BPPV; 18 were categorized as posterior canal canalolithiasis (PC canalolithiasis), and 31 were categorized as horizontal canal (HC) BPPV with canalolithiasis or cupulolithiasis (HC canalolithiasis or HC cupulolithiasis). Postural control in otolith disorders.

It was the aim of the present paper to investigate the influence of otolith disorders on human postural control by different methods. The 33 patients of our study had undergone a minor head injury and suffered subsequently from an utricular or sacculo-utricular disorder as evidenced by vestibular evoked myogenic potential recordings and eccentric rotation recordings of the otolith-ocular responses. Postural control was assessed by performing stance/gait tests (standard balance deficit test, SBDT) and by evaluating trunk sway (using angular velocity sensors). Moreover, classical tests of the po

Three-dimensional analysis of eye movements during off vertical axis rotation in patients with unilateral labyrinthine loss.

3D analysis of eye movements during off vertical axis rotation (OVAR) was carried out in seven subjects with unilateral labyrinthine loss (ULL). The modulation component (MOC) of these patients was not different from that of normal subjects. However, the horizontal MOC was significantly smaller when the rotation was directed towards the diseased side as opposed to the healthy side. With rotation to the diseased side, most subjects exhibited a horizontal bias component (BIC) to the opposite side as compared with the diseased side in normal subjects. The vertical and torsional BICs were also inf
